Signature HealthCARE at Colonial Rehab

The entire staff did a great job of taking care of my mom during her rehab. The facility needs to be updated,but its kept very clean, and everyone there worked very hard to get her back on her feet and healthy. Cant say enough good things about the staff and employees there.

My sister was in this facility and was treated bad. She fell many times and they let her lay in the floor for long periods of time. I also witnessed another patient fell and was lying in the floor and the nurse just kept walking past her room ignoring her til I said aren't you gonna help this patient. Would not recommend to no one

Do not go there. The 1st night my wife was there they gave her false information. They said that they would clean her up and she had to clean herself. No help from staff.

If you care for your loved one, I would recommend looking somewhere else. My mother was here for about 24 hours. In that time, she was not provided her medication, they checked BP and oxygen 1 time (she is on oxygen), when the call button was hit, no one came for 30 minuets and we had to go find a nurse to finally get someone in her room. After 24 hours, we took my mom out and just made arrangements with family members to move her home.

My family and I cannot be more thankful for Signature Healthcare of Columbia!!! From the first phone call until now, we have been treated like family!! Misty Morgeson, CEO heads up the most amazing team!! Some employees have been there 30+ years and excellent care they give shows!! Placing our sweet momma in a nursing facility was one of the hardest decisions we’ve ever made, but we feel the Lord led us here so mom can receive the care she needs wrapped in the love she deserves!!! Thank you for making this transition easier than we ever anticipated!! We love each of you!!

Signature Healthcare at Colonial gives great care to their residents. The food is great and the activites are awesome

Colonial is a great facility with caring staff. The nurses take time to listen to the needs of the residents and their families. There are always fun things to do. Colonial is an asset to Bardstown.

I love the therapy and the activities in this facility. I love the people. I feel like this is a homelike atmosphere.

Very nice country setting with caring staff. Virtual tours provided via the website. Private rooms available. There is always something going on at the facility to provide social interaction for those interested in spending time with others and being involved within the community. Stop by for a tour and meet the leadership team!

weird experience during this last weekend visiting family. nurses kids running around doing tasks for her? is this legal?

My 88 year old Grandmother was brought here after colon surgery. Not one day after, my family goes to check on her. She was covered in feces and had a fan blowing on her. It took over 30 minutes to have a nurse assist her. DO NOT LEAVE YOUR LOVED ONES HERE.

My daddy was sent here for rehab only. To learn to walk again. Within 8 months he lost 70 plus pounds and is now deceased. Wouldn't put my worst enemy in this place.

Signature Healthcare Colonial Rehab & Wellness is a great facility and resource. The staff are warm, friendly and professional. The facility is clean and I always enjoy the activities they have with the residents/patients. They also provide excellent rehab to help transition back home if that is what is needed. Also the administrator is hands on, welcoming and always available.

Colonial has been my 2nd home since my teenage years. It’s just home to me. I love the atmosphere and the great teamwork we have. It’s takes a lot of passion and love to do what we do. We all come together and make our/your loved ones feel safe and at home.

I am a resident here at colonial I live here long term. I think they're doing a great job with activities. There's always something to go. Going outside and getting fresh air. Working on gardening. All the workers cut up and have a good time with us. We're like family. I love living here. Physical therapy does the best they can with everyone and getting people up and walking again. the adies and nursing staff are so caring and work so hard.

Our experience so far at Colonial has been satisfying.The staff is always willing to help and the nurses and aids are amazing they are always trying to make our mom feel welcomed and like family. I have not yet been able to visit inside the facility due to the pandemic but hoping that will soon change . But even though we have been able to make window visits as often as we like to . And I have been to visit on several different meal times and it always seems to be a good portion and food appears to be satisfying and if not they try to find something to make a replacement. Activities seems to have picked up more which we was very pleased to see .

It's a nursing home, I was just there visiting my uncle. They are short staffed, the staff there is too young, just CNA'S that don't understand the value of someone's loved one yet. Phone constantly ringing. Patients left unattended in the hallways in wheelchairs. I stopped and helped one resident get her one wheelchair brake off bcuz she was just turning in circles and the staff walked right by her and never once stopped. I realize they do the best they can for what limited resources and staff and money that go into it. But please get it thru the CNA's heads that this could be them, or their loved one day and have them try to look at it as how would you want your loved one to be treated and taken care of, if they were in a facility like this??? Keep that in mind every time you see a resident. I'm also a CNA, but the only reason I got that certification was because I had to have it in order to further my education and career in the Healthcare Field. I'm not above doing their job in any way. I did my clinical rotations, I know how hard it can be. I also understand that not every person that is employed in the Healthcare Field is going to treat their patients like I do, and may or may not have the same work ethic. I just wish these facilities weren't like this... The residents deserve better, This is their Home at This point. Just want the staff to be compassionate that's all. 🏥🧑‍⚕️🦽🚑💺🩹🩺💉💊

Excellent staff and patient care! Everyone is so friendly, and always concerned about all of the residents needs. All around wonderful facility!

Medication aren't documented properly. Poor quality of care. Patients need basic care needs and proper medical attention. Friendliness is not full care. You must be there everyday to ensure care is given. Poor hand hygiene. Entire wing CDiff.

My mom is a patient in this facility and has been treated with so much kindness. Her health is their number one priority and the medical care she receives is exceptional! I thank everyone for their kindness and consideration but most of all for their excellent quality of care. Thank y'all so very much!!

I have always enjoyed my visits to Colonial House. I have been visiting since i was a child, when our church would go once a month to sing. The residents always have a smile on their faces that brings me joy. The staff has always been great!!
