Jasmin gave us a wonder tour! I almost didn't have any questions which never happens.... We put down our deposit to be placed on the waitlist for about a couple of months and we just moved mom in about 2 months ago and she is loving her new life! I am so pleased!

Jasmine was amazing! During my tour everyone was so nice! She explained so much to me about assisted living. Mom didn’t think she was ready but after the tour we put down a deposit within weeks we were moving in! Mom has been here for 8 months! I couldn’t imagine her anywhere else!

This review is written to document serious concerns regarding the care and oversight provided at this assisted living facility. Serious Safety and Care Concerns A male resident passed away while in their care after being left unresponsive for approximately 12 hours. There were no routine checks during that time. The resident had a known history of falls and required close monitoring. Family members explicitly instructed staff that the resident needed bed guard rails and monitoring due to fall risk and because he was accustomed to sleeping in a king-size bed. These safety measures were never put in place. As a result, the resident fell from his bed multiple times during his stay. Additionally, the resident requested medication or assistance to help him sleep. This request was never followed through, and staff did not go to his room to administer the requested support. The resident was ultimately found unresponsive (duration of 12 hours) and later declared brain dead. The lack of monitoring, failure to implement safety precautions, and failure to respond to the resident’s needs raise serious concerns about neglect. There were also concerns regarding staff professionalism and compassion. Family members report a lack of basic courtesy during visits and, following the resident’s death, no outreach or condolences from facility leadership. Families rely on assisted living facilities to provide consistent monitoring, safety, and humane care. Based on this experience, those expectations were not met. Prospective families should carefully evaluate this facility and ask detailed questions about staffing, monitoring protocols, and fall-prevention procedures before making a decision. -----> Response. My Dad was well taken care of in the rehab facility. He was there two years ago as well. Ashley and the PT crew do a great job with rehabilitation efforts. The nursing staff is amazing too Lisa and Fatu are exceptional as well. Highly recommend this facility. I will say the bathroom next to the nursing station needs updating as do some patients bathrooms as well.

The road leading to your facility is a nightmare. I don't know who owns it, but fix it. Someone has to. Its a joke. I will be sending you a bill for my tire if you dont fix it. Its closer to your property.

My mom has been a resident for nearly a year. She is so pleased with the care she is given. We know whenever we have a question or concern that Lindsey, her social worker, will respond immediately. The daily personal care from the nurses and aides is top notch. Mom looks forward to her PT and OT provided on a regular basis. Multiple medical emergencies have been handled quickly and thoroughly, all the while keeping Mom informed and involved in the decision-making process.

My Dad has been in Francescan for rehab. We are very pleased with the care and attention he has received. Everyone has been friendly, efficient and attentive to our Dad's needs. Shout out to his 2 nurses, Yuliet & Idiaysis, who provide excellent care, combining knowledge and compassion, while actively listening to family questions and quickly seeking solutions. They are top notch 5 star team members! CNA's promptly responded and were friendly and kind. Shout out to the therapists PT, OT & ST who assist Dad in reaching his goals. Facility is clean and well maintained. All staff from housekeeping, kitchen/meal services, dietary and social services were excellent! Well done Francescan ☺️

My husband and I were very pleased with the care he received at Franciscan. Everyone that was part of his care team including the doctors, nurses, OT,PT, Dialysis, Social Services, Aids and others were very caring, thoughtful, and knowledgeable of his needs during this difficult time. I had visited other Health Care Centers, and although they were nice, Franciscan was by far the best. Thank you to the wonderful group of people that make Franciscan Health Care Center such a special place.

Very nice rehab facility. We’ve never been in this situation before so we didn’t know what questions to ask. But toward the end of Moms stay the social worker, Lindsey, was fabulous! She was always polite and so helpful! She answered all my emails and questions. She is very good at her job! I have to shout out to Angie A also. She was so sweet and so helpful! She made Mom laugh and put her at ease when Mom was anxious. A fun personality makes a huge difference! And Cam’ron, a gentle kind person! A friendly smile goes a long way. Thank you all.

I'm a resident of Franciscan Health Care Center. The staff is extremely friendly and helpful. The price is reasonable. They have dining areas, activity areas, and several living rooms that are well appointed and extremely comfortable. I absolutely love my one-bedroom apartment. I went from 1,600 square feet to 437 square feet, so I had to downsize considerably, but it's well laid out and well appointed. It has a kitchenette, a bathroom, and a walk-in shower. There are emergency buttons in every room, which you would expect in assisted living. I have a patio door and a small patio outside, which lets in a lot of light, and I absolutely love it. Meals, cable, internet, housekeeping, and trash pick-ups are included. They do personal and linen laundry once a week. Those are also in the base price. If you need assistance with dressing or other things, that's more of an ala carte thing that adds to the price, but for just plain independent-assisted living it is extremely reasonable. They allow pets, which is a plus for me because I have a cat. The food is OK. You can choose from the menu daily. Three meals a day are included in the price, but I have a full-sized fridge and a microwave, so I can forgo that and fix something in my room. I can do it every once in a while or I can go out. I was in rehab here, and I was able to continue my PT after I moved to assisted living. They have a great physical therapy group. I can't say enough about them.

My mother just recently became a part of the assisted living community. Instantly, staff and administration embraced her individuality. Mona McCubbins has made every effort to make sure my mother is getting settled and receives what she needs. Jasmine is always helpful when I need her. Support staff celebrate what makes my mom unique and incorporates that into her daily living. They offer opportunities for our large family to congregate to facilitate as much normality for her as possible. My mother raves about the way staff go above and beyond to assist her. I was skeptical about the decision when she made it, but now am very comfortable with my mother being at Franciscan Assisted Living.

I was a vendor at their fall festival this past Saturday. What a wonderful place. I really enjoyed the atmosphere and was pleasantly surprised at the quality of life they provide for the residents. They even had a pony named Rocko, who was being led around the festival for all to pet and get their picture with. I was touched by how loving and jovial the staff were and how all of them were very involved with the residents from the kitchen staff to administration. I've vended in many places and I have to say that I would love to live in such a place when I need a little extra care. It's was heartwarming to know there are places that are wonderful for seniors. Keep up the great work!!!

* 3rd update Mon 8/12 ~1045a So far Mom is slowly getting better and the Therapy staff (both PT and OT) seem to be great with her. Pushing her but keeping her happier than I seem to be able to when I try to same. :) The care plan meeting went well. I did have to talk with the Social worker (who at least in person corrected her speech pattern, but blamed it on a soft…(Seems that is the review size limit) * 2nd update Wed 8/7 ~930p There certainly was not any therapy on the weekends. However, the actual therapy for my Mom so far during the week as been good. The food seems so so but not bad for institutional selections. Most of the staff seem great but as with most places there are a few that I question if they are in the right job. I know that this type of job is a challenging one and they are still rightly COVID in the building. But, you either like your job and do a great job.or you really should find something that suits you better. So far the one nurse I mentioned below is the main concern. And the social worker who called me today. She didn’t technically do anything wrong but I am neither a kid nor someone who is so old that I am going to have a hard time understanding you. Please don’t talk to the family members (or for that matter the residents) as if they kids (or dogs maybe)…. It was just weird. * 1st update Sat 8/3 ~6:30p Confirmed no actual therapy today. I don’t understand how that can count as one of the limited Medicare funded days. * Initial Sat 8/3 ~1:00p I expect this review to change several times over what will likely be the next couple of weeks. I am starting with a 3 after less than 24 hours. It is not lower because today is Saturday and Mom just admitted to this facility (for Rehab around 4p yesterday). But, sadly while the staff in general have seemed very nice the rating cannot yet be higher as you will see below. Mom arrived just before me. They had her settled in bed and generally cleaned up. The gentleman nurse who was on duty when I arrived had waited for me for some minor paperwork, which I appreciated as Mom struggles to understand what she is signing, and was otherwise VERY nice. Dinner came (though we arrived late, which is expected but sometimes difficult for the kitchen) but it didn’t account for any of Mom’s diet restrictions (which are critical for her overall care). Other items that night I will chalk up to her first night and they were learning her needs. Today, when I arrived about 11:30a she was up in the chair and had been helped to use the bed side facilities. She has her lunch which again doesn’t look bad but doesn’t account for her diet needs. She has not had therapy, it is the weekend but they still bill so I certainly hope that she will still get care on the weekend. But, when I arrived I heard a noise. It seems at some point they flushed the toilet in the attached restroom and it was just running and running. After messing with it a little I think it likely needs new seals so I stepped out to let them know at the counter that I would turn the water off so they would know and have a chance to say that they would prefer to handle this differently. This was my second (lack of early therapy is the first and largest as that is why we are here) issue. The staff didn’t seem to care at all. It was obvious that it was a nuisance to them that I even brought this to the lady’s attention (pretty sure she is the nurse currently on day shift, but really not sure since no one today has spoken to me). I tuned the water off about an hour ago and so far that is that…. Guess I am glad that Mom will not be using the attached restroom for a couple of days at least. I will add updates above as available. And, will adjust the stars as seems correct. I really hope that this ends up as a 5 star review. That is best for the place and for my Mom but in any case I will provide a real set of updates and an honest rating.

My mother-in-law lived in Franciscan Senior Estates in front of Franciscan Health Care Center and always wanted to move there when it was time for her needing more assistance. The staff are so polite and helpful. They get menus to pick out their food. Snacks are always available. They have activities and games so residents get out of their rooms to meet other residents other than at mealtimes. My mother-in-law loves where she’s at. When she needs more skilled nursing care she won’t have to move again which is wonderful when you are 95!

We chose Franciscan Health Care Center for my mom. It's clean, and the people are friendly and professional. The staff is very knowledgeable and they do a good job. They serve three meals a day. They have bingo, music chimes, a couple of courtyards, and a porch sitting area.

The staff knows each resident by name. Even the house keeping staff knew my Mom’s name and which apartment she resides in! Exceptional care and service!

If I could give this absolutly disgusting place 0 stars I would. Dirty, broken down furniture, Staff is non-existent. Mada would be there all day and you never saw one person come check on her. Food is absolutely disgusting and I had to bring my mother food to eat because it was so nast. I had to choose this place because my mother needed dialysis and I swear to God, it was the worst decision I've ever made in my entire life! Didn't give my mother a bath for 3 weeks! I had to go in and demand it. Bunch of young boys for nursing assistance that have absolutely no clue on how to take care of elderly. Had a covid outbreak and never even had the decency to tell my family, had family night in everything! Well, my father ended up catching covid from visiting my mother in that nursing home and died. When I called and talked to Christie, I told her my dad caught Covid there and she said Yes, I'm not getting a lie we do have a covid outbreak here. How can you do something like that and not warn anyone? Never saw one staff member with PPE gear on whatsoever. Patients have to share bathrooms and I had to have them go in and clean up some disgusting mess a patient In the room next to my mother had left on the floor and it took them a hour to come in there and clean it. Please, if you care about your family member do not put them in this disgusting place. I wish to God someone had warned me of how bad this place was before I put my mother in there. Worse decision I have ever made in my entire life!!!!!

I have never been put in there myself. My brother was and 5 other friends. I was there to visit a friend today. Who they was skipping with the food tray. They kept saying she got breakfast when she said she didn't. I stayed till her lunch tray came. They passed her room up at lunch we had to call and ask for her lunch which came cold straight from frig. I called this evening and they passed her up again. My brothers room wasn't cleaned it got a quick rag across door handles. Hope I never get bad enough to be put in there

I wouldn’t recommend this place to my worst enemy. In late March my husband fell and broke his femur up by his hip. He had surgery and Rod and screws inserted. It was recommended he go to a rehab facility for further rehab. After searching it was determined Franciscan Rehab was where he would be going. On day of transport there were no less than 3 nurses/aides helping him from wheelchair to recliner and they dropped him in the floor! I kid you not! Up to that point in hospital he was walking with walker. There wasn’t a walker in his room when they admitted him to his room. As a result, titanium rod bent and later broke at top and my husband had to undergo another surgery to have his hip replaced. I wouldn’t send anyone there!
