Signature HealthCARE at Jefferson Manor Rehab & Wellness Center

Jefferson Manor has been a wonderful facility for our brother. The thing I love most about Jefferson Manor is that each time we visit there are at least 5 - 10 nurses, aides, other staff available for questions, assistance, etc. Our family members have lived in multiple Assisted living and nursing homes around Louisville so for comparison sake, Jefferson Manor is the most well-staffed facility and we are grateful every day that our brother can live there and enjoy all that Jefferson Manor offers, including a lovely outdoor patio that’s accessible from all hallways and a nice lobby with a piano. Dr. Nick Cooper, the director, is approachable and always available for discussions about medical needs, etc. We highly recommend this facility and appreciate all they have done for our loved one.

My disabled brother (65) has been a resident at Jefferson Manor for 18 months. He initially went there to rehab after an injury, but liked it so much he wanted to stay. I visit often and am thrilled with the care he receives. The care staff and the management team are excellent. They have great attitudes, treat him like family, and do their jobs with pride and professionalism. He regularly states that he hopes to live there forever.

The staff and administrator at Jefferson Manor have been great. They make my father feel welcomed and comfortable. They answer any questions we have and address his needs.

When I first spoke to the Administrator at Jefferson Manor regarding finding a place for my father, I admittedly had my doubts. I had done my research after all; checked all the reviews, ratings and besides, who would know better than me about the best place for my dad. After hearing his passion for the facility and what they are doing there, I was compelled to see what his enthusiasm for Jefferson Manor was all about. But what I would soon discover is this hidden gem, tucked away right here in Louisville, provided far more than just a place to take care of my father. It provided family. From the moment you walk through the doors, you're greeted not only by a spotless, welcoming environment, but also by a warm and compassionate team that makes the entire facility feel like home. The therapy team at Jefferson Manor is outstanding. Their knowledge, dedication, and enthusiasm for helping each resident reach their highest potential is inspiring. You can see the passion in every interaction and the joy they take in each success, big or small. Equally impressive is the nursing staff. They are caring, attentive, competent, and consistently go above and beyond to ensure every resident feels seen, heard, and cared for. You’re never just a number here, you're treated like family. That sense of belonging is something you simply can’t manufacture. It's authentic. But what really sets Jefferson Manor apart is the feeling you get while you're there. 5 Stars I can't say enough good things about Signature Healthcare at Jefferson Manor! From the moment we walked in, the level of care and compassion was outstanding. A huge shoutout to the Business Office Manager Brooke is so helpful, professional, and genuinely caring. They made the entire process smooth and stress-free for our family. The CNA’s and nursing staff are absolute rockstars! Their kindness, patience, and dedication to residents truly stand out. They treat everyone like family, and it shows in every interaction. My dad was in Jefferson Manor in February 2025. He was in another rehab in November 2024 and that was an awful experience. I was worried about what his second placement would hold, but read all the reviews and they were spot on. Jefferson Manor is a great facility for rehab and wound care. The staff really do care and get to know the patients and their needs and likes. My dad is very particular, as are most older folks, and they were so good with him. He really, really like and enjoyed Nurse Tyson on night shift. He was SOOOO attentive, personable, and caring. Nurses Lynn and Kim on dayshift were equally kind, caring, and personable. If my dad asked for anything, staff would do anything in their power to meet that need/request. Everything from chocolate pudding to a different bed. They made it happen!! I couldn't believe it!!! Staff at Jefferson Manor communicated with me every step of the way. Discharge was seamless. The social worker was amazing and super skilled in her job. Our last rehab left us with NO referrals upon discharge, and the social worker never called me back at that facility. The social worker at Jefferson Manor made going home super easy; a true seamless transition. Again, I couldn't believe it!! Seriously, if you need a good rehab place for your family member, please consider Jefferson Manor. I would definitely take my Dad back, if needs to be there. The first rehab was horrible, but the facility was beautiful and impressive. Jefferson Manor is an older building facility, so it does not "wow" you with its appearance. But it is clear that they spend their money on people. People who care and provide great care.

If you are looking for a place to work then I 1000% recommend Jefferson Manor. You walk in and are instantly greeted with smiles. Everyone is extremely helpful, caring, and friendly. This facility is highly recommended.

Jefferson Manor is a top notch facility! Staff is always friendly and it is obvious that they care tremendously about each resident. Their therapy team is amazing and has lots of great success stories! JM is a smaller facility so I believe that the care is very personalized in every aspect! JM has a very personable, positive and professional atmosphere. Everyone works as a team to create the best outcome for each individual!

I was recently able to take a tour of Jefferson Manor. I was very impressed with the knowledge of the staff that answered my questions without hesitation. Extremely friendly, clean and professional atmosphere. I could not imagine sending any of my love ones or myself to any other facility. Jefferson Manor 100%

I will like to thank everyone from the nursing team to the dietary office for the exceptional care my father received. Charles Ward. He went in on his back and walked out 20days later thriving. Truthfully no words can express my gratitude. Thank you everyone. Your facility is top notch. I would recommend it to anyone going thru care for themselves or a loved one.

My father was admitted to Jefferson Manor for rehab with a broken hip and femur. I couldn’t be more pleased with the entire staff! They have been exceptional! Especially the administrator who has been with us every step of the way, always checking in with the family to see what we need.

I had the chance to tour Jefferson Manor with their Administrator, Nick. Finding a skilled rehab and long term care that can show how much they care for your loved one is not easy. However, when I walked around I met so many friendly employees and happy residents, and could really feel the positive energy. The community has been going through a renovation to update its halls and rooms so if you haven't been in lately, I highly suggest you take a tour. Their therapy team is all their employees as well so your loved one will get consistency with the same staff. I was very impressed with this community and will continue to recommend it to anyone needing skilled rehab and long term care.

I cant recommend this nursing and rehab facility enough! The staff is incredibly friendly and caring, and the facility is always clean. The clinical team is exceptional - knowledgeable, attentive, and truly dedicated to providing top-notch care. The rehab team is equally impressive, tailoring their approach to each residents needs. With a fantastic location and such a supportive environment, this is the perfect place for a smooth recovery. Highly recommend!

I have a family member at Jefferson Manor.They are thriving, have made many new friends and enjoy the activities. It’s a very hard decision on where to place a loved one. The fit is great. Our decision was spot on. The staff is knowledgeable and very friendly.

I toured Jefferson Manor, and I must say it has an inviting feeling and the residents that were out and about looked happy. The staff were welcoming, especially the executive director, Nick who took the time to explain the different care areas and services offered at his facility. this team obviously knows what they are doing!

Jefferson Manor is a top notch facility for your loved ones! Not only is the facility clean and well maintained but the staff really care about their patients. The Social Services Department is always ready to assist with any issues that arise and also willing to provide services needed for your family to the best of their ability. Jefferson Manor is a great home away from home. You won’t be disappointed!

If you are looking for a great facility for short term rehab for a loved one, Jefferson Manor is the place to go! Not only do they have high care reviews, but they are also one of the only facilities in the city that offers in-SNF dialysis service, so no more going out to an outpatient center for long hours for your dialysis care! The company has great values, the staff is warm and inviting, and I would recommend this place to anyone!

My 95 year old grandmother came here after a bout of Covid. The staff is top notch, the facility is very clean and smells good. The rooms are large and private. The staff were personable and spoiled her during her stay. I would recommend 1000 times over!

If you want to be uncomfortable and ignored unless family is present...this is definitely the place. Room ceilings are dirty. Food is cold and horrible and staff is either on their phones or busy doing nothing. My brother is very unhappy with the service at this facility. We need to move him ASAP.

Yes I would like to say that my two weeks here have been wonderful between my physical therapy all the people there are terrific I just love them all ❤️ And all the staff and everyone that helped take care of me all the ladies are so lovely and kind and they work very hard I just love them all they all know who they are I'm not good with names because I have a short-term memory problem. Honestly I think everyone that helped me went above and beyond and they deserve a raise because they work very hard and they're very good at their jobs💯 Thank you everyone for your support your love and your kindness and helping me through the last 2 weeks it was a pleasure 🤗 Much love and prayers 🙏❤️ Jackie Huff 😉

Let’s face it nobody wants to go to a facility for short term rehab or long term care or even work at one!! But I picked Jefferson Manor!! I love it!! They have the best caregivers in the business!! The management staff is top notch!! If you have to put your loved ones in a facility, here’s the one that is the closest to a home environment you will ever find!! You will not be disappointed… I wasn’t!! So blessed to be in this community!! Thanks JM!! Doing BIG things one step at a time!!! ❤️

For anyone that needs short term rehab or long term care, this is the place to go! The staff are very friendly, helpful, and they actually care about your needs! They go above and beyond to make you feel comfortable while you are recovering at their facility. The facility is clean and well maintained. Their daily activities help to keep the residents entertained during their stay and it boosts the morale of the home itself. I would recommend a stay at this facility!

Signature healthcare at Jefferson manor has come a long way. The compassion the staff show is unlike any place I’ve ever experienced. The management team is dedicated and take all concerns with the highest regards.

I am the last to write a review unless it’s positive, but I do want it to be known. The mistreatment to the patients absolutely blows my mind. My sweet grandfather begs me everyday to be removed from this rancid facility. The low staff, the horrid food, I could go on. MAKE CHANGES! He stayed in feces for over 3 HOURS!

Jefferson Manor has a wonderful staff and provides a great working environment. The staff always puts the residents and patients first in order to ensure they enjoy their time at Jefferson Manor.

This is a for-profit business. I hope you will invest in some bariatric bed sheets Or even regular bed sheets, so people don't have to be laying on mattresses. As a visitor I was appalled that some of the residents don't have sheets to sleep on. Also, some of the aides are walking around with earbuds on, you can't catch their attention. I think you ought to have that for after hours or lunch break only. This place needs to tighten up.

Came to Jefferson Manor with high expectations after working in rehab facilities through out Louisville. I have been very pleased with the facility and how it treats its residents and staff. Looking forward to a long career at Jefferson Manor.

I initially started with the fear I wouldn’t fully enjoy it, thank god I was wrong. I’ve never felt so at home and relaxed whilst on the clock. I was honestly shocked when I wasn’t dreading going in, but actually looking forward to it. Everyone supports each other and go out of their ways for not only other staff, but also the residents. Whether it be staying after even on a long shift, or still wrapping up when at home to ensure the best outcome for all. I can see myself creating a career and being happy

I was there for 5 months and I will say, don't take your family there if they have Private Insurance. The CNA's and Nurses were awesome. But the administration and business manager will lie to you in your face and tell you that your private insurance has stopped paying and force you to pay out of your pocket...and they're still lieing...I've watched patients die, fall out of bed and was told their conditions and they will tell your family members health business to ANYONE....DO NOT TRUST THEM...they've also been fined numerous times for Medicaid /Medicare fraud. PLEASE DO NOT GO THERE.....Going to contact the State.....have false documents to prove they've lied.....ZERO STARS......

Of all the nursing homes I have been at, this is the best one. All the employees get along really well, and things run smooth. They also keep it very clean, and I really enjoy living here.

My father is currently in hospice care for stage four cancer at Jefferson Manor. My entire experience with this place has been negative. They do not communicate with the immediate family very well at all. My father was sent walking outside to his ride (going to an appointment) without shoes on by his nurse. The front desk attendant I was so unfortunate to come in contact with today spoke to my mother and I like we were children; she refused to let us speak and was condescending and overall not pleasant. You would think a place doing end of life care would have some respect for their patients and their grieving families. I’m hoping to get him moved soon; this place has failed my already low expectations.

So initially my observations and experience with this facility was highly unfavorable, communication was bad, getting things established with the staff was rough, getting the bathroom cleaned was poor. After speaking at length with the administrators and staff everything was straightened out. Things started going smoothly and meds were finally handled properly. I would change my rating to a higher rating and offer an apology to everyone involved. I was highly agitated due to my husbands condition and afraid of what could happen if things went wrong, however everything was fixed and the facility seems to be well in order. I apologize and want to thank everyone involved in taking care of my husband, the fear overwhelmed me and I reacted. Thank you Jefferson Manor and Signature for handling this situation in a professional, customer friendly, caring manner.

I am staying in the covid unit in have been served food my dog won't eat its plain out jail food I was given warm grape cool aid 4 frys in a box its horrible how they can feed someone this slop I took pictures. They pretty much told me this is what I have to eat.

Jefferson Manor is one of the nicest facilities. The staff go out of their way to make sure the residents are happy. The environment is fresh and clean. If you’re looking for the one that goes above and beyond, this is the place. Residents are always clean and dry. To me this is one of the most important things. If they look good they feel good. Keep up the great care!

My brother was sent here from another nursing home. When he returned back to the the nursing home were he was sent from he had sores and a red rear end. Also a uti and kidney infection. People need to be aware how you treat elderly people. We are really upset and would not recommend anyone use this nursing home.

I thoroughly enjoy my time here at Jefferson Manor. The residents are so fun to be around and it truly warms my heart when I see the Quality of Life team cheer up the residents with their creative crafts and activities. Everyone is so friendly and happy to be here. It’s like a family away from my own family. Jefferson Manor would be my #1 pick for my loved one.

Randy Drexler is my brother at Signature Healthcare Jefferson Manor. good morning I want to say ThankYou all for taking care of my brother health wise I am very appreciative that you worked hard to get him a new wheelchair I wish Covid19 would not have made it take so long I am not good with names but I love the lady that answers the phone in the daytime she is so sweet and helpful i also love the way you all set it up that we could bring things to our loved ones on Thursday and leave them at the door the people work so hard to get these to everyone and that is so very kind its been so cold I haven't visited much but I will start seeing Randy more now that it's getting warmer Again thank you all very much for taking good care of my brother I truly appreciate all of you !!
