They were great to me
This Skilled Nursing Facility does a great job! It’s an old hospital so, it’s not the most pretty and modern place you might visit. But, it is clean and smells fine and they provide the best of care! Highly recommend!!
The rehab personal were awesome. Very inspirational and supportive. A couple nurses there were useless, but overall the staff was pretty good. Nurse DaNita and Nurse Teddy were a notch above the rest. They would always make sure you were getting proper attention and meds. I would of given a higher rating but I was admitted during a time of a lot of changes going on in the organization. Which left me with a few unhappy days.
The worst place to even consider for a loved one. My mother was placed for rehab there and she wasn’t given her medication for the 40 hrs she was there. We had to go home and get it for her. Her pee bag was not emptied at all during her short stay. The place was outdated and disgusting. Her call light was on for hours for water which was a pitcher filled out of the sink. We literally went and wheeled her out of that place and took her back to the ER to get her admitted and placed somewhere else. The place needs to be shut down.
I’m just hear to say if I could I wouldn’t give this place no stars at all, there call lights are not working, they are having the residents ring a bell for help, also they DO NOT CHECK ON their RESIDENTS like they are suppose to. My grandma was only thier a few days an now she has ended back on the hospital an that’s where she will stay until she is ready to come home. She was suppose to be at this place for REHAB a she didn’t do a bit of REHAB. They left her lay thier to die, she could get to the bathroom on her own but could not get up on her own due to the toilet was so so low. My heart broke for her an her roommate so bad. I’m a STNA myself an let me just say the staffing we dealt with DID NOT do thier job at all. Very very sad! I WOULD NOT PUT MY DOG in here! I hope a pray this place gets thier act together soon an start caring for thier residents! O an one last thing she got sent to the hospital an they packed all her belongings up as soon as she was sent to the er wat a joke, they didn’t even know if she was gonna be admitted or brought back there! But my family an I put our foot down an my grandma will not be going back here! My heart is just so broken for the residents that are bed ridden. They are being neglected every way possible. I watched them bring in food trays sit them down an they sit down an get on their phone didn’t even try an feed the resident at all sooo sad! The director of nursing was good but the nurses an the STNAS we dealt with were a JOKE. Besides the two young lady’s I dealt with today to get my grandmas belongings they were two caring woman I wish they would of been the two to care for my grandma we may of not ended up in the situation we’re in now if she had caring staff.
I enjoy entertaining the residents there.
Everyone was very friendly.
My first expression is that the aides were caring that I met today. The facility was secure with sign in and out kiosk.
We come down every other month to have a Sunday School class with some of the residents. Miss Sue does such a wonderful job getting everyone prepared and in place. She is truly a saint!!!!
Some of the nurses are lacking the importance of medicine. Right it down but the patient doesn't always get it.

Lack of house keeping. My mom's room is smelly. I bring in air freshener just so I can visit her.
Wonderful place with very caring team! Is handling COVID precautions well and taking great care of residents!
They won't answer the phones even though they called to tell me they had a COVID out break no one is answering
I'm happy Matt's getting his new wheelchair,the dentist coming in April was great . We went to see Matt & he was content not riled up. Overall very happy with the progress that is going on for my Matthew. Thank you everyone.

So far our experience at the Pavilion has been great, but my mom has only been there for a few days. The staff on the fourth floor is awesome. Merry Christmas!
It’s a nice place it’s clean. The nurse and nurse aids are really friendly. I have one complaint about my dad’s legs they’re red and swollen again I had time I tried to talk to somebody about it and they don’t have the answers for it and my dad bid the mattress is broke down. He needs a new mattress. That’s the only place that I got.

Wonderful staff always taking the extra time to go over every part of my dad's care. The family friendly atmosphere really puts me at ease knowing my dad's rehabilitation is in good hands
My husband was admitted from the hospital to the rehabilitation and skilled nursing floor about a week ago. There were some issues at first and the food definitely needs serious attention but the staff took our concerns seriously and acted on them promptly. The staff through the week is very competent and caring though the some of the weekend staff are less friendly or involved with their patients. Family follow up is a must but that is only because a lot of these patients can’t advocate for themselves and the staff cannot be with each individual for hours at a time. My husband has been disabled for more then 20 years now and has been in rehab as well as the hospital often but, when asked, he said he was fine staying at the Pavilion for the remainder of his care. I stress though that family must be involved to ensure the best care possible.
This place is awful. There is hardly any staff and the staff that is there seem like they can’t be bothered to help your loved one and when you say something about it, they blame it on staffing issues. Unfortunately that’s not my nor my fathers issue. My dad I heard them talking bad about him right outside his room and on his first day there, I found a moldy food tray left behind from the previous patient. This place needs to be shut.. I’m pretty sure they only have a handful of actual nurses and none of them are polite.
My mother in law was a patient here and received excellent care. The staff was kind, considerate, and very compassionate. The activities department went above and beyond with the "Mother's Day Tea". This made her stay extra special!

When i was at st e and told me i was going to the pavilion is put this picture in my mine that is a bad rehav the botton of the barrel. i couldnt have more wrong the staff,therapy,and kitchen were great With me a big person they made fell that my weight was no problem some of the machines that i used were handled with up most safty for my safty. I was afraid of the hoya lift but my aides talk to me and everyting was find.if any one aaked me about the pavilion.it a very nice place to g
The only reason I left my mother in there for her rehab was because of insurance. There were constant mistakes concerning her medication, there was even a crack about she shouldn't be turning on her call light at shift change so much to go to the bathroom!! Avoid this place like the plague!!!!
I am very happy with the care my daughter receives at Providence, she has been there since it opened. We have a new administrator & director of nursing, that add personal touch to her care. The nursing staff & CNA s all the way to the front desk are very loving & caring as if in caring for their own loved one. I personally don't know what I would do without them. I would recommend Providence 100%. Kathleen Malicek 💝
