Signature HealthCARE of Hart County Rehab & Wellness Center

Everyone was excellent, food was great!!!

This is my second trip to signature care in Hart County and if you’re ever gonna have a loved one that needs help or rehab rehabilitation. This is the place for you. I give them 100%. They treat you like you’re at home.

The nurses and physical therapy providers are great! I experienced a willingness to solve any issues. Many of the patients are now good friends.

The PT OT STAFF IS REALLY GOOD. THEY LISTEN TO YOUR CONCERNS AND WORK WITH YOU TO ACHIEVE YOUR GOALS. THEY COULD ACTUALLY MAKE THERAPY FUN.THEY ALL GET FIVE STARS. THE NURSING STAFF WAS REALLY GREAT AT GETTING ME THE MEDS I NEEDED. AND WHILE ALL THE NURSING AIDS WERE GOOD, COLTON DESERVES A GOLD STAR. ALTHOUGH OVER WORKED JUST LIKE EVERY ONE ELSE, HE ALWAYS HAD A SMILE ON HIS FACE AND WAS KIND AND COMPASSIONATE. I CAME IN ON A WEEKEND AND THERE WAS MISCOMMUNICATION BETWEEN THE HOSPITAL AND THE WEEKEND STAFF, WHICH LEFT ME PRETTY UNCOMFORTABLE, BUT ONCE THAT WAS ADDRESSED ON MONDAY THINGS WERE A LOT BETTER. THE ONLY OTHER PROBLEM IS,THEY ARE WOEFULLY UNDER-STAFFED ON NIGHTS AND WEEKENDS. THIS IS AN ISSUE THAT SHOULD BE ADDRESSED BY WHOM EVER SETS THE SCHEDULING HOURS. THEY NEED MORE HOURS TO WORK WITH.

If you are in need of recovery from surgery and rehab therapy as I was or even long term care, then you couldn't find a better place than Signature HealthCARE in Horse Cave, Kentucky . At Signature, you're not just a number. From Administration to Nurses & CNA's, and Housekeeping you are always greeted by name and with a smile The Therapy group is top-notch and even when you have a bad day and don't want to workout - they stand firm and won't take no for an answer. Trust me - you are better of because of it. Plus there's always interesting activities to get to know other residents and to keep busy.

Our family has had a wonderful experience with Signature HealthCARE of Hart County. My grandpa is currently there for rehab, and we have been so impressed with the level of care he’s receiving. The rehab team works with him patiently and intentionally, and we’ve already seen such a positive improvement in his mobility. The entire nursing staff has been incredibly kind—not just to him, but to our whole family. They treat him with dignity, compassion, and genuine care, which gives us so much peace of mind during this season. We are truly grateful for the dedication and heart they show every day.

I and my family have had a great experience specially entering lobby Karen is a wonderful greeter love her personality and warm smile she really makes you feel at home she goes above and beyond for the guess and residences she will always have special place in our hearts.

My mom is at the facility for rehab the staff has been very nice and professional. We are very grateful for everything they have done for her while she is there.

Ia m honestly not sure where to start Orstop//..when i first came here I was not going toʻ come in,,I have never been at a place like this before,,, So my husband went to n asked the administrator to come out n talk to me n Already made up my mind,,, I wasn't coming in jason had such a kind being Genivoice n he talked so nice to ,,,he didn't force me ,,,I soon could tell he was Genuine,,, So I started my therapy n everyone was so nice n helpful n frenndly N I am now here 8 months n if my husband stay here with me I am not sure if I would just live here (ha ha) Bui have made so many freinds it's like a big family n jason is still as nice as he was the first day Its super clean,,, (((So I highly recommend n signature)))

Pam assisted with the admissions process for my grandmother and went above and beyond to ensure she was admitted smoothly. Thank you also to the staff for their kindness. The facility is beautiful and well maintained.

Trusting someone else with the care of a loved one is never easy, but this nursing and rehab center has earned our family’s confidence for three generations. From my great-grandmother’s final years to my grandmother’s long-term stay and now my mom’s recent rehab, the care has been exceptional every step of the way. If you’re facing the hard choice of finding a place where a loved one can receive the extra care they need, I can say wholeheartedly that this is a place you can trust. The staff are patient, skilled, and genuinely kind, and the facility is always clean and welcoming. We’re beyond grateful for all they’ve done for our family and would recommend them without hesitation.

Beautiful facility !! Everyone is always very nice and helpful and they never get mad when I forget about the door alarm .

My sister Gail was there for three months, and there is no way to express the great care everyone there was kind and decent to her.

My time here has been a warming experience, every body treats you like family, the staff go out of there way to make sure your safe and all your goals are met.

Super kind, genuine, honest, hard working people who really care about those they take care of. Just a 10/10 experience when dealing with anyone there. And they love to eat! 😂🙏🏼

Great place

My mother in law was at Signature after her heart surgery. The staff was great and very professional. I brought my father in law every day and drop him off so he could be with her when I was at work. The staff not only took great care of her but took great care of him. Made sure he had coffee and supplied him with a lunch tray too. I highly recommend Signature Healthcare of Hart County.

Facility was very Clean! The nursing Staff was Alert and on the floor carrying for the Residents! Very nice thing to see!

Spent 2 1/2 weeks with broken hip. Didn’t know what to expect. Was totally surprised by the experience from the empathy to kindness of nurses to aides to pt. Nothing but kindness from some wonderful people

Horribly managed and understaffed!! I will give props to what staff they do have. But management is obviously dropping the ball. The nurses, or rather should I say… nurse, and other staff members are doing all they can. There always seem to be a couple of “supervisors” hanging around the nurses stations and letting the one or two that ARE working run around like crazy! The two times I’ve been there, my uncle has always had to wait forever to get any kind of assistance. And I always feel sorry for the nurse on staff, because it seems she’s all alone! If they are having trouble keeping people, they need to look at the top! That’s typically where the problem lies when it comes to staffing issues. My aunt is in a home in southern Indiana and I swear it seems like there’s one nurse for every three or four patients. This place has one poor nurse tending to dozens all by herself! Do better!!

Great facility, great staff was there for two weeks they took terrific care of me. I would recommend this facility that needs to go to rehab!

My grandmother was here and she was treated very well. She was taken care of during her stay. Nursing and therapy did an amazing job with her.

They provided great care for my dad when he had to go there for rehab. The floor staff and rehab team are amazing at what they do!

I started having back spasms due to an older injury a few weeks ago and went to see Jason and his team. Working with them even for such a short amount of time has eased my symptoms and has increased my flexibility. I don’t use the word “Godsend” often, but in this case I feel it appropriate. Thank you all so much for you empathy and care. ❤️❤️❤️

Christmas will be a year my mom pass she was there for about year the staff was always friendly to mom and the family I would recommend this place to anyone thank you signature making mom stay a little easier

Love the facility worked there for almost 6 months in dietary loved the job. But couldn't stand the boss LeAnn or her best friend Amanda they are both very unprofessional and shouldn't even have their job because you know why because they like to lie and put words in people's mouths and start drama and her boss Suzie wouldn't even do anything about their behavior.... Oh well their loss

A nurse recommended this facility for my Mom! She said I have had good luck with this team! I am so thankful for the recommendation. We arrived and were welcomed with open arms. When you called and had a question you were answered with friendly concern. The therapist were over the top. They motivated her to try and kept the laughter going and made hard work fun. She got to come home yesterday and went up the steps with ease!!! God Bless the entire staff and Thank you so much!

My mother has been at the facility for a year and she is so much better since going there! Her medicines are finally straightened out and she’s feeling better and happier now. The staff there are caring and professional. Great place!

My family has had multiple stays at Signature of Hart County. The staff was so helpful, kind, & accommodating to the needs of my family members during their stay. Angela Croghan always goes above and beyond to make sure they are comfortable and that the admission & discharge process goes smoothly.
