This facility is A 0. I'm 0 very upset I had a very serious surgery was in there about 5 hours came back to my facility that I'm staying at getting well and they didn't have pain medicine for me they told me it was never ordered actually it was ordered and it was ordered a week ago and they told them they were told they had until 2 days before the surgery pick it up so when they called and they said it wasn't there it was but they blame the doctor the lady at the doctor's office said had been sent and it's 10:15 and surgery ended at 2:15 still no meds because they would not there was nobody to answer the phones at the facility isn't that crazy. Her family members are in the care of somebody that won't answer the phone and it's not because they're too busy they're sitting at the desk they just don't want to answer the phone it's like the call likes at night so now I was told I may have been made medicine as early as midnight or as late as 6:00 a.m. that I was just going to have to deal with it they would put some ice on it because the nurse that's in charge of the car that's not here would not give her permission to look through the medicines find one of somebody else's that's equal to mine and then pay them back it's been done before trust me administration didn't even ask me about it I know it had but known about it they didn't even follow up on it when I finally talked to the doctor she said I've been trying to get in touch with the nurse and the facility she said somebody can call them right now and have it sent you probably need to go find the nurse and have her take care of it I went and found the nurse and she was trying to take care of it but she did not know what time it was going to be here and she apologize for me having a supper but it's not her it's administration communication and not following up on other probabilities that it could have been because the medicine was there an hour after she thought it was never going to be there that was a good enough to answer evidently for administration so the problem was solved without solving the problem it happens a lot around here it does like the lights being known for half the night and nobody going to him or Saturdays when Steph's when the administration's not here and like to sit there and blink for an hour I want to throw the things today you got a bunch of lights blinking and they finally came and took care of it and I didn't even have a I like drinking now I'm supposed to be able to leave a review without any problems my name is William r foster it is Thursday the 6th is 10:17 p.m. and not being rude with the ice is it even touching the pain it might be helping the swelling and while all of the stuff that could actually do something I'm sick at my stomach and there's certain times of the day you might want to walk in here and certain days if you have a loved one more to follow if anything does happen to me for leaving the truthful post I will It is now Sun the last time I received any medication was about 8:00 pm sat night.ivr been asking since midnight the CNA came in after midnight and said I'll let your nurse know I turn the light back on by an hour later fell back asleep woke up this morning throbbing in in pain I push the button on the nurses call line the CNA walked in sometime after 5:00 it says oh she never came back and I said no I need somebody now I just had surgery Thursday and there's finally came in and said oh I'm so sorry I'm so sorry I thought that other nurse gave you your medicine well I've done missed two doses now she said she never saw it pop up on the computer,but she first said she thought the other nurse gave it two me?? I asked you the other nurse was she said Nicole the only Nicole I know that off at 7:00 p.m. Saturday evening I bought my medicine in in a cup and told me to take it at 8:00 or 8:30. her story makes no sense. I told her the pain had gotten real bad cuz I had an ibuprofen too and she said yes and never came back again and now it's 550 in the morning. Something doesn't seem right about the way she's acting

UPDATED AT THE BOTTOM I would love to thank The Admitting Director Karen Vonboael not only for her Ability to make you feel Welcome and comfortable But also handling issues such as Medication being given in a timely manner and the Comfort of my stay she gets 5 Stars!! Also Sha'Kayla Foard And De'Angelo Glore on day shift For there urgerency to get to your needs once the call light is pushed! There jobs can be demanding and even personal with things like bathing washing and grooming and meals!! These two always enter the room smiling and pleasant ready to help you out whatever it is!! And last my new friend Matthew Henderson : Med Tech. He is on time pleasant and always lets you know what he's giving you!! Even having to come back on occasion he never complains just a pleasant how can I help you!! And last Donna Farmer Assistant Social worker! Her personality is contagious and so is her Energy!! She multitask as easily and some do one job. She's always there in a flash when needed and if you happen to be down you won't be when she leaves the room And Also Nurse Heather Stout not only for being kind and caring but for taking the necessary steps to get my medicine on a schedule rather than PRN to try and help get my pain under control also for going above and beyond which she does consistently. And Nurse Racole Robinson who took the times of my Medication to maximize my relief for different activities during the day such as early morning because pain was greater than and after physical therapy she took the time to help it get adjusted to help me feel better and be more comfortable! Also for her empathetic and caring attitude and for going above and beyond as well ,Thank you!! And STONECREEK SHOULD BE GRATEFUL TO HAVE ALL OF THESE PEOPLE LISTED AS EMPLOYEES AND THEY SHOULD BE REWARDED!! SINCERELY,RUSTY FOSTER It wouldn't be fair if I didn't add this. It's Saturday February 1st 2025 it's Saturday and it's 1:40 pm. I have the Displeasure of having the Same CNA I had last weekend!!! I won't mention name but she's 7:00am to 7:00 pm 300 Hall. Maybe she needs retrained or She just doesn't care but she definitely doesn't put the patients before herself. She can be right in the middle of doing something and if her phone or Watch rings she's stopping what she's doing and gonna have a conversation with her mom and it wasn't the first time.. I didn't say a word last week but I spoke to the nurse Angel about it today he's the nurse and comes and helps considerably more than she does!! I went to buy a snack about 9:30 there were 6 lights on 4 were blinking I'm assuming that means urgent or they been on awhile.Anyway I'm headed up front and low and behold she's on her personal phone just laughing as if she was at home or a coffee shop. I even said hey there's lights on she kept talking. I went on to the snack machine and on the way back she was just getting off the phone. There's more but I feel like This is how several people can Work so hard really go above and beyond to give a company a good name and one person who doesn't care can give a place a bad name.

Just got told by nurse that my repeated asking for the nurse to give me my antibiotics on time was harassing her staff. I take antibiotics every 12 hours to prevent infection on my open wound from a surgery. It takes 2 hour for the IV to complete then I have to wait for nurse to unhook before I can rest. I can't go to sleep knowing someone is gonna be in and out randomly until IV is completed. Not harassing just don't want to risk a infection. 3 of my antibiotics have been skipped because they didn't have it. People seem to be unconcerned when it's not their health at question. I got here at 3 in the afternoon and have been 3 full day plus that piece of a day. Someone took a spray bottle of something and sprayed the room and bathroom floors then mopped it with one of those Swiffer mop things.this is the only time a attempt to clean has been made. Still got same sheet on bed also.belonging to someone is setting in the shelf. I assume it is whoever was here before me. First full day here I was brought lunch but no breakfast or supper. I did argue with them about not having my meds but got told how everything was someone else's fault. They had 24,hours notice that I was coming but still didn't have my meds. Pushed button today and when someone arrived I told them it was over 30 minutes ago that I pushed the button and was told she was dealing with someone else. I ask her what if I would of been choking on something and she said what if the person I was with had been choking and I said guess they'd lived and I'd died. Her response to that was (people die All the time).

My brother was Transferred there from the Louisvill hospital for rehabilitation from a severe infection obtained post-op. The Dr orders medication and antibiotics be administered on a precise schedule. He must have other meds that have been ongoing for years prior. He arrived by ambulance at 2:47 pm cdt Friday the 27th. As of 11am onthe 28th no medication or antibiotics had been administered. The antibiotics are thru IV .the care thus far isn't care at all. If he has any complications due to this malpractice the liability is solely on Stone Creek. He is still there as of now. Since they have been forced to do their job they now want to send him home. That is not what the Doctor ordered.

I am still fairly new here. But, am truly amazed by how professional , and helpful all the staff are. Even understaffed everyone still goes above and beyond to help. From the cooks, aids, and administration. I really enjoy working here because even when it starts to get somewhat exhausting. The residents are always everyone's top priority! From a midnight snack ,activities or simply helping a resident get around. I see it, hear it, and apply it every day. Residents first! Great place to work, Grow and be!

I have to agree with the last post food is awful and if you would like examples please letme know. I also agree with the staff being great. They look after whatmy needs are and want. I really think the important part of my stay here is the amount of attention (even being short staffed) that i recieve frome the staff, management, and outside resources they provide. Again if you need some more information on how bad the food is.

In particular, the business office manager, Lisa Deshcaaf, has continually went above and beyond to cater to our needs, even when the tasks range outside the scope of her day to day tasks. I would like to say that from the moment I walked in the door of Stonecreek Health And Rehabilitation I was greeted with professional courtesy by the receptionist Julien Robinson who pointed me in the direction of the Admissions Director Karen Bokel. Karen has been extremely helpful with the transition for my 90 year old aunt from her home to this caring facility. The staff is also polite and friendly. I am thankful there is a place like Stonecreek to help attend to someone I truly Love. Sincerely DC

This review is for two people specifically: Julian Robinson and Lisa De Schaaf. Julian is such a pleasure! Always so patient and kind. Ready to help when I need it, as I have physical challenges. There are times I don't even need to ask, he is there helping as soon as I walk in. I look forward to his smiling face every time I visit. Lisa is wonderful with the residents. Even though my mother's mind is failing, she enjoys Lisa's visits to her room. And I've watched her with other residents. Taking the time to stop and say a kind word. And if I need a shoulder on which to cry, she's there for me. These two are awesome!

This place is a joke. I wouldn’t take my dog there to rehab. They talk a great game but they don’t follow thru. The night we took mom there for rehab I gave them my POA and witnessed them make a copy. She had a doctors appointment that I checked her out for that appointment and when we returned with rehab and wound care orders. A couple of days later they asked me for the very same orders I gave them when we returned from the appointment. It took 2 times to finally get them to begin treating her and changing the dressing but they refused to get the appropriate antibiotic patches, so because my daughter who works at the orthopedic institute that had provided the original orders had the orthopedic furnish the necessary antibiotics patches. My mom has a very critical blood anemia and requires bloodwork every other week and the hematologist provide three times the orders for the blood draw. They never drew the blood and she went into a critical condition and required blood transfusions at the hospital. ALSO, they do not let the family know when the projected discharge date is. I checked on the Tuesday following the Labor Day holiday and asked when her projected discharge date was and I was told they didn’t have a day planned. Now at this point she had been there 2 weeks/14 days but she was supposed to be there at least 21 to 28 days. I had to beg for a discharge care plan and waited 4 days for it. So yesterday Monday, September 11th she had a blood draw and appointment with her hematologist and we were told her blood was critically low and needed to be transfused immediately so, we ended up spending 10 hours in the ER at Baptist Health getting blood which probably would could have been prevented if they had done what the orders that had been faxed to them twice. As I was walking to the car yesterday to pull the car up to load her to go to the lab and to see her doctor the financial officer almost tackled me outside on the sidewalk and told me that mom was going to have to pay private since the 5th. I told her that on the 5th I asked the social worker when we could set a discharge date and that is when I was told there was no date set. It was then she said the earliest date would be Friday, September 8th. NO ONE IN ALL THAT TIME SAID ANYTHING ABOUT HER BEING PRIVATE PAY!!!!!!!!!!! I asked the financial person yesterday how much it was and she said 300.00 dollars a day. So if you are considering putting you loved one at Stonecreek just know they will not do what they are supposed to do and they will start charging way before the actual discharge date is. So you have to bring your loved one home way before they are strong enough to be home safely. PLEASE, PLEASE, PLEASE DON’T SEND YOUR FAMILY MEMBER THERE IF YOU WANT THEM TO HAVE APPROPRIATE REHAB FOR A SAFE RETURN HOME. I called them yesterday to inform them I was sending other family members to retrieve her belongings and we would Not be returning. That call happened to be a conference call and I told them exactly how I felt about how her treatment and them NOT doing their “due diligence “ and take care of the patient and definitely not honest with the patient and family regarding discharge. What They want is to secretly turn the patient Into a private pay without telling anyone. That is WRONG, WRONG, WRONG!!!!!!!!!!! Shame on you Stonecreek……….you all suck. I gave them one star but in all actuality they don’t even deserve 1 star.

Don’t believe a word that any of these 5 star reviews are saying, the employees are told and bribed to do so. This is a greedy facility that cares more about a paycheck than their employees and residents. The new administration is mad that people are rightfully giving bad reviews so they have to resort to their loyal employees, whom are only loyal because they’ve been bribed. If you love your family even just a little, never admit them into this facility. State is called at least once a week for the neglect of residents. This place should have been shut down long before now. Teresa Belt is the admin, don’t believe anything that she says. Be safe and stay far away from this place.

I would leave a zero if possibel. They place is way understaffed and not super clean. No one is checking on them at night, or in the am. 8:45 and he has not been checked on, laying in a soaked bed and clothes only to be told they could not clean him up until after delivering food trays. Understand, the workers seem to be trying but only two people on the floor during the day, one at night. Unacceptable !

Absolutely awful. Do not take your family members here! We just found out that my father-in-law has not received his proper medication (one of which is medication for blood clots) in weeks. The staff is rude and does not care about their patients at all. They rarely clean or do his required physical therapy. When asked why this is happening, the staff just shrugs and says they don’t know. Straight neglect.

Horrible place. The staff doesn't care. Takes too long to get anyone to help when needed. It appears that the 5 star ratings come from staff NOT residents. Residents are left in their own waste for extended periods of time because there is no one available to answer the call for help to the bathroom. It is as if they would rather clean you up after you have soiled yourself than respond to the call for help. During this stay no nurse call request was acknowledged in less than 30 to 45 minutes. Most times it is closer to an hour or more. Not at all sure how this facility got 3.6 average rating. The 5 star reviews I read appeared to be not from anyone unlucky enough to have to stay here.

My brother-n-law has an aunt that stayed at Stone Creek for a hip replacement from Lourdes Hospital. They sent her there for hip replacement physical therapy. She said her sheets and bathroom was not changed in 8 days. Her roommate was real sick and needed attention. They told her to be quiet. His aunt complained to a social worker and they came in to change sheets afterwards. She finally got to go home. Thank goodness.

This facility needs to be shut down. My dad received terrible care. He’s a diabetic and never received a diabetic meal. The main reason he was in the facility was because he had a wound vac. One of the nurses lied in her report about changing it. Some of the nurses were rude. If my dad pushed his call button, it would take at least an hour for someone to come. I had to ask to have his sheets changed after his shower. He only got a shower twice a week. The administrator lied about ordering a wound vac to be delivered to my dad’s house once he was discharged to go home. Nothing was ordered.

I have a family member in this facility, his clothes have come up missing on numerous occasions. My elderly mother purchases his clothes on her fixed income. I was told to speak with Social Services, Housekeeping Manager, l also spoke with the Adminstrator. Still can't find his pants. So we waited for Social Service, they said, "she is here somewhere." Then they tell us she is gone to lunch, l said we can wait, they reply, oh she will be gone a long time. So guess what Chelsea? l have some vacation time l need to use anyway, so l will return. return.

My husband is currently in this facility they do not abide Rda standards amongst other things Food is not kept hot and hot trays kept at proper temperatures before being delivered to the The occupants he has fell multiple times has not had a shower in over a week and become very weak at this facility by lack of nutrient food and rehab this is a poor excuse for a facility anyone can put on a nice show and smile but they all have jobs there to do and the jobs are not being done adequately for the people that are in this facility I will be sending this to further investigation and will not stop until this place is up to spec or shut down for those leaving nice reviews good for you for working there and supporting your job

My mom was there less then a month and almost died two times first time they messed up her insulin the second was when I brought her home she had such a bad uti I had to take her back to the hospital and she fell once office staff was nice until they found out I wasn't going to leave her there than they got rude and hateful seems like for the 300,000 dollar profit they make it would be ran better

My mother went in there for rehab in 2 months she was dead! They did not do rehab, kept her doped up, let her fall 3 times( that I know of) broke her hand, didn't shower her, didn't feed her, gave her no water, she was dehydrated really bad! Wouldn't take anyone here!!!! Look up all the law suits they've had!! Abuse and neglect on a scale you wouldn't believe! How are they still open!!!

My brother is in this nursing home last Sunday was my first visit sense covid started two years ago .i did not get to look around much ,but I,m concern about when family visit they can bring there pack of smokes outside and smoke any money they want .but if a a working brings them out they get two smokes and go in .the patient don,t need a hole pack of smokes to smoke if family are with them this concern,s me over the patient,s .and one more thing I do more but it smell bad out on porch where there smoke CLEAN IT Up BETTER. I love visiting my brother .

If I could give this place no stars I would. FYI- This review is just a tad of what happened to my mother. This place is beyond AWFUL Do your homework before you leave your loved one here. I wish I had known all I do now bc I NEVER EVER would have left my mom there. My mom was in there a total of 8 days and passed 3 days after being admitted to hospital. On Day 6 of being in this facility she was so out of it I could not wake her. Day 7 she could not move any part of her body. They blamed it on a UTI. I saw a bruise on my mothers foot, nurse Y tried to convince me it was an old bruise. I knew it wasn't bc nurse K said she had found nothing on my mama when she did a head to toe exam on day 1. I took a photos. Records show Nurse Y knew about it , she even called me from her personal cell the same night that this accident happened ,Not 1 time did she mention.. My moms food was suppose to be mechanical soft and they sent enchiladas .( I have a pic of that as well) If one needed assistance feeding themselves or drinking they would literally die from dehydration. I would not refer this place to my worst enemy, if I had any. This place needs to be shut down and nurses need their licenses taken away. I do NOT know how anyone working here can close their eyes at night and sleep.

I could add photos, I have a few that shows the absolute neglect of the patients who needs them the most. You can bring it to the attention of the supervisors, show them the pictures of the neglect and they just shrug their shoulders, agree with you that it’s not right, but do absolutely nothing about it. If people treated dogs like they treat their patients they would go to jail.

If I could leave this place less than one star I would. I won’t say all, but MOST of the staff are terrible. They do not care about patients, they are rude to patients and families, and they don’t follow doctors orders. My dad ended up with two life threatening conditions that could have been prevented most likely had this place done their job correctly. They are lazy and they don’t clean the rooms or the patients. If you voice your concerns, you will be met with “if you don’t like it, we can give you a referral to go somewhere else.” That was from the head nurse. She doesn’t need a license. These people will just look at you if you ask for something or tell you to get it yourself. I was told to get my dads tv fixed and his ice water myself on multiple occasions. If the patients pushes the button for a nurse, it will take them more than an hour to get to them 99% of the time. If you have family here that you love, PLEASE take them somewhere else.
