My Dad came to The Willows following a short hospital stay. He was meant to receive rehab treatment that would help him regain his strength. My Mom spent most of every day with him to ensure he received proper care. She noticed immediately she had to advocate for basic things like making sure he was eating and that someone was changing his diaper. He developed a bedsore on his rear need within a few days due to their neglect. He seemed to be getting weaker and weaker rather than stronger. Nobody in the facility seemed concerned about that. His nose was running all of the time and he started holding his head down. He became unresponsive and my Mom had to seek help from the nurses. They came in and checked his oxygen. It was dangerously low so they put him on an oxygen machine. When we asked the nurse if he needed to go to the hospital, she became defensive. She asked if we wanted him to go. We are not medical professionals so we were seeking their advice and instead of answers we received hostility. The head nurse came in and we asked her if he should go to the hospital. Instead of giving us advice or recommending we talk to a doctor, she listed off her credentials and job experience. She told us his vitals were stable and he would be fine, that they would keep an eye on him. The next morning when Mom checked in with the nurse on duty, the nurse from the night before hadn’t even entered any notes about what happened. Within 2 days, Dad became unresponsive again and had to go by ambulance to the hospital. Within 3 weeks he passed away from pneumonia. I can’t say that he would still be here if he hadn’t gone to The Willows, but I can say that he suffered a lot and needlessly because of the terrible “care” he received. Don’t be fooled by how nice the facility looks or the nice area it is in. The staff here do not care about your family member.

I was in the rehab section for 2 weeks and it was a real disappointment from many standpoints. The light switches required that I get out of bed to turn lights on/off. Two evenings I was not served dinner, no one seemed to know why. One of the meds ordered by the surgeon was stopped for no apparent reason, causing complications when I got home. The staff tried but there are not enough of them. The PT/OT staff was not up to par. I could go on. I have been promised a call back from Trilogy but it never came. I would not recommend this for rehab.

My dad is a resident of The Willows at Springhurst. It seemed to be a really nice place. They had everything that he would possibly need. It wasn't my first choice, but it ended up being a good place after we found out he couldn't go to the first place. It's a private room. The salesperson that sort of gave me all the information initially was very helpful. She was nice to deal with. The staff kept me updated whenever there was a change. They'd done a good job of that. They have a schedule listed every day depending on the condition of the residents and what they would be interested in. They've got some things going on and they seem to have good food. I mean, he liked the food, and he was on sort of a special diet, but he said the food was good.

My Mother went to the Willows at Springhurst for rehab after a hip replacement. She was there for over 30 days and went from a walker to a wheel chair and lost about 20 pounds....the facility gave me one feedback meeting that I insisted on and at that meeting told me they were releasing her the following week which gave me little time to find a new facility...she passed away two days after leaving the Willows.

Made a delivery here and nobody was up front. Walked right in the building all through the halls and not one person that worked here was around! It was harder to get out than it was to get it! Such a nice place but definitely not secure!!!!

I went to The Willows at Springhurst a year ago. I liked it, it's nice. The staff was knowledgeable and very friendly. The apartments were very nice. The upkeep was very nice. The dining area was nice, easy to move around in wheelchairs or walkers. I didn't like the cost, it was high. It's not far from where I live now, which is pretty close to my daughter, and you can move around there and walk around there. There's no reason to leave, except for my medical appointments. Pretty awesome, haircuts, pedicures, and manicures, they have that in there. If I do need to go someplace, they have a van that can take me.

My father was in for rehabilitation for three weeks. The facility was understaffed which left my father waiting hours for needed pain medication or bath room assistance. I swept room and cleaned room when I visited. Nurses and aides were friendly but had more patients than they could manage effectively.

The staff is always super late coming in on first shift and they are always late on third shift as well! First shift is supposed to be there by 6:00am they don't get there until 6:40am and 7:00am and third shift is supposed to be there by 10:00pm they don't get there until 10:30/10:45pm and they are never scolded when they are this late coming in it's very unprofessional and very lazy of staff members who are supposed to be caring for the elderly.

Husband was at The Willows from June 11 to June 29, 2022. This facility is fabulous and everyone was gracious, helpful, friendly, caring. I would like to reach out to everyone involved in his care and all the great people behind the scenes who I did not meet. We are thinking of selling our home and moving to The Willows.

My mom is in The Willows at Springhurst. I love it there. My mom is in the memory care center. They have activities, wonderful meals, and good caregivers. Everything has been wonderful. All the rooms are private rooms with their own bathroom. They have physical therapy on the grounds. I especially like the day shift crew.

Since Trilogy Willows has taken over it has been a disaster. If I could rate 0 stars I would and I never leave reviews for anything. Some of the issues my non-communicative wife is dealing with: Rooms need to be cleaned better and get pest control, ants found crawling on her. Billing issues for several, several months now, cannot get a response. They are overcharging on every invoice, medicaid is now breathing down our necks. Restorative therapy is required and not she is not getting it. Sheets need to be put on the bed! We cannot even get sheets put on her bed, even though we have sent in our own! Quality of life has deminished so much, it has caused irreversible effects. Has spent so much time in bed, cannot even fit into wheelchair anymore. These are just a few of the issues...take it from me, don't send your loved ones here and if they are already there, check in often! If anybody answers the phone, that is. Will get pictures next time we are allowed in.

My husband is disabled and incontinent. Was admitted post hospitalization into The Willows at Springhurst for physical therapy. Was put into a quarantine????? He has had both Covid shots and a negative Covid test before leaving the hospital. THEY SHUT THE DOOR AND FORGOT HIM! Meal delivery, medications and changing his urination pad were nor dealt with !!!!! AFTER TWO DAYS, I WENT AND GOT HIM OUT……….HE WAS STILL DRESSED IN PREVIOUS DAY’S CLOTHES AND SOAKING, SOAKING IN URINE …….TOTAL NEGLECT…….DISGUSTING

The facility was great once upon a time until poor upper management started hhappening. Daniel Parr is rolling over in his grave, who was the original founder of Parr's Rest on Cherokee rd. So sad. The place has went to the dogs in the last year. Now managed by Trilogy health services, it is much worse. I wouldn't recommend anyone to work there or bring there loved one to live. When I resigned my the residents seemed so sad and depressed. They constantly complained about the food which was understandable. They always ran out of food and some went away hungry. They also were having trouble getting money from their own accounts. This shouldn't happen. The residents pay a lot of money to live here.

The nursing staff was mean and rude. I could never get a call back when I left a message. My father was medicated so he slept a lot and they wouldn't have to take care of him very much. They answered the button for help at least 30 minutes to (more than likely) one hour. My father was left by himself on a wing that had few patients a lot of the time and it was very difficult to get any questions answered as no one returned phone calls or no one was there to answer questions. I never knew they had activities as my father was never offered to go to any.

My mother was a resident for over 3 years . The staff comes and goes frequently . They are loving compassionate individuals but I’ve been told by them and witnessed first hand horrible upper management guidance . They use a lot of agency CNA staff and LPN Staffing . Most of the time when I addressed my concerns they seemed to listen and try to address but lack of staff never resolved issues . During on visit a CNA was telling the DIRECTOR of NURSING about a resident who wasn’t happy about their new room because rooms had been reassigned . Her response was t” What do they care , they are a Medicaid residency . I addressed this with director and he said he would talk to her about talking where residence families and residences could hear . My response was if this is how she felt she was in wrong profession and nothing was ever done . My mother has recently passed and I went in facility the following day at 11:00 and was told by manager over house keeping that I had to get my stuff out and they needed to clean and have room Ready by 5:00 that day . Showed zero compassion and strictly business . My husband said can you show some compassion she’s just lost her mother and she threw her arms in air and said I have to have this room for next resident . Activities dept is 5 Star and the few personnel they somehow hang on too are 5 Star but agency staff just doesn’t know care plans for residents and its vital to prop r care .

If considering this facility, I would suggest you keep looking....unless you have the time to be present with your family member 24/7. Even the staff speaks poorly of the management. P.T. was good, decor was not uplifting and very dark, aids seemed confused and over worked, and very loud. When I asked who was in charge, I was told Baptist Homes Inc or BHI ....but finding out who or what this organization is is impossible. It's as if they don't want you to know who's the responsible party and I'm Baptist and attend one of the largest Baptist churches in the area. My pastor doesn't know who Baptist Homes Inc. is or how to get to the bottom of things... This was a difficult time and I wish I had found a facility to partner with me on health care for my family member, but it was a battle every day.

My mother has been there for 8 days now. Meals are never correct. She complains that the noise level at night is unbearably loud thereby disturbing her rest. Nurses and aides seem lackadaisical about the care she is receiving. When she queried a nurse the second day she was there about what medications she was given the nurse told her it would be a while before she could get to her chart and look it up. Fortunately I used drugs.com to look it up so we knew what she was receiving. I have witnessed what appears to be an understaffed facility as she has had to wait for close to an hour for someone to assist her when she rings the call button. Rooms are sparse and cold. Beware the nicely decorated halls and dining area....what good is that if no one cares whether you get assisted to the bathroom!

This place is wonderful! My aunt has lived here for about a year and a half (maybe closer to 2 years) and loves it. She has made some new and now very dear friends, she loves the staff and they love her. It's home to her and that makes me happy. Our family couldn't have asked for a better place for her. My aunt and I toured many facilities before we came to Parrs and they had everything that was on her check list and even though the prices may seem high, if compared to other places, it's by far the best value. As for the staff and I mean the entire staff, they all care about their residents and I can't say enough good things about them. They are all a blessing to my aunt and to my family. You won't find unhappy seniors parked in wheelchairs at this place. You'll find vibrant happy folks who are engaged and who are encouraged daily by the loving and devoted caregivers who all seem to genuinely love what they do. Some of the regular activities include: ice cream socials, bingo, monthly birthday parties, holiday parties, live piano music with sing alongs, shopping trips, trips to restaurants and there are several groups who like to get together for regular card games. There's too much to mention everything. It's worth taking a tour and enjoying a complimentary lunch.

After visiting there and talking with admissions, I planned to go there after my knee replacement surgery. When it came time to go...they wouldn't accept me. I'm still not sure why since I am now in another facility and receiving great care. Why would they tell me that I would be able to come there and then deny me admission? I don't trust them and will look into this further and be sure it is known to appropriate persons that this has happened. It was extremely upsetting as I thought that I would be unable to get any care at a rehab facility. BEWARE!

The physical Therapist are the Best! Love them! My mom has been there over a month! She broke her ankle in 3 places and was non weight bering. The Nurse on duty made her stand to be weighed her first day. I have you name but won't post it. They have a wheel chair scale that wasn't used? My mom did as she was told because She was ordered to it trying to explain the situation and knowing it could do damage and put her back in surergy because cast wasn't on yet. Surgeon wasn't happy.Btw, he is a well know surgeon in Louisville. He was informed about it. The Cna's are awesome except for two and they will be reported to administer. I did a lot of work, giving her showers, taking out food trays, taking care of dirty linens ect...., I'am going to give full report. And for the cna that rolled her eyes behind my back....,I saw you! And I'am paying for that??? Trust me you are in the Wrong profession.! I asked for a nutristionist and counselor, never happened! Poor management!

My mother (85yo) was placed at Spr'hurst Health & Rehab for 3 weeks. She was there for PT due to falling. During the first 5 days, I personally discovered that her Emerg call button was 5 feet from her bed while she was in bed. Since I was present only a fairly small percentage of the time, I would have surmised that this situation occurred on more occasions than I observed. She was directed to not leave her bed without assistance, but the apparent negligent staff made it impossible for her to call them whenever she would have needed them. One nurse (RN) could only tell me, after the second of these incidents,that it was a "mistake." My opinion of the matter, as I advised this nurse, was that my mom was not here for staff to repeatedly made basic safety mistakes. My mother was admitted there due to falls, she did not need the help of staff in creating future falls! Calling the staff on the phone was also absurd. I would estimate that out of 10-12 calls I made to this facility, roughly 2-3 were answered directly. On several of these calls, voice msgs to call me back were never returned. In short, dealing with this facility was an exercise in futility, it was like a human zoo. I am currently waiting for a call back from their Med records section to obtain a copy of the Discharge Summary. As of this writing, it has been 3 hrs on a Thursday morning waiting for this return call. I would not be surprised if the return call never came through as it would be totally consistent with my experience thus far. I had my mother go to this facility because it was close to my home. Even if I have to visit her in Fargo, ND, she will never return to this facility. My mom's doctor office, in trying to obtain faxes of info from her stay at Springhurst told me that in their contacts with Springhurst, it appeared that no one really knew what they were doing. I am glad my mom made it out...safely. PS/ As of 5pm same day, my call to Med Rec still had not been returned!
