Excellent staff, nurses were amazing with my uncle.
My dad is on his second day here. He has spatial neglect of his left side currently do to a stroke, so no movement on that side. He’s also has had a craniotomy, so he doesn’t have his skull cap. He is not able to hold his body straight and his memory goes in & out. Basically very, very, VERY fragile. He was transferred from Cardinal hill and they know no info about him. He hadn’t been taking his pain meds or heart burn meds (which he is prescribed since we had an episode that was so bad we got sent to UK for 2 weeks). Well, I visit frequently & today I came and he was in his wheel chair eating what looked like slop. He wouldn’t touch it. His diet got updated before leaving Cardinal hill to small soft bites. But yet they puréed a ham, mayonnaise and cheese sandwich.. can you imagine how awful that looked? I brought him a burger and fries and he scarfed it down. After leaving I didn’t think to check if he had safety belts/alarms on because I mean that is a given right? He can’t hold himself up properly, why would he not have extra safety precautions taken? Well I’m in the hall talking to the desk staff (which do seem nice) and lo and behold my dad falls in the floor!!!! He lays there for at least 5 min before anyone notices!! I had to be the one to come and hold him off the floor until the ambulance arrived. Thankfully nothing was broken, but this could have easily broken something major or worse killed him!!! I will not keep quiet about this. My heart is broken and my poor dad is so sore after something that could have been so easily prevented! Negligence on their part. Updated Review (as of Dec 29): As of November 25th, we moved my dad out of this facility and into another one due to insurance issues. Unfortunately, Fountain Circle did not mention the option to appeal the insurance decision. We pursued an appeal ourselves and actually won it—though insurance never notified us, and by the time we found out, Fountain Circle no longer had any available beds. That said, after being in our current facility, I can honestly say that Fountain Circle now feels like a dream in comparison. At Fountain Circle, staff talked to my dad like a human being. They consistently cleaned him up and he was able to shower about twice a week. I was allowed to bring in personal items for him without issue. When he needed accommodations, they made them. And when I was vocal about concerns, they listened. It wasn’t perfect—no place is, and I understand staff have a lot on their plates—but overall the care was noticeably better than where we are now. They were willing to take my dad to the bathroom, and they even allowed him to roam the halls and socialize with friends, which meant so much to him and to us. Overall, I do recommend Fountain Circle. My biggest advice to families is this: visit often, be vocal, and advocate for your loved one when they can’t do it themselves. If we could get back in there, we would in a heartbeat.
The staff at Fountain Circle were wonderful. They encouraged my husband and kept his spirits up while there for physical therapy.
The hospital across the street refers to it as "Fountain of Death" so...take that for what you will.
Almost 3 weeks ago, my Alzheimers riddled Papa was walking down his hallway and fell on his face. We promptly called EMS and he headed off to the hospital. Days later, he was transferred to Fountain Circle Rehab Center, a place that had caused my grandmother's demise in 1999. We were not notified that he'd been transferred until Sunday though he had been transferred on Friday night after an evening family visit, at the local hospital. By the time he left, 2 weeks (I believe) later, he could no longer walk, had a horrid bed sore and had been taught to poop the bed, wallow unit, until someone noticed the smell and changed his bedding. The poopy bed certainly caused complications with his bedsores (that we were never made privy to). Fountain Circle made us aware that he could no longer stay there bc his Medicare had run out. We were told he'd be covered through Sunday but could only help us get him in if he left on this past Friday, September 26, 2024. We knew he wasn't ready to come home because he could no longer walk & didn't even have enough strength to get him to a bedside potty. Fountain Circle waited until nightfall, had a transport company drop him off at the bottom of my mom & Papa's driveway. We had to call the Fire/EMS Department to get him up the concrete steps, into the house, then up the house steps and into the bed he shared with mom, for 38 years. In the wee hours of September 28, 2024, at 12:30AM my mom had to rest on the couch before heading back to bed with Pop. She fell asleep and awakened at 6:04AM...finding my Papa dead. He was not strong enough to get the covers from his face, after rolling to give relief to his painful bedsores. So, in essence, my Papa comes home from intensive PT at this "rehab facility," but comes back much weaker, in someone else's clothing, wounds we were never made aware of and weaker / frailer than he was before going to the hospital. This death was not an accident; this death was not just a travesty; this death reeks of willful neglect, but more they killed him just as their neglect (many years ago) killed my grandma. Please, don't let this place take your most treasured and beloved. There are good rehab and, even nursing homes out there. Please: • take the time to find the right one • you and your family set the goals and expectations, then find out their plan to met your family's goal • visit often and hold hem accountable to your families milestones, dates and goals. Please, don't let your loved one be another victim of places like these. Do your research; take lots of pictures bc you never know what you're gonna need and stay vigilant! I pray all that read this have a better outcome than we recently did.
I am so impressed in how far this building has come! It looks so good, and the environment is happy! The CEO and DON are so engaged, and the care is compassionate!
Do not bring your family here. The FIRST DAY they refused to give my mother her pain medication. She has stage 4 breast cancer that has spread into her bones. Specifically her SPINE. So you can imagine the pain she feels on a daily basis. I had to take off work and threaten the staff with lawsuits to get them to do anything. I had her oncologist himself call and verify her medication and they told us that they were not part of their network so they wouldn’t do anything until Their doctor prescribed it. My mother sat there in pain for 2 days before informing me. How can you listen to a person cry in pain and not do anything! Her phone charger kept coming up missing or stolen, I replaced it 3 times in a 2 week period. She was told by a nurse to not keep any valuables because they have a history of things going missing, are you joking! Finally had to get her out of there because her feet were swollen, her cognitive ability declined and the weekend nursing staff are absolutely useless. The only nurse there that seemed to care at all was Peggy. The beds are ancient and uncomfortable. The whole place smells like urine. And everyone has to share rooms. My mother had a space about 6’x10’ to live in. Do not take your family members here unless you just don’t care about them. Also the food here is absolutely terrible, usually the food is cold. Everything is just processed garbage, I’ve eaten better food in jail! Constantly had to bring my mother food because what they gave her wasn’t suitable to throw in the hogs slop. They brought her a glass of OJ for breakfast and there was only about 1oz in the glass! Asked a nurse for another glass, and they just disappeared. My mother was only there for inpatient physical therapy before coming home from the hospital. She left in an ambulance, back to the hospital in worse condition than when she got there. I threatened them with a lawsuit and they will be very lucky if I don’t follow through. If anyone else has had terrible experiences like this let me know any we will file a class action against them!
My mom is receiving very loving care here from a staff of wonderful people dedicated to to quality care and life for each person . Thank you mama is in need of rehab care and this establishment is top notch . I had such pleasant interactions with Jessica rehab gal for my mom and who was so kind and patient with her in moms journey getting strength back in her legs. Also Dandy a sweet and vigilant hard working cleaning gal always cheerful doing diligent cleaning in room keep it smelling nice and disinfected . Also Sweet Nurse Peggy we thank you for your oversight with your team of hard working gals and guys there contributing to a very nice stay. for mom .We love you all and take good care of yourselves too as you do a very important job in contributing to quality care of our family members . So my situation is different than most people that come here. Had some concerns at first, but after the staff figured out what my needs were, couldn't ask for better place or people, from nurses, rehab, maintenance, was all you could ask for. They took care of all my needs and would listen to any suggestions I had. They're even pet friendly. Would recommend this facility to anyone that needs care. They try their best to make you feel at home.I would be happy to come back here if I had to. A big thank you to everyone that has taken care of me so far!
I have visited Fountain Circle several times with a Friend who goes and sings for the residents. The building is always clean and the staff are always friendly. I enjoy visiting with the residents when I go.
I will tell that I've been at this facility for 5+ yrs, in it's by far the best facility I've been at the staff are very nice in compassionate in will help the residents in anyway they can they are not only employees they are our family. In as far the therapy department it is highly recommended they are great in my book.. I wouldn't go anywhere else this is my home
Being in the care of someone else can be a scary thing but first night here there's a feeling of home everyone is nice and attentive and in the medical field that can be a rare thing but not here people know you by name you don't feel like a body in a bed and that means a lot

I am a professional within the healthcare industry, but that was 30 years ago. I’ve been a consultant in most industries in the United States, especially government healthcare programs. I was so pleasantly surprised coming over from the VA Medical Center as to the personable nature experts at what they do here in the rehab I was a little bit afraid that at my age of 78 I wouldn’t be able to keep up with the engineer and Taylor their programs here to each individual I would recommend Fountain Circle highly not just for a rehab rehabilitation, professionalism and technical now, but the place is clean and every wing it just shines back. It’s hard to find a facility like this and I know I used to run them.
Great Facility! It's older and could use some updates but that does not take away from the care they provide and the love they show for their clients.

I have been a CNA for 22 years. I have worked at many facilities in those 22 years and I can honestly say this is the best facility I have worked in. The management is great and they go above and beyond for our residents. Our therapy department is top notch! We are more like a family here than co-workers!
This review is to give a shout out to Loretta, a social worker at Fountain Circle! She has been so helpful and caring to our family. She does what she says she’s going to do and then follows up with us. It’s refreshing to find someone with that kind of work ethic! We appreciate you, Loretta! God Bless You

This place is a total disgrace to the healthcare industry. My brother was transferred from St. Joe after being “accepted” for rehabilitation to Fountain Circle, along with 3 other facilities. I asked local opinions (personal and professional) about the ones he had to choose from and no one had anything good to say about this place but because it was closer to home and more convenient for family to come visit, we chose this one. Upon entering the building, the atmosphere was not welcoming and there was a man who had fallen out of his wheelchair and was in the floor yelling for help in his room. I had to tell the nurses he was in the floor. They shook their heads and rolled their eyes like he was a nuisance. The first person we met was an overbearing “nurse manager” named Latasha, whose attitude was extremely judgmental from the time she came and introduced herself. As we were getting my brother settled, Latasha came in and said they wanted to speak with us before we left (meaning my mother, brother and I). We agreed and she came back in with another lady, “the podiatrist” to inform us that the medical doctor (whom we never once met the entire time there), would not continue prescribing the pain medication the hospital doctor had my brother on for TWO WEEKS, that he has been prescribed for over 20 years and that the doctor at the hospital recommended as part of his continued care. The podiatrist continued to accuse my brother of taking someone’s prescription illegally prior to being admitted into the hospital. She was totally disrespectful and discriminatory toward my brother. The doctor that was refusing never once came in to gather any prior health information from my brother before they refused to treat him. Obviously, we decided since they were treating him like this and refused to follow the recommendation for continued care the hospital had provided, it was not a good idea to stay for rehabilitation. The hospital had sent a prescription for 3 days worth of pain medication, when we were leaving, we requested the prescription to be transferred to his pharmacy. We were told that it had been filled by fountain circles pharmacy and they refused to give it to us. When we asked to give us the discharge papers from the hospital back that we had physically handed to them 15 minutes prior, they refused to give us the paperwork (his own personal medical records). I have never in my life seen such a lack of professionalism, empathy and respect in a healthcare setting. We have moved forward with contacting an attorney because this was not okay. Please, DO NOT TAKE YOUR LOVED ONES HERE!!! If they treated my brother like this in front of his own family and as a new patient, who is supposed to feel safe and cared for, think about what is going on behind closed doors for those that don’t have families that can make sure they are not being mistreated.
DO NOT BRING YOUR LOVED ONES HERE FOR CARE. The service, the business, the care. ZERO out of TEN! They are incompetent of taking care of those who cannot take care of themselves. My grandmother wasn’t there a whole 24 hours, before she fell making her injury worse. The whole purpose of her being there was to learn to walk on a walker, after an injury. Which was made clear to the staff, by the doctors who cared for her and family members. My grandmother fell, and had to crawl to the hallway, begging for help. No telling how long she was actually on the floor. Due to the lack of care from the staff in the facility. Who took her fall very lightly. The rooms are filthy and not well kept. When visiting my grandmother, the nurse chuckled and told us, that my grandmother had fallen….. After leaving the facility, we called the facility several times to check on her and no one answered the phone or gave us a call back. Later that night. My grandmother had to be rushed to the hospital, due to the pain of her new injuries that she sustained from the fall. We were not made aware of how severe her injuries and pain were, until 11:30 that night from Fountain Circle, stating that they had to rush her to the hospital. Family was told by staff that she was given Tylenol and taken for X-rays to make sure there were no new fractures and NEITHER of those things were done. We found out later that night at the hospital that she had sustained SEVERAL new fractures from the fall at Fountain Circle. The staff had the wrong information in her chart, which was also a red flag. Although she is my grandmother, they showed her a LACK of dignity towards her as a human being. We wanted to send her somewhere where she would be cared for and treated and they did the absolute opposite. I would not trust anyone who is vulnerable and unable to care for themselves to stay there. If you love your loved ones…. PLEASE do not send them to the Fountain Circle Health and Rehabilitation Center in Winchester Kentucky. Your loved one will only go downhill, after being there. As my grandmother did. And we did not take her back to that AWFUL place that they call a rehabilitation center!!!! The true rating is 0 - 10 !!!! But it made me give them a star to leave my comment.
I will give this facility negative zero, my mother is in the hospital due to negligence on their behalf and a broken hip that was intentional. Elder abuse is a real thing and we need to start looking at all the reviews not the ones they cherry pick to put on their page.
If I could give them a zero I would most of the positive reviews are from employees which I I am sure have been ask to write reviews I repeat absolutely do not take your loved ones here staff is. Absolutely not helpful and very insincere the place is dirty I have seen poor patients wait hours to get help or get to get cleaned up the administration is not helpful at all as long as they can sit in their offices and get a paycheck they are happy the abuse of the elderly that goes on in this place is absurd
I was in an accident and I chose this facility for rehab. The physical therapy team is unbelievable. I want to thank Kim, Brad, Tina and Tracy. Without you I wouldn't be able to have made the amazing recovery I did.

Amazing place!! The new administrator is phenomenal and truly cares about the people who live here. If you want good care for yourself or a loved one I highly recommend Fountain Circle.
There has been a change recently with the administrative staff and I think it has been a positive one. She has been able to answer all of my concerns and questions in a timely manner. I know it’s hard to take care of so many people but they do a good job, I wish more Families were involved with the care but when I have seen is that the staff become their family.

I’m currently in fountain circle rehabilitation section I’ve been here about a month and I have the best day shift nurse Peggy she takes excellent care of me always gives me my medication on time and when I need her she comes as soon as she can I love her to pieces. Also I have 3-4 nurses technicians that I dearly love Krystal Rachel Jamie and Jordan these girl are the sweetest and respectful to everyone you never hear anything bad from them and I know this job can really get next to you but they never show it. They come when called upon the patients me included don’t have to wait long when we call they take care in helping go to the bathroom when you have to leave your room they are very careful handling you. I would definitely recommend fountain circle to everyone if you or a family member needs care rather temporarily or long term this is the place to be if I ever need rehab or long term I’ll definitely come back here
I've been here going on four years, and I have made good friends and received good care.
My mom was there for one day last year. She almost fell due to negligence on their part. No gate belt and no 2 people assist. She just had a hip replacement and they weren't taking any precautions to help with that. She was left on bed pan twice more than 15 minutes. The second time the assistant said that snacks come before bed pans. They wanted me to sign ppw that would not hold them responsible if she fell. Yet the bed had no hand rail on one side. They told me to look in drawer for other one and I could put it on. Many other things to say but will leave it here. They did have a few caring staff members but they can only do what's provided for them.
Was a great experience and i was saved by the staff, therapy was fantastic and Kristie was wonderful to me.
The staff are all very nice people. The food is really good. The physical therapy department is excellent. I been here 3 days and love it here. I'min room104. I will never go to any other rehab place other than Fountain Circle from now on. A+++++

When my brother was at a hospital, he was released to go to Fountain Circle Health and Rehabilitation for some reason. I'm not sure, but it's terrible. It's filthy. There's no good help. He was promised a private room, and he shared a room with someone who had been there for a very long time. To make it short, he was there for nine days and we had to call 911 twice. The first time, they kept him in the emergency room and brought him back. The second time, because he had pneumonia by then, they kept him for a week at that hospital, and that was fine. They were good, but he never went back to Fountain Circle Health and Rehabilitation. It's just a hole. It was awful. I wouldn't put my dog there for one night. That's how bad it was. You would buzz for the nurse to come or something, and they wouldn't come so we have to go down to their nurse's station and get somebody to come help my brother. The food was just awful, too. You couldn't even tell what it was. On one of the menus it was called pizza, and it was just like this round, hard, crunchy thing that you couldn't even bite into. When he got real sick there with his pneumonia, and we kept trying to get them to help him, they said, "Oh he just has a chest cold. He'll be fine." The next day, he went to the hospital in an ambulance and stayed there for a week. They had to drain a liter of fluid off of one of his lungs. He was so bad and they said he just had a cold at that place. I'm surprised they are still in business. The first time he went by ambulance, he was diabetic and his numbers were off the charts. So, my nephew came to the place to check on him and he was doing bad. So, my nephew called 911. The second time, when my brother kept saying, "I can't breathe. I can't catch my breath." They wouldn't do anything. So, he called 911 himself from his cell phone in his room and they came to get him. He had so many things to get well before he could leave and we weren't letting him go back to that awful place.
Do not, I repeat, DO NOT send your loved one here! I don’t even know where to start with this facility… there’s a reason why ALL of the positive google reviews are from employees! Please read the reviews from actual family members before making your judgement. I don’t know where to begin with insincere staff, lack of communication, to rooms reeking of urine with paint peeling and door frames not large enough for a standard wheelchair… just please take this as a sign to not send your loved one here. My father would barely eat the “food” they served and would frequently ask me to bring him dinner… he also rarely received an actual shower. Instead he was given a sponge bath every few days. He would often press his call button and would wait up to 30 minutes for a response. Staff blamed me for my father’s high blood pressure because “you don’t believe in him” when in reality he was homeless and they were discharging him without any plan in place! I found many of the staff to be unprofessional or helpful with my family’s situation. Their idea of “discharge planning” is not at all what it should be. Staff has been very frustrating to work with and doesn’t seem to be understanding of family concerns. Please save yourself the heartache and stress and do not send your loved one here.
This place is absolutely horrid. My aunt is in this facility. She has been neglected to the point of losing her mobility and covered in bed sores and cuts. The staff bathes her and other patience once a week and I’ve witnessed neglect all over the facility. If you have a loved one in need of good care - THIS IS NOT THE PLACE.
Please read entire review before making judgement. My father was admitted 8/26/23 for rehab and discharged 9/1/23. He uses a walker for assistance and has fallen as recently as July 2023. Tree lined, grassy driveway with a large fountain serve as a peaceful, cozy setting. Entrance to building could use some cleaning (lights full of dead bugs and ground was littered with cigarette butts. Foyer and main dining room was nicely decorated but upon entering area where residents live (hallways and common area/nursing station) was very bland/outdated...seemed as if Residents lived in STAFF workspace verses staff working in RESIDENT LIVING SPACE. My interaction with staff was limited but there were a few standouts, unfortuntely I dont have everyone's names: Kristin (RN), Kim (PT) and 2 techs (can't remember names but they'll likely know if they read this) all wonderful! Joel (social services) and another RN (sorry can't remember name) was nice as well. I did over hear some staff using foul language, not in earshot of residents. Concerns/Disatisfactions: 1. On 08/26/23 he sat on toilet in his room and went he attempted to clean his bottom, toilet fell over. I had to sit him and toilet upright. I have pictures of how toilet bolts are rusty and broken. Seal around toilet is broken and appears to have been this way for some time. I immediately let his nurse know what happened and she said she would call maintenance. 08/28/23, at approximately 4:30 pm, I approached maintenance in hallway of facility requesting they look at toilet because it was broken and fell over. 2 maintenance employees entered bathroom, rocked toilet side to side and said they would place a bedside toilet stand as a temporary because it was end of their shift. One stated he would come back in AM on 08/28/23 to see if he could repair and if not he would call a professional plumber. Once they left my father told me they had already shown up before lunch and only looked at toilet and walked out with no words or when it might be repaired. My allegation is that preventive maintenance has not been performed on subject toilet thus causing my father to be in a dangerous situation. He was not hurt physically but mentally was very scared to use toilet. As of 08/29/23, toilet repaired. Smell was putrid! Flooring was loose; I assume toilet had been leaking under LVP flooring. Housekeeping never, to my knowledge, cleaned bathroom floor (I did) and smell never did go away. 2. Unpalatable food - cold upon arrival and had no flavor. My father is on a standard mechanical diet, no restrictions. Some food resembled dog food. I have pictures of food, I felt it and it was cold, I tasted as well. Yuck! 3. Room had damages - a. paint peeling, chipped b. doors damaged c. cabinet door hinge loose d. electrical outlet can be pulled out of wall (maintenance did repair) e. Heat/AC spit unit is old and resident cannot adjust tempetures other than to turn it on or off. If unit is on AC it must stay there even if resident is cold and vice verse as applies to heat. f. bathroom flooring is unsafe as it pops up and down when stepped on g. window has 'blocks' screwed in so that window can't be raised higher than 2-3" nor can it be folded out. h. Awkward placement of bed and TV, might be off-putting for a resident (on an extending arm at head of bedside). 4. Dad said medication was not timely. 5. Billed for a pressure relieving bed but I'm not sure that's exactly what he had. It was very thin and bed frame seemed old/outdated. Will update with pictures. Admissions person was told of food conditions and only stated she would send an email to food services; same employee was told of toilet issue and did not have a response other than I'm sorry that happened. I shared my concerns with Nursing Director or Administrator on 08/29/23. He didn't have any statement but took a few notes and sent maintenance to fix outlet. I am submitting this review because if these are conditions my father is in as a temporary resident, what is it like to be a long term or full time resident?
Fountain Circle is a great place to work
Walk in the place reeks of piss. My individual in their care would be given her meds but would just get sat on their bedside table and not checked on to see if they were taking them or not. Had to wait 30 mins to an hour to pee. Was supposed to be able to get up to pee or be given the bedpan, but instead of answering the call light promptly they were given a brief so they could go ahead and use the bathroom on themself. The reason I was given was “short staffing”. They held their pee so long it set up infection eventually going septic. Was sent out for a blood transfusion, needed platelets and has been unresponsive ever since and is now fighting for their life intubated on ventilation. This place is a joke. Poor management and lazy employees.

Worst nursing home ever. They were cussing patients, leaving them in a bed that has blood all over it and being rude to the visitors. The smell inside this place is disgusting. I wouldn't let a pig stay there...
