Beautiful hidden gem! Stand alone memory care community is so hard to find. They love their residents and I'm so glad to be a part of the team!
It may be easier to point out the positives, rather than publish a novel about the problems at this facility. When I first walked in, the entrance smelled like a skunk… if you know what I mean… Our tour guide was rude and pushy, constantly cutting me off during our conversation. It came off very desperate. Aside from the employees, the building was not kept up. They were renovating one section during our visit, so maybe things will improve. However, I will not be returning to the Pinnacle for as long as upper management stays the same.
I like the fact it is designed for people with memory issues. It would be an improvement if visitors were given a daily code to exit and enter the fenced in outside space. I have and do highly recommend, it's a great community.
This facility has very poor management starting from the top. They were unprofessional during our visits and seemed to have no control over the community. I felt bad for the residents that had already moved in.
The pinnacle of Louisville has been a god send. My mother needed a safe and loving place to transition from assisted living to a full memory care community. The director and all of the staff are warm, kind, caring, and committed to helping the resident as well as the family grappling with this difficult journey. I’m very happy to say that Mom has improved in her connection with people and sense of calm and happiness. Something she struggle with before we moved to the Pinnacle. I tell all my friends and clients how wonderful our stay is going. I am confident as we move forward Mom will be in the best hands possible and that is a relief and peace for me and my family.
Highly recommend this place! My dad has been here as a resident since early 2023 and we are so very pleased with everything! The staff are warm, helpful and knowledgeable. They offer support for families and caregivers as well. The nursing team is especially kind, and very committed to my dad's dignity, health and his well-being. They have a variety of activities, entertainment and events at all times of the year, and always extend an invitation to us as the family. Management's doors are always open, but you're more likely to bump into them in the hallway or rec room on visits to the facility. They are currently doing some renovations, which speaks to their overall commitment, from facilities to activities, to provide the best experience possible for my dad and all the residents. We are over the moon with the choice we made, and would recommend The Pinnacle of Louisville to anyone who is looking for a place to care for their loved one. We are so grateful to the people who provide my dad day in and day out with the high level of care that everyone deserves. Thank you!
They have about an 8/1 ratio of residents to staff. Many of the caregivers are very attentive and care for their residents but they cannot keep eyes on every resident at every moment. They are required to check on them every 2 hours but it is much more frequent than that during the day. The staff and directors are very knowledgeable and provide comfort (hugs, pats, and good listening). The directors are available on weekends and return my texts/calls promptly even when they are out of the building. They are understanding, show compassion to family members, and work with us as a caring team to find solutions to challenges. The facility has recently remodeled one whole wing and replaced carpet with nice looking wood floors. This helps keep the halls cleaner. The common rooms feel like a home with living rooms, cozy dining area and lots of natural light. The light is soft and ambient, not sterile. The man that helps with maintenance is kind to help with many jobs such as hanging TVs and picture frames, and moving furniture. There are thumb locks on the resident doors if a resident wants to shut the door and lock it (because it's difficult to keep other residents from coming into others' rooms and touching/taking things) but the caregivers prefer the doors open and residents to not be in their rooms all day but to participate in activities. The caregivers have ability to unlock doors. There is a very nice walking path outside but a family member must accompany the resident. Each resident's room has its own heat and ac. Another one of the best things about this facility is that they have nurses there on staff. So it is easy to be seen without having to leave.
The Pinnacle of Louisville has been a great home to my grandma. I try my best to come and visit when I get the chance and it’s always been a 10/10 experience. The staff is always on top of things and truly loves their residents.

Overall the staff is very nice and caring, they took the time to get to know my mom and understand her needs and personality/behaviors. The care services provided meet my mom's needs regarding nursing, general caretaking, and specialty services (Speech therapy as an example). The cleanliness is okay but could be improved. I've noticed the microwaves appear to never have been cleaned, the public restrooms are often not as clean as I'd like, the hallways smell like urine at times. The other area that is just average is the food, it is okay but not fresh and very full or preservatives. I would not choose to eat it if it was offered to me. It is a good value for the cost and the staff truly care about my mom and her well being.
My mother has been at the Pinnacle since January of 2024. After mom’s diagnosis of Alzheimer’s my sister and I researched numerous places because we wanted our mother to receive the best care. We tried caring for her but she needed more than what we could provide for her as her disease progressed. We learned a lot from this process. After having mom in two other communities that catered primarily to their assisted living residents and the memory care residents were not a priority. I highly suggested if your loved one needs memory care “The Pinnacle of Louisville” is your selection. Not only do they provide the care your loved needs they do it with so much love! The love and dignity they provide means the world to our family. The disease is so heartbreaking yet the Pinnacle gives us so much peace. Momma is always smiling, something she never did at the other communities at the other places!!
Our mom is loved by all the staff. For the 1st times in years my sister and I can breathe. Hats off to every staff member from the director, receptionists, care staff, nurses, life engagement director, house keeping. Every employee works together for the good of the resident and their families.
Our family member has been a resident of the Pinnacle for 2 weeks today. He has adjusted very well. The entire Pinnacle staff from nurses, to care givers, to office personnel, to activity director, to receptionist, to facility manager are warm, caring and friendly folks. They encourage family members to visit often and at anytime. We do. And we are impressed. Thank you Pinnacle. Keep up the great work.
Fantastic memory care community filled with wonderful employees!
It was beneficial that the facility cares for memory care only. The staff was great. The cost was fair. Room sizes were adequate - not large, but I don't know that memory care patients need the large rooms. Not a lot of amenities, but again - not sure these types of patients need much.
My loved one was previously a resident at The Pinnacle of Louisville and the overall experience for her at this facility was okay. The food was not good. The friendliness was good. The Pinnacle of Louisville was a nice place, but we had issues with the care and with the caregivers.
I think everyone there genuinely cared about my mom. It is a 100% memory care facility. It is small. It has amazing outdoor living space. The activities are totally designed for the residents. They celebrate every holiday and more with fun activities, parades and parties. The staff communicated well with me on a regular basis as requested.
I sing regularly as well as visit a resident at this community weekly so I am in the building often. Every single staff member, from the caregivers to Maintenance to the Executive Director is so kind and genuinely happy to be there. The staff love the residents, they aren't just there to provide care and do a job. You can go to facilities that are bigger and newer and offer more services but when you are looking for a place for a loved one with cognitive decline those are not the benefits that they need, they are benefits that cater to YOU and what you would want. Those with memory impairments need an environment that will encourage interaction, provide loving care and embrace your loved one with warmth and affection no matter the circumstance. They need a home filled with love in the same way that we need it as children. You will find that at The Pinnacle. Don't take my word for it. Go visit, you'll feel it.
Excellent service and a good place to spend time
The Pinnacle really cares for their residents. They treat them just like family. They have nurses, LPN’s and RN’s, on staff 24/7 as well as an 8/1 caregiver/resident ratio. One of my favorite things about the Pinnacle is the huge outdoor area they have for their residents. Residents are encouraged to go outside, whenever they want, to garden or just get some sunshine. It’s a lovely place.
My stepmother was a 13-month resident and I couldn't speak more highly of the care and culture at Pinnacle of Louisville. The Pinnacle team, from administration to care staff were exceptional in caring for my stepmother and supporting us as a family through a very challenging season of life. This was my first direct experience with memory care and it was a very good one. My stepmother received personal care including a variety of therapies that improved her quality of life. Pinnacle is more than a facility they are an extended family. Thank you Pinnacle team!
Always short staffed and the cook shalonda has a very bad attitude. Dean is an excellent cook. Grace is very attentive and wonderful to talk to. Teresa is a good worker. Found Ann the nurse sleeping on the job. Need better management at this facility. Also Shawn is not a honest person. He smiles and tell you one thing and then rude when you meet with him with a complaint. I saw a patient fall to the floor and the caregiver just sat there and said he does that. Dangerous place to leave a loved one. Improvements has been made since 3 years ago. Food is delicious. Place is clean. Staff is friendly. Still need to work on the caregivers and nursing staff. Keeping you from getting 5s.

Care staff and nursing are full of compassion and love on the residents daily. We try to do activities that are “out of the box” so our residents are always offered a variety of things to do. I love my little work family!!
Pinnacle of Louisville is a wonderful facility. We are so happy we moved our mother to this facility. All the staff from head down are so caring & loving. Price is reasonable compared to others & benefits given are well worth it. I highly recommend you consider Pinnacle for your loved ones. You won't be disappointed
Have been so helpful in the care of my grandfather. Constantly pleasant, kind and feel genuine. Have given helpful advice and made the entire process so much easier. I felt like I wanted to say thank you by recommending them to others who might not be sure where to try.
The care here was terrible. It looks great, but if you dig beneath the surface, the employees do not take care of the residents. They are very lax about checking on the residents, and they do not call 911 or doctors when the residents are sick, unless it's very dire. One aide told me that they're specifically told not to call because it looks bad on their record with the state. My mom went into the hospital with a raging UTI and a fever and was discharged the day before Thanksgiving. The director of nursing at The Pinnacle promised me that my mom would receive very good care and that rehab - as the hospital recommended - would not be necessary, that they would do the work to get her back into good shape. I said okay, she's familiar with the setting, so let's do that. I didn't visit my mom on Thanksgiving Day, but came back on Friday, the day after. My mom was still wearing the exact same clothes as on Wednesday, and still had the same tight band on her arm where the IV had been removed on Wednesday at the hospital. They'd moved her nightstand close to her so that she could reach her water. No one had been checking on her! She'd soiled herself. It was horrible. I immediately took steps to remove her from the facility. No one should be treated like that. Do not bring your loved ones here.
My mother was here and while it was fine at first, I had to move her after several months when the directors kept quitting, and the staff became full of temporary workers. After 7 pm there was no one to care for the patients other than temporary aides, and the residents would be scared and hide in their rooms and not be able to get help. My mom was terrified at night and the other residents told me the same thing. My mom would fall and lay on the floor for hours at a time before anyone would find her. Sometimes she would be in the same clothes for three days at a time before anyone would change her clothes. I moved her to a new memory care facility and she's so much happier and better cared for.
This is a great place for patients with memory care issues. Staff is very involved in keeping patients active and invoked in different activities. There rooms are very clean as is all the common places. Staff is very welcoming to family and provide updates on how your love one's day is going. You really get great care for your loved one. I highly recommend Pinnacle, my Mother came from another facility where she was basically laying in bed all day and could barely walk, which she could do when she went in. This she was moved to Pinnacle she has come alive again! She is like a different person, she is active in activities, she is eating better, just can't say how much she has improved!

The staff is always delightful and helpful. There is a wonderful music person ,Danny, who sings beautifully and entertains daily. I recommend this highly. My brother is rehabbing here and enjoys the music sessions very much since music was his first love. There is a very good beautician who gives hair and a shave.
My mom has moved into The Pinnacle of Louisville. Everybody was very pleasant. They seem very professional and very caring. The facility itself is wonderful. They have all kinds of things to keep the patients occupied. They have singing and they play games like cornhole. People come in from different churches and sing. There's an activities coordinator; he sings and allows the patients to do karaoke. A lot of them participate in that. So far, it's just been wonderful. They keep these patients occupied. That's so important with memory care to keep the activities just constantly flowing. I got my mom a studio. She's more comfortable in something warm and cozy. The staff, I can't tell you how wonderful it was. It's just unbelievable, even before we took mother there. During the moving-in process, they came and helped us unload our cars, move the furniture, connect televisions, and even helped me put the bed frame and the headboard. This place just bends over backward to help. The lady even said that they want us to treat them like family. So far, it's just been an awesome experience. It was excellent. The games are part of physical therapy, because doing cornhole, helps them with their balance and coordination. There are so many things they do to help the patients, but the patients think it's fun and games. Like rolling a ball across the table, and they have a little basket. It's kind of like table tennis, only they catch the ball in the basket, so they don't even realize they're exercising. They're moving from right to left and you can do it sitting down because some of the patients are in wheelchairs. They have a courtyard, and it feels safe. They have a ten-foot privacy fence around, and it's kind of built-in a half-moon. So, all the rooms go out, and all the hallways connect, kind of like a starburst in your mind. One end has two or three big double doors that you walk out. When you walk outside, it's just lush green grass, patio furniture, and benches. Now some of this stuff is starting to come up because it's spring. When everything finally takes hold and starts blooming, the gardens are going to be phenomenal. If my mom gets tired of one view in the area, there's like four or five different doors she can go out to, and it will have different patio furniture. As soon as it gets warm enough, they're going to allow the patients to plant. They're going to ask each resident what they like and then let them plant so they can go out every day, water them, and pull the little weeds. It gives them an activity to participate outside and get fresh air and sunshine. They have all kinds of bird feeders out there. They just have it so nice for the patients. They had a beauty shop there, and all the doctors, and nurses. They have all these people that come in so that your loved one doesn't have to leave. It's a nice place. I haven't tasted anything myself, but I asked my mother, and she says the food is really good. She's eating more there than she was eating at home, and she's pretty picky. They don't just fix a bunch of food. They fix a nice meal every day and you can go up and take what you want. They have other options for you like sandwiches, salads, or fresh fruit plates, if you don't like the actual meal.

We have recently placed my father, with Alzheimer's, at Park Louisville. We did a lot of research and visited four facilities in total; we cancelled even more visits we had scheduled once we visited Park. Park just 'felt right' and seemed very homey. It is also a Personal Care Memory Care facility, which is what my father requires, rather than Assisted Living Memory Care. The rooms are adequate and the perfect size and the facility layout on their 2-acre campus is great; the outdoor space was a real selling point for us, as my father feels most at home in nature. There is also a veterans' unit there which was very attractive for us. From Kieleigh with whom we initially met, to Melissa and her nursing team to Danny as the memory care director and finally, to Hagit as the business administrator, all interactions with staff and the team have been positive, personalized and warm. Our experience with Park has been professional and positive and we look forward to my father's thriving at the facility.

There needs to be more consistent communication about loved ones, staff, changes, and management.
I have seen some improvements in the last couple of weeks with more consistent staffing but I continue to feel like there is very limited communication between nursing shifts and staff and getting information to family members. It often seems the right hand doesn’t know what the left hand is doing and no one wants to take on the responsibility of making sure the patient’s needs are met. Having a family member in a memory care unit is so difficult on the immediate family members. They can’t depend on what the patient tells them even when the patient can verbally express some things. Communication has got to improve for meeting the patients needs and the family needs. We put our love ones in the care of the staff and we need to be able to depend on them for supportive care and enhanced communication.
The staff has been great to work with. This is an exclusive memory care community. I don't think there is a better place for memory care residents, than Park. There are residents that are at different stages of their walk with memory care.
I was not really fond of this community. The tour was fine and the person that lead us around was great and very attentive.
One of the things I have noticed is that the first day I was there they sent residents in for lunch, by the time everyone was seated they sat there for almost an hour before the food was brought to them with no drinks. When they were served they were on very cheap paper plates and plastic silverware, so I do think the food service could be improved. I know they are short staffed, but they do really have some great staff members that work here as well.
It's a nice, friendly place. They were great. I would highly recommend it for a family member. I haven't had any issues with the staff. I didn't have a chance to try the food, but it always looked good. All these types of places are expensive, but Charter's cost is about the same as anyplace else.
They still owe me a refund and I have not received it. I took my father to the hospital because he was dehydrated. The staff was friendly but the CNA's did not chart liquid or food intake. My Dad was lost one time in someone elses room. I never saw anyone mop or clean the rooms or bathrooms. Food SUCKED! i am mad that I never received my prorated refund.
We toured this community and the tour was very good. They answered all of our questions and were very informative. They were very accommodating and the staff was very helpful and friendly.
