Do not get any insurance plan administered by Lucent Health! It’s horrible! You’re on the phone constantly getting things covered!

My mother was only at Hearthstone (Legacy memory care facility) for a month. All staff we interacted with and who cared for her - from admission to her passing - demonstrated compassion, respect and professionalism. They truly cared and attended to her needs quickly becoming like family. I cannot thank everyone enough and 5 stars are too little. (Your Director Alex D is a true professional who has my utmost respect and admiration for the job she does. Truly an asset!)

Let me preface my review by stating that myself nor any member of my family has ever been a resident at a Trilogy facility. This review is solely based on my experience with the admission process. In truth, 3 different facilities were our top choices for rehab after my mother in law suffered a stroke. After touring the facilities, I felt they were clean and the staff I encountered all seemed friendly enough. I was hopeful that she’d be able to rehab in one of those 3 facilities, so she could safely come home. Imagine my surprise and frustration when she was denied at all three facilities; not because they didn’t have availability, not for lack of insurance and/or an inability to pay, but because she also has cancer and they felt her care would be too expensive. Even with assurances from the family and hospital that she was not currently receiving nor would she be able to receive chemo or cancer treatments for a minimum of 3-4 months until she had time to recover, she was still denied. I do understand that as a business they have the right to make that choice. However, I hate that an unsubstantiated fear over their bottom line excluded my mother in law from rehab and care that she desperately needs not only to come home, but also to grow strong enough to receive the chemo she needs to continue to fight her cancer. So, if you are a family member searching for rehab for a loved one, I hope that they aren’t considered “too sick” or “too expensive” to gain admittance to a Trilogy facility and they receive the care they need. I can't get anyone on the phone at this place. I was given the number so I could speak with someone regarding a issue I'm having. I've called several times and once I get past the computer directory it was nothing but voice-mail. If I can't speak to an actual human being to resolve this matter by the end of the week my next step is to contact an attorney.

My husband was at Aspen Place in Greensburg, IN. His care was very slipshod most of the time. A few people did their jobs with compassion, but most did not. The Exec. Director was ineffective at best. He was there for 4 months with a 5 day stay in the hospital for surgery in the middle. I had to stay all day everyday for him to get proper care because I did a lot of it. I am not trained in health care, but did a better job than most, and that is a sad testament. He was totally bedridden, yet often his food would be delivered without flatware to eat it with! Sometimes he didn't get the whole meal because they ran out and didn't think to ask him if he wanted something else. The nursing station was vacant during mealtime so he had no one to help him, but me. His medications were messed up several times and since he was primarily there for PICC line antibiotics that was a huge issue. The nurse lied to me in front of the director of nursing and nothing happened. She continued to lie the whole time he was there. Communication was a joke on all levels! I could write a book, but it will do no good. And please do not respond with a canned "I'm so sorry your care was not what you expected..." garbage. Don't tell me to talk to the Exec Director because I did many, many times to no avail. Don't tell me to contact any of your other people because if you'd been listening and doing better, I wouldn't need to write this complaint like many before me have done. What a sad, sad, sad example you have led.

This is a business that I would strongly recommend NOT working with as a customer or for a family member. I could take the time to give all of the details of my experience, but it’s not worth it. Just read the other reviews. Clearly they do not care about getting better, as you can see with their canned responses to the issues people have raised. I tried calling the head quarters to offer feedback and they didn’t care either. They just wanted to pass me to the compliance hotline which is an automated service. They just don’t care thus the poor overall ratings for not only the parent company but also their individual care facilities.

My father was at Violet Springs in Pickerington. The care was not impressive. Numerous issues. Would press his call light , wait 20 minutes with no response, walk the halls looking for assistance only to find workers sitting around on their cell phones . Once he left, they refused to give my Mom or I, his POAs, an invoice, said we had to complete a form and their corporate legal office would review it and decide if we could have any invoicing. Now a year later they are sending invoices with very little information.

This place is the equivalent to the smartest of the Kardashians. In other words, they are best of the dregs of rehab facilities. The staff is low level and tells lies. Worst of all, they currently have Covid in the facility, and yet fail to communicate that fact with signage at the door for guests. Myself and a couple of my family members got Covid, and I know it is from there.

I am writing this today 10/20/22, my mom is in novi lakes facility and they are understaffed in the skilled nursing care area and it is affecting care of patients or residents i was visit my mom last night and her food was left for her to eat on her table but she was laying down and the tray was set up high she could not reach it if she wanted to eat and there was no silverware anywhere if you could eat. i could not believe this. my mom can not move her bed by remote to remedy the situation due to the fact she had a stroke and has cerebral palsy on her left side . then i have been looking for her teeth which have been missing for over 3 weeks so she could eat her dinner the food they served her was not a good match for someone missing teeth it was beef stew and she could not chew it. i have asked about her teeth 3 times and they are still missing. i went to look under the cushion of her wheelchair and her wheelchair is missing i purchased a new one 4 weeks ago and there was an old one in her room and it was not hers . something needs to done these people need help. please do something about this. i will checking on her as much as possible due to the fact she need help eating and grabbing her drink . this is a sad and sorry situation. kim smith.

My mother checked into Novi, Mi campus on March 2, 2022. She left there by ambulance on April 9, 2022. The day we checked her in, the administration staff was not very helpful. To our dismay we realized they did not have her in the correct area of the facility. My mother was going to need a lot of attention. Later we realized they had no beds available for her needs so, she was put in a less care area. We were not pleased. Tim Reardon and Mary Smith called a meeting with us and they showed no compassion about the area where my mother was put, room 407. As time went on, things with the nursing staff were not to our satisfaction. She left by ambulance due to bleeding that started at 9:30am. We were never notified till 3:40pm. My mother was diagnosed with a UTI and constipation. This incident should be investigated. In closing I want to say, the therapy department was absolutely the best ever. The food was great and the activity department was wonderful! Please do your research before you take a loved one to this Novi facility.

I am a clinically certified Speech Pathologist and never again would I recomend a facility own by Trilogy in the City of Louisville KY. I realize it is Covid Time...but my 70 year old mother who has been in the hospital for 17 days ( tested negative for Covid 4 times during her stay) and has improved 90% lacking strength in her gross motor movements requiring Physical Therapy was denied BY ALL OWNED TRILOGY facilities in East Louisville including one she has stayed in before...denied by one...denied by ALL...and she had aspiration pnemonia caused by an EGD at the hospital. Not a Covid risk....improved immensely...but since denied by one facility five days ago before her great improvement...denied by all Trilogy facilities. Shame on you! This is not a Sorority or Fraternity.... hopefully soon you will care more about your patients and short term rehab residents than your Covid numbers or reputation. I will spread the message of what to expect from any Trilogy owned facilities. Thank you for the response from the owner. I appreciate being heard and willing to talk. New Review: 6/27/21 I am trying to add another review at this time. My mother is back at the hospital and being treated for an infection related to diverticulitis. She has agreed to stay at an acute rehab facility for 5 to 7 days for PT to increase mobility and stability as well as transfers. When I looked up the website Medicare.gov.zip code after her D/C planner urged me to do so from Baptist Hospital East. Many Trilogy Health facilities were highly rated. We were very very disappointed that Trilogy declined my mom to stay at Forest Springs back in late Dec 2020. I do recognize that she had Pnemonia issues and this was in the midst of COVID-19. I hope this time around when referrals are sent for acute rehab facilities, Trilogy ( especially Forest Springs) will accept her if a bed is available. She is now vaccinated from COVID-19 ( yes.....all get vaccinated) and does not need any oxygen but room air. I hope when you meet about her referral you will take all my thoughts to heart and she can again stay there. Thank You!
