I arrived under suspicion of oh no what am I doing here. There has been nothing but positive. Staff and physician is great. Therapy is fabulous. Food is a surprise how good it is. Cleanliness is top notch. Everyone is warm and caring. If I ever needed a place to go to in the future I would I choose Park Terrace again.
1 time a week you are in there alittle longer then you would like but other than that they are doing a wonderful job. Thanks for your help.
My mother stayed at Park Terrace Health Campus for rehab. She was there for a couple of weeks. We were also on their waiting list for a Medicaid bed, but their waiting list is very long. We had some difficulty, and there was a lot of miscommunication. She was there for two weeks before she received a shower. There were a lot of miscommunications, so we had to get the director of nursing and the administrator involved. However, if you take that out of the equation, everybody else there was wonderful, including the physical therapy, and everybody we came in contact with. It just happened to be two specific CNAs and a miscommunication that was the problem. I don't think anybody dropped the ball. The nurses are wonderful. I feel like they are probably a little overworked, and I think they're probably short-staffed, but that's everywhere. They just can't get people hired. My mother had no complaints at all, whatsoever. She was fine with all the food, and she's a picky eater, but she seemed to be okay with it. Everything I saw there was clean, neat, and almost fairly new-looking. It's an old hospital that was converted.
The room was clean and a nice size. The food was pretty good most days. Rehab service was great. The patient care as far as hygiene was absolutely horrible-my mom was incontinent bowel and bladder. After a week I had to ask about a shower or bed bath. I did speak to several people in management twice on this issue. I wonder how patients are treated that do not have family to come in and check on them. This facility made us a promise on the visit that the didn't provide. I would have given a score of 0 but that was not an option. So disappointed
Do not send your loved one here unless you are OK with them being neglected. My mother was sent here for physical therapy after a stay in the hospital. Her legs and ankles were extremely swollen, and the goal was for her to have physical therapy twice a day to address her mobility. Specifically, we were told that they would work on her being able to go up three steps and also get herself in and out of a shower . Never happened. Other than one day, physical therapy only came once a day to work with her— and never on Saturday or Sunday. what a giant waste of time. One of the ladies who worked with her upper body strength was helpful, but the other guy just had her sit in a chair and would say lift your leg 10 times and then Walk away from her. He would come back and just keep telling her to do it again and again, to the point that she re-injured herself and was in severe pain the following day. Nobody in the facility would even notice that she was in pain because they never came to check on her. They brought her meals three times a day, the nurses brought her medication in the morning and in the evening, and that was it. When I inquired why nobody comes in to check on her throughout the day, they said she has a button to push if she needs anything. Thank God my father was with her because she could not get in and out of the bed by herself, and if she would have fallen, she probably would have laid there for hours and hours with nobody even noticing. On her second to last day there, I asked to speak to a nurse. I showed him her extremely swollen legs and ankles, and he seemed surprised. My mother said the nurses had seen her ankles every day and did nothing about it. This nurse actually wrapped her ankles and legs with ace bandages, which made a big difference. Would’ve been nice if they had done that the eight days prior that she had been there. But then again I don’t think they really care about the patients here. imagine our surprise when a man walked in one day and said that she was ready to go home. She still could not get herself in and out of bed, could not walk up one step, and they had not even practiced helping her get in and out of the shower. He said that insurance based the decision on the notes from the physical therapists, which tells me that they were not being honest about her progress . I was not prepared for the extent of negligence at this place because Baptist East Hospital said it was a five star place. We will never go back. I would not send my worst enemy here.
!!! Beware!!! Today we had a visit to see what Park Terrace (Memory Care) had to offer. However after asking questions of the administration director Emily Wheatley and hearing their policies we were bewildered. I know Medicaid has some policies that don't specifically advocate for differences Memory Care patients need but the un-empathetic way Emily relaid those to us and her implication saying that they could not meet our expectations to care for my mother we're astounding. As she relayed what the policies were we asked questions, she got defensive, one of which was why can't you have bed alarms to alert you if someone gets out of the bed so they don't fall, she said that that was a Medicaid policy to which I said what if she gets up in the night and falls no one will be alerted. I then asked how many staff there would be at night and she said always a nurse sometimes one to two aids and they round every two to three hours and that's for 28 patients/ residents.... I hope you see my point. She stated that helpful restraints like wheel chair seat belts and bed rails couldn't be used unless the patient knew how to use them, we're talking about Alzheimer's patients in memory care.... She said I was being argumentative but I was only asking questions about the facility I was going to put my mother's care in. This was just a few questions that we had and she basically blew us off and said that they couldn't meet our expectations. I don't know what this facility is like otherwise because we actually never did a tour. Things in a facility are usually run from the top down, and if this is the way you are treated during a tour, I would hate to see how things are ran on a daily basis... Read some of the other negative reviews. I should have just marked it off my list from the beginning. Ask lots of questions and if they get frustrated or defensive ....run don't walk ....run and find your family member a better place to stay. Also this as a For Profit Facility. Very unprofessional! There wasn't a 0 Star.
My mom was in there for rehab for 6 days she only had 2 hours in 6 days of therapy. Hours and hours will go by with out seeing anyone. No water brought to her room, no food in the evenings. Staff sat at the desk playing on phone. Garbage cans nevered emptied. I had to give her a shower because no one would after several requests. Horrible experience all the way around.
Making the decision to move a loved one into a memory care facility is difficult, but not nearly as difficult as finding places for those on a limited income. I contacted Emily at Park Terrace and she was great! Emily stayed in constant contact with us during the process - the tour, the paperwork, the approval etc. My mother-in-law moved in the memory care unit the end of October 2024. We couldn’t be happier with her roommate and family. There are activities every day to keep them engaged and social. My mother-in-law, husband and I are very happy with everything so far. Visit for yourself.
My Mother was here for rehab and the place was so dirty. She fell out of bed and had a big knot on her head. They had ambulance take her to hospital to get it checked out. I would never let her go to this place again
I got to hear the "This is not a hospital " speech while the patient, my mother, sat there with severe edema. They put her on a high sodium diet, then took away her diuretic, all while not treating her pain adequately. The physical therapy is good, and some of the nurses seem to care about patients. Hence 4 stars.
We moved our Mom into the memory care unit in June 2023, and we couldn’t have asked for better care. It was obvious to all of us that the nurses and staff cared deeply about her. We all became friends with the other residents, and the nurses and staff were like family. Mom passed away in late July, and everyone at Park Terrace was heartbroken. We’re beyond thankful for the exceptional care our Mom received while she lived at Park Terrace.
Nurses take their time to respond to bathroom alarms. I waited for a solid five minutes to have someone to respond to a bathroom alarm for my FALL RISK family member. After two going on three minutes, my daughter (nursing student), went to the nurse station to alert the nurse, who was aware of the alarm and was on the computer. It seems like the charting could wait until on hand emergency was addressed. Then go back to the charting. Unacceptable as a rehab facility.
A nurse there named Alicia was rude and always neglected my father with dementia and always got mad when she had to change or help him go to bathroom. Was always telling family members it was our place to take care of him while in there . I wouldn't recommend this place to anyone . They also was pushing father in wheel chair and let him fall out busting his head and face up and needing stitches in hand . A few were really mean to him. And don't need to be working in a place like this.
Well. I am / was disappointed. About a year and a half ago my Mom was at this facility and at that time I simply could not have asked for a better place for her to Rehab. :). They were nice, well staffed, the rehab was excellent, the place was clean and generally well cared for. A couple weeks ago my mother needed a place of similar type and quality so I didn’t hesitate to send her here again. She is in more need of rehab this time but this time arriving late on a Wednesday and deciding to leave Friday. The staff we talked to were nice but extremely under staffed. :(. I didn’t expect much on Wednesday as she arrived very late and on Friday she wasn’t doing well so again I didn’t expect much. But, on Thursday the one full day she was there. No Therapy stopped by. My sister was there from the morning through the end of the work day and I arrived shortly after and stayed late into the evening. And, when she needed to get up to the bed side toilet from the time the call light was pushed to the time she was off the potty was over 90 minutes. Hence after her additional stay in the hospital which was VERY MUCH needed, we elected to NOT return to Park Terrace. It really is a shame as they were so very good 1.5 years ago. I know staffing can change but someone made a terrible mistake allowing this place to go so bad so quickly.
This facility needs a total remodel. Obviously, Trilogy is not investing in it. Carpet was dirty, room smelled, couch was filthy and there were ants. Bathroom was not clean. My mother had cancer, I came in the room and she was in severe pain. She said the nurse told her that they ran out of pain pills. Her chest tube was drained at 3am in the morning. I’m the only one who helped her with bathing. Her bed needed changing and the aide handed me the sheets. She was supposed to be released the next day, but her sodium level got so low from LASIK’s pills and she was rushed by ambulance to the emergency room. Sadly she passed away a week later. When she needed rehab, we were informed that this facility was. 5 Star. If I could give it 0 Stars, I would.
Nice and clean good people
We place my Mother in Park Terrace last December. We already had a bad experience with a different nursing home, so I had a bad taste in my mouth. You really never know if your loved ones are being taken care of. I got the opportunity to stay eight nights with my Mother before she passed and the love and kindness expressed to us was AMAZING! I have so many wonderful memories from all staff from house cleaning to aides to nurses. The stories they shared with me about my Mother, the staff that came in on their days off was amazing. The intake worker Demetra was a beautiful soul, the Director Kevin went out of his way to accommodate our family. My Mother was on 3North the Dementia and Alzheimer’s unit. A big thank you for all the caring staff!! Too many to name but everyone deserves a pay on the back. Blessings, Jenny Peterson

My Mom is 92yrs young in rehab, everyone is very nice, great wonderful staff. She loves the food, that is a plus. If you are looking for a place, consider Park Terrace. Happy, friendly people❤️
My mother is currently on the 2nd floor which is the rehab. I cannot say enough good things about this place from the great staff, great food, activities ("happy hour" to a hair stylist) to the rehab. I am so thankful we found this place and if she ever needs this assistance again, we will be back. Keep up the excellent work!! Thanks!!
The nursing staff we're very pleasant and professional. Rehab department is dedicated to meeting all your goals to return home. Plus the food is good.
My mother is in Rehab there on the 2 floor and I’m not sure where the 5 star rating came from, but they are not my favorite place. The workers are hatful and neglectful towards my momma. She pushes her light for something to drink and they my come 40 minutes to a hour later. She asks for help off the toilet, they come in there after 15 minute wait and then say, “let me go get help” another 15 minutes go by and still, no one. I hate this place with a passion. Thinking about contacting someone for nursing home/rehab neglect. Signed furious daughter
I wish they had a negative star option in this case. My Grandfather was there for 7-8 days and it was a disaster. First they gave him a medicine that he was no longer taking. My Sister found the pill in his bed, glad he didn't need that pill! Then after 3 days of being there, we were visiting daily, we realized he was not being changed. He laid there in the same dirty gown for THREE days. I thought they had the same gowns as the hospital but realized they did not. I pulled the gown back and his chest was matted with food and his bed was full of food as well. We were told he would be taken out of his room for activities/meals but he did not leave his room except for therapy the entire time he was there. We cleaned him up and changed his bed. We then talked to the staff about his care. I will say that after that the staff was much better. There were a couple of people there that I WOULD trust individually to care for a family member but obviously not there. Someone visited everyday except one, because two of us picked up a virus during that time, and we did not want to give it to anyone. The day after not visiting we had them release him. This was because I called before we headed up to see if he was okay. I was told he was discharged!!!! I said well then it was done illegally because my Daughter is his primary POA and I am secondary and we were not notified. I was left on hold for over 15 minutes and then Kevin picked up, he was very condescending. Meanwhile my Daughter was heading there. Kevin spoke to us like we were irrational at being upset that my 87 year-old Grandfather was "discharged" without our knowledge and they could not tell us to whom he was discharged. However, Kevin could give out HIPPA protected information to someone who was not on his "list". As a matter of fact, they never checked any of us coming or going and made the comment that they thought we "were from a therapy service" when we were changing him and his bed. Kevin also rolled his eyes at me and told me he has "HIPPA" training when I called him on his mistake. I think he needs a refresher. "Under the HIPAA medical privacy rule, a hospital/facility is permitted to release only directory information (i.e., the patient's one-word condition and location) to individuals who inquire about the patient by name unless the patient has requested that information be withheld." He certainly gave more than directory information and never inquired to whom they could and could not speak with about his condition. Kevin could also use Google to freshen up a bit on HIPPA. He spoke to a male in our family very differently than us irrational women folk. Keep in mind this was during a time we thought they LOST our 87 year-old Grandfather. I would not board my dog here nor would I hire Kevin to run my lemonade stand. Please do not send your family members here (and my dogs are family.) I will also note my Grandfather has told us several times how mean they were to him since being home. I will say Bianca, not sure I spelled that right, and a few other employees certainly deserve raises and possibly Kevin's position. Oh and my Grandfather gave me permission to post this with his info.
My mother, a resident at Park Terrace, passed away just this afternoon. I cannot express my gratitude for this place and every single employee there! Mom was always treated with love and respect. I will always hold you in my heart!
My father in law, Ray, was at Park Terrace for a week. He had just lost his wife, 4 days prior, has a broken wrist, fractured back and ribs and was getting over Covid. From what we saw and what Ray told us, the food was good, there were a lot of caring people that took care of him. One day they had him ready for his wife's funeral and another day they got him ready for his granddaughter's wedding. The place was clean and bright. A special thanks to one of his nurses, Marie Curry, and many others that we don't know their names. He really spoke very highly of his PT folks, too.
Exceptional, compassionate care. Staff was on top of everything. Clean rooms, amazing food. The rehab specialists, nurses, and case managers were so good with my Mom who was recovering from a hip fracture. We're so grateful for their amazing care.
The rooms are dirty and smell, the wall has feces on it, the carpets were stained and gross My in law was left in a chair in their own feces and urine for 6 hours until I came in and complained. They wouldn't try to get her up, she had to wait for days for a sponge bath because they told me they only give showers two days a week. I moved them to another facility and upon checking her the bed sore she had was worse and her incision was infected. DO NOT GO HERE EVER.
My father has been in Park Terrace Health Campus for a couple of weeks now. So far, I'm happy with everything and it seems like a nice place. I'm not disappointed. He's been in quarantine the whole time and he's just coming out of quarantine today. Today's the first time he'll be out of his room. He's just been stuck in his room since he's been there. They had to put him in quarantine because he had never had a Covid shot. He hasn't even got to do anything or meet any of the other people, which I think will help him when he's around some other people his age. He's been locked in his room and none of the other people except the staff could come in his room. He's in skilled nursing. He's in a private room and the accommodations are nice. The food seems pretty good. I'm always just there in the room with him, too, but they got the nurses that come around every couple of hours and check everything to make sure they're OK. Everything seems pretty good. I wish to see him doing stuff. I see activities going on all the time, but I haven't personally got involved or seen what they were doing. That's part of the problem with him being on curfew, but I see that they have a lot of activities available.
Park Terrace Health Campus looked like a clean, well-organized, and comfortable place. They let me see the memory side care unit, which obviously is behind locked doors. The rooms would have two people. So basically, that was probably the main drawback for me; it's like a hospital. The room is just like a hospital room. It has two beds in it with carpet. It's probably the only thing that has turned me away from that a little bit; i don't like the idea of another person in the room with my mom and not so much privacy. I know I'm not going to do a private room, but I would like to try to see if there are other places with enough rooms to do just one person. There's a hallway that you can walk around, a sunroom, a living room type area with a TV and a bunch of chairs. It all looked nice. They have an activity schedule daily for the memory side, where they can keep the people engaged and busy if they like to be. The moment I was there, there wasn't anybody doing any activities. It was well later in the day, but they showed me the calendar of activities through the day.
My mom loved the food. The nurses and assistants were nice. Here second room was clean. The activity director came around often and invited my mom to participate. Our mother had a stroke and we had no experience with stroke patients therefore asked many many questions. The physical therapy staff seemed bothered by our inquiries. The social worker is where we had our most trouble. Although I was there every day, I was informed of my mom's discharge via a telephone voice mail on a Friday afternoon. She had neglected to order my mom the medical equipment she needed upon her discharge. I spoke with the secretary in administration and was told everyone was gone for the day and she had no way of contacting anyone. I had to call Medicare to stop the process until my mom received what she needed before she was discharged. I voiced my concerns to the Director of Nursing who listened and assured me that mom would get what she needed before discharge. I also asked to speak with the administrator on two occasions, but he was unavailable,.

While my grand mother was there your staff washed her off only once a week, somehow you broke her arm, let her get assaulted by another resident, and then on 05/23/2020 she died while in your care
I spent a few days at this facility in November 2019. While the therapy staff were wonderful, and for the most part the nursing staff was very attentive, I will never use this facility again. Having had knee replacement surgery I did not need as much care as others in the facility. I really try not to bother the staff unless it is just something I can not do for myself. On the Friday evening of my stay I had, had a very rough day. As evening went into night I was having a very difficult time getting comfortable enough to sleep. Sometime in the wee hours, about 3am I think, I called for my pain meds. The Aide said she had to tell the nurse, and the nurse had just gone to lunch so it could be a little while. After laying in bed for so many hours, I decided to get up and sit on the bench while I waited for my pain meds. When I used my walker to sit back on the bench, the bench seat flipped in leaving me stuck with my feet off the floor unable to get up, (remember I am recovering from knee replacement surgery), not a very comfortable position to be in for any length of time. I sincerely do not know how long I was like this, it was somewhere between 10 and 20 minutes before someone heard my yelling. When the nurse came in with my meds she helped me get up and back in bed, she gave me my meds, and I never saw her again that night. At about 7 am Saturday morning I was in quite a state of, sleep deprivation, pain that in no way was being managed, and completely exhausted. The nurse on duty that morning acted like she didn’t know what I was talking about when I told her why I wanted to go home. This told me all I needed to know about it. The incident was not documented. This proved to be true as over the next 4 days no one came to ask about what happened, and no one ever came to fix the bench. My husband had to fix the bench. I had two very bad days here out of 8. As I stated before my troubles really did not come with staff. The last thing that caused me to rate them below avg. is the fact that although each room has its own bathroom, the showers can not be used. I had visited Park Terrace before my surgery and my mane concern was that at other facilities everyone used the same bathing area. When I ask about this the person who took me on the tour said that would not be an issue, each room had its own bath. I was very upset to find out I could not shower in my room. The surprising thing to me was to find out that admissions did not seem to know the showers in rooms did not work. I know it is a common issue at places like this, but is it really necessary to understaff these places so badly. I do not blame the people who are doing the very best they can in circumstances not of their making. Bonnie
People who work there are nice, but did not pay attention to patients and their records. My mother was supposed to have a special diet and they were trying to feed her regular food. That caused her to have more problems, which led to her to not want to eat from the pain and nausea. She also developed pneumonia. Needless to say she had to go back to the hospital. I was very disappointed in her care. The last RN tried to correct the things that were overlooked but by then it was to late.
I honestly could write a book about this place on how great it is! Management , culinary, nursing, administration, and environmental services all will answer call lights !!! I can’t think of anywhere that does that. You honestly get treated with the dignity and respect you deserve as well as get great nursing care and rehab services. I work for this company and I’m a cheerleader for them because they honestly care about every person that comes to live there weather it’s short term or long term you become part of our Trilogy Family !
Make it easier for family member to stay with their loved one. Have a list of restaurants that are nearby that they could go to when cafeteria is not open.
This place is the absolute worst! My mother has been here for almost a week now. She has wet the bed at least 10 times now because she can't get anybody to help her out of bed. She had a heart attack and was on a ventilator for 9 days before coming here so she is weak and unable to get up on her own. I hit her call light and after waiting for 45 minutes I finally walked to the nurse station and they were both playing on their phones. I told them she needed to use the bathroom and it was another 20 minutes before anybody came to her room. As of now, the man next door has had his call light on for 15 minutes. I heard him yelling so I walked over to check on him and he too is waiting for someone to help him to the bathroom. This place was amazing a few years ago when my father was here. I'm not sure what has changed. But if you are concerned for yor loved ones, this is NOT the place to send them. It's not because they are understaffed or busy because they have plenty of time to play on their cell phones.. It's pure neglect!
Our experience with Park Terrace Health Campus has been great. The staff is outstanding, they take great care of my mother-in-law, and I can't say enough about them. The food is not bad. I spoke to the higher ups because it seems they serve some stuff that the seniors have trouble eating. Like food that's not easy for them to cut up, or hard for them to chew. The place itself is very clean.

It's not a "Park" it's a bldg. Full of Doctor's offices, more like a medical plaza. I love my Dr's office everyone there are polite and friendly.(southwest family health center)
My Mom was a resident for rehab at Park Terrace in August. She had a great experience. The staff was caring and made her comfortable. The food was good and she liked the therapy she received there. She is doing much better thanks for the help.
Decent place, Friendly staff but sometimes it seems they are understaffed due to the amount of time it takes for them to answer a pt. call light. Rooms could use updating, brighter colors, less depressing.

My mom stayed at Park Terrace. When we communicated concerns with the staff, it did not seem to be carried over from one person to the next. We wanted her to have milk with every meal and in four weeks, we could never get that to happen. The meals that they brought had nothing to drink. It seemed like a very simple thing. There were times when she was in a diaper, and there were times when we would arrive, and she was dirty. They were understaffed and impacting the patients themselves. They had young people with very little training as compared to the where my mother is now. We encountered problem after problem with her care because there seemed to be no communication. However, the environment was very nice and very clean and without odor. The facility looked good. We were very pleased, but she just was not given the right care. When we pointed out the problems, we were very nicely told that they would be addressed and they were not.
My dad was a resident in the long term care facility from Feb 2016 until June 2016. The people at Park Terrace are a fine group of people." I cant say enough good about them "My dad was not an easy person to take care of." Dad was clean, shaved and showered like he was when he was able to take care of himself. The facility NEVER had the feel or the smell of a nursing home and looked more like a 5 star hotel. A big heartfelt thank you to all the ladies that took care of my dad until the time of his passing!! THANK YOU!!!!!!!!!! The Donald Christian family.
I have never been to a rehab center before, so didn't know exactly what to expect. I was there less than a day when I realized what a wonderful place Park Terrace Rehabilitation Center was to heal after surgery. To the person, I found the staff friendly and helpful, from the nursing staff, to the dietary department, to housekeeping. My room was kept neat and clean, nurses and other personal care givers were responsive to the call light, and each had the personal touch... The call light was answered by a person at the door instead of a microphone and speaker. The physical therapy facility is top notch, as are the physical and occupational therapists. Not once did I encounter anyone who was not friendly, caring, and compassionate. Yes, physical therapists tax your body to the limit, but without them, it's doubtful that most of us would put the stresses on our surgically weakened bodies to effect a swift and complete healing. I can't say enough good about Park Terrace Rehabilitation Center. If I ever need the services of a Rehab center again, I won't hesitate to call upon Park Terrace to fulfill those needs.
