My friend is having a lot of issues with care

Though the facility is a bit dated I can only praise the staff. My mother in law was a patient there for about 4 days. It was a rehabilitation visit. The staff, nurses, social workers and management care was as good as any care she has ever gotten anywhere. Please don’t judge this facility by its cover. Once you see inside you will be amazed at the level of care and compassion. It was a wonderful experience for a woman that needed some special care. She received it at the highest level. Thanks to everyone who gave my mother in law exactly what she needed.

Employees like Ladonna Griffy and Deborah Gamil are why Bluegrass is the best. There both professional but, so kind and empathetic with the residents. One is the business office manager the other social services.They are perfect in there jobs. I don’t know if I could handle the screamers or having my office literally bombarded with residents in wheelchairs. Demanding attention now. Kudos to these two ladies.

Im a former patient at this center for nearly 2 years.i was comfortable during my stay.i felt like a famiky mwmber. I was the one gard headed but i was still treated with respect. the staff was sweet and conciderant as well as compassionate. the place is very clean and neat Id recommend this place for care for needed patients

The staff in this building are so caring and compassionate! The activities are fun and the CEO and DON are very involved!

Heard that you could "yell for hours and no one will come help you." Very sad. Get your facility together please! Lexington needs places like this. Take the comments seriously

Avoid at all costs. Inattentive, poor communication, poor care all around. My dad went in here for rehab after a fall and was sent to the ER after 4 days in their "care & rehabilitation" facility due to severe hydration which shut down his kidneys. Isn't making sure the patient is hydrated the most basic of care?! We noticed blood in his catheter bag and told a nurse. They did nothing about it. 24 hours later he was practically catatonic and no one even called the family to alert us to his decline in condition. Glad I planned to go in that morning. Couldn't get him out of there fast enough. Even the EMS workers told us that this isn't the first time they've seen this at this facility and told us to make sure not to send him back there.

This place is an amazing place to get better and move forward in recovery and staff is amazing people im am very thankful for this place and the staff Nora was great very helpful; Lynn,Jerica,Jerri,Chasisty all the staff amazing

Mother in law is there now and this place is trash. Walked in and the smell of pee just hits you in the face. Told my MIL when she arrived she has missed evening meds and she won't get anything for her pain until 430 a.m.!! The nurses didn't even help her to the restroom made her do it on her own!! She's supposed to be in bed rest for crying out loud!!!! Do not take your loved ones here for anything.

I saw amazing highly skilled nursing, PT, SW and all care staff provide outstanding care

I would 100% recommend Bluegrass Care & Rehabilitation (BCR) for your loved ones, because of their excellent care and compassion for residents. 10 years ago, mom required intense, rehabilitation, and long-term nursing care. She became a resident at BCR. Four years later, dad required the same intense care needs. BCR welcomed him as a resident and allowed mom and dad to live together in the same room. Together again, they thrived from the excellent care received from BCR. In addition to the outstanding care Mom and Dad received BCR makes the Family members feel included, keeping us updated and answering our questions and concerns. I would, and do, recommend Bluegrass Care & Rehabilitation for any rehabilitation care or long-term nursing care for you or your family.

My relative was at Bluegrass for rehabilitation after a surgery. Our experience was very poor. CNAs are understaffed and can take near an hour to respond to call lights. Another relative provided nearly all of the personal care due to poor care. The provider had her on a cocktail of meds that led to delirium, which was only fixed upon family intervention. The building is run down, very loud, and smelly. Their only saving grace is their therapy department. All of their therapists were great.

I like it here. I was here before and they got me walking again. Thanks to Kelly!!!!. Now I live here it's a good place to be.

Bluegrass is a beautiful place. The rooms are large and roomy. They have wonderful staff here.

I like living here I like the food here, and the activities. Good parties. I would recommend to friends and family.

Bluegrass care & rehab has very knowledgable staff who care about the resident’s above all else.

I work within the senior industry and partner with many rehabs across Lexington. Bluegrass has really rebuilt their reputation. Their therapy and staff are super friendly and helpful. We have had two families of ours that are hard to please who would like to be there for LTC when the time is right. Ladonna has always been great with anything we need for discharge! Great work team!

Bluegrass does an amazing job! Every time we are there the staff are helpful and friendly. They are so supportive and kind. We will continue to use them for rehab services!

Bluegrass is by far one of the best facilities in Lexington. I service about 6 different places in Lexington and this by far the best! People actually care for the patients and the Admin Tracy Smith and office manger Deborah Gmail always make sure I have what I need to properly get the right lab results for their patients! My husband is in there. Daughter stopped in other day cock roaches ant ants all over room. Him and his bed. They do nothing as little as they can. Terrible place. Plz do not send anyone there. We have pictures if don't believe it. We didn't either. Sorry gave stars accident. No stars they don't deserve even one. This is not only neglect, we have encountered here.

I had my grandmother here for rehab after a fall. She was treated with the upmost respect from every person there. The cnas and staff were very helpful and meet her every need. I would most definitely recommend this facility to anybody who needs the extra care

My brother needed to use the bathroom at 8am, his nurse told him he had to wai till 12 noon. Not good care there at all

Bluegrass Care and Rehab provides quality of care to every resident at the facility. I have witnessed the caring, kindness and love that the staff exhibits daily. I am proud to be part of this team!

They have always taken great care of my grandmother, and now they are taking great care of my mother. They are very caring and responsive. Have never had any issues!

To share all that I've watched happened since my husband had his stroke years ago, adding complications of hearing and sight loss and finally his hip and ever present temperament. It has been remarkable to see him respond to their encouragement. Watching him not only participate is his rehabilitation but wanting to ask questions. I have not seen that in a long time. Yes, thank you so much. Life is important and improving.

Absolutely horrible experience. Arrived on a Friday night at 7 pm. Mom didn't receive any meds until the next day at 4 pm after I repeatedly asked. STOLE my mother's wheelchair that had her name on it in large 4 inch letters. Had to go in serach of linens twice to change her bed because no call bells are answered EVER! 1 nurse and 1 tech for 28 patients in absurd! I only let my mom stay 3 days and got her the hell out of there! I would not let them care for my dog much less a human that Iove. Mismanagement everywhere!!!!

This place did a wonderful job with my mom and they shown care and my mother given great words about the staff and she said nurse was great was not bad it was all good. Pray they stay open

Bluegrass Care and Rehab is amazing! The staff love and care for their residents. I am happy to be a part of this family. I Enjoy coming everyday and seeing all of residents having fun and seeing them happy.

Quality of care is excellant, staff is compassionate and dependable, residents are happy and they do so much with them, I would use this facility for my own family.

I have seen the therapy department at this facility perform miracles with patients receiving occupational and physical therapy. I am aware the building is under construction in areas at this time to improve their appearance and I am thankful they are making continuous changes towards improvement for their patients!

great facility and wonderful staff! The residents are well taken care of and cared for. It’s hard to find a facility where you can feel safe to trust with your family but this is a great choice !

The staff is very kind and treats everyone like family! The activities they do are amazing. They do activities that match all holidays and they are always smiling during and following the activities. The patients are family ..not just someone they need to watch over . Thank you to the CNA and everyone else

This facility is amazing. The staff are very attentive and friendly. They ensure that the resident has things to do with their activities such as painting, karaoke, and special events. All questions were answered and if the management staff was needed they were readily available. Would refer to anyone.

Do not let the old building discourage you from bringing your loved ones to the facility. Though I have not had family stay at the facility, I have heard great things about what bluegrass has to offer. I personally know some of the staff and by far they are some of the most caring people you will meet.

I love seeing how involved and engaged the residents are at Bluegrass. Activities are planned every day. The nursing staff is kind and attentive to the residents needs. The administration team has an open door feel, and they are available to residents and family.

At Bluegrass Care and Rehabilitation the staff work hard and they love and care about the residents.

I want to give a big shout out to the employees and staff at Bluegrass Care and Rehab. These employees work very hard and make each person feel like family. Teamwork is evident at Bluegrass and everyone is welcoming, happy and smiling. The facility provides excellent care and the therapy program is top notch.

Hands down the best staff around! The facility is well taken care of as well as the residents! They also have the best DON around!!

Please don’t let the older building keep you from not going here. The staff here are caring and are second to none. The rehab team is wonderful! The Quality Measures which are the Quality outcomes are a 5 Star rating. Keep up the great work Bluegrass Team!

Do not believe or trust any of the positive reviews, this place needs bulldozed. I am not one to leave negative reviews but I had to rescue my mother after 4 hours of admission, I could not leave her there. It's heartbreaking to think our elderly have to endure such an experience, this is no rehabilitation, this is a travesty and needs investigation. The building is falling apart and super old dusty furniture, bedding, chaotic with people running around the hallways in wheelchairs, the stench and neglect there is unbelievable. Beware of this facility, should be shut down. Where is the state! Needs intervention or closed permanently.

I spent many years trying to keep my mom at home because I heard stories of nursing facilities. After 13 years of being a ft caregiver I became very ill and needed to place my mom. I was devasted & scared BUT Bluegrass Care and Rehab helped Mom and I through the transition. They love her like family and she's happy & well cared for... They keep me informed & go above and beyond daily to ensure my momma is safe and happy. We have been blessed because the staff genuinely care about the residents.

Never answer the call bells, patient's are neglected, lousy food, dirty. Very bad experience.
