Hillcreek Rehabilitation and Care

30 yrs ago my dad was a patient in this facility. 2 days be4 he was due to come home I found him sitting in chair his legs blue. I had him rushed to the ER. He died fr sepsis 2 days later. He'd begged to go to bed for hours and was ignored. I'm stunned this place remains open c

My Dad has made many friends here. Nurse Kimberly has always kept me updated with his weekly care.. dedicated to there patients. It is a good place.

Do not endanger your family members. They will neglect them. The administration will only gaslight you and pretend like they have provided any solution to any of the atrocities they have committed to the elderly.

My husband needs help nobody comes hes got more wounds on I need to talk to corporate

Please do not send your people to this place. The care is extremely low, the facility is fealthy and smells like pure urine. I'm very surprised this place remains open with all the complaints and low level of care the residents receive here. Physical Therapy is the only plus to this horrific place, it's truly sad that residents are required to pay to be fed what looks like vomit, can not rely on the staff for much help. There are a few employees that do care but not many, food trays are left in the rooms where resident an not reach the tray or open their milk and rarely get help I've seen this several times. Please do not send your people here. I could simply write an entire book of the negative issues within this facility. Zero stars is a proper Rating for the care and a Four Star for therapy Dept.

Friendly staff but I do think they are in need of more staff or better due to the lengthy wait times my mom indured after hitting her call light needing assistance ex., when she couldn't breath ( as she has a trekea and on oxygen) and past out due to lack of oxygen, before doing so she hit light and banged on table scared waiting for someone to rush to her but noone did before she past out. I understand there is alot of others to get too but she needed to be on a critical watch for a while. Agian I get short staff maybe but a patient shouldn't sit on a bed pan for over a hour waiting to be cleaned up while I witnessed staff sitting on there phones not assisting. I appreciate all they have done for my mother I do but this is my review on my experience. I did get lengthy conversation and time with therapy and respiratory and really appreciated and respected there level of concern for my moms well being and appreciate there patients and help while going over and explaining her treatment at length.

This is just a pre-review. my sister is being sent here from Kindred. I've read these horrible reviews. If I see any of the horrible things mentioned here or how poorly my sister seems to be treated going forward, I will call every government service available to inspect this place. I'm hoping that this is not the case. But I see posts here less than 3 weeks old and none are glowing reviews. And responses to the reviews are not satisfactory.

The staff does a good job and are always available to help, they treat me like family. The management team is always available to me and go above and beyond for me and all the residents.

Unbelievably filthy facility that has bed bugs, lice and rat droppings in the dining and kitchen areas. Don’t bother expressing your concerns to anyone in upper management because nobody employed in that facility cares about the well-being of the residents and I say this as a former employee. Please send your loved ones to any other facility except Hillcreek.

Please please please do your research before working here or considering sending your loved ones or family members to this horrible facility. Residents receive the worst care and the facility is so dirty. You can smell urine as soon as you walk through the entrance and visible dirt and chunks of mold and who knows what else are seen throughout the facility. I didn’t even feel comfortable sitting down and decided against sending my grandmother here after the tour. Best decision I could have made.

The facility is filthy and the residents are treated awfully. There are bed bugs, germs, roaches, and mice. I’ve seen them all.

Had to send my grandmother here for rehabilitation and she left with multiple infections that she got while at Hillcreek. PLEASE DO NOT SEND YOUR LOVED ONES HERE!

This place is Horrific! My mother had to go back to hospital for a week and is now at another rehab. The smell of Urine will knock you out when you walk in and they don’t even try to hide the smell??? Run for your life! It should be called Hell Creek!

Beyond horrible care. My mom wasn't given her hospital-ordered medications her first 2 days. It often takes hours and multiple requests to get briefs changed. Not surprisingly, the place smells so strongly of pee that it gags you. Family are not allowed to stay overnight, so I would get calls at 4 am from my mom saying that she couldn't get anyone to change her dirty brief. It usually takes at least 20 minutes to get someone on the phone at night. Despite numerous complaints, I would come in every morning to a brief so wet/dirty that it spilled over and flooded the sheets. Two times her tube feed was just turned off rather than someone starting new tubing. One of those times, it was off for a minimum of hours. The food is so bad it's almost unidentifiable and often stone cold. My mother was neglected by staff even though we were visiting daily. I can only imagine the care received by those patients without family to help them. It's really sad that this place is run so poorly, because the physical and occupational therapy are excellent.

The only thing bad about this place is the food the people were kind and friendly most of them anyways especially the therapist I would recommend it highly

Hillcreek is a very caring and professional rehab. My mother is very happy in their care.

They deserve NO STARS. My mother was in this place. They never informed us when our mother was moved to the hospital. The hospital called us. They never let us informed regularly of our mothers health. When our mother passed away they still never called! The place smelled like urine. Workers just standing at the desk talking. The sheets on her bed were torn and ripped. I had to ask someone to come and change it. NOW! They kept loosing her clothes even though they had her name on them. When they moved her to another room they lost her pictures and some personal items. DON"T sent your loved ones to this place! It is the WORST!

I have sort of a mixed review on this facility. The place is not very clean, smells bad and has some very rude and unprofessional staff. But there are some good as well. I talked to my brother one day and he told me how a lady had dropped her tray in the hallway and the nurses/aides were literally arguing over who was cleaning it up and ended up neither did. Went to visit him the next day and this lady's tray and food was still all over the hall floor. The pics of the food my brother would send me looked horrible, like didn't even know what half of it was. These people are here because they need a place to help not to be made to feel like they are nobody and have no choice but to eat the unknown. Some aren't as lucky as my brother was to have family bring them other options. The last negative and I'll go to the good, my brother was sent here from the hospital and it took them 4 days to get him any of his meds. 4 days he was without blood pressure, pain, acid reflux or phosphorus meds to name a few that he takes. And each staff member he spoke to about this just blamed the next one. That was not ok in the slightest. Now for the good, staff wise Turquoise the nursing director or supervisor not sure which was wonderful in helping him make sure things got done, the social worker was so kind and there was a new pt not sure of her name but he was the first patient she did leg wraps on and she was great as well, she was new and seemed to be thrown into it but she did the best she could I think. My brother was here for 3 weeks then went home and 16 days later he passed but even with the bad I am thankful for how far some of the staff here helped him come before he went home. I would hope that some of the rude and unkind staff could take some lessons from the better ones or move on out of Healthcare because these patients do deserve better.

Everyone there is great! I really appreciate all that they have done for my brother! Just a great place!

Only been here a few days. Am impressed that the medical Director and the Aprn have met with and taken care of my medicine concerns. The staff have been nice the Physical Therapy ppl are already working me. Keep up the good compassionate work.

The care was great and the food was very good. The therapy here was first class!

My aunt is here recovering from a longer than expected Hospital stay. The nursing staff, therapy and management have been Exceptional, Professional, kind and helpful. She will successfully discharge home this week. Thank you for all the healthcare heroes that work at Hillcreek!

The staff and nurses are treating me well And are respectfully I'm comfortable and happy with my choice thank you

RUN, don’t walk, away from this place. If I could rate it lower I would. My mother has received no care here. It’s filthy, the medication isn’t administered, it’s forgotten. food is cold, late and not nutritious or well balanced. No water cups or pitcher. No rehab! Promises made but not kept. My 10 phone calls were not returned. I drove there for resolution. Their excuse? Hmmm. Sorry. I can’t say enough how hellish this is. Why is this facility even open? Hospitals should not refer patients and the health department should be called.

My father was placed in their care a month ago. He has end stages of Parkinson amid other health issues. My mother wasn't allowed to visit the whole time except once when he arrived. She was never able to get anyone to talk to her about what was going on with his care. We were under the impression that he would be become long term because he wasn't doing any of his personal grooming. And was never allowed out of bed. My mother is not able to push and pull on him. They have released him to go back home. They would never answer any of her calls. Even when they said they were going to call her they never did. It took her all day to find out that the ambulance they had wouldn't transfer him because they have steps at their house. They then told her she needed to find an ambulance to bring him home. Not happy that she was treated that way when she was just trying to find information out that was needed. Never again. It's a shame people don't do what they say they are going to do. What happened to accountability? Just saying something is very wrong. Adding info my father arrived home. He has been there a little over a month. He was never allowed out of bed. Which he now has bed sores. But he was not bathed, shaved, of any personal grooming done. His medication was given incorrectly. The reason he was there was to get rehabilitation for a fall. His knee was still swollen and since he was never allowed out of bed is less mobile than before. No one should have ever be treated badly. They gave no info to my mother. She would call and leave messages but no return call. When she did speak to someone they were clueless and said they would have someone call her back which never happened. No one deserves that treatment.

This place is absolutely horrible. My father arrived for Hospice Care at 1:30 and at 6pm his oxygen was finally hooked up. The room was filthy with urine dried on the floor and syringes in his wastebasket. The heat index was 110 and he had no air conditioning. Maintenance finally brought in a window unit for his room. My father didn't have anything to eat until late in the evening. The next morning he was moved out of his bed and caused 2 skin tears on his hands. My dad is bed ridden and should've never been removed from his bed. I called early to schedule a visit for 11am. I traveled 50 miles only to be told that all visits were cancelled due to covid cases in the building! My father left this facility within 24 of arriving. This was a horrible nightmare!

this is the worse place there is no communication what so ever an you can't a nurse to help with questions when you finally get in there to see loved one rude must be something they allow when somebody calls with a question also you are lucky if the Director of this place returns your call in a timely manner because the staff doesn't want to deal with your questions

Please do not take your loved ones here. My uncle received horrible treatment at this place after being sent here by VA. The staff very unfriendly we went to take my uncle food they took forever to respond and wouldn't bring him to the door, place and workers looked dirty like no one cares. My uncle was not cared for properly causing horrible bed sores that they did not take care of. My uncle was moved back home where he passed. Thanks for your horrible care you should be investigated for cruel treatment of the elderly. Poor service for Veterans

They hardly come in and take care of resident,call lights always ringing for long time.

We have just came through the Covid nightmare, but Hillcreek Rehab made it bearable. Our parents, both in their 90's had covid. We will forever be grateful that Hillcreek made a way for them to be together when Dad passed. Corrine, in registration, made the paperwork as easy as possible. Our angel nurse Melissa, helped ease our pain during dad's last days. The other nurses, Kristen (sp?), Jenny, Emma, Heather, and I am sure there were others, loved on them and made them as comfortable as possible. We will forever be grateful to Hillcreek Rehab for helping our family through such a diffcult time. God bless you all.

My friend brought her 95 year old husband to Nortons Brownsboro for a leg problem, they were both tested for covid for precautionary reasons. Both my friend and her husband tested positive for covid. My friend was sent home to quarantine at home and they immediately took her husband to Hillcreek Nursing Home in Breckinridge Lane. My friend who had continously called the hospital for updates on her husband but Nortons would not give her any information. She called Hillcreek and requested that she take home her husband because she can take care of him at home instead of staying in the nursing home. My friend is much younger than her husband and she is very well capable of taking care of him by herself.which she had been doing for a years. She then decided to bring him some nice and warm pajamas and a Ralph Lauren house slippers. For 6 weeks, she had trying to talk to someone with the hope that she can take him home but no one would talk to her. I suggested that she get an attorney or tell Hillcreek that if they don't release her husband, she will get an attorney, this is after 6 weeks of staying there. When she told them her plans of getting an attorney, they immediately told her that her husband can be released the next day. When she went to pick up her husband, he was very week looking and lost weight, she asked where his things were because they have not returned anything, she asked for his pajamas (3 sets) and his house slippers but they told her that they never received any pajamas that she sent him. Now, her husband is back home and she said he started walking around the house while when he came from the Nursing home, he could barely walk. I am writing this review for my friend because I am so upset how this Nursing Home treated my friend. Hillcreek should be investigated on how they treat the families and the patients staying at their facility.

My father is currently in this facility and we have major issues getting updates on care, physical therapy progress, being notified when he falls, etc. This is unacceptable. How can we reach out directly to The Executive Director?

I had my grandmother here, and she was poorly treated. She was neglected, she had a temperature of 102.9 they did nothing about it. She was breathing heavily and the staff acted like it was normal. It is also a very gross place. They barely clean and it smell horrible. And the folks there are sitting there with their heads drooped down and the staff was not friendly. it is nothing like it looks like in the pictures!

I take care of a gentleman when he is home. So I went to check on him in this facility, He was sent here after a stroke for some physical therapy and rehab. He is suppose to receive therapy daily. He is lucky if he is getting 15 mins 2twice a week. There is not hot water in his room. So no real showers are being given and he has been here weeks. It took me 5 mins of juggling the toilet handle to get it to flush. It is out dated and reeks of urine.( the whole facility except the lobby which smells of a overpowering air spray) There were nats flying around in the hall. When I ask for a clean sheet to Change his bed because no one came to check on him the whole 41/2 hours I was there to visit I was informed they did not have any clean linen at all. Not even a wash rag so I could attempt to clean his armpits. I contacted his family we are trying to get him moved now. Please do better. The people in your care deserve better.

Terrible long term facility. Had a couple of good nurses, but that was it. My mother was there, and she has passed, and I am still mad abour her care there, several years later. Keep looking, don't entrust your loved ones to that place.

Not The Best Rehab Or Nursing Home.I Was There & The A.C. Didn't Work Right,They Have Been Trying To Fix Things Around There.There Is Better Nursing Homes & Rehab Centers In Louisville Kentucky.. Sorry.. I Had A Bad Experience While There ,Even After Leaving There ,They Did Not Take Care Of What They Said They Would For Me.,After My Discharge..Not Good.

My sister is at this facility, she first was there for rehab but now is on the main floor because she is not doing well health wise. When she was in rehab I visited her and her gown was always dirty and her bed was dirty as well. When they moved her upstairs to another room I noticed that her gowns were usually clean and her bed was as well, but the room didn't look like it had been cleaned from one visit to another. I really got upset when she needed something to drink and her water container was empty so I opened it up and there was mold and some sort of slime in the bottom of the container, so you know they were not cleaning it! Ever...I went to the nurse station and the aid was told to get her a new water bottle...I personally think they are not a very clean facility, how can a patients water bottle have damn slime in it, that's more than gross it pitiful!

My mother is staying there and has been there a year now. Even though I do her laundry, try keep loosing her clothes. I buy more and they loose them. There are times that the girls do not take care of her the way they should. My mom wears a colostomy bag and she told me that one evening , it came loose and they didn't want to change her bag so they used tape to try to hold it close. The next day mom called me crying that she had made a mess in her bed. I was so mad. There's been other things too. Don't take your love ones there. I wanted to move my mom but she doesn't want to leave now. There are a few girls that she likes.

Hillcreek seems to lack communication and organization. Staff fails to return phone calls and seems apathetic when speaking to them. The Physical therapists/Speech therapists rarely answers their phone, and even if they do return your call, they never leave a voicemail so you have no idea who called you. The director of nurses is extremely rude and quick to speak, and not slow to listen. Highly doubt I will refer someone here or bring another person.

Happy with the care that Hillcreek staff have my husband for a 1 week rehab stay. The staff is kind and professional. Loved Aida, Julie, Toya and all the others.

I have never seen my uncle so healthy or as physically strong as he is, since entering this facility. Awesome staff! They feel like family, keep up the good work!

1 star would rate lower if possible. Not a place you would ever want to put a loved one (unless you hate them). Nurses always under-staffed so they use Temp nurses who don't care since they are only there 1-2 days. The housekeepers are paid very low (think fast food), also housekeepers managed by low IQ rude foreigners (think muslims)

HORRIBLE, HORRIBLE! !!! my mother was here and she fell in her room, broke her FEMUR, she had to watch the clock 2 HOURS, before they noticed her. She had bedsores 4 big BEDSORES, that we had to nurse ,MALNUTRITION DEHYDRATION after removing her from this unkept, under staffed, uncaring, facility.MY MOM LEFT THERE WEIGHING 67 POUNDS If you LOVE someone, never never bring them to Golden living! !!!!! What a complete nightmare
