This place smells of urine as soon as you walk in the door. Patients with diapers hanging off in the hallway. My mom was there for two weeks and received awful to no care. She’s received only one shower. When crying in pain she was told by the nurse she had 40 other patients to take care of. Medication was never the same every day nor per shift. She has a broken leg and needed surgery on her back due to sciatica. Therapy was trying to get her to ride a bike. Which is a big no for sciatica problems. Food was awful! There was feces on the floor and curtain of her room upon arrival. Her first night 15 packages of wipes were stolen. I was told my expectations were their expectations. Clearly that was not true. Her bed pan was never cleaned and put in the bathroom stacked up with other bed pans. My mom got sent out after three days of hallucinations and talking about dying. Turns out she had a UTI. Not surprising since I would need to change and clean her when I would get there. Even throughout the day because they will not answer the call light. Her room mate was a sweet lady who used her call light a lot so their room was avoided. They got a urine sample three days prior and we weren’t told anything. They had her knee, hip, back and bowels all x rayed at different times. We were only told the results of the knee and bowel x rays. Blood was taken, again no results. They acted like she was an inconvenience Please , please tour this place before putting your loved one in here. Don’t trust the reviews like we did.
My mom was in there, this place is nasty, it stinks as soon as you walk in you can smell the funk, nirsing homes is supposed to be as clean as a hospital,I went to see my mom and my mom needed a change it took me and my husband an hour to finally get someone to change her, they was supposed to do therapy with her to get her to walk again since they allowed her to fall in the first place,but they never did,we would call to check on her and be put on hold for over 30 minutes or hung up on,we tried to get my mom moved many of times and they make excuses because this place isn't fit for a dog to live in,we finally got her moved thank God not sure how long we would have stood this mess,I would not recommend your loved ones to live at Madisonville rehabilitation,if I could I give it a 0 rating.
Very nice nursing staff and housekeeping staff was really nice. Very clean place.
Very nice place my mother in law is their and all the staff treat her and loves her like family awesome therapy with her and they are always doing a activity or a craft and I Love that they put pictures on Facebook like family ... I would recommend this place to anyone ..
Madisonville health and rehab is a very loving and caring place. They take real good care of the residence. they have an awesome therapy department and they’re always doing activities with the resident and they all care for each and every resident. I would recommend it to anyone.
I have a spouse in their and the staff keep me updated with everything and how she is doing and all the staff love her and cares for her like family and a very clean and homey place

We will forever be grateful for Madisonville Health and Rehab. They provided the best care, 24/7 for my grandma in her last days. She never had to ask for anything, and her techs and nurses spoiled her rotten. We are so thankful for Carla and her team.
It’s never easy when placing a loved one in a home when you can no longer give them the care they desire. But this facility went above and beyond to ensure my sweet Mamaw was well taken care of and spoiled to say the least. She was a spit fire so at times i know it probably wasn’t easy but all the way up to the very end they loved and adored her and made sure she had the very best care and I am forever grateful for that! They always made sure she felt at home in her room and throughout the entire facility. This staff and facility deserves more than 5 stars in my opinion! When they take the responsibility of caring for your family they are not just a patient, they are family!
We just visited my in-laws there for the first time. The place is absolutely disgusting! It is nasty - the rooms are very tiny. There should only be one patient per room unless they are a couple. The closet is so small that it is crazy! There was such an odor in the facility of urine. We were told that the facility on one end had been remodeled and if it had been - I don’t know where! Supposedly, my mother-in-law’s room was remodeled but again it does not look like they’ve done anything but maybe laid some new floors in certain areas. The guest bathroom in the front lobby was dirty! Apparently there is a bug problem as well. My mother-in-law had mentioned to us a few months ago that she was getting eat up by black bugs flying around. I had mailed her a new robe and gown and she never received it - I suppose some staff took it home with them! I would HIGHLY NOT recommend this facility! I will say the staff we came in contact with were at least nice but not very helpful. We had asked for a wheelchair to take my mother-in-law to the front lobby and she said you can go look for one in the front lobby. She had also asked her nurse to give her a bath this morning because she knew she was going to have company and that was ignored. Also, the hallways have equipment and junk sitting in the halls. They also told us the food was disgusting. They each have lost at least 20 lbs since being admitted to this facility from not eating because the food is so bad. They’ve also asked to be moved closer together or in the same room for the couple of months they’ve been there and they keep saying by administration that they are working on it but nothing has happened. I would not put my pet in this place and I left crying because it is such a disgusting place. ADDENDUM: After posting this review, my husband was contacted by the facility’s administrator that they would look into addressing some of these issues. We shall see….we are not too optimistic.

I can’t get admissions on the phone or even a call back. Business must be good. Taking the whole month of April off it seems like. Going through one of the toughest times of my life and get zero support here!!!
My cousin moved into Madisonville Health and Rehabilitation. The experience is good. The nurses and staff members are friendly and helpful. They give you the feeling of encouragement to try to do stuff for yourself. They seem more like family than a workplace. The meals served looked good. I don't see any stuff on the floor, and it doesn't smell in there. They were doing bingo, and the patients enjoyed it. Medicaid takes care of it. It's a good price.
Recently had dad at MHR for a week+ for rehab after pacemaker surgery. I was very pleased with the thoughtfulness and knowledge of the entire staff and glad the rehabilitation staff worked with him seven days a week. I recommend MHR.
You couldn't ask for a better and more caring facility not only for the patient but also the family. And especially so with the new administrator. I couldn't be happier with the care that my mother gets. At first I didn't trust the staff because of baseless rumors from the prior admin worker but by standing by, observing, asking the staff questions and reviewing my mother's case records, the current rehab staff have proven those rumors to be false. I couldn't recommend them more highly!
Went to an event here with friends. The event was well organized and included the staff, residents, and family members. It’s so nice to see a facility that is as welcoming and focused on residents as you are.

My mom is at Madisonville Health and Rehabilitation. It's decent, but it's not new and clean. The nursing care was excellent. I can't stand the administrator because when my mother had difficulty, what he wanted to do was throw my mom out immediately. When I went in to talk to him about it, and at the time I was getting ready to have a full hip replacement, he left me down the hall and made me hobble after him to get to his office. I thought that was very rude. But the nursing staff is good, and the financial lady is the best. They do physical rehabilitation. They bring them their food on trays, and the food is okay.

It's OK if you want to go thru the mud to get in the place.park in back walk in mud to get to the front door.
The activity department is always coming up with fun and engaging activities. Nursing staff care for each resident like family.
My father was at this facility and I couldn't have asked for better care. He was initially against going to a nursing home even if it was only for therapy to return home. At the end of his stay he was very complimentary of the staff and care he received and was willing to come back in the future if he needed to.
I think it’s a very family friendly environment. Learning, and improving daily.
Madisonville Health and Rehab is a wonderful facility. There is always something going on in their activities department. My uncle was a patient for years and got the best of care.
No patient should be in the same clothes for 4 to 7 days, that means they are not being properly washed, complaining to management over the nursing home only worked a few weeks, then right back to the same neglect, I gave two star's because their are a couple nurses who actually try, but I literally have witnessed myself nurses sitting in the room with patients playing on their phones with the food trays just sitting there,these people are among the most vulnerable, if you have a family member here please check in on them daily if possible, don't be fooled by how some of the nurses act sweet, this place needs someone to really oversee daily who is doing what job ,rant over !
We've had to use Madisonville Rehabilitation several times so my husband could recover from a hospital stay or fall at home causing injury to him.We choose this facility because my husband always gets quailty care and his recovery to home is always their goal along with our family.The staff no matter their position were always kind and helpful to us as a family as much as they were to my husband as a patient.If we are in need of help again I without a doubt would chose them.
Horrible experience!!! So happy our family member made it out of there. We had no choice. She is home safe.
I was admitted here in 2017 for physical therapy after I deconditioned from my debulking surgery for cancer. Being a nurse myself, we often try to find fault with other facilities and staffing. I honestly have to say that I received the utmost care. Amazing staff and physical therapy department. I would gladly refer any family member to this facility, they are that good.
My grandmother was at this facility and every single one of the workers made not only her feel comfortable in her last days on Earth but also the family! Even though my grandmother has passed away my grandfather still goes the facility every week to visit and preach! All of the staff has made him feel like part of the family.
Madisonville Health and Rehabilitation is a wonderful center. The center stays clean, has a lot of meaningful activities for the residents to participate in, successful therapy and compassionate nursing. One of the best centers I’ve had the opportunity to visit.

the social worker was very rude to my husband and hung the phone up on him Mr Belcher is a liar wouldn't take my mom over all we done was ask why we waited for a while and he never called us back so when I called he had someone else to tell me he wouldn't take her there
I have had a family member here for a while now and I could not imagine moving them anywhere else. The nursing staff is wonderful. The CNA's seem to change often, but the RN's have been the same faces for a long while now. The building is old, and really needs an update, but the way my loved one is treated outdoes that by far. I have even hidden a voice recorder in her room at times to see if I was just being fooled, and could not be more pleased with the treatment my loved one gets. Thank you to the RN's and LPN's of NHC. Our family love you all!!
