MY WIFE IS CURRENTLY IN REHAB NOW. THIS PLACE IS ABSOLUTELY AWESOME. I WOULD WITHOUT A DOUBT RECOMMED TO EVERYONE.
My mother stayed at the Clifton location for rehab about one week. She had recently received the same type service at another provider in town with little help. After her stay with Nazareth Home, she is back at her senior living community and doing very well. Like anywhere, one must advocate for their loved ones but this staff-across the board-was happy to share with treatment goals, progress, etc. I saw the CNA’s and Nursing Assistants always trying and always knowledgeable. We had Selena and two others who were terribly wonderful. I’m sorry I don’t have the other names but if you helped my Mom in 616, I am so ever grateful. I was not sure I would see her so healthy again.
Much of the staff is very caring and nurturing but some are horrible. They left my paralyzed friend who had a deadly infection sit there for 4 hours without changing her. She went without pain meds or her reg meds for hrs also When questioned nicely about this the heàd cna for her section, (i suppose she was, she didn't identify herself) for her in the evening said they were overworked, did nt have enough staff and hung up the phone. I heard 2 of them bullying her about using the call light to much There was also another witness to this When I said something on the phone they later didn't respond to her even though she reprimanded me for speaking up on her behalf I suppose they were showing her who was boss. I can understand why she did nt want to say anything when their response was neglect
My Father was admitted for physical therapy rehab and when he came out he was ten times stronger and more capable in his mobility it's like looking at a new man with an awesome Go to attitude I just wanna thank everyone there that put in the time and effort with him! He's My whole world so it's amazing to see his turnout!
As a CNA self employed that got booked a 12 hour shift that was canceled to work after the in house workers show up 45 minutes late. During my 45 minutes I waited to see where I was suppose to work. Day crew left and there was me and only one in house cna there. So I got up and answered call lights for 3 halls. Eventually the scheduler, DON and the owner(s) from my observation rewards goes their in-house late workers to finish their 12 hour shift and sends agency home. This is a very bad business practice from this company. Imma navy veteran, intellectual and 15 year experienced in the medical field. They should treat workers fairly and not use agency when we show up on time , spend money on gas but in my case Lyft rides back and forth and ruin our day when we could of made other plans! This company need to pray to God and do what’s fairly humane. Shifts should be first in-house crew on time otherwise agencies have next in line to work. Stop rewarding late incompetent people!!!

Second rehab,wife attended. Excellent service and care.Staff was caring. Wife was there for in house rehab. I felt she was taking care of,while I was at work.It was such a relief not worrying like in past home. There is quality healthcare rehab homes out there.Str ongly recommend for your loved ones.
Our mother moved in while she was at her end of life stage. The entire staff was warm and welcoming from the very beginning, when she moved in, and throughout her 2+ week stay, when she finally passed away. The Nazareth staff would stop whatever they were doing to tend to, not only mom's needs, but our needs as well. The nurses and CNA's would sincerely ask us how we were holding up and make sure that we felt like we were a part of the Nazareth family. If you are looking for a competent, warm and caring place for a loved one to stay, I unequivocally recommend Nazareth.
They are all taking excellent care of my 92 yr old aunt. I wish I could give them 10 stars !
We like the staff at Nazareth Home-Clifton Campus. They're so caring to Dad and they are so good to us. There are some that aren't, but overall, they are very concerned, very nice, very cordial, and helpful to us when we were there. My mother and my brother liked them, too. The facility is pretty clean. There's a huge courtyard and they're trying to bloom flowers around there. There are benches and tables and it's pretty out there.
Management is terrible.
**WARNING**. DO NOT send your relative to the Nazareth Home Clifton Campus Rehab! My mother in law was sent there recently for post hip surgery physical therapy and rehabilitation and the experience was a complete disaster. No meds of any kind given the first evening there. Complete confusion on how to control pain for the first 3 days. Complete confusion on her meds in general nearly the entire time. Very dismissive and aggressive aide caused her extreme pain, left her in tears, and made her question whether she should remain there. Waiting 45-50 minutes for aides to assist with bathroom or comfort issues even after nursing staff explained there were NO staffing issues. Physical therapy was often so mild it bordered on none at all. One PT person, after she had been there over a week, thought she was there post back surgery instead of hip! Food was extremely poor and no consideration of nutrition or effects of diet seemed present. State ombudsman contacted three times with no response. Some staff members attentive and apologetic. Unknown if this is an issue at the Highland Campus.

Nazareth Home Clifton was Mary Ann Ballard’s final days where she was abled to see and experience nature. Mary Ann loved to hear the birds in the outside garden and enjoyed being wheeled around the outside. Staff quickly learned to see the beautiful spirit that Mary Ann radiated to everyone there. They knew she was special for her smile was always there. I do say there were hard times for us both after being separated after living 48 years together mostly serving others and active in social justice issues. Mary Ann was a healer and a blessing giver which she shared there at the home. Lucky am I for having Mary Ann at Clifton. I am so very grateful for being there daily to visit Mary Ann from March To September 3. Staff knew me well and met our needs with love Laughter and peace. You deserve every award you have for the service that you render to us all. Thank you grateful am I to every one of you. I would add one other comment the house cleaning staff touch me deeply in ways that gave me courage and had such a caring attitude toward me and to their patience. I love everyone of them. I get to visit the Nazareth Home residents in Clifton on a regular basis. The community is well staffed and the folks are well cared for and offered lots of activities in which to participate. Definitely feels like a home with lots of love
Every tertiary care rehab has the same list of issues common to the industry. Staffing issues, long wait for nurse responses, and lack of communication are not unique to this facility. That being said, it's okay. It is clean. Food is institutional, but a limited choice menu is given. The presence of upper level clinical staff was scarce. All employees are well meaning, but with limited resources. The physical rehabilitation was not assertive for years of experience.

My mother received rehab from a fall. The staff is kind, and caring!! I definitely recommend them when you family needs that type of care!!!

I called and spoke with the liaison about rehab services for my dad. She was extremely helpful, kind, and patient with all of my questions.
My husband is a resident of Nazareth Home - Clifton Campus. I like Nazareth home. It's about 12 miles one way, so that's 24 miles if I try to go up there to see him, and with the gas prices and everything going up, I just wanted to have him a little closer to home. It's been a good experience. They're nice to him, but I just wanted him closer to home. I just thought it would be nice if we could run and get him and he'd be really close. The staff has all been very nice to him and he likes them. He's on a very strict diet, so he complains about that. They only give him a certain amount, but it's because of his diabetes. He needed to lose weight and they just want to keep a close eye on his sugars and his food intake. I do take him something every now, and then when I go up there I try to take him up something special, like a milkshake or something different than what I think he will be getting there at the home. But they give him snacks, ice cream, Jell-O, and they even give him cookies at times. They're very good. I've been in there sometimes when it's kind of messy, but I think a lot of that is him. He lays in his bed most of the time and if they come in to clean his room and he'll say, "I'm all right, you can come back later," or something like that. I'm sure they're short staffed, especially after the pandemic, and I told him he just needs to let them clean his room. But I've been in there when the room is really clean, too. They have a courtyard, and they play games with them. They have music, and they have people that come in and play music and sing with them. They do bingo and the price is right for little gifts and things that they can win. He doesn't really participate in that; he's just not like that. Nobody's perfect, it's not always perfect. To me, it looks like it is clean, but I think there's always room for improvement. They're short staffed and that's hard to be dumped off on one or two people. But they have good staff and I've been satisfied. We have Medicaid, but I still pay them a lot of money a month. I know that it's not the home charging me that money, and it's Medicaid telling me what I can pay or what I'm supposed to pay, and that part I don't like, but it has nothing to do with the home.
Nazareth Home - Clifton Campus is nicely kept, everybody's very friendly, and they're willing to try to answer any questions you have. They're really good about printing out activities for the month, for each resident. If we want a copy, we can even get a copy of it. They've been taking really good care of my mother. The staff is very, very friendly. They've done a good job with COVID and in trying to keep the residents that came down with Covid that didn't have to go to the hospital in one section. They sectioned it off and kept them there and kept everybody else safe. As far as the workers there, when they came down with Covid, they stayed at home to recover. They've been really good about making sure that the residents had all their COVID shots and their flu shots. They've been really good about having a meeting with us to discuss what they've been doing for my mom and what the dietitian has been told to serve my mom, in terms of her meals. Everything's been nice down there. I have tasted the food and what I had has been good. Everybody pretty much gets the same breakfast unless they are on a special diet or high fiber diet like my mom. They changed my mom's from eggs, bacon, and sausage to high fiber cereal and she likes that better. They have 3 choices at lunch and dinner that they can select from. They can take whatever they want. My mom goes to the dining room where she can pick what she wants. If she wants a sandwich and she only wants a half then she can ask for it. If they have chicken tenders, she could ask for a half portion. They give all the residents an activities sheet for the whole month. That way, when relatives or visitors come to visit them and they want to know about activities, they can look and see if the resident has one and see what the activity is for the day. They have a lawn care service that comes in and services the lawn. Beyond that, there are maintenance gentlemen that take care of everything else that the lawn care service doesn't do. All the rooms are painted a neutral shade. My mom's in one of the bigger rooms that have a half-circle of four windows. She went to rehab and then they recommended that she stay in the nursing home and we kept her there. The staff is all nicely dressed and I just think they do a wonderful job.
Father in law stayed here. Jeanine in admissions made it a smooth transition from hospital stay. Staff is very friendly and facility is so clean. He had a private room and rehab helped get him back on his feet. We were very pleased and our minds eased.
Employees here are very caring. They take the time to try and understand my mom, who has expressive aphasia. The few times there have been an issue it is resolved in a respectful manner. When the employees are cheerful and say hi to my mom her face lights up and that’s not something people do because of their pay, it’s something they do because they care. The garden is peaceful and beautiful and there are places nearby to visit without having to drive. There are many activities for the residents, including concerts and singing. Even during the pandemic the employees are working hard to care for residents and try to keep them happy during this hard time of isolation.
Because of the wonderful care at Nazareth Rehab we also had our Dad use it for rehab. I would use it if I needed rehab. The staff care and love their patients. Thanks for the great job.
Nazareth Clifton is true healthcare with heart! We choose them time after time for their integrity, communication, smaller units with more one on one care and incredible staff. From Rehab to Personal care & Long term care to Palliative, our seniors and their families are extremely happy with their patient centered approach.
My dad was here for rehab after a hip replacement surgery. The second day he was there, we came to visit him and he was unhappy with the way the head nurse was distributing his pain medication. His doctor prescribed his pain medication to be administered every 4-6 hours. While we were there to visit, it had been over 8 hours since he'd last received it, and this was after requesting it from the head nurse which he shouldn't even have to do. The aides stated, "That's the way the head nurse does it." She gives every patient their medication at the same time, REGARDLESS of whether or not the scheduled time has actually passed. This is completely unacceptable. When I asked an aide when he was going to get his medication, she started giving me excuses about how understaffed they were and that they'd had a bunch of callouts and that it didn't matter how fast they went, if they were behind they stayed behind. I asked where the nurse was and she said she was catching up on her reports because of shift change. I asked if she could pause her reports and administer his pain medication. She said, "No, we can't do that because the reports are very important for shift change." I was in disbelief. I said a patient suffering and being in pain was very important and she again began making excuses about being shortstaffed. No one took any ownership of the problem and continued making excuses. Later the nurse finally brought his medication and said, "Sorry for the wait." What a joke. So we left right after that, including my dad. Clearly, if they are that understaffed and have no backup plan for when employees call out, they cannot properly care for patients. I think they're just used to the older patients who can't really stick up for themselves but it is clear the staff here does not care about the patients and their well-being. His leg was supposed to be slightly elevated after surgery. The pillows and bed mattress were sub-par to say the least. The mattress was so worn that it curved down. During his first physical therapy appointment there, all they did was screen him and ask questions. There was no physical therapy done! They said that the screening takes up the entire therapy appointment the first time. Well then what is the point of going there? They should have gotten all of that information from his doctor instead. DO NOT send your loved ones here for anything, they will NOT be well-cared for. The staff does not care, there is no sense of urgency AT ALL. And the food was the worst hospital food you've ever seen. I don't mean it was awful because it was healthy, it wasn't even healthy. His dinner was cold fried chicken tenders, an old vegetable medley, stale french fries and canned peaches. Save yourself some grief and stay away from this place!
I would not send my dog to rehab in this place. Where they are getting these 5 star reviews is definitely not from someone who stayed here. I waited hours on top of the normal 4 to 6 hours for medications. This is unacceptable. I will be informing my orthopedist and everyone else I can about this place. I was supposed to stay 7 to 10 days. I left after less than 2.
