Signature HealthCARE at Jefferson Place Rehab & Wellness Center

The therapy was really thorough. The nurse Sue was very friendly, kind and helpful.

Jefferson Place is an amazing place to receive short term rehabilitation with a stay in one of the private rooms on their Ecelerated Care Unit. The leadership in the facility has been there a lengthy period of time and very knowledgeable about the day-to-day care and expectations of those served. I would recommend visiting the facility if you have not done so recently. Ask for Megan, she will give a wonderful tour of the facility and answer any questions you may have regarding the care of your loved one.

This is my 4th stay and I wouldn't keep returning if it wasn't an excellent facility. I have recommended three friends that have come here and they were happy as well. The therapy staff here are wonderful and the therapist that were here five years ago are still here. I highly recommend this facility.

TINA managing the wound vac professional care; ERIK, ANGELA, RON & DENITA encouraging the PT/OT/SLP protocols for hip replacement surgery recovery AND for stroke recovery; CHRISTY & KATIE coordinating the skilled nursing and patient care we need; ALYSSA managing the specific high(er) protein diet requirements; MEGAN managing for our smoother patient intake experience; and for AMY assembling such a good team.

Jefferson Place does a great job caring for the people that come through their door! It is by far the most beautiful rehab facility in town!

Beautiful facility with beautiful rooms and people that really do care. My grandmother recently had a fall and then went to Jefferson Place for rehab and she left so much stronger and back to her old self. Whenever she needed an ice pack they switched hers out right away. Her therapy sessions seemed to really help her get stronger. She said that the food was mostly good, staff would go above and beyond to help her, and it was quiet at night and she got good sleep. She enjoyed socializing with other patients and staff too! If she ever needs rehab again, I hope they have a room for her!

I like the cleanliness, the outside view. I love the discipline of the staff on their commitment of doing their job correctly. When I go hone, I'm going to be talking about this place and how wonderful it is! The progress I have made has been excellent! I am doing so much better and I thank Jefferson Place for getting me here! I can't say enough good things about this place and I highly recommend!!

The CNA's are overworked. The nurses, with exception to one, were unwilling to help the patients. I was the patient, and on average, I was waiting 20 minutes or longer for a CNA to come in and help me. The food is horrible, and unhealthy for people living there. I was sent here after my hospital visit, and asked that I could leave as soon as possible due to the poor living conditions there. The social worker is incompetent, I have been home for 2 weeks today and still have not had a home health care visit. This is unacceptable. Come to find out, the phone number given for me, was Jefferson Place's operator line. They didn't even give my cell phone number as a contact number. Please if anything, find someone else to take care of you or your loved ones.

I have experienced some very unpleasant skilled rehab facilities until I came to Jefferson Place. This has been the most pleasant experience. I can't walk and the people here have helped me with everything, including my confidence . There is truly a warmth here, a care and concern. It is truly the best place I could imagine. I highly recommend Jefferson Place!!

It's a great rehab facility. Staff is very attentive and friendly. If I ever have to have rehab again, I will be coming back to Jefferson Place. The food is great compared to the hospital food.

Hello, I am reaching out and leaving a review to say that I was in need of help with my husband's stay and was treated kindly by the business office manager. I was not sure what or how to get help and she talked to me and stayed with me until we got a resolution. We are still working together on his care and she has been so helpful with her time and information. It is truly appreciated to be treated kindly during a very hard time. So thank you.

The people here have been great! Therapy has been good and the meals are good as well. The rooms are very nice! I highly recommend coming here for therapy.

There has not been one person in this facility that hasn't been kind, compassionate, professional and caring! They have helped me to get stronger and more mobile. I use to work in this field and these caregivers have always been accepting of my limitations more than I have. Also a big shout out to their maintenance team who is remodeling the facility and it looks beautiful! Highly recommend Jefferson Place!

If I could give no stars, I would. My grandma was in for several days and was completely neglected. She was able to get out of bed with assistance but no staff helped her shower. She wouldn’t have clean clothing on when family visited despite having several outfits with her. She would push her call light and staff would not respond. Her last day here she had 2 falls. One I might have let slide as an accident or her trying to get up by herself but 2 is on them. She had a broken finger, an open wound on her leg and was bleeding from her head and they were going to put her back in bed until she said her neck hurt, then they called an ambulance. I don’t think they did much therapy at all with her during her stay. While my sister was visiting, she reported staff yelling at other patients and disclosing personal information in the hallway. We were not at all pleased with the way my grandma was cared for. I’m starting with this review but my family will be filing a formal complaint.

Nice staff and great care

Signature at Jefferson Place has been amazing, especially under unusual circumstances. My mother was literally transferred there from a hospital who didn't do the proper paperwork. However, the staff and administrators at Signature Jefferson Place handle that with amazing professionalism. When dealing with a loved one, decisions and choices may be difficult, but the administrators and staff at Signature Jefferson Place are literally amazing at helping you feel at home when you cannot be at home.

Attentive staff! Been to a few rehab facilities and this one is the best! I love the location, it’s homey and I love the courtyard. The therapy is great.. shout out to Angel and Jane! Would also like to compliment nurse Donna and Nick! Highly recommend!

The care is amazing at this facility and it’s super clean… probably one of the cleanest facilities in Louisville, Ky. However, the way I was treated as an employee was beyond disrespectful. I was discriminated against as a LGBTQ woman of color 🏳️‍⚧️

Awesome Rehab Facility. Denita, Eric, Chris, Ron, & Marcia worked with me on my rehab to get back my mobility & reduce my dependence on Oxygen. Nursing & CNA staff were caring and efficient. Can't remember all their names but I do want to thank everyone for the care and service my wife & I received.

Our mother recently moved to Signature HealthCARE at Jefferson Place in Louisville. To say that we are so happy with our decision is an understatement as it was not an easy decision to make. She immediately felt at home and we all consider the Jefferson Place Team family. The Maroon Unit Team is amazing - always puts her first and ensuring that she is safe and happy. The care is ok. Never consistently good. Not enough staff at all to tend to patients. Clean yes, food TERRIBLE!!!!! Physical Therapy I give a 9.

Food is pretty awful. My spouse had a hamburger and gravy for breakfast 4 times during her 10 day stay. Her sheets weren't changed until the day before she was discharged. The only 2 showers she had were provided by visitors. Staff is slow to respond to medical issues.

I love this place and I don’t ever want to live any where else! The staff is great. The nursing care you get is great and the food is good. I absolutely love the activities here! Therapy is good too! It is a very clean facility!! Everybody that comes to visit me can’t believe how clean it is and there’s no smells. I love it here!!

This was our family's first choice AGAIN when we found out dad had to go to rehab. The care and compassion your employee's show is outstanding. This is our 3rd or 4th time here and its been absolutely WONDERFUL! To not have to worry about dad's care knowing he's being treated well and taken care of is a huge load off our shoulders. More facilities need to take lesson from you

We were so pleased with the care our 97 year old Mother received in the skilled care unit of Jefferson Place. Due to our moving to Cincinnati, we needed to also transfer her to Cincinnati to be with us. Our Mother repeatedly said how much she would miss the caregivers at Jefferson Place. We know of no higher compliment or endorsement of an elder care than that! Our Mother has all her faculties and was treated with the utmost kindness and dignity possible. Thank you caregivers at Jefferson Place! We appreciate you! :-)

This has been one of the best rehab experiences for me. I saw only pleasant faces and had positive experiences. Rehab is physically and emotionally difficult and a perceived frown or disparaging remark can be devastating. Bravo, Jefferson Place! Thanks to all the staff. I am very grateful and now have a preference should I need rehab again.

I've been in the majority of the five star nursing and rehab facilities with my loved one in Louisville over the past six years and Jefferson Place has exceeded all expectations with regard to nursing care. I recommend this facility to anyone. It's also close to my home.

Absolutely horrible experience for my Mother's Hip Replacement Rehabilitation, she received zero rehabilitation and basically kicked out and sent home.Do yourself and your loved ones a favor and read ALL of the one star reviews...She's at Masonic Rehabilitation for her Shoulder Surgery and they are wonderful...

I don’t typically due reviews but felt I really needed to make a review here. I had the opportunity to experience the care at this facility and we were extremely happy. Having a family member in other facilities, we had the experience of what to accept and I can say we have never been so pleased with the care our family member received. The staff were extremely helpful and caring. You can tell this is a very well run facility. Are they perfect, no, but if you do have a concern, management addresses it quickly. Therapy was amazing and our family member truly enjoyed the activities. She got to cook when she was there and it made her so happy! If your family member needs rehab, our family highly recommends!!

This wa a very nice place 20 years ago. But I guess Signature has not put a dime into updates. The first room they took my mother to was absolutely awful. Light over the bed didn’t work, bathroom was outdated and just gross. Furniture was broken. Colors were dark and depressing. The bed must have been 30 years old! They put my mom in a different room (after complaining). It was minimally better. Same bed. I went to Kohls and bought a mattress topper to put on that awful mattress. My mother was there for Rehab after a hip replacement. Physical therapy was less than acceptable. The food was just awful. Night time snack was graham crackers. Most of the staff didn’t seem to care about anything. There were a couple of good employees. Stephanie was one of the nurses that actually seemed to care and we really appreciate her. Thank you Stephanie. A rare helpful MA. I wish I remembered one of the good ones name. Most of the time the MAs were sitting in the side rooms doing nothing. Wow I wish I could go back and have had my mother somewhere else. I should say that we met some full time residents that were very happy there. Maybe it was just the rehab program that was far below our expectations. Mom did well and got out of there asap.

Jefferson Place is a wonderful facility to stay at. The nurses and the AIDS care and it shows. I have been coming to this facility on and off for 3 year. I love the activities they have to get you out of your room and it's not just bingo all the time. I have made many friends here with the staff and the resident. I would recommend staying here for rehab.

I had the pleasure of getting to complete community service hours for my exemplar at Jefferson Place. I got to help out in the activities department, which is ran by an amazing director named Emma. The residents always looked good and happy to be there throughout my time there and the staff were very nice too! I have to complete hours yearly and plan to return to Jefferson place, if allowed to do so. I really liked the building, residents and staff. If I have to put my mom in a home it would be this one.

My five months at Jefferson Place has been positive and pleasant. Anytime I had a concern it was answered promptly by the director of nursing or facility director. I appreciate that they were so accessible. The place is kept clean on a daily basis. The CNAs taking care of me were caring, efficient and good at their jobs. The nurses were knowledgeable and also caring and good at their jobs. I benefited from the physical and occupational therapy programs both in room and in the well equipped therapy gym. Jefferson Place helped me get back on my feet and ready for my next phase. They want their residents to improve and do their best to make it happen.

I am only giving 1 star because I have to hit a star. Recently my 85 yr old mother with dementia was dc from the hospital and needed PT so I was told JP was great....WRONG. I visited her daily 2-3xs a day usually 2hes or more at a time thank God bc I usually had to do THEIR job. Communication sucks I waited 3 hrs for her night time nurse to call to be told she's good gave Tylenol she's asleep and hung up. NOOOOOO when a patient has dementia we depend on nurses and staff to fill us in on how they are that is something this place ignores. Trying to get pt reports was a joke I would be told 1 thing my other family members were told something different. One night we visited until about 930PM I asked mom if she was ready to change and get in bed she said no she wanted to finish the movie so I laid her gown on the bed told her to push the nurses button ( which they love to ignore) and walked out I stopped at the nurses station and told them she needed her gown on and help to bed the nurse or aid I told was like her aide should be down there I said there is no one in the hall there that is why I am telling YOU. At 10:55 I get a call that my mom fell he said I didn't need to come she's in bed Tylenol given guess who showed up anyway yes me he wasn't thrilled but I didn't care upon arrival she was groggy and in a hospital gown where was her gown I laid out? I told them an X-ray needed to be done bc she typically breaks bones when she falls he kept trying to say IF she complains NO she's already hurting u will order it. That was Fri night sat no X-ray sun we visited my husband asked about the x-ray he told him it was done THEN walked in moms room looked at her and said they came and did your x-ray right....she said yes he said ok and left ....SHE HAS DEMENTIA YOU CAN NOT ASK HER STUFF LIKE THAT we found out Mon an X-ray had NOT been done. Talked to assistant nurse manager who said x-ray would be done STAT well stat was 4-430 PM. There were a FEW staff I felt chose the right job who care about their patients 1 was Hailey from the long term unit she was helping moms unit mom had been there a week NO showers given she had a Dr appt mon I asked that she get one this girl was AMAZING and caring and not grumpy at helping. Stephanie and the aide who helps her were good Tiana was good and a nurse mom had at night who was blonde with glasses other than that the staff needs to be evaluated standing at desk waiting I'd hear things like she wants to do things on her own and fall then want me to pick you up I'll do that when EMS gets here bc that's who I'm calling to pick u up .....I heard another griping bc a patient paid to have hair done and no one came and got her she said that ain't a nursing problem don't bother me bc you gave them $20 and they didn't get you, you shouldn't have done it. I was there when staff pranked whom I guess is the boss boss before vacation saying state was there and they were not but I heard staff saying man she had us busting butt cleaning rooms and areas we don't worry about.....IF YOU DID YOUR JOB DAILY YOU WOULDNT FREAK OUT IF STATE SHOWED UP. My point is unless your family is able to do for themselves and of sound mind don't take them here

It is a wonderful place. The employees are very nice and helpful. They made a big difference for me with my rehab. Therapy great and nursing great. All get an A. Highly recommend.

Jefferson Place is a wonderful facility with great staff. You always feel welcomed when you walk in the building and it is very clean. They are very compassionate and caring towards their Residents. This facility is one of the BEST!

An elderly family member spent three weeks in the rehab wing and it was a terrible experience. We visited daily and generally found her neglected by the staff. It was not unusual for her be left with food waste on her face and clothes, with a pile a dirty clothes in the corner and a strong smell of urine in the hallway. She didn’t receive a bath in the three weeks she was there. The staff at the nurses station had the ability to ignore the endless signals from patients paging the desk for assistance. On her last day, we met up there with the transportation van to return her to her home at an assisted living facility, only to discover that the staff had done nothing to get her ready to leave. Her lunch arrived late and she had no utensils on her tray, so she was eating with her hands spilling her drink on her self because they couldn’t provide her with a straw. Her clothing was full of food and her underwear had not been changed. The nurses hanging out at the station did not offer any help, and we had to gather her dirty clothes off the floor of the closet. On our third trip to the van with her personal effects, someone finally asked if we needed a cart to move her stuff ourselves. For over $10,000 a month, patients should expect unpleasant food, and hours of alone time without a wellness check from the staff. If you check in a family member here don’t be surprised to find them without a cup of water, or a change of soiled clothes. The weekend nurses in particular would stand at the station and ignore the calls from patients. If you have no choice but to place family in this facility, check on them two or three times a day yourself to feed them or tend to their hygiene.

I have made at least 5 calls to inquire about a statement and cannot get a return call. Today (6/28/24) was the 5th call.

The management staff cares and so do the floor staff. My dad was placed here temporarily and I even had one floor nurse call me just to give me updates because my dad was unable to use his phone. I recommend this place. And if you’re having problems I guarantee that if you talk to the manager they will get everything lined out and handled.

My Mom went to the Jefferson Place for rehab last October when she broke her knee cap she got great care there and she had therapy for her knee twice a day she enjoyed talking to everyone there in rehab and seeing different people there, she couldn’t go back to her apartment and take care of herself in a brace plus she didn’t want to be by herself either and she needed trained helpers to help her so she was able to get a room there at the Jefferson Place and she has settled in her new room she enjoys seeing everyone that works there she talks to everyone that comes in her room and she loves going places with the staff and doing the activities they have, myself and our family are very happy for our Mom everyone there from the house keeping, nurses, the office workers, Laundry workers, and everyone else that work there are very helpful and nice to talk to Thank-You to everyone at Jefferson Place for all the care you give to the residents 😊💖

I would highly recommend Jefferson Place for rehab and nursing care! I can't speak highly enough of the nurse's and aides. I could not ask for better care and I would highly recommend this facility

In the short time we have been dealing with Jefferson Place while they are caring for our beloved aunt, we have experienced a kind, helpful, welcoming staff. The facility is heads and shoulders above her previous long-term care facility. We have been informed by multiple staff members via phone calls, texts, and emails as to our aunt's progress. She's been in JP for 3 days and has already been out of her bed more than she was in 3 months at the previous facility (which was never). Take a tour and see for yourself!
