I’ll tell you right now I tried being positive about this place and there are some nice girls there, but after tonight I think completely different… Who ever Jamie is that works on hall 100 is completely HORRIBLE and has the worst attitude ever!!!!!! The fact that she doesn’t know how to talk to people and not professional at all is baffling oh and let’s not forget hanging up on people!! My grandmother stayed there and was taken care by some of the nurses, but others didn’t care and the fact that they lost her favorite blanket and the only thing she has asked for since being in the hospital is CRAZY!! But they are horrible and I don’t recommend this place to anyone!!!!

(Would give zero stars if possible) My family’s experience with Greenwood Nursing Home and Rehabilitation Center was deeply concerning, and I would strongly caution anyone considering this facility. My mother is only 52 years old, mentally impaired, nonverbal, and fully dependent on staff. During her time here, we observed repeated issues including bruising, bed sores, dehydration, weight loss, and multiple hospitalizations for UTIs. These infections repeatedly escalated to the point of requiring hospital care, which is alarming for a monitored setting. A recent barium swallow study confirmed that she can only safely swallow very small amounts (about a teaspoon at a time) of thickened food, meaning feeding her requires time, patience, and close supervision. Given this, we had serious concerns about whether adequate assistance was consistently provided. Earlier in her stay, we also observed instances where her ordered honey-thickened diet was not followed. Her physical decline while at this facility was severe. Although she already had some decline before admission, she was previously able to sit upright, walk with assistance, and use her right hand more effectively than she can now. Over time, she lost the ability to sit straight, became slumped to one side, could no longer walk independently, and lost significant function in her right hand. While we understand underlying conditions can progress, we believe consistent therapy, positioning, and mobility support were lacking. Basic daily care was also an issue. Dental hygiene was neglected to the point that her teeth are now visibly grey, despite repeated requests. We also repeatedly asked for dental and vision appointments to be scheduled. My mother has worn glasses her entire life, yet no vision appointment was arranged and she no longer has her glasses—meaning she was functioning daily with blurred vision. The environment raised concerns as well: hallways often smelled of urine, call lights remained on while staff appeared distracted, and interactions with some staff were dismissive when we asked for assistance. Because my mother is nonverbal, crying is one of her only ways to communicate. Rather than consistently addressing basic needs such as positioning, hygiene, or comfort, there were repeated attempts to manage this with medication, despite our clear requests to avoid unnecessary sedation. We have since moved her to another facility in hopes she will receive better care as we continue working to determine the underlying cause of her condition. If you have a loved one who cannot advocate for themselves, especially someone young and medically complex, I urge you to research carefully and remain extremely involved. Based on our experience, I would not recommend this facility.

I could literally write pages and pages expressing my gratitude to the nurses and staff at Greenwood. They are absolutely incredible, and even that doesn't feel like strong enough of a word. These are people who genuinely care about their patients as if they were their own family members. Knowing that my dad (who had very specific medical needs) was being cared for and watched over 24/7 was such a burden lifted off of mine and my family's shoulders. The 200 hall nurses quickly became an extension of our family- they ALWAYS took the time to talk with us and answer questions, and never made us feel like we were bothering them. There were many occasions when I would stop in to visit my dad, and run into a nurse or staff member in the hallway who would say, "Oh Shelley, I'm glad you're here! I wanted to give you an update on your dad (etc)". When my dad was refusing to eat, the kitchen staff made such an effort to make food for him that he would actually eat- the kitchen manager even attended my dad's care meeting so that we could brainstorm nutrition options for him. The social services team is wonderful and always kept me updated/informed on any items that needed my attention, and the ladies in the business office literally held my hand and guided me step-by-step through the process of getting my dad on Medicaid. The therapy team also went above & beyond for my dad, and Nancy in particular took such good care of him by keeping an eye on him and truly never giving up on him (even when he was being stubborn). Even the custodian who cleaned my dad's room kept a close eye on him, and formed a bond with him. When I tell you everyone at this facility is an angel with a calling from God to care for people, I mean it 100%. I'm getting emotional writing this, as my dad has recently passed away- but I mean it with my whole heart when I say we are so, so grateful that these were the people who cared for my dad in his last months. You would be hard-pressed to find a more caring, passionate group of people. They have my family's eternal love and gratitude forever.

i been watching for awhile watching the flow of things, first off the way i heard the kitchen staff treating nursing as i sat in the dining room with my family, they are so rude. imagine what a difference it would be if everyone played their part to really help the residents, for them to be so rude to that sweet little cna, the manager made a smart comment to one of the aides seemed jokingly, but overall the way they treated them it just seem to be animosity built up, very rude to the sweet aide. also one aide runs around doing so much as aides are sitting at the desk, more accountability should he held as they are suppose to be here to take care of our loved ones. not all the staff such as the sweet nurses with the dark hair and the ones from the other hallways i encountered but im just so upset i hope and pray i will be treated well if i needed to go to a nursing home after seeing the working conditions and the unhappiness here i begin to wonder… it seems to be a lot of bitterness i wish everyone could be nice for the residents atleast, and be kind.

They have a few good nurses that are knowledgeable and helpful. They also have a few good techs that are efficient and helpful but for the most part they are not efficient, knowledgeable, or helpful. The food is terrible except for breakfast. The doctors are not good and I did not get the care that I needed. After a week I ended up in the hospital swelled up like a balloon. I would hit my light and sometimes wait 30 minutes or longer for help. They did not keep up with my pain meds. I felt like I was in prison. I won’t be back.

We never imagined needing long term care for our mother. When she needed that higher level of care, we were blessed to find a spot at Greenwood. They provided professional, compassionate care for Mom and really for us, her family, who had no idea what to expect. My Mom passed away gently in the care of Greenwood and we are thankful they were there for us through her illness.

I was there 2 weeks. Most of the staff were nice, knowledgeable, and helpful when needed. Very loud many nights with patient's doors open and their TV volume all the way up. The meals were not chosen and brought to my room. The portions were small and it was always, always cold with no place near my room to microwave it hot. I will not return to this facility!

My mom just passed away today at Greenwood Nursing and rehab here in Bowling green Ky. She had great care and I can’t thank them enough for all they did for my mom and for my family that was grieving as my mom went through End of Life care after dementia consumed her. All the nurses and staff were so kind and helpful, from Lindsey in finance nurses and cna’s. Nurse Sadie always kept me in The loop on what was going on with my mom and I appreciate that so much.all the cna’s were good but there is one young lady that went above and beyond taking care of my mom Betty. Her name is Ashley and if you have ever been in 100 hall you would have seen her caring for each of her patients as if she knew them her whole life. She is passionate about her job. I am grateful. I don’t know what the nursing home side was like but the 100 hall rehabilitation are amazing.thank you for all the care you gave my mom and my family. Catherine and Tim Cline

I bought a new car from them and took it to change the oil and they didn't put the filter in properly so a few days ago we realized that the truck was leaking oil and that it almost had no oil left.. Something so simple was not done well, but at the same time something so delicate that the job cannot be done halfway because the engine would have broken down. And even the manager was not worthy of doing his job Good.They said they could change the oil but we had to pay even though it was their fault. 👎👎 I will never buy anything from them again!!!!

My great grandmother lived here for a few years right before she passed and I remember her always telling me on the phone about a nice red head nurse that treated her with so much love and respect. I remember when she started getting close to the end that nurse held her hand right before she fell asleep at night just as my great grandfather did when he was still with us. And although I couldn’t be there when she took her last breath, I knew that angel of a nurse would be there comforting her.

I have been working at Greenwood nursing and rehabilitation for 2 years. The employees at greenwood work with passion and integrity to make sure the residents exceed the expectations of the facility. We are family oriented and do or due diligence to do their best. i worked at a lot of places, and you wont find very many facilities that pride in catering to the individuals needs. I am very humble in working for such a facility that aims at giving customer service, and as a whole is a loving family .

I spent 2 weeks here after breaking my wrist. It hasn’t been renovated in a very long time but seemed to be kept fairly clean. I had a private room with shower in bathroom.. The food wasn’t very good but got to choose what you wanted to eat from a few selections every day. . The menu wasn’t very healthy with fried steak and mashed potatoes and gravy almost every day. However, there was a blue menu where you could choose hamburgers, hot dogs, baked potato, pimento cheese sandwich, fries, etc which were fairly good. There were several excellent nurses and CNA’s including Andrea, Brittany, Felicia, Cheyenne and Mari. The physical therapy offered there is excellent. There were activities including bingo, etc which I didn’t participate in.

I don’t like to write negative reviews. However, I would never want another person to live at this place without knowing what I saw. My mother lived at this facility for approximately 2 months and I cried every time I left. After 2 months of searching we found another facility in Bowling Green that was much better and did a transfer. Let me start by saying the staff is very kind and helpful, as much as they can be. They need more employees with better training. Most often when you enter the facility it smells bad. It is worn and old and many things are not maintained. Drawers and cabinet doors are often off hinges. Ours were the whole 2 months. In November 2019, the guest bathroom looked worse than most gas station bathrooms with things broken and worn. The tile floors are so noisy as carts are pushed up and down them at all times. The carpeted areas looked dirty. The archaic use of an intercom system is so loud and disruptive to patients. It is always loud and noisy. The rooms are TINY! With two patients in a room my mother was unable to turn her wheelchair around in it. She would sit in one place all day and stare at the wall. We had to sit on her bed to visit her. She couldn’t watch tv because the room was so small the patients’ remotes worked on both tv’s in the room. This caused a lot of issues for both patients. The resolution to this problem was for them to take my mother’s remote which left her with nothing to do all day. It was heartbreaking. Food usually arrived at her room 1.5 hours after the first round of meals. It was always cold. We went out to the courtyard one day and got locked out there with nobody answering the door. Another visit I arrived to my mother sitting in the cold hallway in a short gown and bare feet in her wheel chair very upset. She had been sitting their waiting to go into the shower room but they had to wait for another person to exit. She was cold and I asked for a robe or blanket. It was about a 20 minute wait from the time I arrived. I’m not sure why she could not have waited in her room or at least been covered up. I felt like people really lose all dignity there. The bathrooms are not set up well for people in wheelchairs. The wheelchair and the patient could not fit in the bathroom with the CNA. She literally would have to grab a bar about 3 feet in front of the toilet and try to stand up from her wheelchair that was in the door way, and then she would fall back onto the toilet. She was bruised from this. The day she left Greenwood was such a relief.

I'm so sorry to give such a small rating on this facility, because "all" and every one person working in this facility were unbelievably caring, kind and do their jobs to the best of their ability. But the facility itself is primitive....one must share bathrooms, and the toilets are all low. What does it take for the owners of these facilities to come out of the dark ages and install high rise toilets and walk in showers. My gosh, why are older people and their small needs so invisible and uninportant. We are people to....and it's high time America start getting rid of the primitive low toilets and start installing high rise toilets. When you go to a rehab facility with joint problems that is the very least these facilities should have. The rooms are very old, the beds are not comfortable at all...however, they do have a large amount of nice activities for the residents, and the staff are the best group of people working there, and I'm certain it is not an easy job.

I WAS IN THERE FOR ALMOST A YEAR, TO RECOUPERATE FRO HAVING MY HIP REPLACEMENT REMOVED. THE PEOPLE WERE GREAT IF I ASKED FOR SOMETHING AND THEY WERNT BUSY WITH ANOTHER RESIDENT, I GOT IT RIGHT AWAY. AND THE FOOD WAS AWESOME. THEY HAVE THIS MENUE YOU CAN ORDER FROM AT ANY TIME. IM THINKING OF GOING BACK, AS MY CARE GIVERS NOW ARE NOT QUITE UP TO SNUFF.

Parts of this facility could have been more up to date. I loved the staff they really went out of their way to make me feel like I was family. The dining room had a very home like feel to it. I loved the rehabilitation area in this facility. The staff was very quick at answering the call lights. I loved the little lady that I sat with in this facility. She had been in the facility for about six months when I sat with her. Her staff in the facility was constantly checking to see if she needed anything, if she was clean, if she and if she was hungry. I liked to she how happy she was with the care that she was recieving. With being a health professional myself this was very satisfing to see.
