Did you have a horrible childhood? Do you seek revenge on your parents? If so, then this is the place for you. If I could give -5 stars I would and that’s still not enough negative. There is nothing good about the place at all. I can’t think of one good thing to say. Just grateful I got my mom out of there before they killed her. Please do not send your loved ones there.
I've been here a couple weeks, hoping to get out by the end of the month. They need to hire some new cooks. The food they serve is usually bland or has way too much pepper. Last week they served raw chicken to some of the residents. Theirnmedical staff over all are great.
This place helped me tremendously at my worst..thanks staff.
My mother went in on April 19 2024,on a Friday,,she was laughing,talking up a strom to them,joking,she got to go to her room,Up walking around,we took her walking her around the second floor..We was so happy to find a good place.All the people was so nice,,( I thought) .we left,came back the next day..To bring her stuff,,she was sitting in hall..I ask her,what she doing in the hall,said she want to keep a eye on them,,said they never check on her in her room..said they have not feed her,,Thank god,,we brought food,,so made sure she was feed..So her grandson came up Sunday,said she was a little out of it..He had to feed her..She said she was so hungry and still in hall...So we came back up Tuesday,she was still sitting in hall.She was a zombie,,She keep pointing at the nurses.saying bad,,crying..she kept nodding off and on.I kept saying want to go lay down..she say no..she was scared..The nurses said she has not ate, no breakfast or lunch,I told them no wonder,look at her,,I got a some food,and put it in her mouth,it just sits there, so I had to take it out of her mouth..I told the nurse this is stupid,can't feed her stuff,had a diaper on her..When she came in 5 days ago,went to bathroom on her own,ate like a pig..walk..I told them,got to get her off some of this medication.They said they would..Guy named Chris,,called me next day,,rude,,said she falled,and trying to rip her clothes off,I ask him,was she okay,,he said don't know,,he told me that if they can not give her what they want,for me to come and get her..he was pissed,, so we was coming next day to come and get her.Got a phone called said your mother pass away..this morning They did something to her,,it was her falled or they over drugged her..found out she was on second floor,moved her on seventh floor all the way at the end,in 712 B,something not right..please do not bring your family there..I learn the hard way,,Brought her in on a Friday,Tuesday she was dead.April, 25, 2024.There will be a investigation on them..
Landmark is a great place to clear your head and to get a great foundation for sobriety/recovery. Staff (PN Elizabeth and Counselor John along with all the techs and administrators) were extremely helpful and supportive. Had a very worthwhile experience. Would highly recommend it. :)
I live at this facility. I like the care I receive here. The staff is friendly and caring. Housekeeping keeps the building clean. I also like going to casino night here.

My experience with this facility has been overwhelmingly positive. I am impressed with how the staff interacts with the residents. I enjoy observing these care givers’ reflection on maintaining great practices and making improvements to ensure residents experience well rounded quality of care.
I was there a year ago (2022) and my time was miserable. I was told by staff that my medication was not there yet and the staff was very unhelpful and the food was gross. I wouldn't send send anyone there.i had a leg amputated and they were rude about caring for me. There was bugs flying around roaches coming out of the sockets
Care is very poor. Recently had open heart surgery. Came to this facility for my rehabilitation. The staff treat me as if I am a problem for them and it takes them over an hour to come help me with things that I need. I've been in the facility for about 2 weeks now and still have not been seen by a Dr. I'm constantly told that they don't have my meds for me. I will not be recommending this place to anyone.

I am sending this in because I want the community to know Landmark of Louisville is a good nursing home and therapy facility. Now that we have new management, it’s beginning to fill with good workers. I can honestly say when I’ve first got here, I didn’t like the employees and how they treated the residents. Since then, they’ve been through five administrators and there’s one that’s actually made a difference and his name is Mr. Chris Kellogg. Mrs. Jennifer (currently admissions) has been an outstanding employee and was my first CMT and has always cared for the residents.”
The facility is very welcoming and the staff are friendly. Excited to hear about the Cardio Unit opening up on the 3rd floor!
I can say that this establishment has grown over the years. There is so much peace, patience and kindness that surrounds this place. They really do work with you and do their absolute best to meet your needs! 10 out of 10 recommend👍🏾😊
The facility has changed tremendously. The staff and employees work tirelessly to make sure the residents are taken care of. I was highly impressed with the operations , as well as the cleanliness.

Such a loving staff. Right when I walked in I knew this was the right place for my grandmother and they have never let me down. It’s so beautiful and clean. They keep me updated and keep my grandmother busy with all of the activities. I highly recommend this home to anyone who has to make that tough decision for their loved one.

What I don't understand how is it possible Landmark refuses to acknowledge it's mistakes and try improve workplace condition for the patients. I have a loved one here and I am not impressed. It is sad where leadership views this place as nothing but revenue generating facility as opposed to making sure families are very comfortable with their loved ones here. The people here are someone's family who are waiting for them to come home happy and health not having fear of not knowing how they will be treated. If you look at the history of this place it has a bad reputation and it is disappointing that government doesn't step in force them correct their mistakes. I really hope Landmark gets better for the sakes of these patients.
Some people in the world go to work, do their jobs, and go home. Others take those jobs to a whole new level entirely. The staff of Landmark of Louisville is taking “ CARE and COMMITMENT” to a whole new level! Times have changed and we’re striving for greatness! One day at a time!
If you love your loved ones DO NOT send them to this place. They abuse the patients, leave them dirty, doesn't give them their medicine as ordered, don't serve them their food on time if they're unable to walk to the dining room. While visiting my dad I heard patients crying for help and no one checked on them. I heard them screaming no don't hit me anymore. They kept unplugging the phone in dad's room so he couldn't call us and tell us what they was doing. So I bought him a cell phone and everything was going good, until one evening my dad called saying that a male worker slammed him down in a chair and hurt his shoulder.I went up there to checked on him that night and reported it the next day. A day later my father's phone was "lost". I started going up there more (almost every day). However I have Sickle Cell Disease and the moment that I got sick and couldn't go up there to check on my dad i got a phone call saying that my father coded, 30 minutes later he was gone. The nurses lied on the reports about his blood sugar saying they checked it and it at 8am and it was 234 they said they gave him his insulin and checked it again at 11:22am and it was 176 and my dad was disoriented so they called EMS. When EMS arrived they checked his blood sugar it say "HIGH" (which is over 600) my dad died before reaching the emergency room. His blood sugar was still reading high.

My mother was there several years ago with dementia you may get good care and are sane and alert but when your loved one is alone ... beware of shower room its unsupervised other nursing homes showers are heavily watched see for yourself

If there were negative stars i would use im on phone as i text with a good friend who lost his mother due to the treatment and lack there of from this establishment so please if you want a loved one to be safe secure and monitered for good health DO NOT BRING THEM HERE

I believe that landmark provides the opportunity and tools for patients like me to implement in their daily life. The staff are receptive to my needs and the patient engagement specialist are keenly aware and invest a great deal of time and effort into each patient. The therapists here listen as apposed to telling me what I need to do. As someone who has been in and out of treatment, I would strongly recommend this facility to anyone needing help with their addictions.
Most of the staff do not care for patients. Dirty place, patients personal belongings go missing often. Something fishy is going on here.... just go to any floor and see all the employees chilling in the center area gossiping. Landmark is cheap for a reason.

My aunt stayed at Landmark (Parkway Medical Center). The general appearance of the place was OK. It had a pleasant smell and no odor. They had furniture in the lobby area where people could sit. The food could be brought to the room. They had some games for them to do like bingo and movies with popcorn.

My mother-in-law was in Parkway Medical Center for about six weeks. The room was very clean and the attendants were very attentive. It seemed like everything she needed was there. They had to grind up her food because she couldn't swallow. I liked its central location, which is where she lives, so that location was perfect.
Parkway was my chance to make it back to work and live as well as I could after a open heart surgery. the therapy department and staff were encouraging and warm it was home away from home. When I needed them they were there and the food was wonderful thank you dietary for the homemade vegetable soup! Everyone cares about you there.
If I had visited this place before my friends were transferred here, they wouldn't have been here at all. I have visited this facility on several occasions. There is alot of confusion here. No one knows what the next person is doing. There is no communication between anyone. Even the Head Director doesn't keep his word on what is supposed to be done if you request it. I don't recommend this place to anyone Short-term or long-term. There are other places that would be much better and possibly have better communication between patients and others.
I called the facility and asked to speak with human resources! The lady asked me what I wanted! I told her I wanted to speak with human resources, She Replied "speaking" I asked are you with human resources. She replied. "Maybe" I told her I did not appreciate her doing that. I told her I wanted to speak to human resources about employment. She replied, if you would have told me it was about employment. "I would have connected you" to me this is very unprofessional and no one should be pretending to be someone that they are not! To me, this is fraud! I can prove that this happened beyond any reasonable doubt! I am disabled. There is a assistive device I use that records all of my phone calls for notetaking purposes. Since I am unable to use my hands! My notes are time and date stamped! I have a record of this call! If they are just honest about who you are speaking with. I can only imagine, I do not want to imagine!
My Dad suffered a serious stroke and was sent to Parkway after another facility almost killed him. The care he received at Parkway was fantastic. I could not recommend this facility enough. It might not be the most beautiful nursing home in Louisville, but if you want a place where the patients come first and the staff truly knows what they are doing, Parkway is the place to be. Thank you Parkway for saving my Dad.
