You all sent my aunt home to die. My aunt is still in the hospital, thanks to Patient Neglect at your place. She was in a coma right after she got back from Tanbark. She had a raging Kidney infection! Just poor care from you all. I wouldn't send my enemies to Tanbark!! If I could give a rating of zero, I would have chosen zero.
My father was in rehab here. He did not get bathed for over a week. Complained 3 times to nurses and staff. Had to call and complain to director of nursing and CEO. Will never refer anyone to them in the future!
My mother was here for a month. Left in a bad mental state, horrible UTI and bacteria infection, very low thyroid levels that landed her in the hospital again 24 hours after being discharged from this hell hole. Communication was horrible. Under staffed. I had to complain about them saying she was getting showers when she definitely wasn’t. Half of the staff are techs. The nurses don’t care. And my mom has been in the hospital since being admitted back. If I had it to do over, after the first red flag, I would’ve did more. Do not recommend this facility to anyone.
I highly recommend Tanbark Rehabilitation and will go again, if needed, without reservation. I was at Tanbark Rehab following a shoulder replacement. The staff is wonderful! They helped with everything I needed. I was unable to shower by myself and the assistants that helped me made me comfortable. The nurses were professional and caring. The rehabilitation staff were amazing. The PT and OT staff were knowledgeable and engaging. I looked forward to that part of my day. Everyone I meet from the doctors to housekeeping, front office, administrative staff and dietary staff made me feel comfortable and cared for. My only complaint is their medication system. Their pharmacy is in Louisville. Enough said.
My wife was in the Lexington Kentucky facility for close to 25 days. She was given too much of one of her meds which kept her so messed up she wasn't able to do her exercises . She had two utis which almost killed her. The second one was so bad that I called an ambulance to take her to the hospital . I then took her out of the facility. She stayed in the hospital for several days and they billed me for some of the days she was gone. Since her stay was over 21days the rest was my responsibility. Her care was so bad that myself and another family member stayed around the clock with her.
Dad was at the Rehab facility, only two people cover the Rehab facility at night, they have 40 or so patients and many with memory issues. Food is all processed, frozen from a box, I imagine jail food is better than what is served here. Memory care patients are placed in wheelchairs and left to roam around the halls or are placed outside the front entrance on warm days. Sometimes they get a cup of water and a staffer member might check on them. I had several ask me for help because they had no way to call for help when they were placed outside. COVID protocols are not enforced, several patients have COVID and staff members don’t wear masks, don’t sanitize their hands going room to room. The night supervisor is arrogant and dismissive of patient needs. On paper they check the boxes but in reality it’s more like a barn with stalls to house the patients and worse food with no nourishment. Sadly many have no other choice, thankfully my Dad did and he is thriving in his new nourishing clean environment. I lose sleep thinking of those still there. Several working staff members like nurses, and techs did their best to take care of Dad with the severe staffing shortage, they are sick of the conditions there too. In two weeks we say a lot of turnover with staff.
No one is on the same page . can't get Dr. orders correct . Takes up to an hour to get pain medicine after asking. Shift change takes 1 hr with know who your new nurse is they don't introduce the new nurse to the patient.. Need updating badly.
My father was recently at Tanbark after having covid. He was treated with great care and respect. The room was comfortable and the staff treating my Dad was exceptional. The social worker, Latoya, did an amazing job of making sure all of his needs were met. Sandy, Dawn and the two Mikes as well as Mathew the Physical Therapist should be recognized as well. The food was good, as I was offered a guest tray during visits. Tanbark also provided transportation for Dad so that he could visit his doctor during his stay. They also brought a doctor to his room when he was having issues with a wound. Overall we are very happy with the care he was given.
My mother was at this facility for some rehab and asked to go to the hospital several times, for two straight days. They neglected to call an ambulance. On the third day she was in such pain that by the time they got her to the ER she passed away. My mother was young (mid 60’s) and talked of a life enjoying her grandkids. This was supposed to be a few weeks stay to get on her feet. This facility needs to do better, they are letting people die due to the abuse and neglect. Please if you have loved ones here keep a close eye on them but I recommend placing them somewhere else.
My mother received excellent care at Tanbark. The entire staff was most attentive and addressed my questions and concerns in a timely manner. When my mother arrived at Tanbark, we were convinced that she would probably never walk again. However, the skilled OT and PT crew had her walking in under two weeks. I highly recommend this facility.
My husband spend 2 weeks in June of 2022 at Tanbark for rehabilitation after a very hard fall that resulted in a fractured back. We were very pleased with his care and his physical and occupational therapy! Everyone was very helpful and professional! Robert and Ramona Maxwell
My mother has been at tan bark for 4 weeks the staff is top notch always ready to help and the faculty is always clean I would recommend them to anybody that needs rehab
The transition from the hospital to Tanbark was an easy process. When my husband admitted he has to return to the hospital,but on his return to Tanbark the staff were very welcoming and attentive to his needs. The rooms are always clean and they even provided me with a cot when I wanted to spend the night. I would recommend this facility to anyone with any short term rehab needs.
I would like to recognize two of your employees at Tanbark. Social Service director, LaToya and nurse aide Comfort. They have been outstanding during my stay. Please show them my appreciation by recognizing them.
My Mom has been at Tanbark for the past 5 years. She has also used the rehab facilities a couple of times. We have found the staff to be very attentive and courteous. Mom is 102 and really appreciates the help she receives from all the staff, maintenance, the folks that clean her room, the kitchen staff and especially the social director who comes up with innovative ideas to entertain the residents.
The staff is helpful to answer your questions The physical therapy team and occupation theraphy is great.Housekeeping is really helpful and keep the rooms clean. They have excercise class and a craft classes in the afternoons. They also have church services . Tanbark is a nice place if needed for health reasons. I also want to thank the CNAs for all their help. Thanks everyone and God bless.
My mother has been at Tanbark for 3 years total. She was in the rehab section for 3 months and in the personal care side the remaining time. The apt is large , well lit and has a large window to see out of. Except when she was isolated due to Covid restrictions we have been very pleased with the care she has received . Some of the staff and caregivers have been there for several years. The food is very good and you can’t have a better food staff than Brad and his crew. I go see my mom most every day except when I’m in florida. When I’m here away from her, Tammy Smith sends me updates and pictures of mother doing activities. She treats her like family. They have a new activities director who is doing wonders getting the residents to participate in doing crafts . All in all the Ayres family has been pleased with our mom being here. Our dad was also here for his last 4 months and loved Britany dearly who also is very kind to my mom. Her rooms are kept clean. We all love the hair stylist on board now that Covid restrictions are less and loves my mom.
I am writing this review on behalf of a person I believe who should be recognized for her great work ethic and professionalism. Since becoming a part of the Tanbark community, she has transformed into a more successful and happier person. This shows in her new found zest for her job which I have seen in her after work attitude. Less stress and much happier in her after work pursuits. Good job Latoya Morton. I stayed at Tanbark Rehab for 4 weeks to rehab my broken leg. The facility was very clean and comfortable and the food was very good, too. There were numerous people to help with anything you needed. and physical therapy.
The most miserable night of my life post surgery. 2 hours late picking me up from surgery, left me in the wheelchair in my room until my girlfriend showed up and helped me to my bed. Didnt pay any attention to my medications or drainage tubes. My nurses pager died and I had to wait over an hour for pain medication, my girlfriend actually had to call and tell them. Was late to post op appt next day.. if I could leave less than 1 star I would
For me, it was a refreshing moment each day to see my room ambassador. She made a difference each day I was there. Nurses are a professional group led by Justin. Several specific ones are outstanding. The Therapy team are a truly crackerjack team. Noted for their skill, care, and patience. I highly recommend Tanbark for any Rehab needs. Caring dedicated staff.
Shady from the start, they would return your calls but always told you what you needed to hear. I had a family member come in the rehabilitation center and after 11 days they were rushed back to the ER. BP 72/40 kidney issues, dehydrated and more confused than when he got there. I’m not going to blast this facility because others may have better experiences but if I am asked by friends I will tell them the honest truth. I would not recommend sending anyone here. We do thank the one staff member William for actually doing what he could for our family member.
Tanbark, now named Signature Healthcare, is a beautifully planned and arranged rehabilitation facility. My experience here has been made very special by a number of excellent health care employees: JJ, a stunning lady whose energy level is contagious- she can do anything that needs doing-she seems to be everywhere at once, helping out her fellow workers- she does so many different jobs here, i dont know what her title is. Brent, head of Maintenance, also is a multi-tasker and when he is doing something amazing for you, you seem to be his only concern- yet he is helping a hundred different people and constantly repairing things! His wife, Catherine is temporary Director of Nurses here. She is as amazing as her husband! Georges, R.N. should go on to Medical School, we need physicians like him- he is intelligent, professional, quick, insightful and very kind/compassionate. Rachel N.A. You just hope you have her. She is a treasure- you will receive nursing at the highest level plus compassion and kindness. These people define the Tanbark/Rehab experience at its highest level.
My aunt was in Tanbark Health Care Center for rehab. It was fine. (My mom was also there three years ago.) They had wonderful physical therapy, the nursing staff was very nice, and the rooms were very large and clean. Overall it was very nice. It's located in a residential area, it's well maintained, and the grass and everything was well kept. It fits well into the area that it's in. It's not homey. It's small, but it's definitely a rehab center. They have lots of patience; my mom had vision problems, and they were very kind and helpful. My aunt had very extensive physical therapy, and they worked with her really well.
Staff was professional, occupational and physical therapy was excellent. Mds nurse provided all necessary medical treatment and care for me including discharge care. Thankful I was able to come here
I came to Tanbark seeking rehabilitation after breaking my ankle and a subsequent infection. I cannot say enough about the staff and their dedication to every aspect of recovery: they really do care about helping the patient get better and go out of their way to ensure needs are seen to. From the medical staff to the physical/occupational therapy staff to the kitchen staff, all personnel have been helpful, knowledgeable and compassionate. I hope I am never in such dire medical straits again but if I am I have no doubt that Tanbark will be my rehab facility of choice. Many thanks to all the folks there!
My mom stayed in Tanbark Health Care Center for two weeks. I was not pleased at all. I've gotten an excellent review on that. I would not take my mother back there, and even my mother, she was not happy at all. They were not organized, they lost her clothes; it even had her name on them, and the food was not good. They told me that I could come and do a window visit, but I wasn't able to even look in her room because there was a courtyard. I don't think they did any activities, and my mother was not pleased with the food at all. I didn't feel the care and love there.

There is not a doctor on call all the time I think they show up on Monday or Wednesday one of the two my mother was there for over a month I never talk to a doctor they have a social worker Danielle what is she tries but if she does not have the backup as far as personnel that is supposed to communicate with the patients and their families there isn't much she couldn't do because there is no communication with the patients and their families when it comes from upper management you do not know how well they're doing you do not know what they're supposed to be doing when they discharged them it is extremely hard to get someone just to call in medications that tanMark has put them on and when you call you get ahold of her I think her name is Heather she is not a friendly person at all she wants to talk down to you like you're supposed to already have this stuff done and not bother her with it I do not recommend this place for anyone the rehab unit looks more like a nursing home and they push these people out in the wheelchair and put them in the lobby and that's where they stay forever my mother lost so much weight in there because she said the food was horrible and you could not eat it I do not recommend this place for anyone the reason that my mother was there was insurance issues if you can work around the insurance don't send a loved one there
Tanbark has been great for my mother. I’ve felt compelled to rate after seeing some of the reviews from a few years ago. The staff are kind and caring, there are activities for residents to attend, social interaction is encouraged and residents are assisted as needed. Three hot meals a day and medicine delivered just in time and right on time each day. Thank you Nikki and team.
I was here for rehab recently. The staff was wonderful. The occupational and physical therapists are top notch. I would highly recommend this facility to anyone in need of rehab.
I was here for respid care for PT and while my townhouse was putting in the necessary changes after an injury. All of the staff were fantastic, great facilities, awesome amenities (with the beauty salon which is great) and wonderful PT. I was definitely not the typical resident (at 28yo) and I was treated wonderfully by all staff and residents. My aunt ran nursing homes/assisted living and we visited quite often so I have been in and around nursing home facilities for most of my life and this is definitely one of the best.

My 87yo mother recovered here after breaking her leg at home in a fall. The facility is beautiful, spacious, neat & clean. All the staff are friendly, helpful and patient. Her therapy was exactly what she needed, building her arm strength so she could progress from a wheelchair to a walker. That enabled her to begin weightbearing on her healing leg, following doctor's orders with the direction of the nurses and therapists. We have all been very grateful for the care she received. She is now back in her own home with a granddaughter and a home companion, continuing therapy. Thank you, Tanbark!
Mom (age87) was in the Rehab unit after breaking her femur. The facility is always neat and clean. The staff is very personable, helpful, & patient. Mom's rehab therapy was exactly what she needed, building up her arm strength and increasing her ability to use a walker and gradually increase weight bearing on her leg as it heals. We have no complaints and are grateful to the whole staff!
I checked out Signature HealthCARE at Tanbark Rehab & Wellness Center. They were too hands on, more like a nursing home than a senior community, and I don't need all that help. They come around your room every two hours and check on you, and if you want that much help then they're really good. It's a beautiful place and it's gorgeous, but it's a lot more money than my current community (like $1,500 a month more(. The staff was very helpful and very nice. They're gorgeous, but they're kind of too elegant for me. It's a fancy place with plush carpets.
I have an elderly friend who resided at Tanbark (now Signature Healthcare at Tanbark, under different management). For approximately 3 years she resided there. Her entire family is deceased. During her first year in her appartment, she had approximately 5 thousand dollars of jewelry stolen on two separate occasions, without any resolve although police reports were filed under pre vious management. During the past year she was the victim of two additional thefts with no police report filed although the incidents were reported to the social service director Jennifer Jackson. Ms Jackson was not only defensive and condescending with her attitude regarding this situation, she was not forthright or truthful. A third theft involved a Christmas gift we purchased for $80. We filed a police report again with no resolution, and found Ms.Jackson to be completely dismissive. She denied any history of thefts from other residents although the detective handling the report revealed knowledge of an apparent history thefts in fall of 2017 at this establishment. Most of the staff we encounterd were caring and capable. Individual thanks to weekend aide staff, kitchen staff, in particular George, Ryan, Lenny and Jessica. Disatisfaction with Ms. Jackson and the Ombudsman response.

Never take your love ones to this place. Had my mother their for rehab. Its like the nursing done nothing but sit up front party and having a good old time. The nurse aids done everything. Its like the nurses didnt even talk to them about patients. Cause nurses aids had no ideal about my mom. Its really sad csuse 4 years ago my husbands mom was their for rehab and it was a great place but not now. I got told new people brought it. Well new owners suck. They realky need step up and take better care of the place.
WOW! After touring multiple facilities around the Lexington area I knew that these stakeholders are THE BEST IN TOWN by a long shot! Everyone is greeted with a smile and are always willing to help. I believe that this staff truly cares about their residents and their successes!!! Sending a sincere THANK YOU to everyone at Tanbark!!!

Needs more help in all areas
Facility is understaffed. Nurses seem to have a high turn over rate and some seem to be under-educated. The facility is old and has water issues (leaks and hot water problems). Office and managerial staff are very "sell" orientated but offer little in resolving issues to the patients favor (prepare for excuses). Activities are lacking unless your loved one thoroughly enjoys things like connect four or being read to. Care is hit or miss; at times the service is great and at others totally deplorable, etc.. I would be prepared to do a lot of research before placing my loved one here; I suggest reading the reviews across the web, most of which are negative... This was my biggest regret and will be removing my loved one ASAP!
I do not give this place any stars. My father was here and this was the worst experience we had ever had with rehab. He laid 5 days without any therapy at all do to the fact that he was in so much pain and he was in pain because, we found out, he wasn't getting his meds. This place is awful and I will never take any family member here again.
My mom was sent there after open heart surgery and from the minute we arrived she was treated like a family member. She has received exceptional care both physically and mentally. She had a small relapse and went back to the hospital for 2 days and without question she returned to Tanbark to finish her therapy. Every single nurse, nutritionist, cleaning and maintenance employees, and support staff are highly knowledgeable and if they don't know something they seek out the answer immediately....not hours but immediately. I recommend Tanbark to anyone seeking rehabilitation in an atmosphere where they want to feel loved and helped.

If you really care about your loved ones, you will not put them in this facility. They are understaffed and my mother passed away as a result of a fall she had there while in Rehab. No one was watching her. She was also a resident on the apartment side. If a resident rings the call bell, it is sometimes up to 30 minutes before someone comes in. While some of the employees are good people, most look like they are on a work-release program. The billing is so messed up, no one knows what's going on and it is undecipherible. The adminstrator just cares about making money for the facility. Please don't put your loved ones here.
