Love this facilitie's home-like atmosphere,exceptional nursing care and top notch therapy team. I would definitely recommend to any of my friends or family.
I have been 3 weeks and I could not have asked for a better group of people working here! Organization is good, people working here are dedicated, serious and yet funny and that's all important--a balnce of this is so important. Nursing and therapy and housekeeping were fabulous!
It’s a great place to work! Love my residents and getting to make them smile every day !

The facility is great. The staff is friendly residents happy. I would love for any of my loved ones to be there and stay there.
Experience was OK when we first got there. My dad was admitted on a Friday night at 3:00 in the afternoon before Thanksgiving so everybody was ready to go home. Not much was explained to us how everything worked. After finishing the paperwork I walked into his room He was crawled up in a ball with his hands over his ears because the shared roommate had his TV up and my dad cannot take loud noises like that. So I did ask him he did turn it down a couple notches and then I had to talk to The nurses and had somebody come in and turn it down some more. (Which I understand shared rooms are that way). The first nurses that took care of him were awesome especially the night shift. When I was showing up there every day stuff seemed to be okay. But after I left to come back home for a couple days that's when things started getting a little bit worse. And I started getting phone calls every so often about different things that had happened. Then Sunday morning 3:00 in the morning I get a phone call stating my dad is bleeding through his catheter again. And never going to call the on-call doctor. And I never heard anything about it until the next morning when the hospital caseworker calls me. Because they didn't have an emergency contact number and was trying to get a hold of my deceased stepmom for an emergency contact. And then they had to contact somebody and found out that I was the emergency contact. So when they sent him from Signature Healthcare no one relayed any of this to me! And they never gave them any kind of updated contact information! Then about 20 minutes after I got that phone call Signature finance person called me told me Medicare don't pay for both!!!(Pretty rude when she did) And I needed to have a decision whether or not I wanted to hold his room and pay $420 a day while he's in the hospital!!! So after that I had to call the hospital and figure out what he was even there for besides blood in his urine in pain in the abdomen. Then in the meantime I had to determine what to do with him after he got out of the hospital. And I was labeled to locate a way better facility closer to me and ended up having to transfer him up here to where I live at. And have had a lot less problems and he's doing a lot better than he was there both mentally and physically! So to top it off they told me that they had all this stuff in the social workers office. I went there picked it all up and when I got to the new facility I realized that he was missing pants and some shirts. And nobody's seen them now! But that's the minor part of it I can buy new pants and a new shirts and he is a lot better off!
My grandmother was here before she passed. This is hands down the worst rehab/long term care facility I've ever witnessed, and I work in healthcare as well. The staff especially, management here is awful. They don't care about their own employees, they're really cliquey, and just overall immature. They're all really cliquey like it's highschool or something. I know this place has a high turnover rate and now we can see why. The actual facilities however are even worse. I've never once seen any kind of house keeping, the rooms and halls look like something out of a 50s war movie. This place is so poorly kept, call lights hanging off the walls and everything. I feel sorry for anyone who's loved one had to come here, and even sorrier for a lot of the staff there is tons of power abuse. I will be filing a complaint with the better business bureau, there's no reason this facility should even be up and running and they're claiming to take care of patients?? No one could possibly get better in these conditions, I know because my grandmother didn't.
The staff was always kind, helpful, and generous. We appreciate everything the therapy and nursing departments did. We recommend this facility to everyone!

TOP TIER! ONE OF THE BEST! The therapy staff kept me on course and pushing me to succeed. The CNA and nursing staff are top notch! And the food is some of the best!

The staff there is amazing and Nicole, Seth & Debra - deserve a very special out for the care they showed my husband during his rehab. The only critique would be that of the food as it left a lot to be desired, but I think that is likely in most health care facilities. Thank again for all you did during our stay... Mrs. Darling :)
Everyone is really nice here; and the location is central so I'm close to my family so they can see any time. The nursing and therapy departments are so nice and helpful.

I come to Signature for the therapy and I have come here 3 times for my short term rehab! And, I highly recommend the therapy department! I will be back again, if I need to, great experience!
There is no resident line to contact residents here as not everyone would have a phone in their room, only the front desk phones which staff apparently do not answer on weekend nights.

Recent 43 day stay for rehab n the Bowling Green facility after a fall. The therapy department is phenomenal. I had some unique injuries and they worked together to meet my unusual needs.. The nurses and techs made sure your needs were met. I’d love to name my favorites but the list would be too long. Perfect place to get stronger before you go home. Thanks to all
I really enjoy visiting with the residents at the facility. They are fun to talk to and interact with. I’m in JROTC and we had the privilege to do a Veteran’s Day program at the facility. We also got to play BINGO with the residents. Always a great time when visiting the facility.

My experience with this center has been top notch. I have witnessed first hand how they go the extra mile to assist patients with discharge from rehab to home and follow up to make sure needs are met once they’re home.
I was NOT impressed from day 1. The bed was too small for my spouse and I was assured they would get a bed extension for him, Didn't happen even after him being cramped up for 10 days. Same clothes on for 4 days. I asked someone to get him fresh ice water after his cup still had water in it from the previous day. Some may like this place but, I for one will never have him back in there. No they don't check on the patients like they should. After I told them I was taking him home did they say they could get him a longer bed.His ankles were propped up on the footboard so he could stretch out some. As well I didn't go visit at the same times, I popped in at various times and seen the same things day after day.
Everyone is very kind. Staff speak to you when they see you in the hallways. Staff interact well with all of the patients who are at the facility. The nurses are extremely good. The facility is clean. The food is good. I love the therapy team. They have helped me meet my short-term rehab goal to return home. I am going to miss everyone here. I’ve made so many friends. I plan to come back to visit them and play BINGO.
Signature has been my home for a long time. All the staff have become a part of my family and they are all very caring.
I appreciate Signature for everything they do. This is a great place to live at. It’s also a great place for people to come to do therapy. I love the Activities department. They have a Country Store, we can go to buy stuff and are always hosting fun events. The Social Workers are excellent. The Administrator and DON are very involved. Nurses and CNAs provide excellent care.
The staff was very friendly when I stopped by! Facility was also very clean.
Family went for a facility tour, we had numerous questions and wanted to understand how nursing homes work. All our questions were answered thoroughly. You can tell staff really takes pride in their facility.

The Therapy unit here is amazing! I am very pleased with the therapy my husband received. The whole staff was excellent and their work ethic and kindness was exceptional! I highly recommend this therapy unit. A big shout out to all of them—PT and OT.
I highly recommend this facility!! Signature Healthcare of Bowling Green has a friendly and knowledgeable staff that are dedicated to getting patients to their optimum level of function to be able to return home successfully. They are very responsive and go above and beyond to offer a private room for rehab patients and even allow families to stay in the room with there loved one.
As a non-emergency medical transporter, I am visiting different nursing homes/assisted living facilities within 5 counties, almost daily. Signature HealthCARE of BG is one of the best, if not BEST facility I have visited. Walking in, the entrance way, lobby, and bathrooms are always nice and clean with a fresh fragrance in the air. Employees and staff members are helpful and friendly. They really do go above and beyond. My clients who I have spoken with who live here, feel loved here, so it's easier to feel at home. I would recommend this nursing home facility to any of your loved ones.
Signature Healthcare in Bowling Green has the best nurses, therapists, and CNAs in town! Resident are well taken care of in every aspect of life from physical, emotional, and spiritual. When you walk in the front doors you see smiling faces lining the halls. I am very thankful for this facility. I would recommend this facility to anyone who is in need of care for loved ones or for themselves.

HORRIBLE PLACE!!! Would give 0 stars if I could!!!! Had a family member in this place as a last resort due to insurance coverage, and within 3 days they almost killed her! She was within hours from death! Lucky we were able to force them to call and ambulance to take her out of there. Still in the ICU receiving care at a local hospital a week later! DO NOT LET YOUR FAMILY GO HERE!!! THEY HAVE HORRIBLE CARE!!!
I feel that the nursing staff is great, Lynn Snyder, is his nurse and she just does a great job, keeps me informed and my father, who is a resident there, likes her too. All the staff is wonderful here too. Great communications!!

This place is like a slum lord owns it. They use 1 nurse for 60 patients. Your meds won’t be on time & they won’t be corrected. The dr will not see you weeks until he “ feel like it “ & you are at their mercy until he does. They information & charts they get will be all wrong. Honestly I can’t even believe these ppl have s business. They must pay ppl yo give s decent s pre , bc I’m telling you from my own personal experience, this place is horrible. Whoever is giving this place anything other than a 1 is lying . The food is that of scaps off a poor table. They bill insurance fir things they don’t do or give you , right down to butter Patty for a piece of bread that they take straight out of the plastic bag and throw over the top of the garbage food. This place is nothing but a waste of money. I would never trust these ppl to care of anyone I love .
