My husband was there and it was a total crapshow. He called for help while we were there several times, and either he was told, “ I’m only one person” or they took forever. He was on a bedpan and called for help and those were the results, when I wasn’t there. I had to “ babysit “ the staff and make sure they gave him his medicine on schedule. The nurse, Janet, should have gave up a long time ago. She was hateful, rude and mean no bedside manner at all! I paid for a haircut and had to chase her down and was told “ you’re not on the schedule “. I had my receipt and even spoke to her through text for a haircut on Monday, then she was like your right. I know I was. Just an example of how there is no communication there! Oh and the kicker was they noted things that were not true! We left and will never be back!
My Dad was at Boonesprings for over 4 years and recently passed away on June 8, 2025. There were a few hiccups along the way, but overall he was well taken care of and I was notified promptly of any issues.
Scheduled a tour and didn't really feel like the woman that met with us was super knowledgeable about things. The tour lasted for about 12 minutes total, and we didn't even get to see the floor where my mom would potentially be...almost felt like they didn't want us to view that floor for some reason, which didn't make us feel good. It just didn't leave a good feeling for us.

The team at Boonespring takes great care of my Grandpa! Especially thankful for Olivia, Mackenzie and Kelsey who keep an extra eye on him!
I was so impressed by Evan, a social worker, with Boonesprings. He was helpful, professional, gave excellent advice and went out of his way to help my father in his future journey into nursing care in another long term facility. We will be forever grateful to him.
I’m very familiar with all the nursing facilities in the area and Boonesprings knocks them all out of the park. They’re professional and their communication concerning care plans and goals is unmatched. The management goes above and beyond to ensure all residents are happy and taken care of, the aids are very attentive and caring about their residents as well. I would highly recommend this place for rehab and long term care.
My stepmom gets the best care. She has come a long way because they work with her and understands her needs. The staff are really nice and always there to answer any questions or concerns. Very clean place. I would suggest them to anyone.
Roomy and bright, large screen color tv, internet in room/wifi, rooms not curtains separate guests, share a walled bathroom, and share a very large walk in closet for both clothes and personal items Excellent medical care, highly trained nurses, therapists, therapy is great, 3 to 5 days a week, therapy rooms large and bright. Staff are respectful and caring , call me once a week for update. Call me for any changes good or bad. High rating from Medicare. They accept Medicaid and Medicare plus insurance. Good relationships with hospital, which is 14 minutes away. Very kind like being at home with friends
I would definitely recommend Boonespring! The therapist, nurses and the aides are amazing and truly care about my father’s recovery! I can’t praise the staff enough, every request has been fulfilled!
My grandparents received the best care. Couldn't be more thankful for the Director of Nursing, Kelsey Rogers. She went above and beyond making sure our family was taken care of. She was attentive at all times and very informative. She addressed any concerns we had immediately and got in and helped the rest of the nurses firsthand.
My family’s experience has been so positive with this facility. Hailey is so knowledgeable and helpful, and she sure knows how to help make things better during stressful times. Thank you so much.
I would recommend this facility over others in the area. Alyssa in therapy is great. Mackenzie the Nurse manager upstairs is very attentive and helpful, she follows up quickly and makes sure the patients are heard. All of my concerns were addressed very quickly by Hailey in social services and taken care of right when i mentioned them. I’d trust them with any of my family members!
Nursing care and the therapy department is excellent. Any concerns regarding care were addressed immediately by Kelsey. Social services was able to give me more than enough information needed. highly recommended this facility.
My grandma was supposed to be there for 20 days for rehabilitation. They were not consistent with doing her rehab. They also kept giving her the choice on doing the rehab or not (she is 92 of course she doesn't want to do it). They were supposed to help her eat but never did my aunt had to. When she soiled herself she would have to sit/lay in it for hours. Which she ended up having a big sore in her butt and she became so swollen she is unable to urinate and a urinary catheter has to be placed to empty her bladder. She ended up getting aspiration pneumonia while there and was admitted to the ICU. Within 2 days of being in the ICU they already put someone else in her room. I know one of the male nurses were always nice to her and attentive to her. I don't know if there are too many residents assigned to an employee or if they have low care for the residents there. At

I don't see much good out of this place, so far, have been lied too, communication lacks..the staff steals personal belongings, the food they are given is horrendous,people go hungry, especially if they cant feed themselves, I feel badly for the residence here,they deserve better!!

There is always room for improvement! Although I like Boonespring of Union and the care they provide my mother is okay, they are short staffed and it is very evident! The caregivers are pleasant most of the time, but often hard to reach by phone because they are too busy (I live out-of-state). It’s difficult to get anything accomplished with the administrative staff because they don’t answer their phones or emails, and are never available. And, forget it if it’s a holiday, it’s a skeleton staff! This facility could be amazing if they just had more staff that was trained properly to actually provide all the levels care that Boonespring of Union advertises and markets that they excel at! Do better Boonespring of Union!
I wish to personally thank the entire staff, especially Kelsey Rogers, for the exceptional care that was given to my mother. Kelsey immediately addressed any concerns I had and always followed through! I sincerely appreciate her kindness. In my mom’s final hours, she called me personally and offered to stop by and check on us. Words cannot express how much that meant! The staff deals with so much on a daily basis but somehow she always listened and we were never treated as a burden. Thank you Kelsey!

My father was here for 6 weeks on hospice care. The nursing staff was always very kind. They truly cared for my father and had his best interest. His room was always cleaned well. We are happy they we decided to go with Boone Springs.
Both of my grandparents wound up at Boonespring before passing. While always a difficult moment in one's life, the staff were incredibly kind and supportive. My family received excellent care. The nursing and clinical staff were excellent, my grandparents were taken care of well, and the building is clean and inviting. It does not feel antiseptic or stale. While their stay was short lived, and their final few months felt immensely short, I could not have asked for a better facility for them. I cannot thank everyone involved enough. From the kitchen staff to the nurses to the friendly faces who stopped in to check in on them, thank you.
Avoid sending loved ones to this facility. They are dishonest and uncaring. Repeatedly the staff has been made promises and not followed through. Not unreasonable promises but things like providing updates on inpatients, returning calls from patient room so loved ones can speak to the resident. The last straw was tonight, after being transferred twice and hung up on twice I heard a staff member in the background say, “just hang up, they will stop calling back eventually”
My mom is a late stage dementia patient who had been living in Memory Care and was referred to rehab after a mild stroke. I knew full well that she would likely need to transition from rehab to long-term skilled nursing. I discussed this at length with Boone Springs and they gave me a tour through the long-term unit and we discussed payment, etc. Mom was only approved for a certain amount of days by insurance for rehab and at the end, we would continue to self-pay as we did in her memory care. The last 2 or 3 days of her stay, it was decided that she would indeed, require long-term care. Boone Springs failed to inform me that they had no openings for long-term care. WHAT!?!? This should have been disclosed from the start. Transferring a dementia patient anywhere is a very difficult process and usually a huge setback for them and now I would need to find a new place to send her. This lack of disclosure is unforgivable and I blame the Admissions Representative for completely misleading me. The actual staff caring for my mother were only ok. There were certainly a couple of outstanding nurses, who I greatly appreciated but some of those aides need to get outta there right now! I found poop embedded in her finger nails twice, found her lying naked in her bed with no diaper or blanket and every time I visited, she was dying of thirst. Residents are usually sitting out in the hall or crammed into a small TV room in their wheelchairs, smashed together in rows like a parking lot. Mom slid and fell out of her chair twice in that room. It is only attended by one worker sitting at a small desk, usually only paying attention to her phone. I am sorry and so sad that I subjected my mother to this place. Compared to our experience with her Memory Care facility, this was disastrous and I’m so glad I got her out of there!!
My husband was in the Boonespring for several months. I was there daily from mid-morning until after dinner so I'm aware of the high degree of caring, cheerful, dedicated people who attended him each and every day. The food staff made every effort to accommodate his picky eating preferences and the meals were generous in quantity and tasted good! Boonespring made us feel as if we were family and we will be forever grateful to each and every person there.
Nice landscaping outside. The lobby is nice and spacious. The employee at the front desk was very friendly & helpful. There is a beauty salon for residents too. Has many different areas for physical therapy & rehab and they all seem well equipped & clean.

If I could, I would leave a negative number!!!! My family and I had the WORST experience here at Boonesprings! My grandmother was sent here for rehab after surgery. Day one I come into visit her she is in a wheel chair slumped over her bed with no depend on or a chair alone to let staff know if she gets up. Mind you my grandmother just had hip surgery and is not very mobile and needs assistance walking. We pushed the call button to get help to move her to her bed and waited 30 mins before anyone showed up. We ended up moving her into bed ourselves. The staff was super rude, uncaring and inattentive. When asked why she didn't have a depend on the nurse aide said she does have one one. I said no she does not I checked. The nurse aide again questioned myself and my grandmother about having one on. When you ask any of the staff a question they claim to not know and will say oh that wasn't on my shift so I don't know. When my grandmother fell they did not call any of the family members to inform them of the fall nor let them know she was being transported to the ER for medical attention due to the fall where she sustained injuries of a fractured 5th finger. When I called and asked to speak to nurse director Kelsey or the Administrator Amanda about why nobody called us to let us know she fell and was going to the ER, I was told that they were both unavailable to talk at this time and would call me back. I never received a phone call back. The lack of compassion here at Boonesprings is just down right awful! I wouldn't recommend anybody sending their loved ones here! The fact a family member highly recommended them baffles me!
This place leaves a lot to be desired. Most of the staff is uncaring and inattentive. When the patient turns on their light for help, it can take up to an hour for someone to come. Meds are not passed out timely - some nights they aren’t given until after 11. Depends aren’t changed often enough, and patient isn’t always cleaned up when they are changed. I hesitated writing this review because my family member will probably be “punished” because of it.
I’ve had quite a few problems with this place. My Grandmother has been here for over two years now and it’s just went downhill. I go visit somewhat often and have found major issues with the place. My grandma is immobilized and has difficulty doing things for herself. She’s fell quite a few times with no staff checking on her and resulting in hospital visits. She will also be in her chair for sometimes hours because she cannot make it to her bed. There’s also been incidents where she has used the bathroom on herself because there is no one to help to her to the restroom. When a family member arrived there the other day, she pressed the call button and it took 40 minutes and she still had to go get a staff member. Which may I add, they like to sit on their phones in the hallways and do nothing. So she found the staff and got the help she needed. But this is absolutely ridiculous. Not to mention, when we’ll go to pick her up for the day sometimes, it’ll be about 10 and she hasn’t taken her meds, nor has anyone helped her dress herself. Between the falling, her being stuck for hours in one spot, and the using the bathroom on herself, this place is a absolute joke. I would never in a million years want anyones loved one to have to go through what she has went through there. Do not send your loved ones here.

I would like to say if I could put a negative number I would. My mom stayed here for 3 months and passed away September 6th. Boonespring sent my mom to emergency when she got there the doctors and nurses said she was severely dehydrated which is unacceptable considered she was suppose to have 24 hour care. On the day the hospital admitted my mom they called me and said if I want my moms room I will need to pay $360 a day or they would be happy to pack her items. I could not believe it I told Rachel that I don’t even know what’s wrong with my mom and she is there due to there neglect with my mom. She stated “I don’t know about her care but I need to collect funds that is not my issue”. So so heartless. Also my mom used the help call button we waited an hour an half nobody came so I had to find someone. When I walk those halls the older people are begging me to help them. So sad. This system is a racket they take all these peoples savings and don’t care for the elderly. My heart hurts that I put my mom there. PLEASE BE WARNED DONT SEND YOUR FAMILY MEMBER TO BOONESSPRING!!!! My mom told me they were mean to her I wish I had known there intentions. 😭😭😭😭😭😭😭😭😭😭 Then they come up with a bill of 44k in 3 months. Someone needs to stand up for the elderly.
Let's see if I leave a bad review how long it takes to get a response. Apparently they don't respond very quickly to anything, including the call button in my mother's room. She is not supposed to get up without help. She waited over 30 minutes for help to the bathroom and noone showed up so she went on her own. I even tried to call from home to get someone to assist her and all I got was a ringing and eventually a busy signal. The shift change occured at 6pm it's now 8:51 and still have no idea what the nurse looks like. Mom was due medicines at 6pm. Not happy with this place at all.
Wouldn’t recommend anyone leaving your family member here. Staff is not at all attentive, if you need help you better be prepared to wait anywhere from 1/2 hr to 2 hrs before someone answers your call. Then if you do get someone to help and it’s “not” their job then you have to wait and see if anyone else will show up. Food is disgusting we normally brought in food. The only one Good thing I can say is that the Physical Therapist did a good job.
I only gave them one star because 0 isn’t an option! I know the day will come when my Father will need a facility on a permanent basis. But it will absolutely not be Boonespring! It has been the worst experience in every way. The rudest staff I have ever encountered. These people do not care for these elderly people. It’s absolutely heartbreaking and see the lack of compassion shown to them. Needs are not met, no activities for them, horrible food, no communication with the families or residents and expect at least a half an hour response to the nurse call button. This facility offers nothing that a family would want for there loved ones.This place is 💯 about the money and 0 percent about the residents. Please I urge you to do your research before you put your loved ones in this facility that is more of a prison.
The first time my mother in law was there for 3 months and it was fine. The second time she was there it was a terrible experience. They were not giving her a diabetic diet and her sugar levels were over 300. They stated the drs from Christ hospital did not send orders. She had been there before and had her drs. Info from St. E.
My husband was there when he left they failed to give him all of his belongings one important thing was his dentures I guess they threw them away
Absolute worst place EVER. DO NOT send anyone you love to this place. The staff is horrible & lazy, will let your loved one go without meds or food. The management will not return phone calls. The positive reviews MUST be FAKE! AGAIN steer clear of this place....
HORRIBLE! My mom was in there, we relocated her. For a patient that has dementia and can not speak for themself, just show up randomly and check to see if they are dry. Not one time have I visited and my mom was not sitting in the same soiled underwear that clearly had not been changed for awhile. When it disintegrates after you touch it, that's a clear sign. The aids are rude most of them anyway, not all of them. After my mom was removed she has no longer had any rashes or UTI associated from sitting in soil. My advice, check on your family member frequently especially if they're unable to speak for themselves, or look elsewhere.
REALLY nice rehab facility/nursing home. When my mom pushed the button for help, it was answered in a matter of minutes. Beautiful facility. They are working with my mom getting her prepared to come back home - very impressed!!!

My mom, Dorothy is a new resident here from Eastern KY. The staff, doctors, np, nurses, physical therapist and administrators have gone above and beyond to make her feel safe, cared for and that all of her special needs are being addressed. Before arriving I spoke with admissions about her journey here and they were very attentive to her situation. They have done everything that they can to make her feel safe and loved and secure. The facility is clean and beautiful. The team is caring and professional. Day or night I can rest assured that she is in capable and caring hands. Your loved one will be in the very best of care at this facility.
