Very nice place for your loved ones. Your family gets treated with respect and kindness along with making sure they are took care of all the way around. The activity aides go above and beyond for residents to have activities and games to do for the residents and the other staff members make sure that each and every resident has what they need or want. Very family oriented facility where they treat everyone like family.

Don't bring your love ones to this facility. They dnt do their job correctly and are very rude with patients and family members. My dad was out there, and they said he refused therapy and they just let it go and wouldn't give him his therapy he needed to get better. Also they had him sitting in his wheelchair all day except for whwn it's bedtime and his legs were swollen and ankles amd they dis nothing but ignore it. I hate this place and again they won't take care of your loved ones well at all. STAY AWAY FROM THIS FACILITY.

I wouldn't recommend you to bring your family here. Half of the CNAS do not take care of the residents. I was an employee and I was bullied by management and other CNAs. The DON herself has a assault charge and as a nurse legally your not allowed to work in healthcare ever again. Yet shes still the DON. The nurses do not care about the residents when they are sick because they are too lazy to do paper work to send them to the hospital. Residents have died because nurses refuse to give correct care to patients. Management accuses you of every they can and make it look like your fault. This place needs to be looked into and shut down. They will accuse you of stealing residents things and management and nurses are the ones who hide the stuff to make it look like you did it. They accused me of things that I wasn't even there when it happened. Nurses bullied the cnas and if you turn in a nurse for what they did wrong management will believe the nurses over the CNAS. Tim George is the owner. I will be calling the upper management and state. I witnessed a resident fall and they got hurt and the nurses said they were fine and won't report it.

My mother had multiple admissions to the Village of Lebanon, LLC. She was admittedd for rehab in 2015 due to a fall and broken hip. Thankfully she was able to return home for two years after their wonderful care. She did great with rehab and met her therapy goals under the direction of the Rehab staff. She returned from 2017 through 2023 for long term care after we were unable to care for her independently at home due to dementia. It was such a blessing to be able to trust her care unto these people. I must say she was NOT an easy patient to care for as the dementia ravaged her mind and made her strike out physically at her caregivers. They were always kind, respectful and worked to make sure she was taken care of. She never once had a bed sore! I highly recommend this facility and these caregivers.

My dad was sent to the Village briefly. His care was pretty good. The facility was clean. The staff responded to his call within a few minutes, some longer than others. Everyone was friendly. I had no issue until he got worse and then released home on hospice. The family was called for a meeting to discuss care. I show up and the lady up front, who was a main nurse in his care, was too busy looking at and buying Avon to acknowledge me. After standing a few minutes, I finally had to interrupt to ask about the meeting. The day of his release, we were told everything would be in order and his transition would go smoothly. Ok here's where the main issues come in. He was supposed to be released after breakfast, the staff didn't call rescue till noon to pick him up. His prescriptions were wrong and by time he got home, it was late Friday and nobody knew anything. It took over an hour and a lot of calling from the pharmacy to finally get his prescription right to be filled. When the nurse at the Village was questioned about hus meds that morning, we were told "I guess he was given meds this morning." They had closed his chart. They didn't seem to care he was sent home with wrong prescription and only asked if Tylenol was ordered. His discharge was a total nightmare from this facility. The staff need to be more observant to what's going on with discharge and not concerned about buying Avon when family of a dying patient is there trying to find out what's going on.
