My mom is in Crestview and has been for 2 years. They have taken very good care of Mom. The staff and all workers are so great and they really do care about the residents. The activity director Lisa does such a great job finding things for the residents to do and programs for them. My sincere thanks again to everyone at Crestview.
I have been on both sides of the fence here. My Mom was a rehab patient here and had a great experience. I have been employed here for 3 years and I love it. The residents are loved here.

Crestview is a small home-like facility. Being a smaller facility, residents and families can get the individual attention that they deserve. I highly recommend Crestview!!
I have several family members who work in nursing, and so I have a sufficient understanding of the fact that these facilities are NOT ideal. That being said, I would not wish this place on my worst enemy. I have a close family member who was a resident at the facility for approximately 18-24 months. At the beginning of her stay here, there were no serious concerns. Unfortunately, I became aware of several concerns egregious to the extent I was compelled to file reports with the Kentucky Board of Nursing and with the Adult Protection Branch of Kentucky's Cabinet of Health and Family Services.
My experience with Crestview Care and Rehabilitation Center was great. The admissions administrator was very helpful and explained a lot to me. It was very clean and did not have that nursing home smell. The rooms looked very clean. The staff who gave the tour was very nice. They have an outdoor that is like a garden. And it was very nice.
Our mother was there for rehab. Facility was fairly nice, the rooms were clean and spacious. Residents were pleasant to our mother but some of the staff seemed to be lacking in compassion and empathy. Our mother is now doing well at another facility.
My mother’s experience at Crestview was fantastic. She raves about her rehabilitation and the workers who showed extraordinary kindness while caring for her.
My Mom passed away here after a short illness and then my Dad was here for a couple of weeks. I can’t say enough good things about the staff here. They felt like family. They were so caring and took the best possible care of both my parents. The care they received was topnotch. I can’t thank them enough.

This place could use a complete overhaul to both the physical structure and management staff. The building looks to have been built sometime around the 1960's and outside of the community area, could use some renovations. The TV's in the room are extremely small and dark, which comes from the LCD display burning out. The facility itself seemed to always smell like an old elementary school, which I guess could be due to the age, but I'm not for sure. The flooring and walls are in desperate need of repair and a fresh coat of paint. Honestly though, I could overlook all of that if the management staff was at least halfway concerned about their own competency. Getting return phone calls about a family members health progression was almost impossible. The nursing staff also decided to completely disregard our decisions when they conflicted with the nurse practitioner. Since we had a power of attorney, that was technically illegal, but they did it anyway. Also I wish you all good luck with getting all of your clothing items returned if you use their laundry services. I don't know who they use, but they consistently lose items. In closing, I'd avoid this place like the plague if you can, even if you have to drive a little further for your loved one's care.
Never answer the phone or pas on messages can't actually understand most of the reviews I've been reading how people say good things about them the More Oklahoma house is in need of staff change
I have had the pleasure of visiting Crestview on a few occasions. Each time the center was clean, the staff friendly and knowledgeable, and the experience a positive one. I would highly recommend Crestview Healthcare and Rehab to anyone looking for exceptional care of their loved ones.
Crestview is a smaller facility with a home-like feel. They have a lot of staff that have been there for a long time. One of the CNA's has even been there for 39 years. The staff are caring and treat residents like they are their own family. Crestview has a lot of repeat business because of the caring staff.
Crestview is welcoming, kind, knowledgeable, attentive and treat the residents like family! Such a calm, nice, fanily like environment! I would trust then to care for my family!
Facility is clean. Staff is attentive to residents needs. Food offered is good. Activities for the residents to participate in. Hair salon is great.
I had a recent 20 day stay at Crestview. I could not have been treated better. The building is older but was very clean. The staff was caring and helpful. The physical therapy team was welcoming and thoughtful. They even called me to make sure I got home safely. Thank you for such good care.
They were being unfair to a family member of mine that they complain that she had too many unexcused absences.
My mom was at Crestview Care and Rehabilitation Center. I've had some of the food and it was good, but since mom's a picky eater, she didn't like it. The building was a little rundown. They had some holes in the wall, which could easily be fixed if they just wanted to. If you were to hit the button to call someone, it would sometimes take an hour or so before anyone would come. The people were pretty nice though and her room was spacious enough.
Terrible my grandmother was neglected this place is full of crooks
I was not impressed with this place or the staff. This nursing home/rehab will never be considered for another family member. I want go into details. I certainly will not recommend it for sure.
My mother-in-law was placed at Crestview under Hosparus care from Louisville. They were very understanding and worked to make sure she was as comfortable as her situation would allow. They had care plan meetings via phone including several family members so everyone had input. They were very accommodating and even called if there were significant changes. When she passed we were called as soon as they found out and offered to visit if we wanted to. Everyone was great and I know they have a tough job but I think they did the best they could and the whole family appreciated that during our loss. On the star ratings, I didn't get any feedback on the food as she really didn't eat during her stay and obviously didn't participate in any activities.
Crestview took great care of my grandmother on several different occasions. The nursing and therapy department are top notch! The whole facility truly cares about their patients on every end of the spectrum!

This is a small and friendly facility with a caring staff. Can't say enough positive things about this place.
My mother is in Crestview. The staff is AMAZING. It gives me great comfort knowing how well she is cared for.
My mother moved into Crestview Care and Rehabilitation Center. It's small, it has a small community feel to it, homey, and the staff treats her very well. She said the food is okay, she likes her salt, and they do not have salt, but it's fine. They have bingo, somebody comes in once or twice a week with dogs and cats for pet therapy, they have movies, church service, and they have something planned every day.

Both my parents were residents of crestview for a short time while recuperating from hospital stays. The staff was friendly and attentive, the food was adequate, and there were activities available every day.
My mother is rehabing from a broken hip. The level of care and PT and OT have been excellent. Staff is very accommodating and courteous. All of this in the midst of remodeling. They have done a very good job and I am very comfortable with my mother being there.
