Sample Communication Re: Limited Resumption of Certain Activities

KSLA has created a sample document to assist our members in communicating with residents and families about the limited resumption of certain activities.

We started with DAIL's Provider Guidance: Phased Reduction of Restrictions for Long Term Care Facilities and extracted the pieces pertaining to residents and their families. We also included a health attestation form and an acknowledgement and agreement form, both of which could be signed by family members visiting the community.

You may have already created something similar for your community and that's perfectly fine. You're in no way required to use this document. It is being offered as a resource or a "starting point" for those who haven't developed a communications piece regarding the new policies and procedures. We hope you'll find it helpful.
