All staff made my stay for rehab pleasant. The employees are all kind, caring, and efficient that I dealt with while there. They are very professional. The facility runs like a well run machine. It is the best ran facility that I have ever seen. As a registered nurse; that speaks volumes.
Let me start with the good ..the aids and nurses are amazing. I have been in the hospital on and off 18 times in the last 2 years. I have never met people like I found here !!! The love and the way they care for there patients. I had one nurse who's smile just bright your days she would do anything she could to make your day alitte better. Another one has so much love for people more then I ever thought could be in one person. She loves to hug and to be truthful. I truly loved getting them. Every aid was amazing. There was one nurse I waited for all day to talk to(she worked the night sift) just to tell her about my day to see the caring in her face. TRULY THANK YOU ALL FOR TAKING CARE OF ME......room103 Now the bad ..and it was horrible on your meals you received a ticket telling you would be eating. NEVER EVER did the ticket match the food on your plate . In fact they feed you so little i lose 9 pounds in one week. It is so bad the nurses and ais would spend there own money to buy the patience food because they were so hungry. Sad i know. Many day I went with out all my meds (heart meds,blood pressure, sleeping meds,and more) at one time I went without my meds for 4 days. House cleaning: mostly only clean once a week that also means my trash didn't get taken out for 4 or 5 days at a time. I talked to Shawna (social worker )about it but nothing changed The truth is management.knows what's going on but doesn't do anything about it.... think about why give a ticket with your meal but that's not what they feed you (ticket proves to government what we are supposed be getting) we would get so much less. I personally believe that Joanna the head of the whole nursing home.. see to blame. I want to say thank you again to the aids nurses You guys really are amazing
Unorganized, unprofessional and A I don’t care attitude. The administrator (lady don’t know her name) was rude and had the I don’t care attitude by the way she talked to me and her body language. When ask for help for my loved one, no one helped they all looked as if they had no clue on what to do or if we was just going to have to wait until after they were done looking at their phones or smoking. The admin had us wait 20 mins before coming to speak with us just to tell us what we already knew but she told us with a nonchalant iDC attitude not wanting to help. Stay away from this place if you can. I work in the nursing field and the treatment that we experienced at this facility was very poor.
I don’t get being open 24hr but no one is answering the phone or is it just me idk but yeah sucks not being able to talk to you're elders!!..
There is a reason for the 1Star rating from CMS. Grossly mis-managed facility.
My mother has bed soars on every place of pressure and bone that touches her skin. She went to the hospital because of not eating and found out she has stage 4 cancer and is on her deathbed going to hospice. This is the worse place I would never put anyone in there not even a pet . God will get each an everyone of them that has abused an neglected the patients. My mom has always been a good size person and she weighs about 70 lbs and is loosing her hair due to being under nutritions . God please protect those residents and their families from this. God bless everyone

Wonderful building, always welcoming. Staff and residents are wonderful.
My grandmother is a current resident. Since the day she arrived to now - there has been problems with the facility. Bug infestation. Poor care. Low staff. Took a week (of constant calling) to get ahold of administrator. My grandmother now has pneumonia and is in hospital for falling and hitting her face/head, and after 1-2 weeks of coughing and issues that they have not addressed. We were told that putting her in a care center would help - that she would be supervised more closely than being at home. That was not the case. She was put into a room left to herself, served 3 meals a day and her medication. took 2 hours to change her bedsheets after she spilled her food on herself and bed.
The staff truly care about the residents and want to provide care in a way that makes them feel loved and respected. The residents are involved in meaningful activities and are happy.
Staff are very friendly and welcoming. The facility is clean and smells great! The residents appeared to be happy and were enjoying activities during my visit.
Very pleased with the care my husband is receiving. Would recommend to family and friends.
As good I guess as any other place such as it is,none of them have enough help! After covid no one wants to work.
I am a resident at Nicholasville nursing and rehab, most of the staff are great. How ever they are so short staffed that the quality of care. Is not what it should be. I was in the hospital for a week I'm before I left my linens needed to be changed I returned to the same dirty bed as when I left.i have asked to see the new administrator every day that I have been back from the hospital. I still haven't seen him. I would not recommend this place to anyone.
cna Chloe… This child is outstanding! She is one of the nicest on hand cna that I’ve ever seen!! She is so caring and considerate..yall need 100 Chloes!! please do all that you can do to keep this girl.. she deserves a raise and a good one!!$ She has taken excellent care of my 89 ye old mother!! My dad was loved and taken care of so very well at this “home” He was always dressed, up & walking the halls. And the workers were always so kind to him and honestly cared for his well being. I can’t say enough good things about the place.
Everyone is SO nice and helpful. They treated us like family from the very first point of contact on. The facility is clean and well kept, and you can tell that the staff truly cares about the residents and employees.
Wonderful facility. Kind staff, there’s always activities for the residents to do. Everyone always has a smile on their face.

This place is s joke thay let employees do what ever they want. Leave the building for 3 hours. And let ppl come and go as that please. I would not put my dog is this place .
Nicholasville Nursing and Rehab Center gets 5 stars from me because of how they treat their employees and residents! The facility is clean with home like décor. The residents often compliment the kitchen staff and anticipate their favorite meals and deserts. Especially Dion's meatloaf. It has been a great place to work. Care Team Members here are A-1 staff from our volunteers to our management team and everyone in between! Everyone is friendly and courteous towards our residents and do everything they can to go above and beyond to meet our residents needs.
Great facility with homelike atmosphere
My friend there called, he had to sit in his defecations for three hours after calling for help before someone cleaned him up. When I spoke to a worker there and accused them of bringing in more patients than they could care for she simply agreed.
I believe we have a very dedicated and caring staff. We are small which gives us a more home like feel. Our therapy department makes miracles happen.
Great Facility and Staff Is super caring! They keep the Residents happy with Activities for everyone! Great rehab facility as well! Everyone truly cares about the residents and their needs! Highly recommend
This is an awesome facility. Staff is professional and friendly. My grandfather was there for 2 months the therapy department worked with him and he progressed well and discharged home. Definitely would recommend this facility.
A great facility that cares deeply for all of their residents and employees. Lots of activities and involvement for not only residents, but care team members as well. Also, the therapy staff and gym are absolutely amazing! They truly work miracles with the residents! All around a great facility!
This center has beautiful private rehab rooms, a spacious and modern therapy gym and clinical leadership I'm confident in. They care about their residents and families.
The state and federal government agencies need to review this place. Since the pandemic when residents can’t have visitors, the staff are neglecting and are not taking care of the residents or addressing any questions the family has about the residents care. I would not even give them one star, they deserve a zero.
Really appreciate the many caring people at Nicholasville. What I really like about their facility is that it is clean, comfortable, and just the right size where everyone gets the attention they need. (And I am not part of the staff.)
If you are unfortunate enough to have to put a family member into this facility....take a safe that can be nailed to the floor and has a combination, key or some other type of locking device. My mother has been in several hospitals, physical rehab facilities and nursing homes in Nashville, Lexington and nicholasville during the last few years. Every other day something came up missing at Divericare in Nicholasville, Ky including everything from flower vases, clothes, cigarettes and MY cell phone which I lent her so we could communicate while she was there. When she was sent to an assisted living home I spoke with Tom Davis, the administrator and was told that I would be reimbursed for my phone within the following week. But when the following week came around Mr. Davis told me that the cost of my phone, some card replacement including the 400+ minutes on the phone and the cost of the vase would be taken off of a balance she owed from her horrible stay there. My mother is 78 and has Medicare and I don't understand how Divericare can take money that Mr. Davis specifically told me would be reimbursed to me and apply it to ANYONE'S outstanding bill. Can anyone explain this to me!? I am contacting Medicare and the corporation that owns Diversicare and the Ky State AG on Monday. Beware of administrators who are untruthful and facilities that refuse to take responsibility for theft that occurs in the building! I would not recommend this facility to ANYONE!
By far the most unorganized and unprofessional facility in Kentucky. Most of the nursing staff lack the fundamentals required to care for the resident's basic needs and safety. I have witnessed a KMA withhold medication's resulting in hospitalization. The resident's rooms smell of urine and feces. Activity and nursing assistant's violate HIPPA law's and regulations daily by discussing all of the resident's medical status freely throughout the building. The regional corporate personnel are very dishonest as they are aware of all the ongoing problems. My one star is only for the new dietary manager, Phillip. He takes the time to get to know the resident's likes and dislikes. He is an exceptional employee. My last concern is the appearance of the employee's. Most of the nursing assistants and housekeeping staff do not have teeth or wear clean uniforms. Please consider another nursing home before allowing your love one to stay one night at Diversicare of Nicholasville.

The quality of care far exceeds all expectations! The rehabilitation department is phenomenal and patient goals are being met, with them returning to loved ones and home. It is very exciting having this quality of care in Jessamine county. Family members are 5 or 10 minutes away and can see their loved ones anytime.
This is an awesome facility. The staff is helpful and knowledgeable. My grandfather came to the facility for rehabilitation and he loved it. The new gym is amazing.
Been through renovations
Diversicare of Nicholasville has just gone through a renovation and is better than ever. The new administration is is kind, hard working, and compassionate. The team works hard to make everyone's experience as comfortable and effortless as possible. You can feel safe and comfortable knowing that your loved one will be taken excellent care of by the staff. Nurses are strong and competent and very knowledgeable about the aging and disease processes. There is also a new state of the art rehab gym and private rooms for short term rehab. You should really check this facility out if you are looking for your family member a place to live or needs rehabilitation following an accident/injury.
My wife is already on a nursing home named Diversicare of Nicholasville. It was formerly known as Royal Manor. They got some problems, but since they were purchased by Diversicare, they have a number of improvements that they are going to be making. The facility is old but very clean. They have a couple of places to eat there. They have things like TV, bingo, church services, sing-a-longs, and crafts. It seems to be safe and secure. This facility is close to home. There are a lot of plans to upgrade it. Right now they are still in that process, and things haven't been upgraded just yet.
The staff was a mixed bag, there were a few very kind members of staff, but I found many to be gruff, and inattentive. The decor was dismal. People with Alzheimers were left to roam the halls at all hours. It had a terrible smell.
