Never send anyone you love here. Most lie, don't care for the patients properly, very neglectful, these people are here for the paycheck, not because they care. They had the nerve to tell me my mom was faking her condition. My mother is in worse shape now, and is wasting away. This place needs to be investigated...

If you are seeking the best care for your loved one---Signature Healthcare at North Hardin should be on the top of your list! Wonderful, caring staff. Beautiful building that is exceptionally clean and inviting!

Five Star rating isn't enough for this amazing facility. The staff are excellent, kind, and compassionate. A great example of what long term care should like for all facilities.

I visit the elderly people there every once in a while and I will say that place makes your family feel like family. They’re well treated very well cared for, and they are their new home away from their old home

Love seeing the activities and celebrations of holidays for the residents. It’s clear that the staff really care and try to give the residents days that are full of fun, laughter, caring and compassion.

This facility is awesome from the staff to the residents! So much happiness in that place, I took my kids to there Easter program the staff and residents put together it was great thank you Signature!!

Visit Signature frequently.Staff is always welcoming and helpful. They seem to enjoy their job and I love how they interact with the residents. No matter what time of day I visit I always find it very clean and orderly. I enjoy the seasonal events they have which include the community and the residents which opens their world a little. The grounds are beautiful with a pond, ,fish and a duck or two. The big Easter Egg Hunt is coming up which is always fun for the residents and the kids of the community. There is always something planned for the residents, parties,contests, music, art and more. In In my opinion Signature goes above and beyond. 😊

My Dad was admitted to this facility for rehabilitation, and our family could not had been more pleased with the care he received. From the moment he arrived, the staff demonstrated professionalism, compassion, and a genuine commitment to his recovery. Thanks to the dedication and expertise of the entire team, he left rehabilitation feeling stronger and doing better than he had in quite some time. The level of attention, encouragement, and quality of care provided made a significant difference in his progress. Our family is truly grateful for the outstanding support and care he received during his stay. We sincerely appreciate the entire team for their hard work and commitment to their patients.

Fannie has been here this time since Thanksgiving night 2025 and this is her second time being a patient as a stroke victim and has been treated great both times, the nurses, admin, staff and CNA’S have all been very responsive to her needs and concerns, the physical therapist did a great job with her, and especially Neal and Lauren in Occupational Therapy all of the CNA’S on the day shift were great,I would try to mention them by name but would take forever and ,the facility has great workers and especially Nicole a nurse that she has had both times and she went beyond the call of duty to make sure Fannie was taken care of, I want to thank Britany for making sure her paperwork was straight and Julie for ensuring she had a room available when she was discharged from the hospital on Thanksgiving night and Pat who was very helpful with any appointments or help we needed and great coffee. One CNA JayJay (probably misspelled) came in yesterday on her day off to tell Fannie bye and be there for her when she left.

It has been 10 Days since my 83yr old Mother, who had been suffering with VASCULAR DEMENTIA, has gone to heaven. To this day, I continually reflect back on the heart felt, quality level of care that she received during those last couple of months that my mom was still with us. It was on FEBRUARY 13th, 2026 … my mom had passed away in her sleep & was now being rolled out of her room to a waiting transport vehicle. As the family was walking slowly behind her, we could hear soft music playing throughout the hallways and as we turned the corner, to our astonishment we looked ahead to see that both walls were lined up with the Administrative, Nursing & Care Staff to honor my mom with their tearful eyes and many hugs of sympathy for our family. This final goodbye was yet another validation that this was the best place for my mom to have been taken care of during her final days. You see … Signature Healthcare @ North Hardin was not the first REHAB/NURSING HOME that my mom was in, it was her Second One, after a LESS THAN PLEASANT, short 3 week stay at another facility in Hardin County. So, from firsthand experience, I know the difference between true quality level care and, as my mom would have said, “NOT SO MUCH!” So, what was the difference? It actually all started with that very first meeting we had with the ADMISSIONS COORDINATOR: JULIE T. From the warmth of her kind greeting, comforting smile & caring personality … she made the intake process feel like you were with family, while easing the stress that comes with leaving a loved one to be cared for by people that you have never met. This particular SIGNATURE facility lives by the golden rule, to treat others how you wish to be treated, while remaining focused on being supportive and compassionate. It boils down to the facts that my mom was not only provided with a clean, comfortable & healthy environment. The Staff were always positive, attentive and encouraging in a manner that was critical to meeting her needs. Not to mention the many times where they went out of their way just to do something special for her, like painting her fingernails or dropping by her room to give her a hug. And on those days when I inquired about this or that, I was always responded to in a professional and swift manner. I truly could go on, but I won’t do that! If you, or a loved one, are in need of a QUALITY LEVEL CARE facility, for SHORT-TERM REHAB or LONG-TERM RESIDENCY, then this is the place to be. THANK YOU, SIGNATURE HEALTHCARE at NORTH HARDIN!

Just wanted to share our appreciation for the fine job the staff at Signature North Hardin has done in their trusted care for my Dad. Since the first night Dad was a resident there the entire West staff has done a tremendous job meeting our needs and requests. From Allison, to Nicole, to Taylor, to Deborah (just mentioning a few of the many who have been a blessing) the entire staff as a whole has been outstanding. My Dad, has had some issues stemming from mistreatment at a former facility, especially with his breathing issues. Any time he has shown the slightest inclination of needing assistance or needing to go to the hospital, the staff at North Hardin has been on top of it with a text to us or a phone call…they have listened, and carried out our desires to see Dad get back to his old self whereas other facilities seemed to limit or give up on him. They have pushed to better his situation and always go above and beyond to accommodate our desires and his needs. They have paired my Dad up with the best roommate we could have asked for (thanks Roy) and are always open to dialogue about how we can best get Dad on the road to recovery. They don’t limit their expectations, they strive to go beyond what other’s expectations may be. We are thrilled with the staff and the facility as a whole. They are tireless, open to communication, and strive to meet both the family and residents’ needs. We couldn’t ask for better guardians or stewards in my Dad’s care. 5 stars! Beautiful grounds outside the facility reminds you of going down memory lane with the serene view of the pond. Many new updates to the inside of the facility. Rooms are spacious. Residents appear happy.

So my parents were here a few months ago and I was happy that someone was willing to accept my parents especially in the condition they were in. I was out of state and was thrilled. Once my parents had been there for a month at North Hardin Signature in Radcliff I was HORRIFIED by the way thee staff treated others, how unimpressed I was at all the BS that they explained to me on the phone and in person. I was HORRIFIED by the way they left my parents in urine soaked depends all day without anyone changing and NO food when they stated, Especially when I came to visit they havent been fed for 24 hours. I automatically called The VA which had no contract with Hardin Signature of Radcliff not knowing they lost there contract by any and all stupid means! The staff and management lied to my face that they were taking card of things as fast as they could. Medicare and the VA stated "PULL THEM OUT" ASAP! so I did within minutes. Not one CNA or Nurse gave me all there belongings and or laundry and or there medications when i was about to leave. The place is older and smelled like urine was all that you smelled. There was also a person in the courtyard that fell and it was amazing to me if the person was hurt or not because the staff took there sweet time in making an effort in getting to them fast enough. I am HORRIFIED and Baffled that the state hasn't done more to this company that what they look or smell like. DISGUSTING, UNFRIENDLY, INEXPEREINCED, UNCONSCIOUSLY STUPID, and so VAIN. Anyone sticks there family here .....OMG !!

I had the privilege of visiting this community over the Halloween season. They opened up their doors to the public to give out candy to the children. The staff at this nursing home were friendly and inviting. Each staff member greeted us as we strolled down each spooky hallway. As I walked through this brilliantly decorated facility I couldn't help but think about how well maintained it was. The walls had fresh paint, and the corridors were filled with the pleasant aroma of apple cider and the likes of Halloween. The staff seemed very engaged and attentive to the elderly residents in their care. I oftentimes heard and seen the staff playfully talking to and assisting the residents as we walked past. Although I don't have any family members of my own that would benefit from Signature's services, I would recommend to anyone, to at least add this home to your list of buildings to tour, when deciding where to place a loved one. The caring atmosphere alone, was what really stood put to me and my family.

I visited the facility very clean and nicely kept! Everyone was very friendly! The residents that I saw seemed to be very happy!

Had a tour yesterday and was very impressed with the beautiful campus and amazing Caregivers here. Alison and Julie introduce me to many great staff, and Mackenzie made us feel very welcomed and calm with our decision. Met with Debra and Rebecca who truly have a great passion for what they do for their patients. We look forward to a great experience with Signature North Hardin.

I am a former resident of Signature health care North Hardin and I want to say that I will miss some of the nurses and aids there I want to thank them for all they did to make my stay there comfortable

Julie from admin and the nurses and aides have done an outstanding job welcoming my Father to the facility. Brittany has also been a great help in facilitating paperwork between insurance and Medicare/Medicaid. The whole staff always speaks and shows genuine care and concern for the residents. The Physical Therapy is top notch. They spend time working with Dad and listen. I would recommend Signature Healthcare at North Hardin for your loved ones.

They are not doing what they supposed to they don't care about the people my mother is in this place and they have not been taking very good care of her and I am appalled at how they are treating her and the food there is awful they treat my mother like she is a child and she is not a child she is a human being and deserves to treated like one and she deserves respect

I would highly recommend Signature North Hardin. My sister was fortunate to get a bed in this facility. The nurses, aides, office staff, therapists, absolutely everyone treated her with the utmost respect and kindness. They treated her like family and I was very grateful to everyone. Top notch facility.

Both of my parents are now in rehab at North Hardin Signature. A series of events led to this. My mother was already there after being hospitalized and my dad soon followed afterwards. Julie worked so hard to get my dad the help he needed. She brought all the components together to get him admitted and on the road to recovery. I cannot say enough about the staff at this place! Mom and dad are clean, fed, their clothes are clean and put away. They have access to numerous activities outside of their required ones. They have bible study, BINGO, weekly newsletters, exercise opportunities, snacks, and much more. I have questions because that’s just who I am. I’m never given the brush off. If an answer is not known, they end up finding it before my visit is over. Having my parents here receiving the help they need by caregivers who really care, has given me time to take of business that needs to be handled. It doesn’t matter what time of day I pop in, my parents are taken care of and listened to. I’m so thankful for this place and the people that are my Mom and Dad’s caregivers. No place is perfect, but I feel comfortable here and I know that my parents are well taken care of and that these caregivers truly want what’s best for them.

My time at Signature North Hardin was outstanding. The care from the first minute I rolled into the facility until I left was 110% positive in every way possible. The staff was amazing 👏 in every way. Physical therapy accomplished my physical needs. They explained everything I needed to know about my stay. Signature North Hardin's therapy is very good. The staff has listened to our concerns and criticism, and it has gotten better. My mom does therapy and goes to some of the church services they offer. They have a Fair Day where they serve snow cones and cotton candy which my mom really enjoys. Food is one of the concerns because they send out an order form or a menu, and Mom selects things, but she doesn't get the things she selected.

I am a former rehab patient And my room was cleaned everyday It is in the rules now that we shower twice a week and on those days if not before the bed is changed Food is ok the activities are posted on the calendar that is distributed throughout the complex It's posted Through the complex on the walls in the hallways as well as in each resident's roomThe nurses there are few that were my favorites and it was always because they Very cheerful and Had a ready smile for me Some of them They were only there for the paycheck Same way with some of the AIDS Butt There were a few CNAs that were just Tops And they're the best I go there now and they greet me with a smileThe physical therapy staff is also excellent and they work really hard with all of us I would have to say That There has been an instance where I have heard a resident calling for help And no one comes And I have had to go down to the nurse's station and get some one for my neighbor

This place is ABSOLUTELY horrible. My family member has had nothing but bad care since he has been there. I go to visit and his bathroom is a mess, the CNA's, with the exception of one, are useless. If you want to earn a check for free I advise you to seek employment here. Several items has come missing and I would not recommend this place to keep a pet!!! They need to be closed down!!

My uncle was in here for rehab and it was horrible. They had trash all on the floor. The bed never got changed. He has never wore a diaper before and because the CNA didn't want to wait on him to go to the bathroom so she put a diaper on him and told him it was ok to use it. That will break his skin down and it is hard to get that held up. The staff was horrible. He was there for one week of unsanitary conditions. I would not recommend it to anyone.

THIS PLACE IS TERRIBLE!!! Patients don't get a bath like they say, they lay in their wet pads, nurses are rude and act like it's an "inconvenience" for them to do something as simple as getting a blanket. They need to shut the place down. Other patients are yelling for help and no assistance is given.

My husband was in North Hardin Health for rehabilitation. I have to say that after two days, he called me on the phone and said, "Mimi, if you don’t get me out of here I am calling a cab, and I am walking out of here." I went over there, and it took me almost all day to get him out. My husband was with somebody else in the room and when I came to pick him up, he was already sitting on the bench close to the door to get out. They have very little space, and I didn’t see anything for recreation at all. My husband went that bad. I didn’t want to ask him what about it, he just said don’t talk about it.

My aunt is at North Hardin right now and she is doing well there. The people in the facility are outstanding. The place is very clean. I haven’t tried the food myself, but according to her its okay.

This nursing home was all right for what it was. It had a reasonable cost and the residents received basic care and assistance. They provided physical therapy and other medical needs when required. The nurses were overworked and some were very unfriendly. The shifts always seemed understaffed. Unless the residents had standard schedules many were just left to their own devices either sitting in the hallways or wandering around. Some of the more difficult residents seemed to receive the least amount of care from the nurses. The residents who were better able to take care of themselves received the most amount of attention. Some residents were unlucky enough to have nurses who let them lay in their own waste and several residents had developed bedsores. The doctors were nearly absent. In the seven months that I visited this nursing home I saw a doctor only a handful of times. It seems like most residents received the most basic of care and anything further required that the families come in to assist.
