Percentage of Vaccinated Staff & Residents Survey

We recently sent out a link to a survey regarding vaccination status of residents and staff. Unfortunately, any responses submitted after 10:30 a.m. on April 8, 2021 were lost. So we've re-created the survey and have a new link for you to use.
