I have always heard bad things about this place but would always give the benefit of doubt. Staff overload poor management etc etc was probably the cause. My sis law was admitted on Friday afternoon for therapy do to loss of muscle because of a rare autoimmune disorder. By Monday afternoon she was admitted to the hospital for dehydration and high blood pressure. She laid in her own urine so long that she already has a rash. She was so dehydrated when she was admitted to the hospital that they had to use an ultrasound to find a vein. This is uncalled for!! This is ridiculous!! What about these ppl that have no family member to check on them to see if they are ok. What do they suffer at these places at the hands or lack of hands of these ppl. I realize your under paid or your under staffed but you took this job because you have compassion on ppl and you want to be a caregiver. Or that’s why you should have taken the job. Those ppl count on you daily to help to cherish to have empathy on. They don’t want to be there but many don’t have a choice. Many you are their only family. Put yourself in their shoes. Madison y’all can do better!!

So my grandfather is blind and can not eat or drink on his own. When I asked a nurse how often do they fees him or give him something to drink they said they don't have time to feed him. My grandfather has been in and out of hospital a few time for dehydration and infection due to not having his catheter changed.

the trenches. That’s the review. Should tell you enough. Whether you work there or are a resident there. Same thing

This is the worse place ever stay away. They laundry people has wash my clothes then lost 4 shirts of mines it's been 2-3months still they haven't found my clothes with my name wrote inside my shirts they have some dummies that work here or maybe some thieves . Sad workers

This place is a mess. My friend was in there a few weeks. He used the bathroom on himself multiple times, because no one came to help him to the bathroom. When it was the day for him to leave the social worker didn't have his papers ready. It's the next day now and his prescriptions still aren't called in, even though I have called multiple times and asked them if they had done it yet!! The first day I went to visit I asked his restrictions as far as his mobility, no one knew... I asked the aide when was the last time he had a bath. The response was idk. Like I said. That place is a mess!!!

This place is horrible! I have a brother there and he tells me of issues that he has had there and I finally visited to see what he was telling me that was TRUE THAT HE STATED! I was flabbergasted to find that the food was not fit for a DOG TO EAT! I was able to see first handed that the AIDES never answered the CALL LIGHT in a timely amount of TIME! The call light was ANSWERED in NOT 5 minutes BUT! in 30 to 90 MINUTES! That ticked me off enough that I contacted the STATE which oversees this! I didn't like the food (ate some) staff was NOT friendly, nurses were found to be sitting at the station and not helping out with call lights. I never received the correct information I asked about since I had a nurse (stepdaughter) with me which she said was not correct (she did some fact checking such as BP,O2,HEART RATE), and this was teeing me off to a point I wish he was at another place! Sorry for the ongoing sentence, but I do wish I was able to place him at another home! I don't and would not! recommend this place at all!

My mother resided at this facility and received excellent care. The staff was outstanding and always greeted my family and I with a smile on their face. The staff always informed and involved me with my mother's care. Her favorite part of the day was mealtime. She enjoyed bingo and getting her nails painted with activities. If you are looking for a place for your loved one to reside, I would highly recommend Madison Health and Rehab.

I visited my grandmother in this facility last week. I feel like she is always protected and cared for, the nursing staff goes above and beyond for all the residents. All the employees were in a good mood and smiling! The rooms were nice and the facility was very clean. Management was very helpful answering any questions or concerns we had. Several residents got to sit outside and enjoy the nice weather during that day. All together I would definitely recommend this facility to anyone needing help with a loved one, they will definitely be treated like family!

I love this facility. My grandmother was in this facility on multiple occasions and was always treated with respect. The staff have been great:) I would highly recommend this facility!

Food not good. Portion sizes too small!

Unspecified · July 2023

My Mother is currently a resident at Madison Manor Health and Rehabilitation Center. I am saddened to say I absolutely cannot stand the thoughts of her being there, but in Madison County our options are limited. The food is usually not fit to feed the dogs and is usually cold. As of 7/14/2023 the administrator is not truthful nor are many of the staff members in charge. The aids are afraid to speak the truth out of fear they’ll lose their job. If I wasn’t disabled I’d bring her home and do my best to care for her. The owners should be held accountable. No adequate seating for visitors. The East end has nice rooms and furnishings but the west end is horrible. Not sure why there’s such a difference in the rooms. So sad. The activities are childish. Wish I could say more good things.

My family member consistently complained about how awful the was food. No sure how food service workers were selected but they need to do a better job of selecting and screening.

Unspecified · November 2022

I would advise NOT placing any family member here atth9s horrible place! Now I hope this gets posted as another I know theirs never did!! I visited someone and I talked to them regularly on the phone and I just blew it off the way they described how everything was done and the treatment they were getting! It had been a year that they were placed here by a hospital not by their family and I wish I could have did a tour before they were shipped off to this awful place!! I rarely ever talk someplace down but people this is ridiculous how they treat people. I received a first-hand look and saw some issues that need the state to take up and react! Food was awful and under to over cooked; all 3 meals and the snack was not at all nourishing as was the food and very small portions! I don't see how they get away with what they do, and something needs to be done! Rooms I have to say were clean and tidy, but the hallways were not and there was carts and wheelchairs lined up along the walls and makes it hard for the residents to maneuver thru. I was just flabbergasted at what I saw and ate a bite of. I just wish there was a way to get them out and to another facility. Whatever you do be sure to take a tour of this place if you decide to place anyone here! Lord help them all, they need it! Another thing my daughter just wants to add, the activities are very kidlike and not for adults!

Unspecified · June 2022

Take a tour and talk with some of the residents eat some of the meals and see how they are taken care of. You then may not want to place your relative there after you have done all of the request there. The place is clean but the help is terrible, food is undesirable to eat, call light is seldom answered, and there is no rest due to some that shout constantly or did the 16 hours I was there visiting with a friend. I know I would not want to be there and glad I am still quite young.

Madison Health And Rehabilitation is a JOKE!!!! They are ALWAYS short on staff so the employees are killed and the residents are the ones who suffers!! I’ve seen employees eating the residents feeders food because they can’t talk to tell on them. I’ve heard employees saying mean things to residents, I’ve seen employees lie about giving resident showers. The people in the office falsify documents and I’ve heard them ask other CNAs to falsify on charting when the CNA has no idea as to what the residents were doing because they wasn’t at work so how would they know what they all ate, or if they had a bowel movement. Madison need to REALLY REALLY be evaluated but not with them now that they’re being evaluated like for someone to dress up like they’re coming to visit a valuated that way it’s only way they’re going to see the real true side of the people at Madison Health And Rehab!!!!!! DO NOT, I REPEAT DO NOT SEND YOUR LOVED ONES to MADISON HEALTH AND REHAB!!!!

Kenwood Health and Rehabilitation Center is convenient for us because it is in our hometown. The staff is OK, and I haven't found fault in what they are doing. Mom enjoys the food there, and the facility is clean.

My uncle is currently in this nursing home. I am hoping to get him moved soon. This nursing home is DISGUSTING! The floors are dirty, the walls are dirty, paint is chipping, door facings are chipped, the patients are not bathed, the beds are dirty, the rooms smell......and that's just what I see with a casual glance. I can't begin to fathom what I would find if I looked hard. It's disheartening to see how little most of the employees care about the welfare of their wards. When asked to do something that is needed, they tend to act as if it's a bother. And I know they work long hours for low wages and have numerous patients. But we don't ask for anything unreasonable.....can he get a shower, can he get a shave, do you know where his clothes are. I hope we are able to get my uncle moved to another facility soon. This facility should be closed until the facility is cleaned, new staff and management are brought on board and/or training/refresher courses for the current staff.

My father lived here, happily, for 3 years until his death 2 years ago. The staff was always very helpful & friendly. We never had a complaint other than from the daughter of one of the residents who caused the facility dog, Lucy, to be removed from the facility! The dog had apparently peed on the floor in her mother's room & she complained loudly. Very immature! Her mother probably peed on the floor more than the dog did. As a result, a very lovely dog, that cheered up my father & other residents, was removed. I hope that if I am ever placed in a home, there will be a dog on the premises. I love dogs & find it very cruel when people who don't live there have the right to make it miserable for people who have very little left! This happened 5 years ago & I've always been very upset about it!

We had Mom transferred to Kenwood as it seems to be a much cleaner place. Many more caring attendants and they stay in touch with me often. Just makes us feel more secure with her there.
