I had a very positive experience with the care provided at this nursing home. The staff was consistently kind, attentive, and respectful, creating a warm and supportive environment. It was clear that they genuinely care about the residents and take the time to get to know each person. The therapy team was excellent—knowledgeable, patient, and encouraging throughout the recovery process. They worked closely with me to build strength and mobility, always making sure I felt safe and comfortable during sessions. The facility itself was clean, well-maintained, and organized. The communication between staff, residents, and families was also very reassuring and helped build trust. Overall, I’m very grateful for the care and support I received here. I would confidently recommend this nursing home to anyone looking for a place that provides both quality care and compassion.
My husband was there for eight days. It was the worst place to have to send someone you care for. The nurses staff was very uncaring and unhelpful. In those eight days, not once did he get any kind of a bath. Never had anything cold to drink, they said they didn't have pitchers to put any ice or water in. His call button most of the time was pinned to the bed where he couldn't reach it. The bed was broken when he first arrived. He ended up with a roommate. His wheelchair was put right by the other patients bed, he couldn't even see his own TV. I could go on and on. Just know if you can find another place to go, please, please do. My husband ended up passing away, and I blame 95% of his passing on the lack of care he received at this facility.
I am writing this letter with a full heart as I prepare to leave Louisville East Post Acute after three years and five months. I wanted to put into words what this place has meant to me, in hopes that it might encourage others who are considering coming here. I arrived in October of 2022 in very difficult circumstances. I had survived cancer in 2019, during which a good portion of my left thigh was removed. Although I recovered, I was left unable to stand for long without falling. I was also carrying significant weight — nearly 278 pounds — and I was deep in grief following the loss of my husband. When I first arrived, I could not get out of bed on my own and I was incontinent. I want to be honest about that because I want people to understand how far I have come. The therapy department at Louisville East is simply the best I have ever experienced, and I tried many before coming here. Diana, my physical therapist, practices what she calls "hard love." If I said I can't, she said yes you can — and that is exactly what I needed. She would not let me give up on myself. Tony, my occupational therapist, had me lifting weights and riding the bicycle, doing something every single day. It got my mind off my grief and my depression and made me feel alive again. I was moving, and I was living. The nursing staff, the nurses' aides, and my doctors all treated me with great kindness and professionalism throughout my stay. After about a year I was given a private room, which became my own little sanctuary. The food was wonderful, and the dietitian Kathy worked with me personally to arrange fruits, vegetables, and a managed diet that supported my health goals. Over the course of my time here I have lost approximately 70 pounds. My blood pressure, which had been dangerously unmanaged and may have contributed to my falls, is now well controlled through proper medication and diet. I had been using a walker before coming here and was still falling regularly — but through the care I received at Louisville East, I no longer fall. My walker remains my trusted companion, but I use it now with confidence and stability. The activities program also played a healing role that I cannot overstate. Under the leadership of activity director Angela, her assistant Jackie, and the wonderful volunteers who give their time to our community, the crafts, get-togethers, and special dinners created a sense of joy and belonging that supported my recovery in ways that go beyond medicine. I am now transitioning to independent living, something I could not have imagined when I first arrived here barely able to get out of bed. The moving trucks come today. Someone comes for me tomorrow. I leave with gratitude in my heart for every person at Louisville East who showed me kindness, gave me their knowledge and skill, and helped me find my way back to myself. My deepest thanks to the entire staff. Louisville East Post Acute is a truly healing place, and I would recommend it without hesitation to anyone in need of care. I recently spent 5 nights at this facility following ankle surgery. I was given a room on the 100 hall which overlooked the courtyard. During the tire stay nursing staff both licensed and CNA’s were very nice and attentive. Although food is almost always a concern at nursing facilities, mine was quite good , served hot and when asked for extra condiments they were always available. Therapy staff was helpful and helped me reach my short term goals. I slept well and meds arrived on time. Staff made a point of stopping to see me prior to discharge to wish me well. My spouse paid privately for the stay and it was well worth the money. If I need to get some short term rehab, again I would definitely recommend LEPA!!!!!
If you care about your loved one, I strongly advise against placing them in this facility. Our experience was deeply troubling and raised serious concerns regarding the quality of care and patient safety. My significant other was initially placed on the Pathways unit, where the staff was exceptional. However, after her transfer to the 200 Unit on Louisville East, the level of neglect and unprofessional behavior became apparent. We frequently had to advocate for her to be changed every 2–3 hours, which is the expected standard of care. Instead, she often went four or more hours without assistance. On multiple occasions, either I or another family member had to turn off her feeding tube because staff failed to do so during changes, creating a risk for aspiration. Several aides were openly rude, dismissive, or acted in an intimidating manner when we requested basic care. These same individuals repeatedly attempted to make us leave the room while providing care, which we eventually refused due to concerns about her safety and proper feeding tube management. Fortunately, staff members from other units—who were not assigned to her—took it upon themselves to check on her. They consistently reported finding her soaked, confirming our concerns regarding ongoing neglect. Additional safety issues were present. Some aides intentionally turned off the air conditioning at night despite being fully aware that she could not tolerate heat, causing her blood pressure to rise to dangerous levels. While we understood that her roommate preferred warmer temperatures, we attempted to compromise with fans. However, the room became unbearably hot. Repeated requests to switch bed locations or move rooms were denied by management, despite clear evidence of the medical risks involved. The bathroom toilet remained broken for weeks. Although we notified staff numerous times, and were repeatedly told a work order had been submitted, no repairs were ever made. Urine remained in the toilet for an extended period, eventually leaking into the adjacent room. Despite this, her wheelchair continued to be stored in the same bathroom, even with urine on the floor. These issues appeared to extend beyond her unit. During time outside, other residents openly shared their own experiences of neglect and mistreatment. We also had to supply basic care items such as chucks, which frequently went missing or were taken for other residents, despite being clearly labeled with her name and room number. I also purchased an expensive waterproof wedge to keep her elevated during showers—an important medical accommodation—which also went missing, along with several other personal items. I am incredibly grateful that we were able to remove her from this facility as quickly as we did. The level of neglect and lack of accountability we witnessed was unacceptable and deeply distressing.
Not a negative comment about the facility except they should be more cognizant of the APRN they have on staff. Vicki Raizor, APRN, was probably one of the rudest healthcare providers I have ever spoken to. Simply asking her a question about a family member’s condition put her on the defensive. There’s usually a reason people are defensive and rude and afraid to answer simple questions about patients. Perhaps she is in the wrong field.
Please use caution before considering this facility for your loved ones. Pathways itself is a 5-star experience — the care is wonderful, and every employee from the CNAs to the nurses treat patients like family. Unfortunately, the 200s unit was a completely different experience for my sibling. On this floor, care was often refused. CNAs rarely checked on my sibling unless we hunted them down to ask for help. One CNA even told me that “Q2 turns are not a thing” after refusing assistance. The bathroom in her room was out of order for three weeks, with a toilet full of urine and water dripping. Every time we asked about repairs, we were dismissed. Many times I had to call the front desk after my family member went four hours without any CNA responding. Worse, some CNAs acted aggressively toward my family. They mocked and verbally attacked my parents when we asked for help. I often had to secretly ask CNAs from other units to check on my sister because she was being purposely neglected — and they told me she was consistently left dirty. Basic care was ignored. CNAs repeatedly forgot to pause her feeding tube, and we often had to step in to stop her from choking. Her belongings, even clearly labeled with her name and room number, were stolen weekly. Such as clothes, her chucks ($50 weekly missing), soaps, lotions, other misc items. This neglect was not an isolated incident but a repeated, ongoing problem on the 200s floor. Families deserve transparency and safe care for their loved ones.
Only familiar with the short-term rehab side of the building, not with the nursing home section. The rehab team is very helpful and informative. A few of the aides were very nice and kind to us
It’s a fair looking facility and the staff we talked to were nice overall. Therapists were very helpful. My SIL had been to a few other rehab buildings in the last year and they were awful compared to here. If we had to go through rehab again with her, we’d likely come here if given the chance.
Don’t let the pictures fool you. My sister there for therapy, very little improvement in her reason for being there. Very rude staff, my sister needed to be changed and I asked her when the last time she had been changed she stated that she had not been changed sine they got her up at 630 and it was 2 PM. My sister is one for UTIs was saturated and doubled briefed. Lazy so they don’t have to changed them. Nurses always talking down the aides, very out dated facility pictures are far more better than the facility. I would not recommend this facility at all. It is nasty
As a patient here I have to say the staff leaves you hours on end with a wet or dirty diaper and patients hurt themselves trying to get to the bathroom to clean themselves up. It's awful. They don't care. Number 1 excuse is "I don't have you today" it's pitiful. I wouldn't put my mother in this place. Going forward if I ever need a place like this please don't ever send me to one. Let me go be with Jesus before that happens. Their rehab is good but I can do that part as long as I didn't have to come in contact with a CNA or nurse. Never again will I check myself into a place like this. Worse experience of my life.
Once again I am giving a review because I am not sure how someone can be RUDE to the elderly people at this place. The person at the front desk today was very rude to people older than her. Her name is Cathy. I felt sorry for the person she was rude to. I was walking out with something I need to take to my car and there was an elderly person who was waiting to go out they were pushing someone in a wheelchair and were trying to go out. Cathy was telling the person you have to sign out before you can leave and the person said my sister is going to do it. She signed us in and Cathy said you can't go out yet because you are not signed out. The lady had a look on her face that was kinda heartbreaking to see. I don't know how Cathy was so rude. I know she is not a nice person to most people anyway but I really was upset at how she was treating the other person. I was trying to get out my self and pushed the button by the door like I am used to doing the door didn't open right away but I know how to get out and she was telling me I had to wait for the door to open which was neither here or there but she was refusing to press a button at the desk to let us out. The door did open not sure because I pushed it open or she finally pushed a button to let us out. I really felt sorry for the other people she was being RUDE to. I just don't understand why she is that way. The other person just looked so sad. I don't think Cathy deserves her job here. Like I am so happy my mom is leaving this place so I don't have to put up with Cathy rudeness. I can kinda deal with her rudeness to me which she shouldn't be but when she is RUDE to people older than her I can't not get upset about it. That is not a good person to have working anywhere to be honest. I am just leaving this review to tell you all how rude she is and should have more respect for other people

*Mom stayed a week. *Very little improvement-yet positive change in her issue for being there. * I have never been told “it’s not my job” by so many staff people. Ever. Anywhere. *Vast differences in the front line care staff. We had several - off the charts great nurses and assistants-. But some left my Mother in soiled clothing in her bed after requesting help repeatedly with no response. *If you send someone you love there, be sure to be very proactive about their care.
We thought this place was a good place for our Mom to go for rehab. Some of the people were nice but not everyone was The food is trashy. My mom is one of their best patients. However one day she was laying in her bed and complained about feeling a rod where she was laying. It was hurting her back I kept saying she had to sit in her wheelchair and that not comfortable either. She was in it for a couple hours. Her husband came to visit her and he went to the nurse station and told them twice what the deal was they did nothing. So I went to the nurse station and approached them about it. Well finally the nurse manager came to see what was up she claimed she didn't know anything about it but that was a lie. So I kept going to the nurse station and then was told by a sibling to get the nurse email cause we were filing a report. She said she could not give me the email cause she didn't do anything wrong. I told her you know exactly what I am talking about. So finally she got intimidated by me which I didn't care she called the director of nursing and gave me the phone I told her what was going on and she finally gave me her email. I still had to keep going to the nurse station and they were no help. Moridiat was the nurse manager and she was aweful finally she came for a minute acting like she didn't know what the deal was. They put my mom back in the bed so she could relieve her bladder. The nurse manager asked her if it was ok cause they were trying to pump it up. My mom said no I can still feel it a little bit so the nurse manager said oh it ok now I said NO she will soon feel bad again. The nurse had the audacity to say they could put a bunch of blankets on the bed so she would not feel it. I was h***l no you have to bring a new mattress She walked out and I didn't wait long to get on her again. So like another half hour went on I had to keep finding her. Finally she came back and asked if it was ok to bring a regular mattress. I'm like yes this is what we been waiting on. I told her you better have it here in half hour or I will be on you until you bring it So finally a half hour later she brought one.my stepdad and I were talking about how she wasn't doing a good job. She was like what is the problem you saying I am not doing my job right. I told her mind your own business. She was fully aware we were filing a report. But once again I would never want to see her again if she doesn't know how to do her job. Like I said Moridiat is the worst nurse manager I have ever met. She works nights.
Tried to get my brother a water pitcher the stated they only get cups, then we was looking for a wash cloth they looked at my niece like she was asking for a million dollars they stated they don’t have access to them my niece well can I have one!! What is going on in this place!!hope he gets out of this place soon!
My husband was sent there for rehab to become stronger after a severe infection. While he was there he hurt his knee cause he was encouraged to peddle around on a wheelchair with no feet. His foot got stuck under chair and it cause him to twisted his knee. They never even looked at the knee he could not put any pressure on it. The pt people cam in to do therpy when he told them that he refused to do therpy. The had him transfered out of there back to hosptial. They called me and only said he seemed confused and agitated. Well at the hosptial had had 4 strokes. That the rehab did not notice. He shared a room but it was so damn hot in it. The roommate was about 120 pounds and my husband is over 300. The room mate bed was right next to ac that he did not like on and his bed was twice the size of his. There was one really nice nurse that only woored the weekends. The night nurses were very loud and acted like they were throwing a party. My first week there i had no problem with staff until he was injured. PLEASE DO NOT SEND A LOVE ONE HERE!!!!

Cockroaches and no care or respect for human life
Nice staff, great environment, they have an amazing unit for the brain injury patients. They have a total of 5 different types of units from long term to short term.
Worst case of Communication break down I have every seen. Called once and was put on hold, with no one answering the call, left 2 messages with no response from the facility. When I finally did reach someone they were rude and uncaring that I was calling from a doctor's office trying to get additional information for continuity of care on a patient. I wouldn't sent my dog there.
My brother is there. He is a veteran of Air Force, served over 20yrs. VA is paying for his care, which in my eyes is worse than POW camp. He's not the only one lying in a urine soaked cot.( not even a bed.)his whole wing smells so bad you can hardly breathe.
My mom was here and 1 day is all it took for her to sign herself out. We weren’t happy at all. She wasn’t being assisted to the restroom. Patients phones aren’t working, and when my mom needed them and pushed her button no one ever came to her aid. One of the nurses told my mom that she could put her in a wheelchair and sit her on the sidewalk. Be vigilant people when it come to your family members.
A close friend of mine stayed here recently for short-term physical rehab after being in the hospital for a long time from a nasty fall in her bathroom. I was worried about her being in a facility, but it was actually wonderful for her. Staff were often busy with their jobs but when we had questions for the nurses or therapy or administration, we could get answers when we needed them. If someone didn’t know an answer, they found someone who did or got us in touch. I cannot put into words how wonderful the therapy staff is! It was like a miracle. They got her up and walking so much faster than she or I expected. It was amazing!! Now she is moving better at home than she was before she fell. We got to have a meeting with the rehab unit nurse manager, social worker, and rehab supervisor to go over the plan, how to get her goals accomplished, and answered our questions. They were all very nice and professional. They ordered her some medical equipment to take home that I picked up and got her therapy at home too. Overall a great experience and surprised us. She and I would return if we needed to for more therapy!!!
The room was infested with water bugs and I had to bring my own bug traps and dirty. The staff always said they would help and be right back, but never came back to help me it would be hours before they returned. As far as amenities there none that I can speak of. The rehab people were first class. The food was okay.
Don't send anyone here if you want good care. Pretty bad when a new patient arrives to no sheets on the bed. Doesn't take 5 hours to turn a room.
Do NOT allow your family to stay at this substandard facility. It is very unsafe. One nurse per 24 patients. The staff is more interested in socializing with each other than caring for their patients. My husband was there for rehabilitation after spine surgery. He was there for 4 hours. He had significant bleeding and called for his nurse using the call button. After 20 minutes, he struggled to the hallway and called for help. No staff responded but could be heard talking, laughing, socializing. After 45 minutes someone came to his room stating “that’s a lot of blood” Eventually EMTs were called and he was taken back to the hospital. If this place is recommended by any hospital, do not allow your family member to go there. It’s dangerous.
My father went there after having major spine surgery. First; these moronic, medical misanthropes took all day to get his transfer papers prepared, as well as transferring his prescriptions. He gets there and his nurse tells him that she will check his back in just a a few minutes, then leaves for an hour and a half. It turned out that his nurse didn’t even know that he had a procedure. Later, he had to go to the bathroom but could get help, so he hobbled to the toilet on his own. After he made it to the bathroom he began peeing a large anoint of blood. He was in pain and needed help, so he pressed the emergency button, but no one came. He yelled down the hallway and still, no one came. Eventually, other patients started yelling for someone to help him. Eventually some showed up and they stuck him in an ambulance.
My momma was transferred to Louisville East Post Acute following breaking her hip and having surgery. Momma had to be transferred to a memory care facility 2-1/2 weeks ago. I received a phone call from one of their nurses from Louisville East, Paola, who called AFTER MOMMA WAS GONE for 2 WEEKS, just to check on her to see how she was doing. To me, this is a nurse that is not just helping patients for a paycheck but because she TRULY CARES about the patients - this is ABOVE AND BEYOND!!! Thank you, Paola, for TRULY CARING about my momma! The Administrator goes above and beyond to work with you as you and your family have to adjust or readjust to the various phases of life you have to go through with your loved one. A high thank you to him as well.
My dad is here for long term care, but still in short term until there is an opening. He has been in 3 different nursing homes and so far this one is by far the best. Weekend staff is a little iffy with care, but I have always been able to reach out to someone for a resolution. Joni and Jake have been great.
When my mother was first transferred to the Nursing side from Rehabilitation and I wasn’t able to see her because of Covid, I would call just about every day to get a report on how she was doing and I was always told fine or ok. It’s like some of the staff couldn’t tell me anything pertinent. Now there are some friendly staff people but I still don’t know how my mother is doing other than visiting with her. Like I was never told why she is on a Puréed diet. I do understand that it could be because of choking hazards. I don’t know when my mother lost her teeth but it happened way before I was born. My mother has always been able to eat food that most cannot without teeth. I know that my mother is used to not moving around but I was always told the more she moves, the better she is going to do. She wears clothes that don’t seem to fit her as well. If I knew her weight then I could bring her pants that may be better fitting. The receptionist is always nice and I like the Social Worker as well. Overall I don’t really have a problem with the facility it’s just that certain areas could be better. If I could have kept my mother at home I would have but I know that she needs more care than I can provide.
My daughter has been a long term resident at Louisville East and overall, we have been pleased with the care she receives. As with all nursing homes, the aides are spread thin a lot of the time and help is sometimes a bit slow in coming when help is requested - usually because they are helping other patients. The rooms in her wing were recently updated with old carpet replaced with vinyl type flooring and that was a great improvement. The food served is nutritious and patients get a good variety in meals. The staff keeps us well informed of any incidents they feel we should be advised of, such as non-injury falls, need to go to a hospital for any reason, commencing therapy, etc. We have frequent contact with the Social Director, who is always helpful and responsive. The nursing home is receptive to visitors and my daughter’s entire family and friends are appreciative of this.
My dad was in there for a couple weeks. His room was always dirty. They really don’t give your loved ones the attention they need, he made no progress while he was there. The last day he was there he asked me to help him bec he was in pain from a bedsore, i asked if he paged the nurse he said ya and no one came. I paged them and they didn’t come so i went to the desk and there they were playing on their phones. I said do u not hear the alarm, my dad needs help and the girl behind the desk said with a major attitude that they are short aids and so i said so no one here can assist my dad?!? And she rolled her eyes and asked what the problem was and i told her amd she was like o u need his nurse and i asked where she was and she pointed to the chick next to her. Thats when they came and a little later dad was holding his side in pain so i had them call an ambulance and his bp was 80/60 something. And was dropping, pain usually makes it go up. I truly believe that he would have laid there and died that day had i not been there. That is just speculation but thats what I believe. Do no send a loved one there if their recovery is of any importance to u.
This is one horrible place and if you send your parent there you would be very sorry. They Threw my mom on an ambulance late at night and was very nasty and hung up on me once I asked the nurse where are they taking her to. She showed up at Baptist east with Urine all over her and her brief was hanging on by a thin thread. They are some very rude people and the Director of Nursing do not care at all. I'm telling you if you have a family member you care about do not send them to this place. They are not fit to take care of anyone.
My mom was there after hip surgery. Everyone was friendly and willing to help. The place was clean. Everyone had great bed side manner and willing to help. I was impressed
My aunt stayed here and said the room wasn't sanitized nor cleaned.The food was disgusting and none of the staff were of help. She said when she was discharged no one assisted her out. Overall, terrible stay.
When my mother was discharged after recovering from a stroke, a walker and bedside toilet was supposed to be ordered by this facility for her to have at home, turns out it was ordered too late and wouldn't be delivered until 2 days later to her home, which took a few hours to find out. She needed this equipment to be safe at home, and the facility was supposed to look at loaner equipment for her during this time but no response back. During the discharge process, the staff and nurses were unhelpful and didn't seem to know anything. My mother also complained about waiting long periods of time when she called for help to use the bathroom - she wasn't supposed to get up on her own. She is now receiving additional rehab at a much more competent facility, and is very happy so far; she couldn't wait to get out of this place. Overall, communication and services are poor with an unhelpful staff.

You won't find a staff more helpful, thoughtful, and willing to go the extra mile than you will at Louisville East Port Acute Care. We were in a difficult situation and had bad experiences at numerous other facilities, but the team here went the extra mile in support explaining the situations, options, and coming to a resolution. Thank you to all involved - really a great team and facility!
My mother was at Louisville East Post Acute with a broken shoulder. She uses a walker and needed some extra assistance and rehab while she was recovering. The entire staff at Louisville did an excellent job from start to finish. The people in the business office and the social workers did an excellent job guiding my family and I through some processes that would have been very tough to navigate without there help and expertise. I always felt the nursing staff and aids truly cared for her and they were very attentive to her needs. The rehab my mother received was excellent and they worked very hard get her better as quickly as possible. Throughout her stay at Louisville East I always had the peace of mind that she was getting the care that she needed. My family and I are very grateful!
Place was super clean when I came here to visit my uncle who was in here temperory after recovering from a car wreck! The staff was always nice and the facility is beautiful .
I believe this facility is very un-professional. I gave this Facility 4 stars only because of a compassionate social services worker by the name of Brian. He has went over and beyond to help with the Care of my Mom. He doesn't put us off, answers questions and gives you a un-sugarcoated answer.
DO NOT ever send anybody, not even your worst enemy to this place. They dropped my family member in the floor trying to get them up for therapy. After the fall my family member complained with their side hurting and they supposedly did an x-ray but they did it of their back so they could report no injury and not document in their chart about what really happened to avoid being sued. Low, pathetic place. Meals aren't hot and there's not enough food fill a person up. Only drinks they offer is milk, water or some horrible fruit juice or off brand soft drink occasionally. Call lights are usually not answered or they come in and turn them off, ask what you need and never return. Whatever your situation is please pass the place up.
Some of the aides were good. Some good nurses too. Food was inferior at best, mostly served cold. Being short staffed so times the call light was answered half hour to 1 hr later. Unpleasant experience. The rehab experience was great friendly and caring staff.
We sent my grandfather here for PT after his hip replacement. Awful choice on our hands. He is blind, diabetic, and is not to be left unsupervised; they let him: fall out of bed, lay in his own filth, left him in a wheelchair unsupervised and let him roll all over the place . The toilet in the room was always clogged up and over flowed constantly. The food was horrible, looking like someone had chewed it up and spit it back up to be served. the people they placed in the room with my grandfather were terrible; one brought ants into this room with all the food he ate ,the other was unhygienic. He wouldn't bathe ,wouldn't wear clothes, let the bathroom and his bed a mess, would turn off the air conditioner , and would flash his private parts . One of the workers had a nasty attitude and would not call for an ambulance when he fainted. After bringing him home, two weeks later ,he is back in the hospital for having stools backed up .

Louisville East Post Acute is a beautiful short-term rehab and long-term care facility. The facility was recently remodeled with a very modern look and feel. The Administrative staff have been very responsive to my needs. I love that there are several very nice sitting areas to visit privately with loved ones. The grand lobby at the entrance is so welcoming and I love the grand piano and sunlight day room. Louisville East is adept at caring for many kinds of patients including basic orthopedic patients but also complex dementia and brain injury patients. I saw patients in their dedicated memory unit having a riot in their specialized activity program. The brain injury patients have their own rehab gym space which is very nice. My feeling is that Louisville East Post Acute is a phenomenal comprehensive rehab and long-term care facility with dedicated staff members committed to serving our Kentucky communities.
I am so happy my grandmother got the excellent care she deserves at this facility. She'd had difficulty eating and they succeeded significantly to help her get back on her feet to a faster recovery.
