Making the decision to place a parent in an assisted living facility is a daunting and frightening task. We all have some pre conceived idea of what's going on. However, from the first phone call to the moment we walked in the front door we had a great experience. Our questions were answered thoroughly with patience and kindness. The staff and residents were warm and out going and made it a point to stop and say hello and many introduced themselves with a warm welcome to boot. The facilities are bright and open and well maintained. The grounds are more like a resort than you would expect at an assisted living facility. When we asked our family member and perspective resident what she thought, her answer pretty much is the best recommendation I can think of. She said, "I feel comfortable and safe here already".

The Forum is not only welcoming -its full of wonderful well manicured green spaces. The staff is supportive and kind and their kitchen serves top quality meals that would satisfy the most demanding diner. A variety of activities are included. What do we enjoy about life at the Forum? Well there are always the meals (both quality and variety); Saturday conversation; Friday Forums; trips; the friendly staff; the friendly residents. We tell people that when visiting various facilities to make note of the noise level during meals. Conversation levels at Forum’s dining room make it a very noisy room. What that tells us is that the Forum residents enjoy their time here. In other words residents call it home and wish to share their feelings with others.

The staff is very friendly and helpful, from the administration to the drivers, the maintenance staff, the housekeeping staff, the kitchen staff and everyone here. The Forum makes the transition to independent living so easy for the residents and their family members! Almost immediately my parents were participating in all kinds of activities and events and continue to tell me how nice everyone is. I have lived at The Forum at Brookside nearly five years and am pleased to offer this facility a rating of A+. The accommodations here are pleasant and well maintained by the experienced and conscientious staff. Eating in our dining room is a pleasure not only because of the excellent food choices but also because of the jovial atmosphere and conversations among fellow residents. We have fun and enjoy one another’s company. Our activity directors and professional staff are very friendly and provide many opportunities for exercise, entertainment and socializing. I am pleased that everyone’s unique temperament and interests are respected which allows each of us to be as sociable or private as she or he wishes. One activity I especially enjoy is taking frequent walks around our well-maintained 40 acre campus. It is a rare treat to be able to live on such a beautiful plot of land with a brook flowing through the property - all within the city.

I’ve been able to former activities going and have been very pleased with the results. The relationship between the residents and staff is warm and friendly. I am in the Personal Care area and have a great experience. The food is so good with lots of variety. But the best is the staff- from nurses, support staff, dietary and even maintenance. It is a great place. It is independent living and we have been reminded of that in situations when I expected her to have some help. The meals are good. Since it is independent living, there isn't communication with the family. I recommend it if the senior can't take care of a house but is functional on their own.

My mother has been at the Forum for over 7 years - in the Villas and now at Ambassador Suites. I cannot say enough about the high quality of food, activities and now, the healthcare. The staff at Ambassador Suites is compassionate, caring and professional. They are attentive to my mother's needs and keep her involved with many meaningful activities. I would happily recommend Ambassador Suites to any of my friends whose parents may be looking for an assisted living facility.

Great staff….beautiful campus….fun events planned with outstanding Activities group…. An old facility needing much updating:… old cabinetry with drawers that don’t work,..broken pool equipment OFTEN, …antiquated gym equipment, …..a Bistro in dire need of reorganization for utilizing wasted space & room for preparing food…. Many new facilities in competition & with all new amenities….people are going to those! A closed Health Care area….losing people because of this also……

I have been a resident here at the Forum since July snd I can honestly say I wouldn’t want to be anywhere else! The staff is so thoughtful, caring and makes living here a pleasure. It’s a fun place with many varied activities and evening entertainment. The food is very good with a wide variety of choices and special requests are honored. My fellow residents are all so nice it’s hard to believe it’s real. A real place of peace and fun!

Yesterday was my very first time visiting here in my life! This place is absolutely beautiful on the inside especially the lobby when you walk in the main doors! This whole place was super clean from top to bottom! I came here yesterday for a job interview and a tour! Alex was so professional and helpful she really is a great team member and I enjoyed having her as my guide! I was coming to fill a spot as a CNA but the position was only PRN! Hopefully when there's more availability I will consider joining the staff at the forum at brookside! My wife and I have chosen The Forum at Brookside but we haven't made plans of moving. We have toured the place and we just liked the atmosphere of it, the whole aspect of the cleanliness, we felt that it was modern, and just because of the amenities they provide. We just felt that they matched our needs and desires. We looked at their independent living. We just thought we liked the totality of all of it. They have a library, exercise with the indoor swimming pool, and also accommodations for taking you shopping and to your doctors. We liked the ease that if you were in a villa or an apartment and you couldn't get out, they would bring your meals to you. We liked the layout of the facilities on the property. It's got a good walking area and a very attractive one too. We liked the scenery around there and the privacy of it.

The Forum at Brookside was great. I liked the people better. The facility was maybe not quite as new and not quite as nice as the other one my mom stayed at, but the people were accessible. It was easy to stay in touch with them. You could always get somebody to answer the phone. They seemed concerned to share information with me that was critical. My mother, in her 90s, moved into the Forum about a year ago. Although she was reluctant to leave her home, she is now much happier at the Forum where she has daily companionship, excellent care, and no worries about caring for her house. The staff members are fabulous; they truly care about their residents. The food is very good and individual requests are honored. (Thanks Jerry!) The apartments and grounds are well cared for. Our family feels so fortunate to have the Forum.

My father is a resident in one of the apartments. My father and our family are concerned that visitors are now allowed in the residents’ apartments. We feel that it is too soon to allow non-essential visitors. The reason why is that the residents just received their second Covid vaccine shot last week. There should be a few weeks of time elapsing after the second shot to ensure the vaccine is effective on each resident. After this time elapses, then the residents’ bodies would be less susceptible if exposed to a contagious visitor. Also, we feel all non-essential visitors should have gotten both required vaccines before being allowed to visit a resident at the Forum. As an example -- What if a resident is walking down the hallway and an “outside asymptomatic visitor” coughs or sneezes near that resident? The contagious spores could possibly infect the resident. It would have been safer to wait approximately 3 weeks after the residents received their second shot before allowing non-essential visitors to enter the buildings. We know everyone is anxious to see family and friends. However, waiting a few more weeks of ensuring a safer environment would be better in our opinion. We are pleased with the transparency and other aspects of my father residing at the Forum.

My parents have been in this beautiful facility for years. After my dad passed, my mom was at the mercy of the management. Who in their right minds would force a 96 year old woman to move from her apartment? The Management's needs always outweighed her needs. It happened before, too. Shame on them.

My wife spent 6 months in the skilled nursing center. She couldn’t do anything for herself including talking or understanding when you talked to her. The staff did an exceptional job taking care of her. I spent a lot of time there and saw how difficult a job they have. Any issue I had was addressed immediately. They have many caring and dedicated people working there. Several have been there for many years. They certainly knew how to care her.

I like The Forum at Brookside very much. I will be moving there. It's very large, upscale, and nice. It looked absolutely lovely. It's on 40 acres. I'm going to be moving into a patio home, and they have a big apartment building as well. It's independent living, assisted living, and a nursing home, so you move from one stage to another. The food looked very good. They gave lunch to prospective people that was absolutely delicious and very upscale. The staff member was lovely and has been there for 12 years, the chef has been there for 11 years, and the manager just retired after 30 years. Everybody was very nice and very accommodating.

The Forum at Brookside was a very nice place. They have a lot of scheduled activities. It's built with a center which is very wide open and nice for group activities, and from that, there are areas going off of it for the apartments. They have a studio apartment which includes a full kitchen with a stove, refrigerator, microwave, and washer and dryer. It was an impressive place. We talked to one of the ladies who are there and she's completely happy with the food. The people I met were very nice. The rooms even have nice views and they have little balconies that are protected. They have windows that open into a common areas in the middle and it's one of these branches that went out, kind of like fingers. There's a grassy area between those fingers and walking areas and benches to sit down on.

My father loves The Forum at Brookside. It's perfect. His needs are met. His room is pretty and he feels very comfortable there. It's convenient. They have concerts, travelogues, classes like churches of the ancient world, a Derby Dinner group comes in and entertains them, pianists and cello players come in, there's bingo, book clubs, and pinochle. The food is delicious and he loves it. He's eating better there than he has in a while. It's a pretty facility.

The Forum at Brookside is a wonderful place for elderly care. It was difficult when my mother's physical health failed and home care was no longer a consideration. The Forum at Brookside provides wonderful meals, medical care, great rest, a social environment where the residents can get involved in activities, and a staff who truly cares for my mom. The staff at The Forum at Brookside engages my mom in banter, speaks to her kindly and truly cares for her needs. My mom requires total physical care, as well as bandaging of a wounds issue acquired before she became a resident there. They care for her impeccably and I can't express my appreciation enough nor acknowledge highly enough the great peace of mind they have provided me with the quality care they provide for my mom. Sara, an administrator there has provided unprecedented one on one kindness and consideration for me and my mom, and with all considered, I have to recommend The Forum at Brookside very highly!

My father came to the Forum independent living on January 5, 2018 and after 3 months my sister and I think it's one of the best decisions we've ever made. Dad loves it. I love it. The activity director, Mark is really good and there is tons going on all the time. High quality food. You can have your pet if you want. You do not need a car. There are weekly shuttles to grocery and other stores. They will take you to doc and other appointments. Tipping is not allowed. They have trips to the theatre, restaurants, orchestra, Louisville sports and many others. Out of town over night trips too. There are craft classes, creative writing, games, exercise, swimming, aerobics. A podiatrist comes to the Forum. They have a hair salon/barbershop you do need to pay for. There is a happy hour twice a week. You cannot wear jeans to dinner. There is no alcohol at meals. There are religious services, computer classes. They seem to have all different types of living situations. My dad has an apartment. We got him a scooter and he zips around everywhere. The ages seem to range from late 60's to over 100. The campus has plenty of green grass, lots of large mature trees, a Brook. Oh, dad has a patio too that overlooks the quad. This place is a blessing since mom's death. I highly recommend.

My parents moved into a duplex home at The Forum in 2006. As they aged they decided to scale down and we moved them into the main building where they lived there for twelve years until February 28, 2018. The Forum had decided it was "time for them to move on", meaning they were going to exercise the 30 day notice of termination clause in the lease that they signed upon moving in. Their reason for terminating the lease was that they needed more care than The Forum could offer. My mother is 92 and my father is almost 98 years old. I can't tell you how hard it was moving them from the place they had grown so comfortable in and had hoped to live out their final days. Last year on two separate occasions they had to deal with bed bug extermination. Since they never left The Forum I have to assume the bugs were acquired on site and not brought in by my parents. With all this being said my main complaint is with the Executive Director, Bill Husley and the Resident Services Director, Darlene Fehder. Neither one of them ever bothered to take the time and check in on them during the move-out nor even to bid a farewell after twelve years of living there. Two more spineless people I have never seen in my life.

The meals are delicious, the staff are amazing, the campus is gorgeous, the many programs are interesting and stimulating and our villa is beautiful. Best of all- the residents are amazing. I lived at The Forum for almost four years now, and I really liked it here. I’m very pleased, it’s priced right, the food is outstanding, and the staff is really great. It has villas, apartments, and areas for nursing home and for assisted living. I have a studio apartment, everything is really great, they have tremendous service, and really good activities. We have an indoor swimming pool, and the price is all-in, but not many people know it, because it’s way off the highway. The property is probably 30 years old, but I wouldn’t live anywhere else; it’s perfect.

I am very pleased with The Forum. They are well staffed, knowledgeable, and friendly. They listen and take care of my mother’s needs. They do what the family articulates to them. It is a very nice facility that encourages the residents to socialize as far as they can. They have a lot of activities and encourage interaction. She is very limited as to what she can participate in, but there are a lot of activities for them. She had a birthday last week, and I inquired if family members can assemble and bring a cake and so on and so forth. They set up a room, decorated it with banners and had birthday activities. They are extremely accommodating and go above and beyond the call. It is a multi-level senior living facility. They have patio homes where people can live independently and just have maid service if they desire or go to a dining room and eat. They have about five different levels of care. It is a great facility.

This is a wonderful neighborhood of friendly, caring, people, both staff and residents are very kind to my wife who has no memory from her 9 year bout with Alzheimer's. I feel more comfortable here, than I would if she was in an Alzheimer's facility.

I was impressed with The Forum at Brookside. They were probably the best facility that we have in the city. They had very nice rooms and apartments, but they were more costly than the other places. The dining room set up was very nice. They had group activities for people, and they provide transportation for the residents. The staff was very helpful.

The Forum was wonderful. I couldn't find any negative thing about it. They seemed to be excellent. It was clean, well run, and secured, and the clients that we saw seemed to be driving and happy. We ate dinner there, and they were very gracious. My brother and his wife went with me, and we thought the apartments were nice. They had a lot of activities, and that was one of the good things that we were looking for. They had an indoor pool, and they had water aerobics.
