Summerfield is now under a new management, Israel and Brandon, who have made a difference. The past is now behind us. Every resident is welcomed like family. The last State Survey proves it all. The food is incredibly varied, always catering to every diet. Residents have fun every day with our lovely Vicky. It’s a place where, upon entering, you can see clean, fragrant hallways and rooms thanks to our top-notch housekeeping manager, Paula. There are so many people who care and work their hearts out. God bless you, and you’re welcome to our home!
The food is so good I was visiting this facility and could not believe how beautiful their food is!!! This is definitely not hospital food!!! Their staff is friendly and approachable I was amazed to find they work under Chefs and hold events for the residents!!!
I have been hear for almost 2 years and I have had trouble getting my showers like I am suppose too.and now they are starting over again I can tell them
I have enjoyed my stay here from therapy to activities, from housekeeping to nursing and dietary I couldn't have asked for any better care. I highly recommend.
Room is great Moe and Ralph are the best. No place is perfect but this place is as close as it gets

I want to share my family's troubling experience with Signature Summerfield Nursing Facility on Farnsley Road. My 55-year-old sister was admitted there in December for rehabilitation following a below-the-knee amputation. She was supposed to stay for a few weeks, but her stay turned into a nightmare due to the facility's unethical practices. The social worker assured my sister and our family that her discharge date was pending approval from her Molina Medicaid Healthcare plan. However, we later discovered that the facility had contacted the state, falsely claiming my sister was a long-term patient. This caused her Medicaid plan to be terminated and replaced with a state plan, which provides fewer benefits. It’s clear this was done out of greed, as the facility would receive more funding from the state. To make matters worse, the nursing home took nearly all of my sister’s Social Security check, leaving her with only $30 for January. This has left her unable to pay her monthly bills. Despite knowing she was not a long-term patient, the facility still reported her as one, misleading the state and causing significant harm to her financial and medical situation. When we tried to resolve this by contacting the social worker, our calls were repeatedly ignored. My sister’s short stay was further marred by poor care. Her call light would remain on for hours with no response, and she even fell in her room while an aide was present—a situation that should never have happened. The only positive aspect of her stay was the physical therapist, Carter, who was professional, dedicated, and truly helped her. Beyond his efforts, our experience was overwhelmingly negative. We are deeply unhappy with how this all unfolded and will be filing a formal complaint with the state about this nursing home. We have already spent countless hours trying to reinstate my sister’s benefits and Social Security check, which should never have been disrupted. I urge anyone considering this nursing home to stay away. It’s concerning to think how many other residents may be suffering due to similar practices. This kind of behavior must be investigated and addressed to protect others from being taken advantage of.
If you care about your license stay away from this facility..
My great great grandfather is a patient here. He is 94 years old, has bad memory and a broken ankle. He was sent here by norton’s hospital for rehab. My mother in law and cousin came to visit him today and found him on the floor, blood coming from his head and covered head to toe in his own feces. This is sick and demented. These people did not come to his room once. Nurses coming in asking to take his weight while he’s on the floor covered in feces and blood. Every single nurse and cna need to be retrained or fired! 4 people are the nurse station and not 1 came to check on him. This place should be shut down. Too many people going into the medical field for the money and not to care for people!!!!
Poor service,they don't care about people health and well being,food is horrible,staff rude and will lie on patient.need to be investigated by state.

My brother stayed at Signature HealthCARE at Summerfield for a week. It was awful. It was an older facility. One day, a pipe or something busted, and they had to move all the patients on one wing of it to someplace else because the hallway was flooded badly. The first time I came in and saw this water, I didn't see anybody working on it. They had a little thing posted to be careful and watch your step. But then the next day, it was still bad. I did see a couple of people that were trying to work on it, but it was awful. It was that way for like 2 or 3 days. The staff members were not very friendly at all. There was a lot of socialization amongst the staff, which, to me, prevented them from giving the patient proper care. One time, my brother needed help and called for a nurse, and it was close to an hour before they came. His response to my brother was that he was busy. My brother leaves his room door open, so he can kind of see what's going on. He saw a lot of socialization between the staff rather than caring for the needs of the patient. They didn't appear to have any sense of urgency about anything. They were laughing and having a good time, but not really doing what they were supposed to do. They have a lot of people pushed out in wheelchairs and just sitting in the hallways.

This place I would never send your love ones to at all period point blank . What they did to my grandma that’s died from there mistakes …. What a shame on this places how pissed and angry I am and how they don’t care how they treat or abuse the patients !!! Karma will bite them big time for what they did to my grandma what a shame !!!
My mother was brought here from the VA hospital, hesitant at first, but the three weeks she was there, she, her sister, and I all loved the staff. Everyone was welcoming, friendly, and the PT staff was excellent as well. Thank you for taking such wonderful care of my mom
My sister, Hannah Millen, is receiving very good beside patient care at the Signature Rehab on Farnsley Road, Louisville, Kentucky. She feels well taken care of. Being legally blind, sometimes she requires a little extra help, and she has received what she has needed.
They were understaffed and the aids didn’t seem to care. I would ask for my meds and I might not get them until 3 hours later. I couldn’t walk or use a bathroom and I pushed the button and told the aid I needed my diaper changed. She said she was taking care of 42 patients and I had just been changed 1/2 hour ago. I wet myself again and someone came 2 hours later. I also didn’t like the food. I was there for rehab but don’t put your loved ones there, if you can please keep them at home.
A family friend is a patient at this home and they are horrendous. They leave her unattended, don’t change her linens or diapers, and deliver food that does not match her allergen listings. They should be shut down and ashamed of themselves for running a place like this.
Very clean and fresh air no urine smell
This place is an absolute hell hole. My 78 year old grandma came here to get better. She got worse because of the care she was in. They didn’t change her bandages. They didn’t bathe the poor woman. She fell and is now in the hospital about to die. Her legs are so sore that they can’t help her anymore. So to anyone who wants to put their loved ones in this place don’t. This is where people come to die. This place is absolutely horrific. I will never forget how horrible you treated my poor grandma.
This place was so unprofessional l had to clean and sweep my mom’s room when l went to visit. I took her an expensive flower arrangement and that same night she was rushed to the hospital we went back for her things and just as l suspected it magically disappeared over night along with her other things. Disgraceful
Terrible experience when wife stayed there from a spine surgery. Nurse left her on Friday and Sunday in soiled sheets at 9:00 a.m She left due to infections. Do not want her to ever go back to horrible conditions. Some nurses did do a good job. Weekends non caring. No supervisors on weekends
My sister was there for 3 nights before being readmitted to the hospital. This facility deserves no stars whatsoever. If you think those pics on their website represent the facility think again.
My family was treated great. Staff was very responsive and addressed all needs.
My father wasn’t there not even 24 hours and had to self check out because when he arrived, his bed wasn’t working, his tv remote wasn’t working and they took forever to arrive. When he called for help they took forever to arrive. When he requested his medication they didn’t even have it in the facility.
I’m currently staying here and if I could give this place no stars I would. They haven’t bathe me but once and it was just a mere wipe down and I’ve been here since Thursday night today is Saturday night. If you need assistance to the restroom you might as well pee on yourself because they ignore your call for help. The food is homeless shelter status at best. No one seems to know anything about anything around here. I’m basically taking care of myself because they are either understaffed or just don’t care. Update: 09/06/2023 I was discharged today and since posting this review Desirae Hawkins the CEO has made a tremendous effort to rectify the issues that I was having with the facility. She has really stepped up and made sure that I was taken care of and I thank her for that. My hopes are that the patients that live there permanently get the best care and treatment possible and that their complaints don’t go unheard.
Is there zero stars? My 85 year old grandmother is there. What a horrible neglectful place. They have left feces on a rag on the floor for days. They nurse was going to lift her to use her Walker to go pee. She is there because she is a fall risk and can’t use her Walker. My mom came by she was nearly comatose hadn’t eaten her meals due to being overmedicated.,she is neglected. Can’t feed her self due to overmedicated and diabetic . She’s not there for any cognitive trouble but you would think she was . She has a UTI & they did a test days ago said no results yet. Hmm forgot . This is abuse . Complaining to the medical board. How is she doing PT when she can’t feed herself suddenly!!! Nurse didn’t even know her condition . Excuse after excuse that this is “typical” . It’s neuropathy . Her condition is neuropathy. They have given her muscle relaxers 3x a day. She can’t even feed herself now. A dog is treated better. Where is the Dr? Shame on these people!!!!
In two days my wife has waited for over an hour for assistance with the bathroom, wasn't fed breakfast or lunch, and any time we've hit the button for a nurse it has taken 30-90 minutes for a response. They put her on the wrong bed, didn't lock the bed in place before placing it on her and she almost fell. She had not gotten most of her medication at all and what they have given her has been irregular. Easily the worst medical care I've ever seen. EDIT: First, I want to say that most of the nurses have been good to interact with, but my god they must be understaffed critically. They didn't come when my wife needed help last night to use the bathroom, and she called and had her light on for multiple hours sitting in her own urine. They have forgotten to feed her literally half of her meals in the first two complete days she was there. Getting them to bring her medication, respond to an alert, or get assistance for using the bathroom results in 30-90 minute waits constantly. She was left sitting on her bedpan last night for over an hour. I cannot fathom how poorly ran and staffed this place must be.

Overall very satisfied with my visit . Looking for care for my loved one has not been easy . Signature staff blew me away . So knowledgeable ,friendly and welcoming. The star of the show is definitely Mrs.Young . Out of all facilities previously visited. Signature is the first with actual on site social worker . If I could review Mrs young she would get 5 stars herself !! Great staff and facility . Felt like home and family!

There is a social worker there who is just awesome. Shes very knowledgeable and helpful. Nursing facilities sometimes get a bad name but with employees like the social worker at summerfield I am at ease knowing that my relative is on good hands.

Summerfield with the addition of the Excelerated Unit has some of the best physical therapist! Our therapist work hard to get our seniors back into the community!
Its Great to back to such a wonderful facility! Summerfield has great things going on with some great new management additions. I can wait to see the positive changes.
