5 for all, we have been pleased with Creekside. We liked it because of the helpful staff. If we need something they try & help. She likes the food they serve. There always something she can do everday if she wants to-like bingo, trivia, movies, etc.
It could definitely be a great place if they had enough staff to patient ratio there is like two staff members per 20 I think, so these residents are not being checked on when they're in their rooms. It's impossible to care for that many people with only two staff members. The doors to the rooms are extremely heavy for elderly people to open and close. My mom could not get in and out of her room. She didn't have the strength to open the door and close it. It's very heavy. The rooms are very clean. The facility is very clean. The food seemed to be good. They gave them plenty and all they wanted to eat however, not enough staff to push fluids, again you have two staff members trying to feed six or seven people that cannot feed themselves so by the time they get around to everyone, the food is cold. I witnessed this on a daily, feeding one person at a time and then getting to others and the food is extremely cold that's not acceptable for the amount of money that you pay for your family members care. One other huge problem is they have nurses aids giving medication's. I didn't even think that that was legal, but it may be in this day and time which is also unacceptable these ladies and gentlemen have no idea what they are administering to residents and again the medicine was never given on time because they have so many residents. The activity director was amazing!
All 5, except for food, I am actually not that aware of what they serve. I only saw one meal served there and it was okay. I did walk through the dining room and saw that they had normal looking meals. Nothing extravagant or extraordinary so I would say a 4, but again that's because of lack of knowledge. The staff there is so nice and kind. The cleanliness is great!
my husband has never really been out of connecticut. we couldn’t afford the assisting living there. [name removed] at creekside was a gift from heaven. I flew out from westchester and toured both properties decided on creekside.
We do not have anything to compare it to. She has only been at [facility removed]. But hearing other stories of other places they all seem to have the same issues.
Our loved one didn't have any meals, she didn't do any activities and she was only there for a little less than a week.
The place & people was very nice. Additional comment: it was just too far away from her friends. They were all older ladies who just couldn't drive that far .
We chose Creekside on Bardstown because of the friendliness of their staff. They were very welcoming and made you feel like you were at home. There are only a couple of workers who have been less than friendly.
It's small but they obviously care about the residents. I got to actually talk to a couple of them and they seemed happy there. Value for the money and no level-of-care fees.
I would recommend the facility. A lot will have to do with the individual, though. They have outings and activities. It is a small place, everything on one floor. That was appealing due to possible emergencies, ex: fires. Some of the places that we visited were very nice. But they were multi level buildings. More expensive and second level rooms. That was a no go for us.
When I first brought my mom who was suffering from Alzheimer's and dementia to Creekside our biggest concern was her safety as she would escape my house while I was at work I was unable to be with her 24/7. Noted she was as fully able to care for herself when she first went to Creekside The first couple months things seemed to be going great until They failed to give her life saving medication as prescribed by her doctor. The first time she spent a week in the hospital as a result of not receiving her meds. We trusted the mistake would be fixed only for it to happen again. She spent another week in the hospital the 2nd time and needed a rehab. She never recovered and unfortunately passed away on Oct 4th from the lack of care she received at Creekside.
Their night communication could be better - that could be an area of improvement. We do like all the activities they have for the residents there as well as the security they provide.
The staff and residents were nice. The community had a nursing home feel and smell to it. Plus it was smaller than we wanted for our loved ones.
They are a very engaged group of caregivers with a passion for caring for the elderly. Generous, kind, welcoming, service above and beyond.
For the most part everything has been very good. I appreciate the friendliness and cooperativeness of the staff.
The workers are wonderful. They take very goodcare of the residents. They make sure everyone is happy joyful. They are very organized. The staff creates events for the residents to do and it's really fun for everybody.
I found them to be unprofessional. When I came in to visit, the staff would be doing personal things at the counter - doing their hair, eating, etc. - and when I would ask for help, they would tell me they were unavailable. I was not pleased with the care. They weren't washing my loved one as promised, so I had to do it myself. I would come in and find the staff in his room when we was not there, which they should not have been. We had a very poor experience.
Our family had an in person tour and we were just disappointed in the overall quality, and the experience was total chaos. The residents did not seem very well kept, and the location is just not in the best area.
They treated my grandmother horrible. They did not believe her ever. She was in severe pain all the time. I don’t recommend them. My mamaw died after 10 months of being there and I reached out many times to try and get her better care.
Only complaints really have to say is that dad says the food is not good, he is not happy with it or the options.
Without A Place For Mom I don't believe I would've been able to handle the situation we were put into on my own as easily as I did without their help. A Place For Mom it's a valuable resource for families put in a situation whose loved one needs more care than a family member is capable of performing . Creekside on Bardstown allowed my mother to enter the facility knowing she would be under the Hosparus program . [name removed] took care of our every need and addressed my mothers personal concerns helping to reduce her anxiety . And when my mother passed on May 16th all of the employees showed amazing professionalism and much appreciated compassion . I would highly recommend Creekside on Bardstown .
Our family is having such a wonderful experience it is almost to good to be true. Our loved one due to their health is not able to participate in many of the activities but from what we can see they seem to be great. She loves the meals, she says breakfast is her favorite, lunch is a little on the lighter side when it comes to what they offer but dinner is usually really good. We would definitely recommend this community based on our families experience.
During the duration of our loved one being there it was a wonderful experience we really liked everything about Creekside on Bardstown. The love that it is a smaller community so the residents and staff are very close knit. It was exceptional and we have already recommended this community to others and we will continue to do so

rudest people i met in my life. [name removed] in HR should learn how to talk to people or just go talk to his mommy like that. it’s unacceptable for a work environment. wake up and smell the flowers sis we’re not living in your world!!
Location was in a rough area, the facility was older and the rooms were like a dorm room. The lady giving the tour was wonderful.

We had a good tour experience at Creekside. I don't feel like my loved one gave them a fair try though because he had a family member who used to live there back when it was Bashford Manor Nursing Home. Even though the ownership has changed, he just couldn't get past that.
We chose Creekside on Bardstown because of the convenient location to family and I was really impressed with the cleanliness and the director was absolutely great, she was also a nurse at one time which gave us comfort if she needed any assistance. They were great with communication, they did zoom calls and sent us letters to keep us updated on what the protocols were and what was going on. They were always really good when I called about checking in on her and letting me know how she was doing. They are really on top of everything here and really care about their residents. I would absolutely recommend them.
My family is in the process of securing a suitable living arrangement for our brother a disabled veteran. Creekside was a recommendation based on our needs. Their representative ([name removed]) was personable, knowledgable and professional. The services and the physical state of the residence was impressive. I would recommend that any family looking for a Personal Care Home for a love one inquire about Creekside ( Community is less than one year old/Jul20).
They say they are equipped for memory care but they are not. I would not recommend this community and we had to move our loved one to a different community. We were very disappointed in the creekside.
Creekside has really good staff. They address quickly any issue you bring to them. They were very proactive regarding Covid -19 in closing the community to visitors. They have been great with communication to families and send lots of pictures. They have a good layout. My step mom is in their memory care. The have lots of activities and field trips. It has been a good experience for our family. Every time I see my step mom she looks better than the time before.
Since our mom has been at Creekside, the past 18months or so, our family has had a great peace of mind The staff is great !!! They place is so clean and beautiful!!! They are so caring and professional. They take very good care of our mom. When we go to see her she always looks happy and healthy. During this time of COVID-19. It is especially great to know that she is in a place where the staff is so caring and attentive. When they did their COVID-19 tests, EVERYBODY- staff and residence-- test negative!!! They have zoom meetings every Thursday to address any concerns! They are the family away home for our mom!!! Thank you [names removed], and so many other wonderful staff at Creekside!!! I have recommend Creekside to others looking for a home away from for their mom
