If I could give zero stars I would. Stay clear of This place the director of nursing let A patient died because she was too focused on her own ego. Arguing and Gossiping proved to Be far more important Than being bothered with a Patient Emergency. The director of nursing then proceeded to terminate the nurse who attempted to render life-saving aid to the patient.

The therapist are excellent. They are very kind and knowledgeable.

Our mom had a stroke in May when we needed a rehab for her we talked with Shannon Ford at the hospital the liaison assisting us . She contacted Tonya Hawkins the admissions coordinator helped us and explain how things are. She walked us through every step on what to do about getting mom taken care . Tonya has been working in this field for many years and we felt very comfortable with her and the facility. She knows ALOT about patient care and what’s best for all parties involved . We have had several meetings with Jessica McKinney Social Services and the nursing staff , the dietitians, and Pt staff about how mom is doing . Any time there is a change with her they call to let us know . They are really good about keeping you informed .Very pleased how my mom is cared for. The facilities are great . Would like to thank all members of the care team and staff who have come into our lives to take care of our mother .

I would not recommend this place to anyone. My father in law was moved here for rehab. After breaking his hip and 6 ribs. He had a small blood blister on his heel when he arrived. While he was there. They let that turn black and then peel off treating it only after we told them about it. They let him sit in his dirty diapers because they would not come when he pushed the button. My husband went up there and witnessed himself. He had been pushing the button so my husband helped him to the restroom. He relieved himself. My husband cleaned him up got him back to bed and about 20 minutes later 2 girls show up. He feel 5 times getting up to go to the bathroom on his own because they didn't come. Once he had to go to the ER. His bottom side stayed red and raw because they didn't make sure he was clean. He was diabetic and they didn't make sure he ate. And we had to buy him new clothes every week because his went missing. Yes Tanya is a real nice lady. Professional friendly and good at her job. But the facility really needs work.

The facility is beautiful. The nurses are nice. My biggest concern is the food. I wonder whomever fixes the food has tasted it? Seriously, prepare like your feeding your grandmother.

Broke my leg was in hospital for a week. They transfered me for rehab her by ambulance. Got there around 330 or 4 they put me in a room n told me not to move. I'm trying to change my pad with my panties pulled down to my knees n 2 ladies came wheeling in wheelchairs. I had to go to bathroom n my bed pan was across the room. I pressed the call button n it took them 1hr n 20min to finally come. Getting get any of my meds from 4pm till 3 am. Dinner was cold hot dog warm coleslaw n pork n beans. Not once did they come n check on me. The next morning my daughter came n took me home. I would never ever recommend this place just because you have a new building doesn't mean the place is good

My father had a long battle with Alzheimer’s and when he began losing his ability to walk it was clear my mother could no longer care for him at home. Having been referred to Signature Health my family had the opportunity to meet and work with Tonya Hawkins. She was very knowledgeable and personable making our decision and process of moving my father into Signature Health. Even when we were unsure of my father’s care and the whole new world of nursing home care, Tonya was there to connect us with the correct personnel to get and keep things on track. And was always there for our emotional support. Unfortunately my father was only a resident for just over a month but during his time there we met many compassionate and capable staff members. As with any business these days there at times with too few staff members; which is all the more reason families MUST make efforts to be there for their family member on a consistent basis. Thank you Tonya and Signature Health Staff.

My husband was a patient here for a little while, our very first visit we were let in by saying “it’s open” and told to go to the front desk where the young girl NEVER looked up from her phone telling us “he’s down the hall” she didn’t know if I had a gun or anything, this place isn’t fit for animals let alone people. The stench of urine is just so sickening and overwhelming that the smell just clings to your clothes and permeates your soul. ALL of my husband’s brand new clothes were stolen from here, his Skechers Go walk shoes, brand new underwear, Levi jeans, socks, pajamas, slippers, bag of socks as well as new T-shirts, when they discharged my husband on 5/23/23 they sent him with clothes that other residents had obviously left behind when they either passed on or were discharged. They discharged my husband without any shoes on his feet, when confronted, the Director Wesleyan, who needs to be directed out, was unprofessional, disrespectful, rude, nasty, and very disgusting regarding my husband’s belongings. NO residents items should be stolen, lost or misplaced, she had no concerns about his clothes being stolen, WHAT kind of director acts this way? To this day they have YET to compensate me for my husband’s stolen belongings . DO NOT allow your loved ones to be placed here, DON’T allow those you despise to be put here. Ms. Tonya is the BEST part of Signature Health otherwise this HELL HOLE isn’t worth the time it takes to blink‼️

Such a great facility. Very clean and attentive. Tonya in Admissions has been such a blessing with getting my grandfather settled in. Couldn’t ask for better help! Thank you

Friendly staff, beautiful facility! Our loved one is very happy and feels at home there for rehab. Tonya Hawkins is simply the best Admissions Director. She is genuine and there to help the family through every step. We went in stressed not knowing what to do, she took time for us and helped us in so many ways. Dealing with facilities and hospitals for the past 7 months we’ve not received the service anywhere that she gave. Such a blessing. Her compassionate caring service she provided was extraordinary. She is focused on patient care and what’s best for all. She is a difference maker going above and beyond in all she does. Shannon Ford was amazing liaison assisting us at the hospital and the transition, very focused on the patient and helpful. The staff has been amazing! Very impressed and blessed!

I wanted to give a shout out to our REBOC here in Derby City, Greg Collier. I really can't say enough good things about him. He is so supportive and helpful. He is very knowledgeable and goes above and beyond to make sure we are all successful. He is definitely one of the BEST at Signature!

Incredibly Beautiful facility with a talented and caring staff to match. Tonya Hawkins has made everything so easy getting our loved one situated and comfortable. The process is a tough decision but we know they are getting the care and support they need.

Ever since moving to the new facility the care and attention to its residents have been poor. I’ve had to help workers take care and clean my loved ones because of shortages in staff. I’ve called numerous times with no answer.

All I can give this place is a one star. I am not happy with the treatment that my father in law gets. The staff let's him sit in his dirty depends all day, he has a horrible bed sore and they only shower him twice a week which is totally unacceptable. He's a large man and needs more than two showers a week. Some of the staff should NOT be working here because of their horrible attitudes. Been to the new facility which is beautiful. Just because it's a new facility doesn't mean the the treatment will be any better!!!

Tonya , I want to thank you for everything you did . You were supportive , kind and help us to understand each step. We don’t know what we would have done without you . You are so special we will never forget you. Also want to Thank your staff , they are a special team. I didn’t meet anyone that wasn’t friendly and smiled and helpful and they were sincere . They took very good care of Ruth. Ruth knew them all by name and told us how good they were to her. I am so thankful for each one of them. They even came in today to tell her they loved her . That was so special. Please tell your staff how grateful we are . God Bless you and your staff !! Thank you so much!

I reviewed the comments left on Signature before placing my loved one By reading those reviews I called and spoke with Tonya. She answered all my questions. During the admission she put my family at ease for making a hard choice. She has been helpful answering all my questions. She is honest and makes me feel like I'm not a bother. I would recommend to everyone. This will always be our nursing home to recommend to everyone we know. Signature of Etown is the best ever.

My husband’s grandmother was a recent patient here for short term rehab. She had nothing but great things to say about the employees. They took great care of her and got her back on her feet!

Employees have been great when talking care of my elderly sister!!! She needed rehabilitation to strengthen her legs after a stay in hospital. I recommend them highly

Tonya Hawkins was very helpful to me and my family. Provided great care to my loved one. Always there to answer any questions and put my mind at ease.

A rating of 1 is even too high for this facility. I have never seen the lack of care that my grandparent has rec’d. Any concerns brought to them or things needed for BASIC care is met with attitude and an instant reply of what they cannot do for you instead. The fact I had to ask for ambulance transport back to the hospital she was discharged from that same day just to get her pain level at a bearable level is absurd. She was moaning in pain when I got there, not one nurse, and there were several standing around laughing and discussing dinner options for themselves, tried to assist us in any way. Furthermore, when I had asked she be given her meds, I was told they weren’t there yet, (she had been there for HOURS and was past due) even though they were ordered, and they had to call for pharmacy code in Louisville to try and get her something , but pharmacy wouldn’t answer phone and all they could do was shrug their shoulders. I work in healthcare, I’m aware of the stress and staffing problems we face daily, this place has no control over their patients nor do they care to. I witnessed patients outside their rooms trying to enter any patient room open, one patient yelling for help repeatedly. The EMS workers had one person who they had to ask for assistance to transfer and then again yell for someone to open the door so they leave with her. Disgusted. I fully plan on reporting this pitiful facility and their staff to the board.

Signature HealthCARE took outstanding care of my grandmother. Tonya Hawkins went above and beyond the call of duty for my family. She instantly made my Grandma feel at home, and provided outstanding support for my mother during the whole process. She treated us like family, and I cannot express how much that means to me. She always had a smile on her face, and met every challenge head on. She worked extra hard at arranging a comfortable stay for my grandma. When circumstances arised where my mamaw had to once again be admitted again Tonya jumped through hoops to get her where she needed to be. She made stressful situations easier. She is well organized, and extremely sincere and caring. She made things happen quickly and efficiently. I appreciate the staff for providing a safe and clean environment for my mamaw to heal in. Signature made her feel right at home. Thank you!

Even though we were not elated with a placement of Rehab at a nursing facility, it actually ended up being a god send . Physical Therapist, nurses aides, nurses are all awesome. Too many to name, but everyone a companionate and helpful caretaker for our mother to become independent after a hard fall at home. All are friendly, caring and very knowledgeable. Signature provided Many planned Activities which were fun and helped with memory and cognition recovery. Tonya, director , always ready to help, especially with all the paperwork, easily accessible- just make things right! No one likes to be in nursing home atmosphere, yet there are times it is needed. Our experience has been nothing but awesome. Thank you for taking such good care of our mom.

I can not thank the staff at Signature Healthcare enough for taking such good care of my Dad! My Dad was at this facility for rehab and received the best care! The staff is amazing!

I recently visited my sister in this facility. She has been well taken care of. Everyone was very pleasant and very attentive to my sister. All the nurses and caregivers alike we're just great. The facility is clean and well kept. I was overall very pleased. Thank you to everyone.

Honestly the staff members are wonderful. They took excellent care of both my elderly relatives. And if I needed anything all I had to do was call me them. Tonja is awesome and the nursing staff are always there if we needed anything. Super happy with this facility. Can’t wait to see the new one being built.
