Never answer the phone on weekends and nights. It seems there are more patients there than help to help the patient.
If they tried really, really hard they could almost be average.
My 85yo mother was there Fall 2022 for 8 weeks. After a long bout with sciatica, she had grown frail and weak. The PT team was outstanding! They were constantly moving patients in and out, working with them, providing encouragement, and there is a friendly dog that one of the PT's brings for the day. I cannot say enough good things about the PT quality, at least in 2022. When she was discharged, she was strong and confident in her walking abilities. There were other hiccups though. They were changing Food Directors at the time. So lots of mishaps with meal quality and delivery times. One day, no dinner ever came for mom and her roommate. So the staff member went to McDonalds, I kid you not! And brought back food for them. Probably broke some rules there but that person gets an A for problem-solving! I would send a family member there again, no hesitation.
I would have given 0 if possible for the Rehab Facility. The person who said that aesthetics was the only thing going for it was correct. They had trouble getting night meds upon arrival and were just going to skip them until I said that wasn’t acceptable. Continually wrapped wounds incorrectly from what Wound Care At Baptist prescribed. Even missed getting a breakfast one morning until she called at 10. Had only one sponge bath in two weeks and was supposed to be helped with bathing twice a week. Supposed to be checked on by aide every two hours by an aide and that never happened. Pain medication not given at correct times when going out for wound care even after a call in the morning to remind them of appointment and when to give medicine. Talked to numerous people to get help and address concerns even the Nursing Director and things got worse instead of better. Rooms never cleaned or bedding changed regularly in the 2 1/2 weeks I am not sure how this place had 5 stars it is very misleading in the hospital printout when trying to pick an appropriate Rehab Facility. Only saving grace were two wonderful nurses on the weekend who were very good at their job and took care of their patients. I practically had to be there every day or every other day to see the standard care was given and even with numerous discussions sadly things did not improve.

I am often angry about the care that my mother-in-law receives at Forest Springs Health Campus. Some of the more egregious offenses have included the inability for anyone on staff to ensure that she received the latest Covid booster shot. It took me four months of advocating for her in order for her to receive the latest booster, in spite of numerous Covid cases among employees and residents. Only when I told the administration that I had contacted the health department did they follow through with multiple requests from me, from my mother-in-law, and from her husband. She is supposed to be bathed twice weekly. This does not occur. She is sometimes only bathed once per week and only after multiple requests. I could go on.
My Dad was there and it was the saddest experience ever. He came in infection free and is now back in the hospital with a wound vac trying to overcome what took place there. They are way understaffed and I would have sympathy for that except they want to fill every be there. Stop taking patients you cannot possibly take of.
Horrible people in Rehab area! My dad waited over an hour after hitting call button. They spoke harshly in a loud voice to try harder on a peddle bike thing. He had only been there 3 days. Worst of all they yelled at my 74 year old uncle for helping my dad into bed after waiting over an hour for help from the staff and listening to dad hurting from sitting. The facility looks very nice, the staff and care ( at least for the rehab portion) are horrible. I would have never known this if it weren't for my dad having a roommate and them yelling at my uncle.
Where to begin...I'm going to keep this as short and sweet as possible. The aesthetics is the ONLY thing going for this place. If you truly love your parent, family member DO NOT bring them here. They let them lay in urine for hours. I have witnessed residents be dropped and residents unable to feed themselves, trays dropped in front of them with no assistance. My dad was here for rehab after having a severe stroke and my sisters and I rotated coming every day to ensure our father was properly cared for. Even with us changing him, bathing him, they still did the bare minimum. The upper management including ADON La'Keisha James is incredibly unprofessional. I requested to speak with someone because my father's bed was saturated in urine and he was very close to falling out of the bed. The aide tells her and her response is "WHAT DO THEY WANT" in a very nasty condescending tone. I've spoken to many residents to see how they like it there. They all say the same thing...they are horrible. My dad had to go to the hospital because he got COVID when the ER nurse at Norton Brownsboro Hospital that cared for him found out where he came from she said OH they are horrible at Forest Springs and that she has cared for many people that have come from "that place." My dad also got a pressure sore while at Forest Springs and my sister informed the nurse before it got bad that the skin on his foot was breaking down. It snowed and we were unable to get there for a few days and when we finally were able to get there they had done nothing for this bed sore and it had gotten much worse. Pictures attached....this happened with us coming everyday. The kitchen staff blasts rap music in a senior citizen home and you can hear it throughout the halls. I have many horror stories, but I will say there are a FEW really good staff members and they are aides 1st shift Elisa and 3rd shift Patty and 3rd shift nurse Emily. They treated my father with respect and dignity and went the extra mile every time they worked with him. Thank you to these very special ladies. If you love your family DO NOT bring them here. You have been warned! (I guess the photos I attached are too graphic for google, but STATE will love to see them)
Very disappointed. I have a family member there. We are unable to see him due to COVID. I have left 7 messages requesting a nurse to call me. I have yet to receive a call. We also dropped a bag of his belongings off. The lady at front desk said she would take it right to his room. Well... he never got it. I would not recommend this place for your loved ones.

My Dad is currently a resident here, the staff have always gone above and beyond in their care. The facility is lovely, the food is very good. A happy Dad is a happy daughter.
We liked Forest Springs Health Campus. Everything was topnotch. The staff was really nice and the residents we talked to all really liked the place. The apartments were well-done and well-detailed. We liked the meal plan for the people who are living in the independent living side.
This facility felt more like a traditional nursing home than a residence to me. The tour was informative but probably not what we are looking for.
Forest Springs Health Campus was very nice, but they didn't have independent houses. They have apartments or a triplex. The facilities were very nice. Even the eating facilities were wonderful. The rooms were great and the staff was wonderful. They treated me like a king. They had a wonderful facility for playing cards, bingo, pool tables, they go on trips, and they have people who come in to put on shows. I liked the place a lot. However, if I live here for three years, which is over $80,000, I will get nothing out of it other than a hotel room basically.
This place is first class all around. Friendly knowledgeable staff, professional management, clean facilities, you name it! Thanks Forest Springs.

My husband was there for a month of rehab after breaking his hip. The staff was wonderful.
My Mother is really enjoying her stay here. She is truly happy here and I’m glad we picked this facility. The staff is wonderful and they are taking care of all her needs. No matter what, they are always around if my mom needs help. There is not one staff member you walk by that isn’t friendly and personable. The food is good and she enjoys it. They do serve 3 meals a day even if my mom only chooses to eat 1 a day. The aesthetics of the facility is very clean. The apartment she is staying in is more like a patio villa. It has an attached garage and a full kitchen. It’s a 2 bedroom 2 bathroom apartment with ample amount of space. For activities, they have festive parties for different occasions, they have people that come in and do educational classes, arts and crafts, and there are exercise classes 3 days a week. I would highly recommend this facility.

I'm moving into Forest Springs Health Campus. It has independent living villas, individual houses, and one-bedroom units. They take care of all the things in the rent. The only thing you have to pay for is your phone. They do housekeeping twice a month. If you don't feel like cooking in your kitchen, you can go to the cafeteria and get a home-cooked meal for $5. It's really nice. There was a man walking with his dog. They have a clubhouse that you can have a party in with no charge except you have to reserve the place. They have a library and a beauty shop, and a lot of activities. I got in the mail an itinerary of what they have this month. They have people and doctors come in and give lectures, and they have merchants come in and sell their stuff. I only associated with one person, and she was very nice. They gave us a meal and it was very good. The facility was clean.
My parents started in a villa which was a wonderful location for them. After my mom passed and then my dad had health issues he moved to the full time nursing care and is now in the assisted living area. Everywhere they have been on this campus has been wonderful. The level of care is fantastic and the people are great. He could not be happier. There is always something going on to keep them busy and entertained. I call daily and get updates on what he got to do during the day. We could not be happier with the location they chose and how things have progressed as the year has passed. A special thanks to all who work there!

Forest Springs Health Campus is the nicest place I've seen. The staff is very personable, very nice, and wants to help you any way they can. The food is really good. They have housekeeping coming in twice a month, and the units are all updated and beautiful. We have different activities every day.
My aunt was at this community for her rehab services. Unfortunately they did not provide the level of care I was comfortable with. I feel they were under staffed. There is room for improvement in this community overall.
I would give this 0 stars if I could. This place is terrible. My great Aunt was there who is 90 years old and has had a beyond horrible experience. She fell in the bathroom and it took her 3 hours to crawl 2 feet to pull the emergency string which means she hadn't been checked on in over 2 hours which is their policy... which baffles me being that she is there for REHAB. Then they basically blamed her for not yelling for help... SHE IS 90 YEARS OLD SHE CAN BARELY TALK!!! I'm just glad she wasn't there perminantly. The only good thing is, is that one of their physical therapist was very kind and extremely helpful! I wish I could remember her name. It was hard to get anyone to answer my questions because I think may have seen 3 staff members the the entire 2 to 3 hours I'm there anytime I go. Let's not forget that they found her at 6ish in the morning and nobody in the family was contacted till almost 12 or 1. Ugh. I hope they get their staff on the same page.
Forest Springs was very clean. The people were very helpful. I learned a lot about senior living, senior care, and rehab care. It was my very first experience with them since my wife was a patient there. I ate there three or four times and never had a complaint. I would highly recommend this place to anybody.
Forest Springs is a very nice campus, fantastic, and very clean. The staff are very professional and organized. The thing that I like about it is the location since it is close to my house. They have three levels of care and brand-new patio homes, which is what we chose for my father. You can transition from assisted to long-term care. They have great activities for the residents. It met all of our needs.
My dad stayed at Forest Springs Health Campus for rehab. I thought they were understaffed. I felt like I had to follow up on nurses and aides and couldn't just leave instructions and get it done. I had to continually follow up to make sure that it had gotten done.
Beautiful building, friendly, helpful staff
Forest Springs has four levels of care, it is brand new, the personnel are very nice, and we think we’re going to like it there. The food is good, and the staff is very friendly.
