Nursing facility is a beautiful place. People are so sweet..It’s clean and so big . But you can see that they really care about there workers and there residents.

After two rehab stays that went very well, a family member now lives at Signature. I've been very pleased with everything about the facility, starting with Tonya, the Admissions Director, who worked with me to get a room there. The facility is always clean and pleasant, and the rooms are very nice and spacious, unlike some of the other facilities in the area. I can't say enough good things about the staff, from the nurses and aides through the administrative staff. They treat my family member like a member of their own families, and they take great care of her, not only tending to her daily needs, but keeping an eye out for her health and well-being and getting her treated when necessary. Highly recommended.

I had some bad experiences in other Healthcare settings. Once Tonya called me I could tell this place would be terrific if it were compatible with how she made me feel. I read reviews on line about her and wondered how one person could make this big of a difference. She came in the room when we got there and greeted us and felt like family immediately. The conversation was started like I had known her forever. I felt good about mom coming here for rehab. This experience will always be what I think of when I think about Signature. All staff were good. Beautiful place to go but knowing you have someone like Tonya that cares about you and your family is the best feeling in the world when you're in this point of your life. I plan for my mom to go back to signature again and the idea of returning makes me feel happy rather than sad. I know she will have the best care and someone checking in on her and talking to me when I call about my mom. Life is hard but people like Tonya make things seem easy. Definitely has our 5 star rating. If you're looking like i was for my mom, choose Signature. Tonya will take care of you. Thank you to you all.

The staff are caring and compassionate. I know I made the right decision trusting them with the daily care of my mom. She is in the hands of people that treat her like family. Her needs are met and fulfilled.

Signature Healthcare of Elizabethtown is the best facility to place your loved one. They offer all therapy services. Very clean, friendly staff. 5 star service. Food is great, activities offered for every challenge or need. Everyone helpful and kind.

This is the second time that I have used this facility/service. The facility is clean and modern and very well maintained. But the admissions and marketing director, Tonya Hawkins makes the entire process Of getting your loved one settled in and welcome seamless. She provides personal one on one support to the resident and family. She maintains exceptional communication and is very much a people person, a tremendous asset to this organization and your loved one’s healthcare

This is for Tonya Hawkins, she deserves a good review she’s 1 of the most amazing people I’ve ever met, she’s always so caring, and helpful, and can definitely solve any problems u ever have. For a new facility they suck. Very understaffed. Don't expect top care for your loved one here.

I appreciate that Tonya is readily available to answer questions and provide information. She reaches across all platforms to ensure her patients and their families have the resources they need for best health. Always welcomed with a smile and a Good morning at Signature. Signature Healthcare of Elizabethtown is the cleanest nursing home I've ever visited, very beautiful. Rooms are always well cleaned and organized. Hallways never have a bad smell, and are always clean. The Nursing team members are friendly and full of compassion for their residents by treating them like family. The Rehab team works hard with the residents and provides top notch services. The Social Worker team advocate diligently to provide needed services and supports for all residents and always there to support the wellness of the residents. The business and medical records team members show comparison to family members during difficult financial planning situations and always try to provide support, guidance, and compassion to families when needed. Over all the facility and staff are incredible at Signature Healthcare of Elizabethtown. I would recommend them to anyone looking for recommendations for care.

This is a nice place to be, but there is no place like home. The cleaning and maintenance crew were great. The nurses are very caring and attentive. Some were of course more attentive than others and I think I got to tell the most of them before I left. One I called an angel and she knows who she is. Therapy is great and considerate. Everyone was very kind. I would go there again if I needed too, but hope I never need too. If anyone asked me I would say I was treated well. Great CNA's and a special thank you to Syrene who was always ready to help.

I've had two really good experiences in Signature's short-term rehab unit. The nursing staff are always friendly and helpful. I like the fresh, clean environment and appreciate the efforts by the housekeeping and maintenance staff to keep things running smoothly. The Rehab department has helped me through my recovery process and I will be able to function more independently at home. Overall, I would recommend Signature Healthcare at Elizabethtown for you or your family's healthcare needs.

The facilities, the nurses, the van driver….. 5 stars all day long. Tonya went above and beyond to help our family. Social services on the other hand, I’m not even going to give them 1 star. The stress and heartache they put my wife through while trying to get her mother top notch health care is inexcusable. Making false accusations when they had wrong information to screening calls and not answering the phone because they knew it was my wife. It is stressful enough when trying to make sure a parent is taken care of without having to deal with rude, uncaring people.

I will start by saying the nursing staff is amazing. They are all exceptional .When I went for my tour Tonya walked me through everything. She was compassionate and patient. Tonya treated us like family. She listened to my concerns and the needs my Mother needed and every one was met. The building is super clean along with patient rooms. Physical therapy is top notch. I did however have problems with social services. They are extremely rude and uncaring. They ignored my phone calls. They did put in referrals for me but it was the way I was treated. I have never been in this situation, and they sure didn’t make it easy. I would never treat anyone the way they treated me. They made me feel less than and acted like I was a burden on them. I hope this is addressed so no one else ever treated the way they treated me. Other than Social Services; everyone else was great.

Skilled PT, OT, and ST along with Restorative Nursing Programs assist with return home for short-term residents and with maintaining current levels of function for long-term residents. I have been here 3 times. I would never go anywhereelse except here. The girls are good. And Tonya is one personI come back because of. She loves what she does. Tater is good and Dani my nurse.no place better than here. I will always come back here. Hands down.

The staff there are phenomenal! I would recommend this place to anyone that sincerely cares about their loved ones. I have several relatives who have admitted to Signature. Not only is the place beautiful. There are many beautiful people inside too. People who care about what they do. My family is dedicated to signature from here. I would recommend this place to anyone. By far, the very best. Tonya walked us through every step of the way. Her door is always open, and she remains inviting. You can tell she cares about what she does and the people she serves. We have no negative words. This is not what a nursing home has always been. We love it.

Michele and the Signature staff have been a huge help with father. Despite there being complex variables requiring extra work and non- standard approaches at times, my concerns and requests are always addressed quickly.

I am a resident here and Tonya and Michele has been the best people to help and listen. They have been the biggest support systems for me. I would recommend the facility to anyone. Ashley is my good aide.

Signature Healthcare is a lovely facility with a great set of staff. While everyone makes mistakes, the staff here work diligently to correct an problems that may arise. Tanya, the Admission Director, has worked well with me to make sure my father go a place to stay. Nurses are kind, punctual, and knowledgeable.

This review is for Nurse Mallory. I had called about some questions and concerns from medications not being restarted at the hospital. She was quick to take care of that and then called me back when it was taken care of.

I had occasion to be required to spend six weeks with Signature in E'town. The experience was generally good with just a few areas that need improvement. The noise level at all hours was difficult to live with, but management met this challenge wit a room change to a quieter area. Nursing care overall was excellent with just substitute nurses contracted from an agency creating the largest challenges. Physical and Occupational therapy helped immensely in maintaining my balance and dexterity. The kitchen staff was very good as well although some selections weren't advisable for a diabetic patient.

Caring staff and Great team, residents are always doing something and enjoy their activities. Beautiful facility that offers a wide range of services and care.
