My mom has been a resident at this facility two different times for the Rehab to Home Program. She has received top notch care from all disciplines including the therapists, nurses, aides, dietary and housekeeping! In my career I have travelled to several different facilities and Liberty Care is one of the cleanest and nicest care facilities that you will find. I’m very thankful to them for going above and beyond to provide such wonderful care for my mom.

This place is nothing but a scam! They do nothing but milk people’s insurance, lie to family members in every way possible, and let your loved one lie in their own urine all night and day! Pathetic! No excuse! Our family member was transferred to this Liberty, KY facility from the Lake Cumberland Hospital in Somerset for the main purpose of Physical Therapy and rehabilitation. However, the director/manager of the Physical Therapy Dept has been on vacation for the two weeks our family member has been there and has only had physical therapy 3 or 4 days during the 2 & 1/2 weeks she has been there! When asked why she hasn’t been taken to the therapy dept for exercise, their standing reply is we will give her a double work-out tomorrow! But that workout never comes! But they tell the insurance company, and anyone who inquires that they are giving them physical therapy every day! I call that lying and lying to the insurance company! That is FRAUD! The patients are not receiving the care they are paying for, and their insurance company is paying for! FRAUD! We have had nothing but lies! When we sit with our loved one all day and evening! They lie to your face, when we all know better! All of the facilities like this are full and you are to believe you don’t have a choice on where to send your loved one—believe me- you have a choice! But don’t choose Liberty Care & Rehab! They are a JOKE! They are Liars! I actually feel sorry for the employees there who must put up with the lying management! They try to do a great job but are prevented from really caring about their patients! It’s sad! This facility is nothing but an insurance milking station where they falsify the records to the insurance company and do little or nothing for the patient. When the insurance is up, they kick you to the curb, even if you have had a stroke and cannot take care of yourself! It’s sad. It’s pathetic. They tell you everything you want to hear, and it’s all lies! Please don’t subject your loved one to these lies and terrible lack of care.

This place is the worst I’ve ever seen. They care nothing about the patient. My FIL was there for 4 days and he was neglected. Sadly he passed away. His son and I was there the day he was admitted and no one came to check on us the 4 hrs we were there. A new patient and absolutely no one checking to see what he needed. We went to the store and got sprites, chocolate covered peanuts and poly grip because he couldn’t keep his teeth in. He never got to eat his peanuts because they never took the time to put his teeth in. They were kept in a drawer. We requested they make sure he gets his sprites and not one time did they give him one!! His daughter was there on Friday and his sprites and teeth were in a drawer. His son and I came on Saturday and found the same thing. His teeth hadn’t been in water and hadn’t been cleaned once!! He had no gown on and when I ask why they said he wouldn’t keep it on. Ok so you just let him lay there with nothing on but a pull up!! He had dementia but yet they would let him make the decision If he wanted his pain and anxiety meds. He got very anxious Sat afternoon so I pushed the call button to get help because my husband and I had been trying to get him comfortable and couldn’t. We had to push the call button 5 times and no one ever came so I had to go down to the nurses station and get the nurse sitting there while the call button was still going off! She and 2 more came to the room and got him turned and ask him if he wanted his meds and he said no. Remember he had dementia so how was it ok to let him make that decision? Had we not been there I’m sure he would’ve laid there miserable but my husband told the nurse to give him his meds he needs. Sadly we left and got a call a few hours later from the most kindest, caring nurse letting us know that he had passed away. We were in total shock to say the least. We were not expecting that news at all! Now we have to live with the thought of how long he laid there before the sweet nurse that came on the night shift went in and found him? He wouldn’t have known to push the call button for help and I’m sure it wouldn’t have done any good any how. I pray every day for the residents that have to be in this place. I will say the nurse that called us Saturday night was very helpful and she is is a nurse that has the residents care at heart. Precious lady and I thank you

My parents moved to Liberty Care and Rehabilitation Center. They're doing therapy on and off. It's not an apartment. It's a nursing home. There's two of them in a room and the rooms could be bigger. They do keep it clean. It doesn't smell at all. The staff, I don't like it because they get agency nurses from time to time, so they don't know what's going on. They're always shorthanded and they're not always checking in the room to see how my mother or father are doing. We have a lot of unhappiness about that part. I tried their food a couple of times and my mother hates their food. They are hired out from the company. They're not employees of this nursing home but they have tried working with my mom on trying to find things that she can eat. If they don't like plan A, then they try to go to plan B.

Poor management. Absolutely not equipped to run a nursing home. All department heads are rude and inhumane. Hateful to workers , residents and visitors. The inside of the building is beautiful, but signature should look for that same quality in the staff they are hiring.

My aunt has been sick for many months and she had been to many doctors and they all told her she was fine. We finally convinced her to go to the Liberty Care and Rehab to help get some strength back. After sending her to the nursing home they they were quick to assess her and determined that she needed to go to the ER because she had internal issues not once but twice so I really would like to say that the STAFF at this facility is wonderful and care for the patients as if they were their own family. We pray that with the help of this nursing home and hospital that she will begin to heal and be able to come back home one day.

My husband was there for therapy. They have an awesome therapy department as well as nursing and other staff. Everyone treated you like family. Activities always had something fun going on. Good food was served from the kitchen each day. My husband came home in much better health and a lot stronger!!

Liberty Care and Rehabilitation is a facility that loves deeply and cherishes the Elders. Making the Residents stay here, whether long term or short term, feel like home is imperative in the Elders well being. There are many activities for socialization and everyone is called by their name. No better place to work or live!!

My father was placed in there care ruffle 3 weeks ago. In my opinion this place should be closed down he is a Vietnam veteran and had fallen at home and had surgery on his knee he was sent there for rehabilitation on his knee they did more damage to the knee than good they reopened the wound and failed to notify a Doctor or the surgeon. He left 2 days ago and we had a home health care nurse come look at it and she stated she could not belive the gross negligence of this so called Liberty Care and rehabilitation. I will be contacting a lawyer. I would give them a negative rating if at all possible... please be aware.
