Signature HealthCARE at Rockford Rehab & Wellness Center

I see my negative review has been deleted. I was inpatient here for 28 days. Here is my experirce in short terms. Not a rehab Physical therapy is 15 minutes a day not counting weekends Food and nutrition is horrible No menu. No choices The same food just reincarnated to the next meal. Trying to get ice or water is an act of god. It takes hours for a response. Daytime workers are fine After 7 uou are on upur own and all weekend I had fractures thst kept me from being able to walk. So i had to be waited on. I cried e ery dsy. The lafy named Megan who visited each day wzs thr only o e who seemed to listen and care. Beds and rooms are outdsted and depressing. No professional drs at all. Not once did any dr take my complaints serious Do not reccomend at all. The na named Mary and the day time RN were fine. Others were just there for a paycheck Sad to see people have to be treated as yhough they are incarcerated

Staff is great.

My dad spent right at 2 months at this facility. They accepted him on a Medicaid Pending status when other facilities wouldn't. I couldn't believe our luck in what he experienced with dad's care. This facility does all the hiring so their workers are vetted thoroughly and it shows. Dad had the best care imaginable from the moment he was accepted until the time he was called home. Marie in admissions is wonderful! She helped us get in the door. Christina, the social worker helped with myriad of requests and did so with a smile on her face. Dad loved his Nursing assistants Mary and Jasmine. What's so special about this is you could tell they cared about dad and they cared about my mom and me. They updated us with how he's feeling and acting and eased our worries when those worries overcame us. If you have a loved one in need of top notch care by an overwhelmingly talented, thoughtful, caring team, you should consider choosing Signature Healthcare Rockford. I'm so fortunate, but more importantly, my dad was so fortunate to be under their care for his final days. Thank you Rockford, you'll always be in my heart!

My Father is a resident at this facility & I couldn’t reach him, but a wonderful lady named Susie went over & above to make sure I was able to talk to my Dad! Blessings to all who go above the call of duty!

My mom has been at Signature Rockford since November. They have been great to her. Working with Christina, and Sharon, and Lisa, the OP/PT staff, and all other staff members has been easy and pleasant. A huge shout out to Christy, one of mom's nurses, who is always on top of her game. She communicates with me constantly and mom loves her. She has been a great care provider to mom and awesome for me to work with. Mom's only complaint has been the food at times LOL. But she loves the staff and the care she has gotten.

I feel that this a great place for patient's! It is also a clean facility. My fiance works there and I know that she goes above and beyond for the patient's she takes care of.

I work at signature Rockford. I love working here, we the staff truly care about our patients. I personally go above and beyond for them. Management is great, always somebody to ask for help if need be. No issues in this facility at all.

They treated my aunt with excellent compassion and care.

Signature Healthcare rocks. Grandpa was in the facility for a hip replacement. Admissions for rehab was excellent. All staff and employees were nice and friendly to him. The facility is clean and professionally ran. Highly recommend Signature Healthcare for rehab.

This place smells like urine, 24 hours a day. Is this location understaffed. Is this location subject to budget reduction due to lower income client's. A surprise state audit might cure that issue. Supervision could be weak as hell, who knows. You folks need to get yourself aidit compliant at all times. Is this a form of piss poor management for some clients, over other clients. This place is pitiful. Making money off the lower income patients, to support state mandated polices to treat all patients recieve the very best care. Get your damage control activated immediately, from this point all. If patients start getting discharged from this facility you are on the radar. You are being monitored. I don't want folks to be fired, I want them to understand that eight hours of work doesn't include talking on personal telephones and birating patients. Some of these employees don't give a *** about the patients with urine stench throught out the minority corridors verses others

I have worked at Signature Rockford for 32 years !!! I absolutely love coming and caring for all of our residents and treat them like family. It truly doesn't feel like I'm going to " work" I feel like I am walking into my home away from home. Everyone at Rockford cares and loves our residents so much !!! I have had 3 family members at Rockford because I knew they would get the best care. Our managers are there for us and treat us with respect and recognize us for doing a good job. I could go on and on how much I love working at Signature Rockford. We are like one big family.

I have worked at signature Rockford a year this month and I love it here everyone is so kind respectful and very helpful I enjoy seeing the residents and staff smiling faces everyday

My brother moved into Signature HealthCARE at Rockford. He's not happy with it. They didn't start his rehab right away. I don't know if that was part of the order for them not to do that. My brother needs assistance, like going to the bathroom, and he's had problems pushing the call button, and nobody comes for an extended period of time. Several times, he hasn't had help in time. The food is horrible. I've had to carry meals for him to eat. They don't seem to have the incentive to give their best to their patients. Every patient might have complications or difficulties, but when you're in that field, you have to learn to work past that to provide adequate care. I just don't think that this facility does that for anybody, not just my brother. I've seen others that seem to be neglected. I don't want my brother there, but I want him to stay until he's physically able to handle things. At this point, it would be hard to persuade him to go to another facility if he needed to. It's an older facility. The faucet in his room doesn't set off. It's moderately clean. I think it could be better at keeping it cleaner and fresh-smelling. My brother is in a private room. It's a long walk to get to his room. I don't see anybody engaged in any kind of activity.

Good afternoon I have been a patient here at signature healthcare at Rockford Lane on Quinn drive. And the care I have received has been phenomenal, and the entire staff has been wonderful they have been kind caring. Whenever I have turned on my light they have been there within a minute very helpful. The food is delicious, it's very very clean here. They have activities for everybody here no matter what level you need All the therapists are great they have activities for everyone which is fun. I definitely give this place a five-star rating. I am almost back to normal, because of the staff and everybody here the cares thank you to everybody here and definitely recommend this place 💙💚

Signature Rockford has very caring Nurses and C.N.A.s I recommend them highly. The teamwork is excellent.

I recently transferred to Signature Rockford! What a hidden gem! The staff has been so friendly and inviting. I've watched interactions between staff, residents and families and it was refreshing to see how loving and supportive everyone was. I am super excited to see the great things in store for this building.

The social worker here is very informative and helpful! Definitely one of the best I've ever encountered!!

CNA's have the hardest job and still continue to be dedicated and dependable and they truly display care and kindness for all the residents. Therapy department absolutely has a great turnover the best in the business hands down!!! I would not only recommend this facility to a friend or family member if need to be I myself would chose this facility for my long term care or rehabilitation

I love it it helped my grandma live a good life and she was able to come home with a full recovery

The facility has had nice upgrades, and looks like it is committed to continuing to improve inside and out, Pleasant Nursing staff and amazing D hall CNA's. Therapy experience has been amazing, going the extra mile not just for therapy outcomes of the patients but for their well being and peace of mind. Love getting to know residents and staff. Had an amazing time this past weekend at the tunk or treat with my siblings at Rockford Rehab!! Lots of fun with the staff, nurses, had good energy. The karaoke added a good vibe to the place. The seniors were all dressed up and they were passing out candy. They nursing staff seemed to care a lot about the elders . I would recommend Rockford.

Signature Healthcare at Rockford is a great facility! I would place any family member of mine in this facility with full confidence they would receive the best care. The building is very clean and all of the staff are friendly and quick to respond to any questions I have. I would recommend Rockford to anyone looking to place their loved one in a Nursing Facility.

Bed bugs!! dirty rooms , dirty shower rooms , under staffed, the nurse don’t help their aids.

I was hired by a patient to transport him from this place to Chicago. He told them at least 4 hrs before I got there that he needed a wheelchair & his meds to take with him. I still had to wait an hour & then I had to demand that they bring him the front. The staff was completely rude to boot & then didn't help the patient out to the car. I feel sorry for anyone staying here.

This place was nasty bathroom was nasty food nasty. Place was a Nightmare horrible. They didn't discharged me properly. i almost lost my check and care. This place should be shut down. Please do not put anyone in this place. This place is a joke not funny joke either and it took them 30 min or more to come to assist with client's the other reviews was nothing but lies and bull

Horrible service. Very unprofessional staff as having relationships with each other. Facility has been notified several times of this relationship and have done nothing about it. He’s a director of housekeeping and she is a business office manager. How can this facility allow this to go on? This is a very unprofessional atmosphere along with the unsupportive upper management allowing this continue.

I was unable to visit a dear friend of mine recently due to the pandemic but the facility took great care of her..she really enjoyed her conversations with her CNAs and time with her therapist..but missing her doggie was good motivation for her getting better and coming home...thanks yall

Amazing Nursing Staff, Great CNA's, the Food from dietary is delicious, Therapy is hard at work helping patients reach their individual goals, Housekeeping is doing a fantastic job keeping us all safe with all the extra cleaning procedures.. A few words on here can not speak to the volume of work, passion, Love and caring that goes on in Rockford Manor, The therapy team is hands down the best. A great facility with amazing staff.

VERY HAPPY WITH This place its absolutely amazing , my dad is there as we speak and he is very pleased with this place and the people that work there - everything about it. He has been to a couple places and had always complained- not this time, he has not one time complained about this place. Highly recommend. Thank you to the staff there. 🙏🙏

Understaffed, 1 nurse can not possible pass medications, do treatments, assess , take vitals, chart, oversee the aides work, admissions, call family to report changes in patient medicine or condition. Therefore a lack of reasonable care exists on a daily basis. State rules and regulations are broken routinely . Because of this a great turnover in staffing occurs. I have seen 5 administrators, 5 DON'S, and a host of nurses and aides in the past 12 years. Signature focuses on profit not standard of care. Before you place your loved 1 in a Signature facility please research complaints and investigations by the state. And just so you know numerous good ratings are posted by senior staff......

I do have to say that this is the nicest facility I have visited. My grandma was there and she had nothing but good things to say. I was greeted by a super helpful lady at front desk. The whole place seemed to in sync with each other but I have to say the physical therapy team was exceptional. Thank you all so much and will definitely recommend this place highly to everyone.

Staff is caring and respectful. I would recommend anybody sending their loved ones here. I would stay forever if I could they have been heaven sent.

Horrible experience. We toured the facility and all of our questions were answered, but not truthfully. I can't say enough bad about the Nursing Staff starting with the Nurse Manager . They really do not seem to be concerned AT ALL about their patients, and they will not take suggestions from family. Absolutely scary experience. Mom was taken out in an ambulance in LESS than 2 days. I urge anyone not to consider this facility for anyone you care about !

Signature Healthcare at Rockford provides exceptional care to their residents. Rockford provides both long term care, and short term rehabilitation to those individuals needing assistance while recovering from an injury or medical procedure. The building is clean, and the staff members are helpful and friendly. Rockford Rocks!

I have worked at Rockford for over 20 years. My Father and my Brother were both in short term rehab and received excellent care. The staff is very knowledgeable and compassionate.
