Signature Healthcare at Colonial is a wonderful place. They really care for their residents. They keep the residents happy with fun activities. I think Colonial is a great place.
If I could give 0 stars I would. Don’t put your loved ones in harm’s way..
Do not go there. The 1st night my wife was there they gave her false information. They said that they would clean her up and she had to clean herself. No help from staff.
Unspecified · February 2024 If you care for your loved one, I would recommend looking somewhere else. My mother was here for about 24 hours. In that time, she was not provided her medication, they checked BP and oxygen 1 time (she is on oxygen), when the call button was hit, no one came for 30 minuets and we had to go find a nurse to finally get someone in her room. After 24 hours, we took my mom out and just made arrangements with family members to move her home.

Misty Morgeson, CEO heads up the most amazing team!! Some employees have been there 30+ years and excellent care they give shows!! Placing our sweet momma in a nursing facility was one of the hardest decisions we’ve ever made, but we feel the Lord led us here so mom can receive the care she needs wrapped in the love she deserves!!! Thank you for making this transition easier than we ever anticipated!! We love each of you!!

My 88 year old Grandmother was brought here after colon surgery. Not one day after, my family goes to check on her. She was covered in feces and had a fan blowing on her. It took over 30 minutes to have a nurse assist her. DO NOT LEAVE YOUR LOVED ONES HERE.
My daddy was sent here for rehab only. To learn to walk again. Within 8 months he lost 70 plus pounds and is now deceased. Wouldn't put my worst enemy in this place.
Unspecified · January 2023 Signature Healthcare Colonial Rehab & Wellness is a great facility and resource. The staff are warm, friendly and professional. The facility is clean and I always enjoy the activities they have with the residents/patients. They also provide excellent rehab to help transition back home if that is what is needed. Also the administrator is hands on, welcoming and always available.
Colonial has been my 2nd home since my teenage years. It’s just home to me. I love the atmosphere and the great teamwork we have. It’s takes a lot of passion and love to do what we do. We all come together and make our/your loved ones feel safe and at home.
I am a resident here at colonial I live here long term. I think they're doing a great job with activities. There's always something to go. Going outside and getting fresh air. Working on gardening. All the workers cut up and have a good time with us. We're like family. I love living here. Physical therapy does the best they can with everyone and getting people up and walking again. the adies and nursing staff are so caring and work so hard.
Our experience so far at Colonial has been satisfying.The staff is always willing to help and the nurses and aids are amazing they are always trying to make our mom feel welcomed and like family. I have not yet been able to visit inside the facility due to the pandemic but hoping that will soon change . But even though we have been able to make window visits as often as we like to . And I have been to visit on several different meal times and it always seems to be a good portion and food appears to be satisfying and if not they try to find something to make a replacement. Activities seems to have picked up more which we was very pleased to see .

Unspecified · August 2018 Medication aren't documented properly. Poor quality of care. Patients need basic care needs and proper medical attention. Friendliness is not full care. You must be there everyday to ensure care is given. Poor hand hygiene. Entire wing CDiff.

Unspecified · August 2012 I have always enjoyed my visits to Colonial House. I have been visiting since i was a child, when our church would go once a month to sing. The residents always have a smile on their faces that brings me joy. The staff has always been great!!
