This building is top notch phenomenal!! The staff are very generous and kind and we’re very patient with my father in law during his stay here for rehab! I seriously have no complaints about this place ! I recommend this place to anyone who needs rehab or long/ short term stay!

My mother was admitted to this facility for rehab purposes. She was admitted on a Thursday evening. The first four days she was there, the only "therapy" she received was to evaluate her. All this to say if you are looking to place your loved ones here for the purposes of rehab be sure to take into consideration the day of the week they are going to be admitted because this location does not provide any therapy on the weekends, even though the clock is still ticking for Medicare coverage purposes. During her stay, my mother's feet and ankles swelled so badly she got cellulitis and had to be given an antibiotic. She was released to go home in this condition. While her insurance gave the final denial on her coverage, it was based on information provided by the staff. My mother is on oxygen 24/7. As you might imagine, her nose can become irritated by the cannula. As a result, she requested a humidifier be attached to her oxygen. On three separate occasions, the nurse did not hook the humidifier up properly, which resulted in water being pulled up into the breathing tube in my mother's nose. Apparently, the staff considered it serious enough to give my mother a chest x-ray after one such event. My mother now has severe anxiety attacks if she thinks her oxygen is not working properly. Last but not least, on her final night there, my mother asked the night shift nurses aide to help her get settled into bed. Instead of walking around to the other side of the bed, this woman grabbed hold of my mother's arm so tightly she left fingerprints, which turned into bruises, and she fell across my mother's stomach. This aide was a larger individual. With the health issues she has, my mother does bruise easily. However, at no point did any of the other staff members at this facility cause a bruise while helping her. Additionally, my mother was told by other staff members that other residents had been complaining about how rough this aide was being with them, too. After this event, even though she was approved for one more night, my mother refused to stay. As we were getting ready to leave, the Director, Head of Nursing, and the Social Worker came into my mother's room asking what had happened. My mother gave her version of what had taken place, and they looked at the bruises on her arm. They did not take any photographs of the bruises, but they assured my mother and sister that a full investigation would be conducted to ensure residents were not being mistreated. Staff shortages and lack of training seem to be part of the problem here. However, I do want to say my mother also had kind, caring staff members who took care of her, and she absolutely loved the food.

In response to their comment they left back, I NEVER applied at your facility. I actually came there through agency. And it was so HORRIBLE, I cried the whole shift and called my agency to tell them to never send me back. Wow the lies you all tell, I think your long list of 1 star ratings and inability to keep staff are a perfect representation of this facility.

It is a nice, new facility that has very friendly people working there. Everything is clean inside. The front entrance is large, covered, and quite easily accessible, which is great if your loved one who is a client there needs wheelchair accessible transportation.

Would not go to coldspring, when you meet them they are great. Place stunk of pee. Some of staff acted like they didn't know what to do, kept his water away from bed. He was getting sicker each day. Wanted to bring him home on 4th day, he was very ill. Took him home that nite had to take him to er, he had covid, they didn't seem to care. Even so called house doctor never to see why , called they the nurse said no one there had it, but yet he got it. PLEASE DON'T GO

My mother is in Coldspring Transitional Care Center for rehab, then she's going into their nursing home facilities. They have rehab and a full nursing facility. She'll be staying in the same room even. We like the proximity to my home in Alexandria, so the location would be the best. The rooms are private and spacious. They have very few activities. They have bingo and some game night. My mom hasn't been to any of those yet, but I see as I go by there what they have going on, but it's very limited. The staff seems professional and helpful. The food is very tasty. She's been happy with the food. I've been eating dinner with her just about every night. It's good portions. They have a varied menu. She's only been involved with rehabilitation exercises and she's very happy with that.

Would not recommend this facility for your loved one. My dad was there for a week and a half before he passed away for therapy. We were lied to by the social worker there during our care conference. My dad said he was ready to go home after 5 days of therapy and was told that it wasn't up to them or the doctor when he could go home that it was up to the insurance. I don't know what they call extensive therapy, but doing therapy once a day in the mornings and leaving him in his room the rest of the time isn't that for me. The person working at the front desk was rude every time I went in. The hallway always smelled like urine really bad walking through. My dad had called home multiple times saying he rang for a nurse a long time ago and no one has came in yet. When we found out that we were lied to about him being able to come home anytime he wanted, we were going up there to bring him home, but he didn't last that last day before he passed away. No one ever called about his updates after the care conference. It's all about money there, not about the patients.

My mother has moved into Coldspring Transitional Care Center. She has been in a couple of other facilities before she got to this one, and it's much better than those. Still, it could be better. She has her own living space, so that is very nice. I call it a semi-private because it shares a bathroom, but it's designed more like the newer dorm rooms in college so that she doesn't see her roommate when she's in her bed or in her living space. They only share a closet and a bathroom. Their actual bedroom is not shared, so it's very, very nice that way. The staff is nice. Upkeep and maintenance are very nice. What I like best is the therapy.

My husband spent over 4 weeks in this facility as a rehab patient and though I give it 2 stars for cleanliness and the rehab team seemed top notch, the general care was spotty at best. Very short staffed and communication that is nonexistent especially from the social workers and upper management. You can try calling the separate departments but you have to be persistent to get a call back. Would not recommend this facility for your loved one. Once they get the insurance money flowing they avoid contact.

This place is terrible! Don’t t ever send your loved ones here! My mom was there for six days and transported by ambulance right back to St. Elizabeth for fluid on her lungs and on top of that tested positive for Mrsa! Never fed my mom the special diet she was supposed to be on. She was supposed to be on low sodium and sugar! Sent her wrong food all the time. Plus with sugar and salt packets on the tray! Couple nurses actually told me that they treat patients like it’s their home and give them what they want even if it’s not on their diet. I went off and said well if that’s the case then my mom will be right back in the hospital! That’s exactly what happened six days later back in the hospital! DO NOT go to this facility!!

My father went in for rehab on a broken femur and less than two weeks broke his other femur and damaged the work on the previous break. I was there to visit every morning and every night before and after work. One night his catheter bag was completely full , and had to ask the nurse twice to empty it. One nurse repeatedly told me they had one nurse for 15 patients, every time I asked her about something! This is not everything. Be very careful on your decision of leaving your loved one here. They are also removing the negative reviews on this site.

DO NOT USE THIS HORRIBLE INCOMPETENT PLACE! THEY PRESCRIBE MEDS AND DO NOT MONITOR OR CHECK FOR ADVERSE REACTIONS OR SIDE EFFECTS! THIS PLACE IS ABSOLUTELY INCOMPETENT. The wait for help to the bathroom or to empty a catheter bag is 1 to 2 hours! My father went there to recover after a broken femur surgery and 2 weeks later he has two broken hips after falling at night because they did not help him. My care was excellent from the staff at Cold Spring Transitional Care Center during my 3 week stay. Kelli Lucas and the staff was very caring, ( Thank you does not seem enough). The staff with Physical Therapy is incredible, helping me to progress each day. Adam and his staff is both caring and as well as extremely informative about my progression. I would recommend this facility to anyone.

I am deeply concerned both of my parents have to go there today. I do not live in the ky and I have very mixed feeling's about them being there. they both have been together for over 50 years I worry about them not seeing each other even though they're rooms are attached. my siblings live there close by along with a large number of family so I pray that they get visitor's to help them thru this mess

We had my mom here for rehab twice now...1st stay was August 2019 with a broken hip and in Jan 2020 with a fractured back....her stay was amazing....they have a beautiful building, her room was large and very comfortable, the nursing staff is very kind, professional and knowledgable. They have an in house Nurse Practioner, Susan Cordrey on site. She actually found that mom was having a reaction to a medication that was causing dementia like symptoms....she gave me my mom back ! The therapy department was wonderful....mom is now home walking with a Rolator walker and actually misses being there ...she really enjoyed all the activities ! Our community is blessed to have such an amazing place for our loved ones who need Physical Rehab or a Long Term Stay !

I was there for a week for rehab for having both knees replaced. The therapist were wonderful . The rest was very disappointing. Waiting for someone to help me to the restroom for up to an hour was the norm. I just started to do things for myself risking falling on my 2 new knees. No one came after my table flipped onto the floor despite the loud noise. I couldn’t wait to get out of there. I felt degraded by some of the employees. I complained to anyone who would listen. I just worry about the ones who can’t speak up for themselves and told them about that as well.

This place is terrible my wife stayed here they didnt tteat her right when she would call them to go to the bathroom would have to wait 30 to 40 min some times you just dont treat people like that when they have to go to the bathroom you should get to there room right away most of the aides and nurses were nasty if you dont like your job you shouldnt work there you should work there because you love people and want to help people that need help ..

I want to say how great the Occupation and Physical Therapy Team is at Coldspring Transitional Care Center. I can't believe how much stronger my core strength and my range of motion has developed. Very pleased with all the staff has taken care of me. I am getting better every day and getting more independent in taking care of myself.

As a patient for 4 wks at Coldspring Transitional Care, I'm in one of the best place I could possibly be. I feel I've been treated like royalty. It is one of the best places I've ever seen. Currently there are some staffing issues which keeps me from giving a 5 star. In regards to cleanliness and compassion there is not a finer place a person could be. Additionally, I would give the therapy department a 5 STAR PLUS as these are without doubt the finest that I have worked with in 30 plus years.

Coldspring Transitional Care Center is great and fantastic. The staff is great and the care is fantastic. My mom has been there for 30 days. They have all kinds of activities. They have a cafeteria and the food is great. They have tablecloths and cloth napkins. It's very nice. They have an outdoor area for you to take the patients out.

Both of my parents were in this rehab facility at different times. All of the staff were attentive to their needs and were very respectful to our whole family. My siblings and I visited at various times throughout the day and didn't have any issues during her stay. I would recommend this facility to anyone in the NKY area that needs strengthening before going home from the hospital. From their admission to their discharge, everyone was so caring and wanted the best for my parents. Overall great facility!

The aides, nurses, dining staff, and therapists were all professional and treated my mother with respect and compassion. It is never easy to see a loved family member have to leave their home and enter a nursing facility; but it is a relief when you know they are safe and treated with compassion. Thank you.

My husband has been in Cold Spring out-patient therapy since November of 2017 entire staff Lauren-OT, Joel-PT, Katie-ST and all the back up therapist have been wonderful. A staff of dedicated people. I have recently had a meniscus repair and told my orthopedic doctor Cold Spring therapy is the only place I would go. I have and would highly recommend this great group.

My mother, 82, spent 12 days at CareSpring in Cold Spring, KY, for rehabilitation after having severe pancreatitis and her gallbladder removed. The facility itself was clean and organized with many activities available for the patients if they chose or were able to participate in them. The therapists were helpful and always willing to answer any questions that she (or our family) had. The only thing that my Mom didn't like about it was that when she was lying in bed she was unable to reach the switch to turn on the light above the bed. My neice solved that problem for her by tying a ribbon to the chain so that she could. Overall, a nice facility to be in when rehab is needed.

My grandma had a recent stay at Coldspring in March. During her stay there, she made great progress and was able to gain the strength she needed to return home and live independently. A very special thanks to the therapy staff who worked closely with her to give her back the confidence and strength she needed! The staff at Coldspring went above and beyond to make sure she was treated with the best care. Everyone was extremely patient, caring, and helpful throughout her stay. Grandma continues to talk about the ones who impacted her during her time there. My cousin was here a little over a month and fell 3 times, the last time landing her in the hospital with a head injury. The place looks like but I wouldn’t leave my worst enemy there!! They were HORRIBLE!!!!

When I entered Carespring Transitional Care Center in Cold Spring, Kentucky, I was practically immobile. I felt helpless and hopeless and could see no possibility that I would ever recover from a virtual body paralysis. I could not move but a few muscles. I could not raise my feet, draw them to my waist, sit up on a bed, push myself with my arms, or even transfer from the bed to a wheelchair by myself. I saw my future and very bleak. Then I was placed in the care of the staff of Carespring who turned all of that around. They managed to give me the hope so desired. They showed me that I could take back my life and my future. Their ways inspired me and gave me the drive I needed to regain my arm and leg movements, all of it. I will not deny that it was a painful, slow and depressing recovery, and it was slow-going; more than once I was ready to throw in the towel, but these folks at Carespring did an amazing thing for me. They reversed my attitude and my outlook, not just with efficient therapy techniques, but also displaying a firmness in expectations with humor and cheerfulness. I am very grateful to all at Carespring who attended to me, and I am especially grateful to the core team who attended to me daily, during therapy sessions and while recuperating in my room. The truth is that these folks became more to me than just therapists, they became friends. I have included picture of the six who most influenced and encouraged me to better things and to whom I attribute my success. For them I remain eternally thankful.
