My mom Helen Prewitt)was there 14 months and the staff was fabulous. They treated my mom with the best care. She loved all the nurses and staff and they loved her. The day she went to the hospital they put her on the vent and the staff came by to see her the next 2 days to say there goodbyes. I will never forget them and the love they showed my mom.

The Masonic Rehabilitation Facility in Shelbyville is nice. My Mom likes it. Therapists are nice. It should rate 4 stars, but we have a big issue with care and communication and that is why I give it a one star. My mother is an nsulin dependent type 2 diabetic. She was released from the hospital rather abruptly to the rehab facility. Somehow the rehab facility did not realize that she was diabetic for the first six days. I believe that the hospital failed to clearly communicate that Mom was insulin dependent. Intake at rehab failed to note that Mom is diabetic and insulin dependent. Mom keeps a list of her medicines with her and did ask about insulin. She did not receive glucose testing, insulin and other standards of care for a diabetic for six days. UPDATE: The Facility reached out with an honest desire to improve by educating staff and improving their end of the hospital to rehab handoff. They were able to catch other cases of patients with incomplete hospital records being released into their care from hospitals. Mom is doing ok.

I couldn't be more thankful and appreciative of the excellent staff that takes care of my Mom! From the Janitorial, CNA's, Nurses, administration and Food Service employees - everyone makes this a clean and loving environment. Very friendly and attentive to residents and family members! Having my Mom at Masonic and so close to my home has been a blessing for my family! Thank you!

When my mom first got to Masonic, it was wonderful. She was in the rehab portion and everyone was great particularly Katie. However when she moved to the personal care section, things got bad. First there was a mouse in the room and they acted like that was normal. Secondly, I feel Sally Garcia was very confrontational with my mother and probably shouldn't be working with seniors. Lastly, they said she couldn't get physical therapy in personal care and I found out later that wasn't true. I am really disappointed because this facility is close to my home and I really wanted my mom to live there.

I was recently a rehabilitation patient at the Masonic home of Shelbyville. I did not know what to expect, but my bar was set pretty low. I couldn’t have been more wrong, the staff was professional, courteous, and patient . The residents could be very trying (no fault of the residents or staff.) But the staff retained their composure, if I ever need that type of support again I know where to go.

During my Mother’s time here, the entire staff was friendly and courteous and worked a miracle on a weekend for us. Director Sally Garcia in particular was exceptional and became our friend first and director second.

I am the daughter of a 4th generation Master Mason, I was raised to believe that “no matter where you are in the world , you could always trust in The Masons to help if ever in need” 1 year ago my Mother chose to come here because of that promise, and as her children we trusted also! She went in for physical Therapy , and after only 9 days of which she was in quarantine, she was neglected and mistreated to the point of a severe sore that covered most of her right side from the waist down! She was sent to the hospital with no notification to the family, they claimed to have called,however, not one of her daughters were contacted and all were listed as contacts.Unfortunately, due to her health issues and the added neglect, she passed away just 3 short weeks later. If only that promise of Brotherhood were true…..

My grandmother went there sometime last year. She was perfectly at herself and could talk just fine. She could feed herself or sit up on her own. One night we got a call saying she sat up and fell out of bed on her knees but then said she was fine and they just put her back in bed. Mind you she couldn’t sit up or turnover by herself. So how the hell could she have sit up and fell out of bed. they later sent her home. she was only home for 2 days before she then had a heart attack. They found out she had a major brain bleed. The two days she was there we were as gentle as could be and didn’t turn her that much. I know the brain bleed happened at the place. There’s no possible way it didn’t. I definitely would never recommend coming here.

A culture of deception, obviously. When you see reviews that are more about a single person than the facility itself, and especially those from ppl claiming close relationship for many years, they should not even be allowed to see public light. (If you even see that soneone correctly spelled this woman's name, which sounds like "Kiana", these are obviously from friends instead of customers). So, if they publicly deceive the public and company with such obvious fake reviews, what other less-obvious deceit is going on? . Can we trust these ppl to correctly report injuries or other state-reportable issues? Are these bedbugs and scabies issues given in some reports here properly addressed, or are they likewise deceptively mislabeled (allowing personnel and visitors to be exposed to them)? Are other issues common to nursing homes, like mold, similarly given false names there? . I look at these reviews as a nurse and former DON, as well as an ex-cop and safety manager, and they scream a serious need of deeper investigation to me. Just these reviews give me reason to sound alarms. . Oh...to those checking these reviews, any post sharing the name of a patient is a HIPAA violation, and therefore should not be allowed in this public display format. .

Thank you so much for the care and rehabilitation in helping my Mother, Ruby Mae Puckett, in her recovery. As soon as you walk in the door there is always a kind person with a genuine smile. It's a beautiful home with many extremely large welcoming formal, not intimating, living rooms with lovely couches and chairs. It has fireplaces that are cozy and large movie screen flat TV's. I have personally worked with the Administrator and several staff members on multiple occasions through the years. You will not find more dedicated, knowledgable, and compassionate caregivers. Highly recommend this facility!!

Where do I began I can not say enough positive things about Conjuna Collier (Administrator) that currently works for Masonic Home Shelbyville Ky . In my 30 years of health care she’s the only administrator I have ever seen on the floor doing the work of any of her staff members in her facility she keeps her phone on 24/7/365 . And don’t we ALL deserve a Vacation I would say she should get one quarterly .Any staff members could call her at any given time Conjuna Collier always knows what is happening in the facility at any giving moment her caring/compassion is beyond superior for the little people/staff in her facility ... I wish ALL facilities had #BESTADMINISTRATOREVER

I can completely confirm what Nancy Smith is saying, and actually, their courage to stand up and say something has sparked my anonymous review as well. I personally can vouch that the BED BUGS, ROACHES (not “water bugs”), MICE, BLACK MOLD, and BATS are everywhere and that there ARE “sick” residents WITH SCABIES (being told to families as “contact dermatitis”) as well as SHINGLES who should be put on isolation to protect our other residents/staff. Just like there are some residents who get placed on isolation and then are allowed to come outside of their room when they’re not only exposing themselves to more germs, but exposing our staff and other residents to these various illnesses to the point that family members have personally came to me outraged and wanting to know what is going on. Our own administrator even said in an all staff meeting that “we are NOT to tell people that there is black mold here. All it is, is water particles that got trapped inside the wall. So tell all your friends and family that Masonic Communities does NOT have black mold.” Staff members are even getting called to the administrators office if she finds out that they’ve been talking about it which is a shame because people deserve to know.. especially if it’s concerning their loved one. Residents are even getting moved from their rooms and placed into other rooms stating that their previous room is “unsafe” to go inside and blocking it off from all staff/residents/family until it’s treated.. yet we “do not” have black mold? And there are still places that are and have been “under construction” when really they’re closed off due to this situation such as the big living/sitting room on personal care. It’s sad that our team of administration would rather visit Cancun multiple times a year and post pictures of them all going out drinking and having a great social life when we have actual needs around the facility that could use that money/attention such as our nursing, environmental, dietary, and therapy departments. Most employees are afraid to reach out and say something about the living conditions that residents are having to endure as well as the working conditions were put under for fear of retaliation from management (specifically, the administrator). I can personally say right now that I’m scared to post this anonymous review, but it needs to be shared and “Nancy Smith” doesn’t need to stand alone on issues that ARE true and factual. I really hope this sparks someone else to comment too so that we can spark some change. It’s sad when social agendas and personal bias overshadow the overall good and well being of the company and it’s hard working employees as well as it’s neighbors and their families. If this were a review about the housekeepers, cna’s, therapists, nurses, dietary aides, and other people who work on the floor and understand what is actually going on within the facility and work hard to keep its doors open, then this would get 5 stars, but because this review is for the management, it gets a 1.

This is a wonderful place. I have visited Millie resident there twice since she has been there for rehab after shehad fallen @ her home after Christmas. Then she had to have surgery on foot to correct a problem Friday Jan 18th 9am, she did very well was transported back to the home same day! She has had therapy twice saturday she is doing great! This home is very clean, very friendly staff & helpful as well. I would highly recommend this home for your loved one needing care. I see why they stay full Capacity!
