My Dad had 2 stays here and both were wonderful experiences. We would highly recommended Emerald Trace. The staff was so kind and very engaging with Dad. He enjoyed getting to know them and it made his stay more personal. The therapy program was just the right balance for him, and he was able to work his way back to living independently in his home. He also had great things to say about the meals they provided. We are very appreciative of the care Dad was given!

What a great place for my aunt. She’s been there a week and has been taken great care of. She is rehabbing from a fracture, loves how clean it is, loves the staff, loves her room and its coziness. The staff are extremely nice and helpful. She and my family give Emerald a 5 star!
My dad suffered a very horrible stroke and fall. After being discharged from St. E and going through Gateway he was still a long way away from walking again. They told me he had 100 days of short term SNF rehab. I looked at every SNF short term rehab in Northern Kentucky and parts of Cincinnati and Emerald Trace had the best numbers and statistics. They did not disappoint and while dad went through 85 of hos days he still cannot walk BUT that is just because every individual is different every stroke is different based of severity but I saw him "wake up" and progress farther than I thought he would. The nurses are the best and so are the aides and other staff. Second floor is top tier and I recommend this place to anyone with SNF short term rehab needs or long term needs. Everything is nice and clean and everyone is always friendly. I am convinced this is the best place in NKY and the best staff in NKY. The social workers and even the cooks are the best also. The therapy department I am convinced is the very best also. My dad has moved on to a different long term care but only because his medicare wouldn't pay longer than 100 days. We were really attached to this place and wish he could just be there but that cannot happen since it was short term. I recommend anyone go here because they treated my dad just like family. They cared for him as I his son would.

I think Emerald Trace is wonderful. I have a multiple health issues. I chose Emerald Trace because I have been here before. They do a great job at making a skilled facility very homey. They have a big kitchen table that many residents eat at. The food is excellent and it is not all the processed crap. The staff is very good. They are very responsive to each residents needs. Nothing is perfect but they make a big effort to respond asap. But in any health care situation one can’t always expect immediate service. Emergencies happen and they take priority. The facility is newer and they are doing some upgrades. I called Lena in admissions and asked some questions about being treated with dignity, medication management. It’s very peaceful. I didn’t want to have to hunt down a nurse to get my medication. The rehab unit is set up like an apartment. You have a refrigerator in the entryway. I almost hate to post this because I don’t want them to get so busy that I can’t get back in when i need to.
I am sorry to report this but my family’s experience with Emerald Trace was not that of a top rated short term rehabilitation center. My family member went to Emerald Trace for rehab after being hospitalized for a fall. They caught COVID and the staff allowed them to have aphasia for 12-24 hours without transporting to the hospital for stroke protocol. I arrived at the facility to visit and found my relative speaking word salad. The daily staff was unconcerned. Said this had been happening since the day before and it was Covid. I thought I was watching them have a stroke. I insisted on seeing the NP that was in the long term care building and she instantly said to call 911 and asked how long she had been in this condition. Response was, since yesterday. She said this is a huge change and should 911 should have been called as soon as it was noticed. The paramedics asked how long she had been like this. The response was since yesterday. He said we should have been called as soon as this was noticed. At the ER, they went into stroke protocol and had a cat scan and an MRI. Luckily it was not a stroke. It was multifocal pneumonia from untreated Covid. We had been told there are no treatments for Covid by the dr assigned to my relative. (Note: The Dr is out of Middleton and mainly focuses on urgent cares. Google the Dr if your loved one is at this facility) we are very disappointed in the care our family member received at this facility.
Visiting a relative, I heard a lady yelling HeLp HeLp me for at least 20 mins b4 someone came. I even went and told them a lady needed assistance. I wish my relatives' experience here was better, but it was not. And the phone never worked in her room. They are not in any hurry to get to you, but whether you need changed or in pain, you'll be waiting long wait times!

I am currently in Emerald Trace for rehab. They're bathing me, taking care of me, and giving me my medicines. They're very good to me here. They're very friendly and very, very helpful. I just push the call button and then somebody will come in and help. They're very kind, very gentle, and always very pleasant. I'm very impressed with them. They serve 3 meals a day. And then if I want a snack or something, they will supply that as well. The food is good.
Best nursing home I have ever worked at staff was super friendly I would go back anyway
I have a patient staying at emerald trace. From the time she's been in there to current day it's been an awful experience and it's not even my family. The staff are neglectful and have attitudes. I've even witnessed a nurse bring my patient her medication and had the wrong dosage, luckily she's cognitive enough to know or else I'm sure with that malpractice continued her health and well being would diminish. The food is awful. The phones never work which is disheartening for anyone who is trying to speak with someone who staying there. None of the issues you bring up are corrected. And my patient even tells me of awful ways that the night shift nurses are treating her. My client has awful bed sores now thanks to neglect. I'm taking note of all accounts in hopes to prosecute this terrible establishment.
My father was a resident here and had a horrible experience. I would not recommend this facility to anyone. We would wait an hour and then have to search for someone to take my 88 year old dad to the bathroom. That is unacceptable! This went on for weeks! The food was not nutritious, it was like cafeteria style fast food. The only good thing I can speak of, is the PT department. It was excellent! I did write a letter to administration, but never even got a response. Do better!!!
My dad has been staying at Emerald Trace. The nurses go above and beyond for my father. It is a nice facility. My dad's room is very roomy. We can probably put eight guests in there at one time to visit. It is just enough for him, but enough for visitors. The nursing staff is Johnny-on-the-spot with anything that my dad needs. The whole place is very nice. I would recommend it to everyone. The dining area is communal. They want all the residents to eat together. They have an activities director. They always do things during the day. My dad plays a lot of bingo. They have a lot of puzzle games to keep their minds fresh. We have to furnish nothing.
Had to place my 87 year old mother here for a few weeks for short-term rehab following a hospital stay. While the staff is incredibly friendly, I found the overall experience to be horrible. I had 1 care call with her therapists a week after she was admitted. Beyond that, I never heard a peep from any of the medical staff regarding her progress. I tried calling on MULTIPLE occasions where the phone would ring and ring with no answer. Every time I visited, I couldn't find any of the medical staff anywhere. Occasionally I would see an aide, but that was rare. Nobody could ever give me updates on her therapy, her overall medical condition, or her mental health. I felt like my mother was shoved in her room and forgotten for weeks. While in their care, my mom suddenly started suffering from severe hallucinations. I asked the staff multiple times if she had a UTI (which can cause hallucinations, apparently) and I was told no. Now that my mom has been removed from Emerald Trace and placed in assisted living at a different facility, she hasn't had a single hallucination and her mental clarity is starting to come back. I find this to be very odd and quite disturbing. As far as meals, my mom needs to have her food cut up into bite size pieces in order to eat. I can't tell you how many times I walked into her room at mealtime and there was a large piece of meat or large vegetables sitting on her plate, which she couldn't cut up herself. Also, I made it quite clear several times regarding her dietary preferences (i.e., she doesn't like coffee or juice, she needs someone to put sweetener in her tea, etc.). It became obvious that they don't pay attention. I'm really not clear how there is a huge sign in front of their building claiming they are the best short-term rehab facility. If you want your loved one to receive the care and attention they deserve, go somewhere else.
I toured Emerald Trace and visited the memory care unit. I liked the way it was laid out and designed. They have their own bathroom, closet, and a little area to put a microwave or something. Then you walked into the main bed area. It felt more like an apartment living situation than a nursing home.
Place need Phone Service for Rehab Customers. Call Room Extension 100 time s. Phones is place where the Costumer can't reach it and getting a nurse on # 2 floor never answer phone. My wife of 62 years is in Rehap and (I am Home Bound. like to talk to Her Once a Day at lease )
Emerald Trace on Turkeyfoot was a great facility. It's pricey, but it's very nice, and the staff was professional and caring. For that kind of facility, the food was really good and everybody had a private room there. I liked that place pretty well. I had a small apartment on my own where there's a bedroom, a bath, a shower, and a small kitchenette with a small refrigerator, sink, shelves, and stuff. Overall, it's clean. If there's a problem, they fixed it.
My father was admitted for rehab after his leg was amputated. His normal, every day medications were poorly monitored so his blood levels became way too thin. Pain meds were given THREE HOURS after requested, and my father was in tears by the time it was received. He was supposed to receive PT AND OT every day, and only received OT two times during his 7 day stay.
Brought my husband here for a few weeks rehab after major back surgery. They kept him 4 days and let him leave without his surgeons release. Contract with facility took 2 hours for paperwork, got him moved in and even though he was taking major pain meds, they let him decide when he needed to go home. Now we will have to arrange for home care, which is very expensive and not covered by our insurance.
The staff are well trained and actually care for your family here. The place is clean and well kept. A variety of activities are available as well as outings. The facilities are up to date and has great security and nice places to sit outside and inside with family visiting. Private dining room for famiy for parties. You feel welcome by all when you come to visit family or friends. There is a hair salon as well as a rejuvenating theraputic whirlpool bathtub. Church services as well as entertainment is provided. Movies played in in a cost community livingroom. Crafts,games,puzzles and bird feedings. Walks outside for exercise as well as senior exercises lead that can be done in a chair or standing. Holidays usually are celebrated with refreshments , trears,gifts and music. Soft Christian music is played during to daytime. I the cafeteria/activity area.
I recently had a family member staying here. Although I could only visit twice as I live out of town, from my experience on those visits as well as the positive things my aunt had to say about her visit, I was very impressed! The facility is up to date and modern and very, very clean. I’ve been to other facilities that have had horrible smells like urine permeating the halls but I never had that issue at Emerald. My aunt couldn’t say enough about the nursing and therapy staff. I told her I would be writing a review, and she asked me to mention her profound thanks to Crystal in housekeeping, Christy in therapy and Donna, Stephanie and Alicia in nursing. Again, we can’t thank you all enough for caring for your patients like their your own.
Recently my Father was in Emerald Trace rehab. I cannot say enough good things to say. The place is 1st rate and beautiful only upstaged by the fine people that work there from housekeeping all the wY up to administrators. Lena is such a kind caring person as well as all the other people that were involved in my Dad s care. I can never thank them enough!
Emerald Trace Got it Right! They say first impressions are lasting. You know - things like a welcoming and friendly staff, professional and courteous interaction, competent skilled services and an atmosphere that promotes well-being. I look at these things through the eyes of a daughter placing my mother into the care of others for short term rehabilitation services. I am also a professional rehabilitation clinician (elsewhere) and very focused on the right balance of all services. My first impression remained for 19 days and every day I noted the following : calm (almost serene) atmosphere with genuine and caring staff at all levels with smiles on every face I came into contact with. Emerald Trace Got It Right : Therapy services were excellent, housekeeping services were excellent, food and meal preparation staff were excellent , nursing and nursing assistants were excellent and all of this made possible by a personal referral that I should contact Emerald Trace for help with my mom. I am grateful to everyone that helped to get my mom back home. I wish I could bottle up all that good stuff and sprinkle it around. Final thought: They say ordinary people can do extraordinary things : Thank You Emerald Trace for being extraordinary.
I have been in many rehab facilities and I must say this is by far the best. The staff were very attentive, caring and considerate of my needs. The therapists listen to my concerns and promptly addressed them. I really appreciated the homey atmosphere and private rooms. I wish I could name all of the amazing staff that cared for me, but instead I'll say everyone from Lena the admissions admin down to Crystal that cleaned my room, everyone in between were a privilege to meet. My mother recovered at Emerald Trace after a fall. She got the therapy she needed to go back home by herself. She liked having a private room and liked the staff.
My Mother was very well taken care of. The facility is very clean and homey. Very comfortable.
My father was there for one day and night, they would not feed him, the nurse was really mean to him and rough with him. They moved him to St E because he was having complications. They did not call us to let us know they were moving him until the NEXT morning!!!! What a joke!!! I will be filing whatever I need to file.
My visit at Emerald Trace on Turkeyfoot was spectacular. I love the atmosphere of the place, the people, the floor plan, everything. There wasn't anything that I disliked. They showed me two different studios. Since it's a brand-new facility, everything is immaculate. They take the residents on outings or to eat at restaurants, they play Bingo, and all kinds of things. We didn't have an appointment; it was just a surprise visit. When we first got there, we met one of their staff members and she was already leaving to go home, but she took time out of her day to find somebody to accommodate us. I was really impressed with that. Julie was the person who worked with us and she was phenomenal, she was courteous, highly intelligent, and knew all about dementia patients and what to expect with my father. She was accommodating with our schedule. I just can't say enough good things about her.
I have been a nurse for almost 37 years and my elderly parent was recently at Emerald Trace. A beautiful facility and good ancillary staff but at the end of the day it is all about the attending physicians covering this place.....I will say no more.
Emerald Trace is a nice place and brand new, but the food is not very good. My mom complains that it’s cold all the time. She does therapy in trying to regain her strength. The facilities are all very nice.
