From urinals without lids to workers screaming in the hallways at night. Broken equipment and losing my dad's pain meds. He went 24hrs with a broken back and being post operative without meds which makes zero sense. We won't be going back.
Ms. Ruth is such a happy and caring nurse. I began working in housekeeping a month ago and she was so nice to me and is very God centered and cares a lot about her patients/residents and staff. She carries herself professionally and enjoys her work. I love her positive demeanor. Always look forward to her presence. 🙌🏼🩵
This place is horrible. Smells of pee REAL bad around the nurses station. They let a dementia patient just walk right out of the building without any questions.
I'm a retired Registered Nurse and I came to Signature East rehab to get stronger. My stay here has been great, the nurses, therapy and admin check on me often. I used to personally know one of the administrators for Signature but I believe she has moved on. I would recommend aignature.
My mom has been there for sub acute rehab for the past few weeks and has gotten exceptional care ! She says she loves the people, the food, the activities and most of all, she feels she is safe while getting great care. The team has been very helpful and supportive while helping us with this journey. We couldn’t be more appreciative. Our mom is our treasure and it means the world that she is happy and well cared for ❤️
The staff are very helpful and the aides are nice. I feel that the regular staff are attentive as they listen to me. Over the past four years, I've been at four different places and Signature East has been the best. I'm greeted each morning and there are things going on each day so there are things to do if you want to be active. Most importantly, the staff at Signature East, treat my visitors like family.
Run from this place! My father was there for rehab, for 3 days. There are some mean nurses, and other people that work there. I stayed with him because I was so uneasy about the place. The nurses station was across from us, they were having a party all night long I asked about quiet hours, and was told by a nurse it was not that type of facility, that people holler( meaning the patients), I only heard the people at the nurses station. I was told that family members didn't usually stay at this facility. So I guess they don't care if the patients get sleep, either. The place was never cleaned, only trash was taken out, it was filthy. Urine was not cleaned up, and tracked on his floor, what a fabulous way for someone to fall. He also had a heart attack there, he called me to let me he was going back to the hospital, and he was scared. They were not compassionate towards him at all. They found out he was having chest pains, and the head nurse walked in there and said "Take him away". Her exact words. No compassion. The food was horrible. I wish I could give 0 stars. They should shut this place down. RUN. I spoke with the Administrator whom was new that Monday. But, I seriously believe nothing will be done.
This place is like terrible horrible and nasty. Workers don't have respect like they should. My mom was in the hospital and lost her strength she couldn't do much by herself but needed to go to rehab before coming home. One day I got there and went to her room She was sitting up in her wheelchair which to me was encouraging. I told her cool Before I had got to her room a nurse was like your mom is going to the hospital cause she has something wrong with her blood work. I made everyone leave her room. They tried saying they had to be in there I said get out NOW. They finally did. So I asked her about what was going on She told me what she didn't really understand what was going on but that they told her she needed to go to the hospital and they had already called the ambulance She said she would go and she told me she agreed because she wanted to get out of there. So I said ok I will get your stuff together and get to the hospital soon. I followed them there. She said they wouldn't even call my sister that could have a say in it. I finally got hold of my sister and told her what was up. We got to the hospital and she had lost weight and was still weak otherwise. They had not done good with her in PT. When she left in the ambulance some of them said see you when you come back I looked at them and said she is not coming back here. The problem was they should have notified us before they called the ambulance because we could have taken her myself. It was good her insurance paid for it. So far we have not gotten a bill for it so it must have been covered. If it would not have been covered we would have done everything we could legally to have them to pay for it. Yea they are not the best place to go to.
Everyone here from Receptionists, Nurses, and all Social Workers especially Lenisha Stribling are amazing. They care for my father from their hearts.
I am pleased to be her, I had a good experience here, therapy is terrific, staff are nice, I enjoyed activities and happy hours, made some friends , food is ok for most , nurses and nurses aide are hard working. I am enjoying my stay here. I would recommend Siganture East to anyone.
This one of the best places I have been at, staff are nice, they helped me alot, place is clean, everyone here is helpful, my therapist has helped me gain my strength, hopefully I will be able to walk out of here on my feet, and get back to my usual business. I appreciate every single staff member that helped me and went above and beyond. Thank you all

This place is shameful and is an absolute disgrace to healthcare. Every staff member here should be ashamed of themselves. My father almost died because of their lack of care. They constantly "forgot" (meaning neglected to) give my dad an emergency button. Before my dad was sent out to the ER, he had spent the entire night in a freezing room with shortness of breath. He asked a staff member for a blanket earlier in the evening but no one ever came back with one. In fact, no one came back to check on him for 12 HOURS. The nurse acted like she didn't know what to do about my dad vomiting before they finally called the ER. They didn't even bother to call me and tell me he was sent out until I called and found out he was gone. Don't send your loved ones here for any reason - it will only make them feel physically and emotionally worse.
First of all, the year my mother in law has been there has been so beneficial because of her growth in recovery. When she was admitted, she could barely hold her head up or barely walk. Today she is like a new woman and she is so confident when she walks and talks that it is absolutely amazing. I called today to have staff let my mother in law know we were coming to visit just so she could get ready and dressed and prepared for our visit. When we got there someone had printed out a sign that said “Cruze family gathering reserved” I thought that was to toughtful and the nurse who did that has my deepest gratitude. We had 3 kids that got to play games and a basketball ski ball thing and read books and play puzzles and just enjoy company outside her room. We appreciate your staff and the amount of support and love you have given her because it has nourished her and helped her grow so much! Happy Thanksgiving from us to you all! We appreciate all you do!
My husband was a patient at the East location. The staff and Mr. Ben Z. Were wonderful. He has since passed but I give them 10 stars out of 5. Thank you for everything. EB-James.
**Edited** My aunt was just transferred here from Frazier downtown Louisville, and was told by her medical POA (my other aunt) that she was not given any meals during her stay but one time for lunch, she has been there almost 2-3 days now; very unprofessional and neglectful of a place that you’re suppose to trust with your loved ones to get back to feeling their old selves! I am not too pleased so far with this move! DO NOT trust these people to take care of your loved ones! My aunt has had a bad experience from the beginning of her hospital stay at Norton Suburban, then Frazier, and now this! PT & OT was only there for 12 minutes (today), sat her up on the bed! Breakfast tray was given, her meds side effects were sick if no food in stomach, she threw it up (3rd time after she got sick before being able to eat), therapists advised family that nurses need to change her better as she is getting a bed sore for negligence!
I was scared of going into a facility but when I got there I was very pleasantly surprised if the caring nature of the staff. They were very interested in my improvement. They went out of their way to help me and keep my spirits up. They are very knowledgeable about rehab and the therapists were wonderful. They knew exactly what I needed. I can't recommend signature enough.
I cannot give enough praise to this facility. The CEO visits with and knows each individual patient. The nursing and medical care are outstanding! My sister received physical therapy, occupational therapy, medical care, medication management, Emotional and physical support. All of the support staff, went out of their way to be kind and helpful. This facility Is definitely the best and we are so grateful. My sister was actually able to return home, due to the progress she made at signature healthCARE of East Louisville!
I can't say enough good things about Signature East Louisville! When my dad broke his femur, we were worried about finding the right place for his rehab. But from day one, the team at Signature East Louisville put our minds at ease. The rehab team worked with him day to day, pushing him to reach new goals and regain his strength. And the nursing staff? They were like family to us. I knew my dad was in good hands every single day. If you or a loved one are in need of rehab after a break, I would 10 out of 10 recommend Signature East Louisville. They truly did an amazing job for my dad, and I'm so grateful for everything they did for him.
I already had an in-depth convo with the Admin and Dir of Nursing about the following problems, so they are aware and want to improve some procedures. First, kudos to an aide named Silas. He was smart, friendly, and gave extra attention to our blind mother. Michelle in social work was another outstanding employee. The RN's were all awesome, friendly, and helpful. My 87-yr old mother, who is frail and blind, did not do well at Signature. She came directly from a week in the hospital, where she WAS walking, although weak. So hospital referred her to SIgnature East because they had open beds. We were very hopeful they could help her get strong again with daily PT. The PT team did not keep us informed at all how she was doing. After a 4-week stay, she was promptly discharged. It was only then, that we learned she was not able to stand or walk. So she actually DECLINED while at Signature! If only the PT Mgr would have told us that she was unable to make progress after the first 2-3 weeks, we could have pulled her out of there sooner. But there was no communication. We never got a report or anything, only a list of PT goals, which she did not achieve. During those 4 weeks, she laid too many hours in the bed and nearly developed a bedsore. They didn't catch that - a paid caregiver noticed. We found her soaking wet a few mornings. We requested a meds adjustment, which took 3 wks to accomplish, so the final week she was there, her meds were finally adjusted and she felt better. Our mother was so weak and confused, she was not able to feed herself. The aides don't have time to sit 30 min with someone to feed them. They are always in a hurry. My advice is, if your loved is frail and not able to speak up for themselves, it is best to have an advocate, a friend or a family member to be there as much as possible.
Signature Healthcare of East Louisville is a beautiful facility with several new management staff. Upon arrival today, I was met by a friendly staff, in a clean, odor free environment! I am happy to know dialysis and a Respiratory Therapist are both onsite. I would recommend anyone 'shopping' for rehab or skilled nursing services to tour the facility. The facility has a 5-Star rating in Quality Measures - the highest star rating available!

My Fiance is in your Facility for Rehab...I was not impressed my first visit but before I left the Nurses Aide was very attentive to his needs and very helpful to me..I hope his care & well being is your "Top Priority" for his recovery.....We did not have a good experience at Signature South.... He will be doing Physical Therapy at your Facility.
My stay here was an awesome experience. Everyone was nice, courteous, and very responsive to my needs. I would recommend Signature Healthcare East to anyone In need of rehab.
This is my second stat here at Signature East, there is improvement compare to my last stay, which mean they are going in the right direction, everyone is friendly, therapy is phenomenal, East is always my first choice.
My aunt was a recent resident of Signature East of Louisville, Ky located on Six Mile Lane. I just want to say, we have been very, very pleased with the service and therapy received. We left a hospital thinking she would be on oxygen and bipap but the RP here have worked wonders. No more of either of those. The PT department is equally as good. She has bounced back to her old self and is walking again. The Social Worker Lenisha and Administrator have just been wonderful. The nursing staff, aids and nutrition staff can't be beat. We highly recommend this rehab.
Very dirty, staff is not attentive to the elders living there. A nurse came and glanced at her patient and didn’t even bother to clean the snot clearly hanging all over her patient’s face. My grandfathers finger nails were not getting cut and his hair had not been washed in days. He looked so dirty.
Very nice facility, they have a very compassionate staff, transition was very smooth from the hospital to their facility, thanks to their admission director, who was very nice and comforting, helped us navigate through this difficult journey, therapy department was top notch, every staff members we come across was professional and helpful, I would recommend this facility to anyone who is looking for speedy recovery and top quality care. Can't ask for better care for my father.
The changes over the last year have been tremendous and positive. I’d admit my family member here anytime. Great leadership, kind and caring staff. Great job Signature East.
The service was great, all staff accommodated all my needs, Therapy was excellent, Favor my Physical Therapist was my Favorite, he was passioned about his job and he helped me gain my strength, this is my fourth time here at East, it is always my first pick for rehab, I recommend this place to anyone who really need rehab. I thank everyone who helped me, Social Services, Nursing staff, Housekeeping, all were great. Thank you.
I'm 50y rs old I was scared to death to go to a nursing home facility, but this place is much more. I came here for rehab. I came in on Friday evening and things were kind of hectic at first. By Saturday morning 90% of things were worked out. By Monday morning 110% of the things were worked out. 98% of all staff went above and beyond to help me and make sure my needs were met! This includes the nurses, aides, management, therapy, and even the housekeeping staff. Activities have also been great in getting me out and socializing with other residents and staff to improve my mood and outlook towards recovery. I was really surprised about the facility and would definitely encourage others to come here.

This place is amazing and really cares about their patients, and communication is awesome!! Very clean and updated with medical care available 24/7!! The staff is honest and caring. My dad stayed here to be evaluated after a sudden seizure stemming from a stroke. He called me nearly every day as the rooms had working phones!! He had PT every other day and was allowed to walk outside to see sunlight weather permitted. I am super happy with the care my father received with this care facility.

I absolutely love this place, the Management team is amazing and really making positive changes, who are genuinely passionate about Nursing and the care of the residents.
Horrible experience! My mother was injured there. We weren’t informed for 2 days and only because she had to go to the hospital gushing blood from a mysterious wound. Please do NOT send your loved one there. I wouldn’t send my enemy there!
Today is my last day here staff was great and rehab got me walking again .very clean the nurses in the300 was excellent

Improved services ranging from short term rehab and long term care, with onsite dialysis and 7 day a week respiratory therapy coverage to meet the needs of complex pulmonary care needs. Clean and professional atmosphere with continuity in care. Great place to work if you’re in the profession of caring. Great place to come for rehabilitation and/or long term care. We aspire to provide great relationships, quality care and exceed expectations.
The staff here went above and beyond for my dad. Everyone of you took such great care of him. From David in physical therapy, Nurse Megan who started his days off with a sweet good morning, and last but not least the case management and social workers that did everything in their power to make my dad's stay here a comfortable one. You all are a blessing and a true testament of what front line workers do. We are forever grateful and appreciate everything you did for us.

My Dad was there for rehabilitation after falling and having a half-hip replacement. The nurses and therapist's were very patient and caring in their efforts to get him rehabilitated. Brooke worked with insurance to provide an extension of his stay for further rehab, so as to alleviate my father's fears of not being quite ready, and also put him in the best position for a successful return home. She then set up additional ongoing home care and therapy to facilitate his final rehab at home. Thanks so much for taking good care of my Dad.

I have been with signature since mid October 2020 and I love love love everything about this place!!! From the staff to the residents!!!! I couldn’t have asked for a better job!!!
This is by far the worse place to have a loved one placed. These people discharged my paraplegic daughter without giving us the courtesy of finding another placement for her. They demanded that I pick her up without giving me the time to find transportation to accommodate her condition and when I arrived they wheeled her out to my car and instructed a staff member to pick her up and place her in the back seat of my car to take her home, with no means to care for her condition. They allowed me to take one of their old wheelchairs to get her up to her apartment which by the way I had to sign for it and give them my drivers license. Upon our arrival to her home we almost dropped her because we were not strong enough to lift her into the wheelchair. To make matters worse she had a horrible and seemingly untreated ulcer on her back. We will be seeking legal action against this facility, pray for us and all who have to endure these uncaring and cruel people.
My mother was a patient here for about 2 months. While she was there she had gotten a terrible bed sore and was kicked out. Not to mention that no one called me to tell me she was being discharged until the evening before they released her. And this is after so many conversations with the staff informing me that it would be impossible for me to care for her alone, and they were convincing me that it would be best for her to stay in a nursing facility long term to ensure she’d get the correct care.So of course as far as setting up all of the care that she would need at home, I was not prepared. Very upset with myself for even having my mother stay there in the first place. I would get calls everyday from the staff complaining about her. While I know that my mother can be difficult at times, plenty of healthcare facilities often deal with much worse than just a bad attitude from their patience. Very disappointed.
My mother has been here for 2 months after suffering a stroke. She needs dialysis 3 times a week. Her right arm and leg malfunctioning and she can not speak well. My issue is the sores on her bottom and legs. The staffnever called me about them. and she can barely speak.
